Mode of Birth and Stroke Risk After Childbirth Among Women With Moyamoya Disease.

IMPORTANCE: There is limited large-scale evidence to guide the optimal mode of birth for patients with moyamoya disease (MMD). OBJECTIVE: To evaluate whether the mode of birth (cesarean vs vaginal) is associated with stroke risk after childbirth for women with MMD. DESIGN, SETTING, AND PARTICIPANTS: This cohort study evaluated stroke outcomes up to 3 years after childbirth. A nationwide, population-based analysis was performed using data from the Health Insurance Review and Assessment Service of South Korea. Individuals with MMD from January 1, 2002, to December 31, 2023 were identified. Amo

Gestational Diabetes and Postpartum Cardiovascular, Kidney, and Metabolic Disorders.

IMPORTANCE: Gestational diabetes is associated with future cardiometabolic risk, but postpartum evidence across the cardiovascular-kidney-metabolic (CKM) disorder spectrum remains limited. OBJECTIVE: To characterize the timing and magnitude of incident postpartum CKM disorders after a pregnancy complicated by gestational diabetes. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used data from April 1, 2007, to October 12, 2023, from the Merative MarketScan commercial database. Participants were commercially insured females aged 12 to 55 years with a single delivery and no

Peripartum Cardiomyopathy in a Woman Who Conceived Shortly After Bariatric Surgery: A Case Report.

Peripartum cardiomyopathy (PPCM) is a rare but potentially fatal form of heart failure occurring in the peripartum period without an identifiable cause. We report the case of a 36-year-old woman who conceived 2 months after bariatric surgery (BS). At 37 weeks of gestation, she underwent cesarean delivery and developed acute respiratory failure and seizures immediately after extubation. Further evaluation was performed to exclude other acute obstetric and cardiopulmonary conditions, including pulmonary embolism, and transthoracic echocardiography revealed global left ventricular hypokinesia wit

Case Report: Postpartum diagnosis and microsurgical management of a cervicothoracic intramedullary spinal ependymoma clinically manifesting during the third trimester of pregnancy.

BACKGROUND: Intramedullary spinal cord ependymomas are the most common intramedullary tumours in adults. However, clinical manifestation during pregnancy is exceptionally rare, and diagnosis may be delayed because pain, sensory disturbances, and muscle weakness are often mistakenly attributed to the common musculoskeletal changes associated with pregnancy. CASE DESCRIPTION: A 29-year-old multiparous woman developed progressive back pain, numbness, and weakness in both lower extremities at 30 weeks of gestation. She was initially managed conservatively. Following an uncomplicated spontaneous d

Pregnancy in a patient with glycogen storage disease type Ia (von Gierke disease): case report and management strategy.

Glycogen storage disease type I (GSDI) is caused by biallelic pathogenic variants in the gene encoding the glucose-6-phosphatase complex. Deficiency of this enzyme in the liver, kidneys, and intestines leads to glycogen accumulation in these organs and, consequently, to inadequate glucose production due to impaired glycogenolysis and gluconeogenesis. The condition is rare, and only a limited number of cases have been reported in pregnant women with favorable maternal and perinatal outcomes. This is partly because GSDI has been associated with significant morbidity, including maternal metabolic

Perforated Duodenal Ulcer in the Immediate Postpartum Period: Diagnostic Pitfalls Following Caesarean Delivery.

Postpartum abdominal pain is frequently encountered and is most often related to obstetric causes such as uterine involution, infection, or postoperative discomfort following cesarean delivery; however, more atypical etiologies can also occur. Perforated peptic ulcer disease in the immediate postpartum period is uncommon and may be overlooked, particularly when symptoms overlap with expected postoperative findings and recent surgical history. We report a woman in her early 30s who developed severe upper abdominal pain four days after a primary cesarean delivery performed for maternal exhaustio

Reversible cerebral vasoconstriction syndrome: a medical challenge in the peripartum period requiring a multimodal approach: a case report.

Reversible cerebral vasoconstriction syndrome is a rare but potentially life-threatening neurological disorder that may occur during pregnancy and the postpartum period and represents an important diagnostic challenge because of its overlap with other obstetric neurological emergencies. We report the case of a 35-year-old woman with a dichorionic diamniotic twin pregnancy who developed severe preeclampsia requiring cesarean delivery at 30 weeks of gestation. On the fifth postpartum day, she experienced thunderclap headache, generalized seizures, and refractory cerebral vasospasm complicated by

Recurrent Episodes of New-Onset Supraventricular Tachycardia in Severe Early-Onset Pre-eclampsia and Fetal Growth Restriction: A Report of a Complex Cardio-Obstetric Case.

Supraventricular tachycardia (SVT) is one of the commonly reported sustained arrhythmias in pregnancy. The rapid progression from early-onset fetal growth restriction (FGR) to severe pre-eclampsia is a well-recognised manifestation of placental dysfunction. The coexistence of both leads to significant maternal and fetal morbidity, adding substantial complexity to management. We present the case of a 27-year-old primigravida of Caucasian origin with an otherwise uncomplicated pregnancy who attended at 28 weeks' gestation solely because of a maternal concern that her baby appeared small. This se

Right Gonadal Vein Thrombosis in Acute Intermittent Porphyria: A Rare Thrombotic Complication.

