Extensive thrombosis revealing a Leinden factor mutation.

Factor V Leiden (FVL) mutation is the most common hereditary thrombophilia and a well-established predisposing factor for venous thromboembolism, while its role in arterial thrombosis remains incompletely understood. We report the case of a 30-year-old postpartum woman who presented with extensive multifocal thromboembolic disease, including bilateral pulmonary embolism, renal infarction, left ventricular apical thrombus in the setting of peripartum cardiomyopathy (LVEF 20%), and acute ischemic stroke-all attributed to an underlying heterozygous Factor V Leiden mutation confirmed by PCR. Reverse Takotsubo Cardiomyopathy Mimicking ST-Elevation Myocardial Infarction in a Postpartum Woman.

Takotsubo cardiomyopathy, also known as Gebrochenes-Herz syndrome, transient apical ballooning syndrome, apical ballooning cardiomyopathy, stress-induced cardiomyopathy, or broken heart syndrome, is a rare disease triggered by stressful events, such as a high plasma concentration of catecholamines and their metabolites, which resembles acute coronary syndrome. It is characterized by transient regional systolic dysfunction of the left ventricle mimicking acute myocardial infarction. The term Takotsubo originates from the Japanese word for an "octopus trap," which resembles the shape of the vent

High sensitivity for peripartum cardiomyopathy among large language models during differential diagnosis consideration.

BackgroundPeripartum cardiomyopathy (PPCM) is a rare, potentially life threatening condition with nonspecific heart failure symptoms, often leading to delayed diagnosis. Large language models (LLMs) may support differential diagnosis suggestion.ObjectivesTo assess whether commonly available LLMs can suggest PPCM and differential diagnoses based on typical symptoms.DesignComparative evaluation of three LLMs using standardized clinical scenarios.MethodsChatGPT (GPT-5), Gemini (2.5 Flash), and Claude (Sonnet 4.5) were queried with prompts describing a 34-year-old woman with PPCM-related symptoms,

Pregnancy-associated extracorporeal life support: insights from a nationwide dual-cohort analysis.

BACKGROUND: Extracorporeal life support (ECLS) is used as rescue therapy for refractory cardiopulmonary failure. Population-level evidence on prognosis and long-term sequelae in patients requiring ECLS during pregnancy or the peripartum period remains limited. METHODS: We performed a nationwide retrospective dual-cohort study using linked Taiwanese databases (2001-22). In the delivery cohort, 247 ECLS pregnancies were compared with 3 970 948 non-ECLS pregnancies. In the ECLS cohort, 247 pregnancy-associated ECLS cases were compared with 2112 non-pregnant women of reproductive age receiving EC

Soluble fms-like tyrosine kinase-1 and peripartum cardiomyopathy: an exploratory meta-analysis of association and diagnostic performance.

INTRODUCTION: Peripartum cardiomyopathy (PPCM) is a rare cause of heart failure in late pregnancy and early postpartum, and biomarkers for risk stratification are lacking. Soluble fms-like tyrosine kinase-1 (sFlt-1), implicated in PPCM and pre-eclampsia, may be associated with PPCM and may have potential diagnostic relevance. This study aimed to evaluate the association between circulating sFlt-1 levels and PPCM and to provide an exploratory assessment of the diagnostic performance of sFlt-1 overall and according to pre-eclampsia status. METHODS: We searched PubMed, Europe PMC, and ScienceDir

Heart Failure Occurring in the Perinatal Period: A Scientific Statement From the American Heart Association.

Heart failure in the perinatal period remains ambiguous in definition and management despite its recognition as a unique disease state. The true incidence and prevalence of heart failure or left ventricular systolic dysfunction during pregnancy and the postpartum period are unknown, although a prevalence as high as 1% to 2% has been reported in the general adult US population. Assessment of heart failure can be challenging in the pregnant or postpartum state, during which symptoms affecting physical function (eg, dyspnea, exercise intolerance, fatigue, and lower-extremity edema) are prevalent

Concurrent Eclampsia, HELLP (Hemolysis, Elevated Liver Enzymes, and Low Platelets) Syndrome and Peripartum Cardiomyopathy in a Primigravida: A Case Report.

Hypertensive disorders of pregnancy can rapidly progress to multi-organ failure. Eclampsia and HELLP (hemolysis, elevated liver enzymes, and low platelet count) syndrome are obstetric emergencies, while peripartum cardiomyopathy (PPCM) is a rare but potentially fatal cause of heart failure in late pregnancy and the postpartum period. We report the case of a critically ill 32-year-old primigravida at 35 weeks' gestation with treated hypothyroidism who presented with generalized tonic-clonic seizures, severe hypertension, and pulmonary edema. Laboratory evaluation demonstrated thrombocytopenia a

Postpartum Complications Modified TBL.

