Postpartum thyroiditis.

Postpartum thyroiditis (PPT) is a syndrome of transient or permanent thyroid dysfunction occurring in the first year after delivery or abortion. It is the most common thyroid disease in the postpartum period with incidence between 5 and 9%. In essence, it is an autoimmune inflammation of the thyroid, caused by changes in humoral and cell-mediated immune response. It has a characteristic biphasic course with an episode of transient thyrotoxicosis followed by transient or permanent hypothyroidism. Of all predisposing factors positive titers of thyroid peroxidase antibodies have the greatest impo

Thyroiditis: an integrated approach.

Thyroiditis is a general term that encompasses several clinical disorders characterized by inflammation of the thyroid gland. The most common is Hashimoto thyroiditis; patients typically present with a nontender goiter, hypothyroidism, and an elevated thyroid peroxidase antibody level. Treatment with levothyroxine ameliorates the hypothyroidism and may reduce goiter size. Postpartum thyroiditis is transient or persistent thyroid dysfunction that occurs within one year of childbirth, miscarriage, or medical abortion. Release of preformed thyroid hormone into the bloodstream may result in hypert

Selenium and the thyroid gland: more good news for clinicians.

The thyroid is the organ with the highest selenium content per gram of tissue because it expresses specific selenoproteins. Since the discovery of myxoedematous cretinism and thyroid destruction following selenium repletion in iodine- and selenium-deficient children, data on links between thyroid metabolism and selenium have multiplied. Although very minor amounts of selenium appear sufficient for adequate activity of deiodinases, thus limiting the impact of its potential deficiency on synthesis of thyroid hormones, selenium status appears to have an impact on the development of thyroid pathol

The first case of association between postpartum thyroiditis and thyroid hormone resistance in an Italian patient showing a novel p.V283A THRB mutation.

BACKGROUND: Postpartum thyroiditis (PPT) is characterized by the development of postpartum thyroid dysfunction, which may occur up to 12 months after delivery. The syndrome usually presents with transient thyrotoxicosis, followed by transient hypothyroidism. The association of this condition with resistance to thyroid hormones (RTH) has never been described. PATIENT FINDINGS: In this report, we describe a 30-year-old patient affected by RTH due to a novel p.V283A thyroid hormone receptor-β (THRB) heterozygous mutation in exon 8, which affects the ligand-binding domain, never before described

[Thyroid autoimmunity].

This review of human autoimmune thyroid disease (AITD) focuses mainly on the epidemiology and pathophysiology of this very common disorder, although some specific clinical situations are discussed. One peculiarity of AITD is the existence of two contrasting phenotypes: hypothyroid thyroiditis and hyperthyroid Graves' disease. Graves' disease is characterized by the presence of anti-TSH receptor antibodies capable of activating the TSH receptor, leading to thyroid hypertrophy and hyperfunction. In contrast, autoimmune thyroiditis progresses slowly, through necrosis/apoptosis of thyroid cells an

Lingual thyroid: diagnosis using a hybrid of single photon emission computed tomography and standard computed tomography.

AIM: To present a rare case of lingual thyroid detected using a hybrid of single photon emission computed tomography and standard computed tomography in a young woman with hypothyroidism. MATERIALS AND METHODS: A 29-year-old woman was screened for autoimmune thyroid disorders in early pregnancy, and found to have subclinical hypothyroidism. No thyroid tissue was found in the anterior neck upon ultrasonography, raising suspicions of an ectopic thyroid. The patient was treated with levothyroxine throughout the remainder of her pregnancy, and developed postpartum thyroiditis requiring an increas

New-onset Graves' disease in the postpartum period.

During routine evaluation for lactation failure, hyperthyroidism was discovered in a postpartum woman. Although postpartum thyroiditis and Hashimoto's disease are relatively more common than Graves' disease during the postnatal period, this young woman was found to have new-onset Graves' disease. Distinguishing between normal postpartum symptoms and thyroid disorders can be challenging. Utilizing history, physical examination, and laboratory testing, the provider can identify the etiology of hyperthyroidism during the postpartum period. Treatment options differ depending on the cause of the th

[Epidemiology and management of thyroid disorders in pregnancy].

OBJECTIVE: Determine the incidence of asymptomatic decrease of thyroid function at pregnant women in the first trimester of pregnancy in the Olomouc region. MATERIAL AND METHODS: The authors examined 461 women in the first trimester of pregnancy. There was measured serum concentrations of thyrotropin (TSH), free thyroxine (FT4) and thyroidperoxidaseantibody (TPOAb). RESULTS: TSH concentration > 2.5 mIU/l was in 21% women (96/461). Reduction of FT4 < 9.8 pmol/l was found in 15 of the 96 women screened with a TSH greater than 2.5 mU/l. For women with TSH 2.5 mU/l, however, was found reduction

[Disorders of the thyroid gland in pregnancy].

