Multisystem challenges in reproductive medicine: A case report of bicornuate uterus, fibroadenoma, and postpartum thyroiditis.

RATIONALE: A bicornuate uterus is an uncommon mullerian duct malformation strongly associated with recurrent early pregnancy loss and adverse obstetric outcomes. When combined with endocrine disturbances such as hyperprolactinemia and postpartum thyroid dysfunction, fertility becomes even more challenging. This case highlights how coordinated reproductive-endocrine management can overcome multiple intersecting barriers to conception and successful pregnancy. PATIENT CONCERNS: A 24-year-old woman presented with a history of 3 first-trimester miscarriages, difficulty conceiving, and prior breas

Evolving autoimmune thyroid disease in the postpartum period with incidental identification of papillary thyroid microcarcinoma: A case report.

Postpartum immune rebound may precipitate autoimmune thyroid disease. The sequential development of postpartum thyroiditis, Graves' disease, Hashimoto thyroiditis, and papillary thyroid carcinoma in a single patient has not previously been described. A 30-year-old woman (G3P1204) presented five months after a preterm dichorionic-diamniotic twin delivery (at 32 weeks 2 days) with thyrotoxicosis and weight loss. Laboratory evaluation showed suppressed thyroid stimulating hormone, positive thyrotropin receptor antibodies, and elevated thyroid peroxidase antibodies. Radioiodine uptake was low, con

Simulation of large-angle scattered light propagation in non-rotationally symmetric optical systems.

Large-angle scattered light propagation in non-rotationally symmetric optical systems is difficult to simulate because a wide angular spectrum can impose conflicting requirements on spatial sampling and computational-window size. We propose a sectorized angular domain synthesis method (SASM) to address this problem. The target scattering angular domain is determined by downstream aperture acceptance and divided into overlapping local sectors using partition-of-unity weighting to preserve the prescribed angular power distribution. In each sector, the large deflection carrier is removed, the res

Autoimmune thyroiditis in pregnancy: epidemiology, clinical impact, and contemporary management challenges.

Autoimmune thyroiditis represents the most common cause of thyroid dysfunction in women of reproductive age and is frequently encountered during pregnancy. The presence of thyroid autoantibodies, particularly anti-thyroid peroxidase antibodies, has been associated with adverse obstetric outcomes even in the absence of overt thyroid dysfunction. However, optimal screening, monitoring, and treatment strategies remain areas of ongoing debate. This study aimed to review recent evidence on the epidemiology, clinical impact, diagnosis, management, and emerging research directions of autoimmune thyro

Thyroid disease as a teratogen.

Maternal thyroid hormones are critical for normal fetal development, particularly brain development. The increase in metabolic demand of pregnancy results in critical physiologic thyroid changes which ultimately lead to an increase in thyroid hormone production. Placental iodothyronine deiodinases tightly regulate the transfer of thyroid hormone to the fetus. As such, too little or excess thyroid hormone can have a teratogenic effect. Therefore, diagnosis and management of thyroid disorders in pregnancy is necessary to prevent adverse maternal, fetal, and neonatal outcomes; however, it remains

Thyroid Gland and Pregnancy - Clinical Update.

Thyroid function undergoes significant alterations during pregnancy due to changes in hormone levels and higher metabolic demands. The thyroid gland in pregnancy enlarges by 10%; however, in iodine-deficient areas, this growth might reach 40%. Elevated levels of human chorionic gonadotropin (hCG) in early pregnancy leads to increased thyroid hormone production. While triiodothyronine (T3) and thyroid-stimulating hormone (TSH) do not cross the placenta, thyroxine (T4) does. Thyroid hormone demands peaks between weeks 16 and 20 of pregnancy and remains high until delivery. There is a rise in the

Pregnancy-associated thyroid disorders: the role of genetic, epigenetic, and oxidative stress factors.

Thyroid inflammation during pregnancy, particularly Hashimoto's thyroiditis (HT) and postpartum thyroiditis (PPT), has a strong genetic and epigenetic basis. Susceptibility to these conditions is associated with specific HLA haplotypes (HLA-DR3, DR4, DR5) and immune-regulatory genes, including CTLA-4, PTPN22, FOXP3, as well as thyroid-specific genes such as TSHR, TG, and TPO. CTLA-4 polymorphism (CT60) is linked to increased thyroid autoantibody production, while PTPN22 R620W variant disrupts immune tolerance, exacerbating autoreactive lymphocyte activation.Epigenetic modifications play a cruc

Cost-Effectiveness Analysis of Universal Screening for Subclinical Hypothyroidism in Pregnancy Based on the Czech Nation-Wide Pilot Screening Program in Light of Current Evidence.

OBJECTIVES: In 2023, the Czech Guideline by Endocrinology society introduced universal screening of thyrotropin (TSH) in the first trimester of pregnancy. We modeled the incremental cost-effectiveness ratio (ICER), budget impact (BI), and new endocrinology referrals (ER) for universal screening of (1) TSH for subclinical hypothyroidism compared with the current testing and (2) extension to anti-thyroid peroxidase antibodies (anti-TPO). METHODS: A decision tree was built assuming that levothyroxine reduces the risks of gestational hypertension, miscarriage, and preterm birth and anti-TPO test

Sexual Functioning and Depressive Symptoms in Levothyroxine-Treated Women with Postpartum Thyroiditis and Different Vitamin D Status.