AUDIENCE: The intended audience for this modified TBL are resident physicians of all years, PGY1-4. INTRODUCTION: Biological females represent 50% of the population we encounter in the emergency department (ED), and following delivery, the ED is often the main resource for postpartum patients to seek urgent/emergent care. The US is one of the most dangerous developed nations for pregnant women, with about 1/3 of pregnancy related deaths occurring in the postpartum period.1,2 It is presumed that many of these deaths are related to lack of support or insurance for close follow up after delivery

Severe peripartum cardiomyopathy complicated by multiple left ventricular thrombi and systemic embolic events: a case report.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a potentially life-threatening cause of heart failure occurring towards the end of pregnancy or in the months following delivery. Severe PPCM may be complicated by intracardiac thrombi and systemic embolization, favoured by left ventricular (LV) systolic dysfunction, ventricular dilatation, and the hypercoagulable state of the puerperium. CASE SUMMARY: A 36-year-old woman presented 1-month postpartum with acute decompensated heart failure. Transthoracic echocardiography showed severe LV systolic dysfunction, with a LV ejection fraction (LVEF) of

Lupus myocarditis mimicking peripartum cardiomyopathy: A diagnostic challenge and successful clinical management.

Lupus myocarditis and peripartum cardiomyopathy can both present with acute heart failure symptoms during the peripartum period, leading to diagnostic uncertainty. Accurate and timely recognition of lupus myocarditis is crucial due to its therapeutic implications and high mortality risk if misdiagnosed. This case report presents a 34-year-old female patient with systemic lupus erythematosus who presented to the emergency department 1 week postpartum with palpitations. In the presence of cardiac enzymes and a low ejection fraction on echocardiography, the patient was initially diagnosed with pe

Cardio-Obstetrics Care: Challenging Scenarios in Heart Failure and Future Directions.

Cardiovascular disease (CVD) is a leading and increasingly prevalent cause of maternal morbidity and mortality worldwide, with heart failure (HF) representing a major contributor before, during, and after pregnancy. The age at first pregnancy is rising, with a growing proportion of women aged over 40 years, and hypertension, obesity, and established coronary artery disease at the onset of pregnancy are no longer uncommon. In parallel, advances in the management of congenital, inherited heart disease and childhood cancer have resulted in more women surviving to adulthood and entering pregnancy

Peripartum-Associated Heart Failure Develops Independently of RHOT Proteins.

Pregnancy-associated hemodynamic overload and hormonal changes induce hypertrophy and metabolic remodeling of the maternal heart. Mitochondrial motility, mediated by ras homolog family member T (RHOT) 1 and RHOT2, is essential for cardiac adaptation to increased workload, cardiomyocyte hypertrophy, and sarcomere maturation. To test the hypothesis that Rhot1/2 expression is required for pregnancy- and postpartum-associated adaptations of the maternal heart, female mice with tamoxifen-inducible, cardiomyocyte-selective deletion of Rhot1 and Rhot2 (iRhot1/2-KO) were mated. Following gene deletion

Severe dilated cardiomyopathy in females with dystrophinopathy : a case series of nine patients.

Females heterozygous for a variant in the DMD gene may develop dilated cardiomyopathy; however, progression to severe heart failure is uncommon, and heart transplantation in this population has rarely been reported. We describe nine females with dystrophinopathy and severe cardiac involvement: five underwent heart transplantation, one received a left ventricular assist device, one died from end-stage heart failure, and two remain on medical therapy. Overall, heart transplantation was well tolerated and effective, with 8-11 years of follow-up available for four patients. Immunostaining of expla

Trends in Initial Cardiac Function, Treatment, and Outcomes in Peripartum Cardiomyopathy.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a rare disease that causes heart failure during pregnancy or in the early postpartum period, and there were no reports on the trend of clinical condition and treatment in Japan. This explanatory retrospective study aimed to investigate the changes in clinical conditions and treatment in patients with PPCM. METHODS: Twenty-three patients (aged 22-43 years; mean age, 32 years) with PPCM from four hospitals in Nagasaki Prefecture were included. We compared the heart failure status and treatments of PPCM between the 10 years from January 2002 to Dec

Pregnancies in Women With Long-Chain Fatty Acid Oxidation Disorders: Results of a European and North American Survey.

Long-chain fatty acid oxidation disorders (lcFAODs) are genetic disorders of energy metabolism that are associated with a risk of metabolic decompensation, especially during catabolic episodes. With improvement in diagnostics and treatment, more women with lcFAODs now reach child-bearing age. So far, little is known about the risk and outcome of pregnancies, particularly in women with more severe forms of lcFAODs. We performed an international web-based survey among health care professionals involved in the care of individuals with lcFAODs and collected data on 89 pregnancies in 39 women (mild

Stroke in pregnancy.

Maternal stroke, while rare, leads to significant maternal morbidity and mortality. Physiologic changes in pregnancy and the postpartum period disrupt cerebrovascular homeostasis, predisposing to pregnancy-specific syndromes and tripling stroke risk. Mechanisms of pregnancy-related stroke are diverse and include cardioembolism, vasculopathies (cervical artery dissections, reversible cerebral vasoconstriction syndrome), intracranial hemorrhage related to vascular lesions or preeclampsia, and cerebral venous thrombosis. Heterogeneity of presentation and limited experience treating pregnant women