Functional disorder of the thyroid gland affects individuals of all ages. Incidence and nature of the disease is influenced by supplying iodine, genetic and immunological factors and other external effects. Increased function occurs in 1-2% of the adult population, reduced function in the younger population diagnosed in about 5%, in women aged over 50 years we have been at 10-15%. The incidence of thyroid diseases is higher in women than in men (5-8:1) and significantly increases with age. Approximately as common as overt forms of thyroid disease are their subclinical forms. Interventions for clinical and subclinical hypothyroidism pre-pregnancy and during pregnancy.

BACKGROUND: Over the last decade there has been enhanced awareness of the appreciable morbidity of thyroid dysfunction, particularly thyroid deficiency. Since treating clinical and subclinical hypothyroidism may reduce adverse obstetric outcomes, it is crucial to identify which interventions are safe and effective. OBJECTIVES: To identify interventions used in the management of hypothyroidism and subclinical hypothyroidism pre-pregnancy or during pregnancy and to ascertain the impact of these interventions on important maternal, fetal, neonatal and childhood outcomes. SEARCH METHODS: We sear

Thyroid disease in pregnancy: (Women's Health Series).

Pregnancy is a state of many hormonal changes that can make interpretation of thyroid function tests difficult. Measuring trimester-specific reference values of thyrotropin and free thyroxine is recommended. Because overt maternal hypothyroidism negatively affects the fetus, timely recognition and treatment are important. Women taking levothyroxine prepregnancy require a ≤50% dose increase during pregnancy. Hyperthyroidism can result from excessive human chorionic gonadotropin or Graves disease. Radioactive scanning should be avoided during pregnancy. Antithyroidal drug therapy should consist

[The impact of thyroid function in women of reproductive age: infertility, pregnancy and the postpartum period].

This article reviews the management and diagnosis of thyroid dysfunction during pregnancy and postpartum, which was published by any of the endocrine societies in 2012. The author presents human data based on these clinical practice guidelines, however, there are also many unresolved questions. Especially, there are inconsistencies about screening using plasma TSH measurement. In pregnancy the main causes of hyperthyroidism are Graves's disease and gestational transient thyrotoxicosis. Generally, gestational transient thyrotoxicosis does not require medication, whereas Graves's disease needs a

Approach to the patient with postpartum thyroiditis.

Postpartum thyroiditis (PPT) is the occurrence of de novo autoimmune thyroid disease, excluding Graves' disease, in the first year postpartum. The incidence of PPT is 5.4% in the general population, and it is increased in individuals with other autoimmune diseases such as type 1 diabetes mellitus. The classic presentation of PPT of hyperthyroidism followed by hypothyroidism is seen in 22% of cases. The majority of women with PPT experience an isolated hypothyroid phase (48%), with the remainder experiencing isolated thyrotoxicosis (30%). Up to 50% of women who are thyroid antibody positive (th

Cost-effectiveness of universal and risk-based screening for autoimmune thyroid disease in pregnant women.

CONTEXT: Hypothyroidism in pregnancy can lead to adverse maternal and fetal outcomes. Although screening of high-risk women is advocated, universal screening remains controversial. OBJECTIVE: The objective of the study was to compare the cost-effectiveness of universal screening of pregnant women for autoimmune thyroid disease (AITD) with screening only high-risk women and with no screening. DESIGN, SETTING, AND PARTICIPANTS: A decision-analytic model compared the incremental cost per quality-adjusted life-year (QALY) gained among the following: 1) universal screening, 2) high-risk screening

Subacute, silent, and postpartum thyroiditis.

Subacute, silent, and postpartum thyroiditis are temporary forms of thyroid dysfunction caused by thyroid gland inflammation. They classically present with a triphasic course: a brief period of thyrotoxicosis due to release of preformed thyroid hormone that lasts for 1 to 3 months, followed by a more prolonged hypothyroid phase lasting up to 6 months, and eventual return to a euthyroid state. However, the types and degree of thyroid dysfunction are variable in these disorders, and individual patients may present with mild or more severe cases of thyrotoxicosis alone, hypothyroidism alone, or b

Thyroid disorders in pregnancy.

The thyroid gland is substantially challenged during pregnancy. Total T(3) and T(4) levels increase by 50% during pregnancy owing to a 50% increase in thyroxine-binding globulin levels. Serum TSH levels decrease in the first trimester and increase in the second and third trimesters; however, not to prepregnancy levels. Hypothyroidism is present in up to 3% of all pregnant women. Subclinical hypothyroidism during pregnancy is associated with an increased rate of miscarriage and preterm delivery, and a decrease in the IQ of the child. Overt hyperthyroidism is present in less than 1% of pregnant
