Plasma 25-hydroxyvitamin D (25-OHD) was measured in cord blood taken at delivery and in fasting blood taken from 44 Asian mothers in the first day of the puerperium. The maternal deaths in a supra-regional reference hospital between 1971 and 1977 are reviewed and avoidable factors discussed. Improvements in ante-natal care have diminished the number of deaths from obstetric causes and as a result deaths from associated diseases are assuming greater significance. Particular attention is drawn to the enterocolitis group of diseases which account for 15% of the total deaths in this series. The effect of pregnancy on amoebic colitis has not been widely appreciated.

In this study 310 cases of infundibulectomy via laparotomy performed at the Castle Street Hospital for Women in Colombo, Sri Lanka, were analyzed. The influence of oral doses of iron on the hemoglobin regeneration of pregnancy anemias and postpartum anemias was examined. The examinations were carried out in 45 pregnant women and in 70 puerperae. Other hematologic parameters besides hemoglobin were determined before, and 4 and 6 weeks during treatment with 110 mg iron sulfate daily. In pregnancy anemias, hemoglobin regeneration took place at a much slower rate under the iron therapy than was the case in postpartum anemias. The clinical and renal biopsy findings from two patients in whom renal functional abnormalities developed in the late postpartum period are described. Both biopsies showed fibrin deposition in the renal vasculature, in one case marked and in the other mild. The patient with the more severely damaged kidney subsequently died, and the other is alive but with evidence of slowly progressing renal damage. The clinicopathological spectrum and pathogenesis of late postpartum renal failure are discussed.