Background/Objectives: Hypothyroidism and thyroid autoimmunity have a negative effect on women's sexual health, which is only partially reversed by thyroid hormone substitution. Sexual functioning in thyroid disorders after delivery has been poorly researched. The aim of our study was to compare the effect of levothyroxine on sexual response and depressive symptoms in women with postpartum thyroiditis (PPT) and different vitamin D status. Methods: The study population consisted of three matched groups of women with the hypothyroid phase of PPT: two groups with subclinical and one with overt th

Impact of Thyroid Autoantibodies on Pregnancy Outcomes in Euthyroid Women: A Prospective Cohort Study.

Background Thyroid autoantibodies, including thyroid peroxidase antibodies (TPO-Ab) and thyroglobulin antibodies (TgAb), are commonly found in euthyroid women and have been linked to adverse pregnancy outcomes. However, their role in predicting complications in euthyroid pregnant women remains poorly defined. Objective This study aimed to evaluate the association between elevated TPO-Ab and TgAb levels and the risk of miscarriage, preterm birth, and postpartum thyroiditis in euthyroid women. Methods A prospective cohort study was conducted with 150 euthyroid pregnant women, with blood samples

Vitamin D in Endocrine Disorders: A Broad Overview of Evidence in Musculoskeletal, Thyroid, Parathyroid, and Reproductive Disorders.

Vitamin D is well established for its skeletal effects, being a cornerstone of several endocrine disorders. In recent years, it has come under investigation as a potential disease-modifying drug in several endocrine disorders through its immune modulatory and anti-tumorigenic action, particularly in thyroid disease, gynecologic disorders, and general fertility. Vitamin D supplementation is well established in the treatment of osteoporosis, osteomalacia, hypoparathyroidism, and primary hyperparathyroidism. In autoimmune thyroid disease, there is a negative correlation between 25(OH)D3 levels an

Effects of Trace Elements on Endocrine Function and Pathogenesis of Thyroid Diseases-A Literature Review.

The thyroid gland is an endocrine organ whose hormones enable the proper functioning of the organism. The normal function of this organ is influenced by internal and external factors. One of the external factors is trace elements. Trace elements in appropriate concentrations are necessary for the proper functioning of the thyroid. Fe, Cu, Mn, I, Zn, and Se are part of the enzymes involved in oxidative stress reduction, while Cd, Hg, and Pb can increase ROS production. Cu and Fe are necessary for the correct TPO synthesis. An imbalance in the concentration of trace elements such as Fe, Cu, Co,

Concurrent Thyroid Storm From Postpartum Thyroiditis and Lymphocytic Hypophysitis: A Rare Case.

Postpartum autoimmune thyroiditis (PAT) and lymphocytic hypophysitis (LH) are rare autoimmune conditions occurring during or after pregnancy. We present a case of a 35-year-old woman who developed thyroid storm because of PAT followed by LH, resulting in panhypopituitarism. Initial presentation included hemodynamic instability, altered consciousness, and hyperthyroid symptoms. She was treated for thyroid storm and subsequently developed hypernatremia, suggestive of diabetes insipidus. MRI revealed features consistent with LH. This case highlights the complex interplay between immune dysregulat

Sexual Functioning and Depressive Symptoms in Women with Postpartum Thyroiditis.

Background/Objectives: The presence of autoimmune thyroiditis was found to be associated with an increased prevalence of sexual dysfunction. Women's sexual health was not investigated in postpartum disorders of the thyroid gland. The aim of this study was to assess female sexual functioning and depressive symptoms in postpartum thyroiditis. Methods: This study compared four groups of non-lactating women who gave birth within 12 months before the beginning of the study: women with postpartum thyroiditis and overt hypothyroidism (group A), women with postpartum thyroiditis and subclinical hypoth

Qualitative and quantitative CEUS assessment for differential diagnosis in thyrotoxicosis: destruction vs. hyperfunction.

PURPOSE: Thyrotoxicosis results from either thyroid hyperfunction, as seen in Graves' disease, or tissue destruction, as in subacute thyroiditis (SAT). Differential diagnosis is essential but can be challenging, with thyroid scintigraphy traditionally used to distinguish between these types. This study investigates the potential of Contrast-Enhanced Ultrasound (CEUS) to provide a non-invasive alternative for differentiating hyperfunction and destructive thyroid disease. METHODS: In a prospective study, 42 patients with untreated thyrotoxicosis were evaluated using CEUS to analyze both qualita

Postpartum Psychosis as a Consequence of Thyroiditis Versus Relapse: A Diagnostic Dilemma.

Thyrotoxicosis can exhibit overlapping symptoms of psychosis in the general population. Each of these pathologies has well-established workups and management. Rare presentations of thyroiditis and psychosis in the postpartum state have been seen in case studies mostly, but data on the prevalence of postpartum psychosis in association with postpartum thyroiditis are not available. Here, we present a unique case of a patient with a history of bipolar disorder who originally presented with postpartum thyroiditis that was worked up and managed appropriately. However, on follow-up, the patient was
