Peripartum Cardiomyopathy in Obstetric Practice: A Case Series.

BACKGROUND: Peripartum cardiomyopathy is a rare but potentially life-threatening form of pregnancy-associated heart failure that occurs in the last month of pregnancy or within the first few months following delivery in women without preexisting cardiac disease. The clinical presentation often overlaps with normal physiological changes of pregnancy and the puerperium, leading to delayed diagnosis and increased maternal morbidity. Imaging modalities such as chest X-ray and echocardiography play a crucial role in early recognition and confirmation of the disease. OBJECTIVES: To describe the cli

Prognostic value of late gadolinium enhancement in peripartum cardiomyopathy: a systematic review and meta-analysis.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a rare but life-threatening condition occurring during late pregnancy or the postpartum period. Although late gadolinium enhancement (LGE) has prognostic value in other cardiomyopathies, its role in PPCM remains unclear. This study aimed to evaluate the prognostic significance of LGE in PPCM. METHODS: Three electronic databases were systematically searched to identify studies assessing the association between LGE and clinical outcomes in PPCM. Outcomes included reduced left ventricular ejection fraction (LVEF), arrhythmia, heart failure decompen

Fulminant Postpartum Weil's Syndrome Presenting with Refractory Seizures and Multiorgan Failure: A Case Report and Critical Care Management.

Leptospirosis is a globally prevalent zoonotic infection with protean manifestations, while severe disease (Weil's syndrome) is uncommon and rarely reported in the postpartum period, where it can closely mimic obstetric emergencies. We report a rare case of severe postpartum leptospirosis complicated by sudden-onset seizures and multiorgan failure. A 32-year-old woman developed generalized tonic-clonic seizures two hours after vaginal delivery, followed by septic shock, hyperpyrexia, refractory seizures, acute liver failure, acute kidney injury, coagulopathy, and stress-induced cardiomyopathy.

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Nationwide Outcomes of Heart Transplantation for Postpartum Cardiomyopathy.

BACKGROUND: Postpartum cardiomyopathy is defined as an incident of acute heart failure in the postpartum period in the absence of any other cause. Up to 10% of postpartum cardiomyopathy may need to undergo heart transplantation later in life. This study aimed to provide a present-day perspective on all-cause mortality and transplant-related complications after heart transplantation for postpartum cardiomyopathy. METHODS: A retrospective analysis of the United Network for Organ Sharing (UNOS) registry was performed for adult patients undergoing heart transplants (01/2001-01/2023) for postpartu

[Postpartum dyspnoea : an inaugural and atypical manifestation of preeclampsia].

Hypertensive disorders of pregnancy and recommendations for their management are currently well defined. However, postpartum manifestations are considerably less so. Postpartum preeclampsia is a poorly studied pathological entity. It is not certain whether it is a distinct entity from antenatal preeclampsia. Diagnostic criteria include the new onset of hypertension within 48 hours to 6 weeks after delivery, associated with at least one other clinical or biological criterion. Most cases of postpartum preeclampsia occur within 7 to 10 days after delivery. Risk factors are similar to those of ant

Pregnancy-Related Cardiovascular Diseases: A Radiological Overview.

Pregnancy induces significant hemodynamic changes, and cardiovascular diseases (CVDs) are one of the leading causes of non-obstetric maternal morbidity and mortality during pregnancy or the postpartum period in developed countries. The effective diagnosis and management of CVDs in pregnant women require a thorough evaluation that considers the health of both the mother and the fetus. Imaging plays a pivotal role in this evaluation, offering essential insights into the most significant pregnancy-related CVDs. However, due to concerns about fetal exposure, the use of contrast agents and radiatio

An unexpected battle with peripartum cardiomyopathy: a case report.

