Cardiac Considerations in Pregnancy: A Spotlight on Peripartum Cardiomyopathy and Pregnancy-Associated Spontaneous Coronary Dissection.

Cardiovascular diseases complicating pregnancies are major causes of peripartum morbidity and mortality. The field of Cardio-Obstetrics has evolved to treat women with acquired and metabolic-related cardiovascular diseases in addition to congenital heart disease. Peripartum cardiomyopathy (PPCM) is a heart failure etiology specific to pregnant and post-partum women. Pregnancy-associated spontaneous coronary dissection (p-SCAD) is the most common cause of acute myocardial infarction in the pregnant and post-partum population, highlighting the importance of evaluating chest discomfort. Postpartum cardiomyopathy with severe complications: multimodal management and long-term recovery.

Postpartum cardiomyopathy (PPCM) is a rising research interest in idiopathic cardiomyopathy with heart failure secondary to systolic dysfunction of the left ventricle, diagnosed when no other origin can be established. As the diagnostic possibilities progress, the frequency of PPCM rises, becoming challenging even for the multidisciplinary team. This case goes beyond the rigid definition of PPCM, as it is hard to say with certainty if the PPCM was "only" initial syndrome, or final diagnosis. Two elements of the treatment procedure should be highlighted as crucial for a patient's recovery: effe

Risk Factors and Temporal Trends of Maternal Heart Failure: A Population-Based Study Over 2 Decades.

BACKGROUND: Maternal heart failure (HF) is increasing in several countries, but trends in Sweden remain unclear. The extent to which changes in maternal risk factors contribute to this trend and the proportion of cases linked to associated cardiac conditions are not well established. OBJECTIVES: The purpose of this study was to estimate the annual incidence of maternal HF in Sweden over the last 2 decades, assess the proportion with associated cardiac disease, identify risk factors, and examine whether changes in risk factor prevalence explain temporal trends. METHODS: National cohort study

REBECGA: Brazilian Registry of Heart Disease and Pregnancy. Multicenter Epidemiological Study of Heart Diseases in Pregnancy: Retrospective Cohort.

BACKGROUND: Heart disease, which affects approximately 4% of pregnancies, is the leading non-obstetric cause of maternal death. It is estimated that 40% of these deaths could be prevented with a better understanding of pregnancy risks. OBJECTIVES: To develop the Brazilian Registry of Heart Diseases in Pregnancy (REBECGA) to study the prevalence, complications, and maternal mortality associated with heart diseases during pregnancy and postpartum. METHODS: A multicenter retrospective study of pregnancy and postpartum follow up until 12 months after delivery in women with heart diseases, includ

Don't go vaping my heart: e-cigarette exposure during pregnancy promotes peripartum ventricular arrhythmias and sympathetic dominance.

Electronic cigarettes (e-cigs) are commonly presumed as a safer alternative to traditional cigarettes despite limited understanding of their health risks. This assumption may lead people, especially those in at-risk groups, to switch from traditional cigarettes to e-cigs or to use e-cigs as their first form of nicotine consumption. Despite that pregnancy increases susceptibility to adverse cardiac events, a considerable fraction of pregnant women use e-cigs during gestation and postpartum. Pregnancy involves profound physiological changes that increase the risk for developing cardiac arrhythmi

Pregnancy and breastfeeding in the mother with cardiomyopathy.

Cardiomyopathies are diseases of the heart muscle, characterised by structural and functional abnormalities of the myocardium that are not caused by ischemia, valvular problems or congenital heart disease. They are responsible for one-third of pregnancy-related cardiovascular deaths. A woman may enter pregnancy with a pre-existing diagnosis, or the cardiomyopathy may emerge or develop de novo during pregnancy. The most common cardiomyopathies encountered in pregnancy are dilated cardiomyopathy, hypertrophic cardiomyopathy and peripartum cardiomyopathy. All cardiomyopathies can be complicated b

Peripartum cardiomyopathy in a 28-year-old patient-case report.

Peripartum cardiomyopathy (PPCM) is a rare but serious form of heart failure that occurs toward the end of pregnancy or in the earlypostpartum period. We present the case of a 28-year-old woman who developed severe biventricular heart failure and cardiac arrest shortly after childbirth. Despite treatment with mechanical circulatory support with veno-arterial extracorporeal membrane oxygenation (VA-ECMO) and intra-aorticballoon pump (IABP), her condition did not improve, and she was urgently referred for heart transplantation. Histological examination of theexplanted heart revealed noncompactio

Reverse Remodeling in Late Presenter Peripartum Cardiomyopathy: Strain and Myocardial Work Analysis.