Acute intermittent porphyria (AIP) is a rare metabolic disorder characterized by neurovisceral manifestations, most commonly severe abdominal pain. Thrombotic complications are not typically associated with AIP and are infrequently reported in atypical venous territories. Gonadal vein thrombosis (GVT) is an uncommon condition most frequently observed in the postpartum period or in association with pelvic infections and malignancy; its occurrence in patients with AIP is exceptionally rare. GVT is a potentially serious form of deep vein thrombosis, often underdiagnosed due to its nonspecific pre

Postpartum retention in opioid agonist treatment for opioid dependence: A population-based cohort study.

PURPOSE: Opioid agonist treatment (OAT) is the first-line treatment for opioid dependence during pregnancy and recommended for at least one year postpartum or until a strong maternal-infant bond and stable family environment is established. Evidence on postpartum OAT retention is limited. We examined retention rates and associated maternal characteristics. METHODS: We linked OAT prescription authority to perinatal, mortality, and other data sources. We identified all opioid-dependent women who gave birth in New South Wales, Australia (1 January 2004-31 March 2020) while receiving OAT. We defi

Postpartum Psychosis: could genetic vulnerability to insomnia or short sleep duration be protective?

Postpartum Psychosis (PP) is a severe and understudied perinatal mental illness which disproportionately affects women with bipolar disorder (BD). A relationship between sleep disturbance and PP is often assumed, but is poorly understood. From a cohort of 2099 individuals with BD, 343 parous women were identified and screened for perinatal psychiatric complications. We compared 117 women who developed PP with 226 who did not. Polygenic Risk Scores (PRS) for BD, schizophrenia, insomnia, short sleep, long sleep, sleep efficiency and sleep duration were computed using PRS-CS. Logistic regression

Adverse pregnancy outcomes as predictors of postpartum psychiatric illness: a retrospective cohort study.

PURPOSE: Severe maternal morbidity (SMM as defined by the Centers for Disease Control and Prevention) as well as other adverse pregnancy outcomes (APOs) may confer differential risk for postpartum psychiatric illness (PPI). PPI is often underrecognized, and it can have lasting and harmful effects on both the mother’s and the infant’s well-being. The purpose of this cross-sectional study is to explore whether specific pregnancy complications act as potential predictors for PPI, and how this differs across important social determinants of health (e.g. race/ethnicity). METHODS: Participant data w

Antenatal depression and its relationship with birth outcomes and postnatal depression in Rural India: A longitudinal study.

BACKGROUND: Depression is the most common mental disorder among women of reproductive age, with maternal depression the second leading cause of morbidity worldwide. Despite the serious effects on both the mothers and their newborns, it is still a neglected issue. The purpose of this study is to explore the relationship between antenatal depressive symptoms, birth outcomes, and postnatal depression symptoms. METHODS: A community-based longitudinal survey was conducted among pregnant women in the rural Chatra district, Jharkhand, India, between April 2023 and February 2024. Participants were se

The impact of cognitive behavioral group therapy for anxiety on GAD related sleep disturbance during the perinatal period.

BACKGROUND: Generalized anxiety disorder (GAD) is highly prevalent during the perinatal (pregnancy and postpartum) period. Sleep disturbance (SD), a core symptom of GAD, and a common difficulty during the perinatal period, has been overlooked as a specific treatment target This study examined (1) the efficacy of cognitive behavioral group therapy for perinatal anxiety (CBT-PA) in reducing SD among individuals with GAD, and (2) whether SD functions as a mechanism of treatment change. METHODS: Perinatal participants (pregnant or ≤12 months postpartum) with GAD enrolled in a randomized trial (n

Risk factors for birth trauma and postpartum posttraumatic stress in the United Kingdom: Results from the international survey of childbirth-related trauma.

INTRODUCTION: Recent research shows that 40.6% of women in the United Kingdom experience childbirth as at least moderately traumatic, and 5.9% develop childbirth-related posttraumatic stress disorder (CB-PTSD). However, risk factors for birth trauma ratings and CB-posttraumatic stress symptoms (CB-PTSS) in this sample remain unexplored. Therefore, the aim was to understand risk factors for birth trauma and CB-PTSS in a UK sample. MATERIAL AND METHODS: A cross-sectional survey of traumatic birth and CB-PTSD with women (N = 339) from three hospitals in England, two in Wales, and three in Scotla

Psychiatric disorder diagnoses and adverse psychiatric outcomes among pregnant and postpartum adolescents in Ontario, Canada: a retrospective, population-based cohort study.

BACKGROUND: Adolescents appear to be at increased risk of mental illness during pregnancy and the first year postpartum, although most studies to date are small or limited to depression or self-harm. We aimed to compare the risks of psychiatric disorder diagnoses and adverse psychiatric outcomes during pregnancy and the first year postpartum between adolescents and adults. METHODS: In this retrospective, population-based cohort study, we used health administrative data from Ontario, Canada, to identify all singleton livebirth or stillbirths after 20 weeks of gestation between Jan 31, 2013, an