Peripartum cardiomyopathy (PPCM) is a rare cardiomyopathy marked by systolic dysfunction that presents in late pregnancy or the early postpartum period with an ejection fraction (EF) of less than 45%. Diagnosing PPCM often presents a diagnostic dilemma due to its nonspecific clinical presentation, which usually resembles physiological changes of pregnancy or peripartum pulmonary embolism. Echocardiography is frequently used as a diagnostic modality of choice with management following the GDMT guidelines and delivery. This case presents a 23-year-old patient with a delayed diagnosis of PPCM, fo

Circulating follistatin-like 3 and its association with postpartum cardiovascular dysfunction and severe maternal morbidity.

OBJECTIVES: Despite postpartum cardiovascular dysfunction being the leading cause of pregnancy-related mortality in the United States, it is difficult to identify at-risk patients. The objective of this study was to determine if antepartum follistatin-like 3 levels correlate with postpartum cardiovascular dysfunction and maternal morbidity. STUDY DESIGN: This observational cohort study included pregnant patients ≥ 18 years old and singleton gestation < 41 weeks who delivered at the University of Chicago between May 2017 and November 2020. MAIN OUTCOME MEASURES: The primary outcome was postpa

Rumen microbes affect the somatic cell counts of dairy cows by modulating glutathione metabolism.

Healthy mammary glands are essential for high-quality milk production in the dairy industry. The relationship between somatic cell counts (SCCs), rumen fermentation, and microbiota interactions remains unclear. This study integrated physiological indicators, high-throughput 16S rRNA gene sequencing, and metagenomics data analysis to investigate the mechanisms linking rumen microbes and mastitis and to evaluate the changes in milk production and serum cytokine levels in cows with low (L-SCC) and high (H-SCC) somatic cell counts. Compared with the L-SCC group, the H-SCC group exhibited significa

Peripartum Cardiomyopathy as the Initial Manifestation of Undiagnosed Sickle Cell Disease: A Case Report.

Peripartum cardiomyopathy (PPCM) occurring in the context of sickle cell disease (SCD) is exceedingly rare, particularly as the initial presentation of undiagnosed SCD. We report the case of a 39-year-old African primigravida at 36 weeks of gestation who presented with a 1-week history of heart failure symptoms. Her antenatal course was largely uneventful apart from severe anemia, for which she was transfused 2 units of whole blood followed by prescription of hematinics. Laboratory investigations revealed hemolytic anemia, a positive sickling test, and confirmatory hemoglobin electrophoresis d

Respiratory physiological changes in pregnancy.

Pregnancy induces a wide range of physiological changes in the respiratory system that significantly affect pulmonary function and gas exchange. These changes are mediated by both mechanical factors-such as the expanding uterus and elevation of the diaphragm-and hormonal influences, particularly from progesterone, estrogen, relaxin, and prostaglandins. Progesterone and estrogen increase minute ventilation and reduce arterial PaCO2, while relaxin enhances ligamentous flexibility, allowing ribcage expansion to accommodate the growing fetus. Anatomical adaptations, including diaphragm elevation a

Trajectory of Premature Ventricular Complexes With Pregnancy and Cardiomyopathy.

BACKGROUND: Premature ventricular complexes (PVCs) are influenced by hormonal fluxes during pregnancy and postpartum. However, the burden and trajectory of PVCs during these periods and subsequent pregnancies are not studied. CASE SUMMARY: A 35-year-old woman developed frequent PVCs and nonischemic cardiomyopathy during her initial pregnancy. Her PVC burden and cardiac function were closely monitored during postpartum and subsequent pregnancy. Catheter ablation was offered, and she reported a second pregnancy before ablation scheduling. An implantable loop recorder was placed to track her PVC

Risk Factors for Peripartum Cardiomyopathy: A Systematic Review and Meta-Analysis.

BACKGROUND: Peripartum cardiomyopathy (PPCM) refers to heart failure following the last month of gestation or within 5 months postpartum, which is a major cause of pregnancy-related death. However, the risk factors for PPCM remain unclear. Therefore, this meta-analysis aimed to identify the risk factors for PPCM. METHOD: All studies on risk factors for PPCM published up to April 2024 in Embase, PubMed, the Cochrane Library, and Web of Science were systematically searched. The quality of the included studies was assessed with the Newcastle-Ottawa scale (NOS). RESULTS: Eighteen studies involvi