Peripartum cardiomyopathy (PPCM) is a condition of myocardial systolic dysfunction with dilated cardiomyopathy phenotype which occurs in the last months of pregnancy to several months postpartum period. There are several predictors of poor recovery in PPCM, especially severe left ventricular (LV) systolic dysfunction at initial presentation. We reported a case of a 41-year-old woman with late presenter PPCM and the presence of poor predictors of recovery from echocardiographic parameters. Echocardiography showed severely reduced ventricular function and global hypokinetic LV with severe tricus

Migraine and pregnancy-related headaches as a risk factor for cardiovascular and cerebrovascular events in pregnancy: a systematic review and meta-analysis of over 94 million pregnancies.

BACKGROUND: Migraine is prevalent among women of childbearing age and is associated with increased long-term cardiovascular and cerebrovascular risk. Pregnancy, a hypercoagulable state, may potentiate these risks. This study aimed to quantify the association between migraine or pregnancy-related headaches and cerebrovascular and cardiovascular events during pregnancy and the postpartum period. METHODS: We conducted a PRISMA-compliant systematic review and meta-analysis of observational studies comparing pregnant women with and without migraine or pregnancy-related headaches. The Efficacy of Sacubitril/Valsartan in the Treatment of Peripartum Cardiomyopathy: A Retrospective Analysis of Four Cases.

Peripartum cardiomyopathy (PPCM) is a rare form of cardiomyopathy that develops in the last months of pregnancy or during the postpartum period and is characterized by a decrease in left ventricular ejection fraction below 45%. The diagnosis and management of PPCM can be difficult because the symptoms of the disease may be confused with normal pregnancy symptoms. This requires physicians to maintain a high level of clinical suspicion. PPCM can be fatal if not correctly diagnosed and treated. In this case series, we describe the clinical course of four PPCM patients treated with sacubitril/vals

Clinical Outcomes in Peripartum Cardiomyopathy Complicated by Cardiogenic Shock: A Retrospective Multicenter Cohort Study.

Peripartum cardiomyopathy (PPCM) is a rare but life-threatening condition that occurs in late pregnancy or early postpartum and leads to heart failure with reduced left ventricular ejection fraction (LVEF). A severe complication of PPCM is cardiogenic shock, and its incidence has increased in the recent years. We conducted a retrospective multicenter cohort analysis to evaluate the 180-day clinical outcomes of PPCM complicated by cardiogenic shock (PPCM-CS), with a focus on the role of mechanical circulatory support. We identified 733 patients diagnosed with PPCM-CS between January 2010 and Ja

A Case of Postpartum Preeclampsia With an Unusual Presentation.

Preeclampsia is a hypertensive disorder of pregnancy that usually presents with elevated blood pressure. It may also occur in the postpartum period with atypical features, which can complicate timely recognition and adequate treatment. We report a case of a 33-year-old female patient who presented five days postpartum with chest tightness and shortness of breath. Initial findings were atypical for preeclampsia. She was severely bradycardic and normotensive, though hypoxic and tachypneic. Workup excluded postpartum cardiomyopathy and pulmonary embolism (PE), but revealed small bilateral pleural

Peptidyl-tRNA hydrolase 2 is a negative regulator of peripartum cardiomyopathy with heart failure in female mice.

The peptidyl-tRNA hydrolase 2 (PTRH2, Bit-1, BIT1) gene plays a pro-survival role during development with loss of function gene mutations causing congenital infantile multisystem disease (IMNEPD). In wild-type female mice hearts, Ptrh2 protein levels significantly increase during pregnancy and decrease postpartum, demonstrating a protective role in response to pregnancy-initiated cardiac stresses. Peripartum cardiomyopathy (PPCM) is due to dysregulated protective signaling in the pregnant heart. The genetic and molecular mechanisms underlying PPCM remain poorly defined with no specific therapi

Advances in biomarker discovery for peripartum cardiomyopathy: current status and future directions.

INTRODUCTION: Peripartum cardiomyopathy (PPCM) is a severe condition with nonspecific symptoms and limited diagnostic biomarkers. Conventional markers such as BNP and NT-proBNP lack specificity, complicating differentiation from other cardiac disorders. AREAS COVERED: We conducted a comprehensive literature search in PubMed, Scopus, and Web of Science for studies published between January 2000 and August 2025 using keywords related to PPCM and biomarkers. Recent advances highlight novel markers in prolactin-mediated and angiogenic pathways. Cathepsin D shows therapeutic potential, while miR-1
