Interactive tele-mental health interventions for maternal depression across perinatal stages: A systematic review and meta-analysis.

BACKGROUND: Interactive tele-mental health (TMH) interventions have been increasingly used to address mental health issues during the perinatal period. However, their effectiveness across perinatal stages remains unclear. This study aimed to evaluate and compare the effectiveness of TMH interventions for depressive and anxiety symptoms across pregnancy, the perinatal period, and the postpartum period. METHODS: PubMed, Embase, Web of Science, Cochrane Library, PsycINFO, CINAHL, ClinicalTrials.gov, CNKI, and Wanfang were searched up to December 2025 to identify randomized controlled trials (RCT

From childhood to adult trauma and negative birth experiences: distal and proximal factors associated with childbirth-related posttraumatic stress symptoms.

BACKGROUND: Childbirth-related posttraumatic stress symptoms (CB-PTSS) have been recognized as a serious postpartum complication. Although childhood maltreatment (CM) is a well-established vulnerability factor for stress-related mental disorders, including posttraumatic stress disorder (PTSD), its role in CB-PTSS remains largely unexplored. OBJECTIVE: This study aimed to investigate the association between CM and CB-PTSS at 12 months postpartum, while also considering other major risk factors for CB-PTSS, such as adult trauma and subjective birth experiences (SBEs). In exploratory analyses, w

Facilitated Transitions to Postpartum Primary Care and Mental Health Care Services: Secondary Analysis of a Randomized Clinical Trial.

IMPORTANCE: Postpartum anxiety and depression are common complications of pregnancy but often go untreated. Primary care offers an opportunity to provide mental health care services after the immediate postpartum period, but it is underused, in part because of ineffective transitions after delivery from obstetric care to primary care. OBJECTIVE: To evaluate whether an intervention to improve transitions from obstetric care to postpartum primary care is associated with increased mental health care utilization for individuals with mood and anxiety disorders. DESIGN, SETTING, AND PARTICIPANTS:

Emergent simultaneous cesarean section and maternal cardiac surgery.

BACKGROUD: Management of pregnant patients with cardiovascular disease is a challenge, and limited experiences exist to date in the literature. Particularly, valve surgery is still at high risk, with a consistent rate of preterm birth, postnatal physical or mental disabilities, and fetal mortality. CASE PRESENTATION: A 38-year-old woman, who was 37 weeks pregnant, was admitted to the emergency department with severe dyspnea and hypertension. A transthoracic echocardiography revealed a 4 cm mobile mass in the left atrium, attached to the interatrial septum by a stalk; the mass protruded into t

Differential Eating Disorder Symptom Trajectories Across the Perinatal Period and Associated Psychosocial Factors.

BACKGROUND: The perinatal period is a critical risk period for changes in eating disorder (ED) symptoms. However, extant research is mixed regarding the direction of these changes, with some studies identifying relief from ED symptoms and others finding evidence of worsening symptoms during this time. To account for heterogeneity in ED symptom change among pregnant and postpartum individuals, the current study aimed to explore ED symptom trajectories and their psychosocial correlates during pregnancy and the first postpartum year using person-centered statistical approaches. METHOD: A communi

Naloxone Prescribing and Naloxone Administration in Response to Overdose Events Among Women With Opioid Use Disorder During Pregnancy and Postpartum Period - MAT-LINK, Seven Clinical Sites, 2014-2021.

OBJECTIVES: Opioid use disorder (OUD) and overdose among pregnant and postpartum women pose severe risks to maternal and infant health. Naloxone, a life-saving opioid antagonist, effectively reverses opioid overdoses but remains underutilized in this population. We examined naloxone prescription or receipt and administration during overdose events among perinatal women with OUD from the MATernaL and Infant Clinical NetworK (MAT-LINK). METHODS: We analyzed data collected through 7 MAT-LINK clinical sites from pregnant women with OUD and known pregnancy outcomes between January 1, 2014 and Augu

Management of Chronic Hypoparathyroidism and Hyperparathyroidism in Pregnancy/Lactation.

PURPOSE OF REVIEW: We summarize and discuss evidence on management of chronic hypoparathyroidism (HypoPT) and primary hyperparathyroidism (PHPT) in pregnancy and lactation to provide guidance for clinical care. RECENT FINDINGS: Women with HypoPT are at increased risk of certain pregnancy complications, though most pregnancies are uncomplicated. Conventional therapy with activated vitamin D and calcium is continued throughout pregnancy, but there are unpredictable changes in the required dosages warranting regular surveillance during pregnancy (e.g., all 3 to 4 weeks). Data on parathyroid horm

Hypertensive Disorders of Pregnancy and Primary Aldosteronism.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) affect up to 15% of pregnancies and are linked to adverse maternal and fetal outcomes. Primary aldosteronism (PA) affects up to 25% of patients with hypertension. In women with prior HDP and PA risk factors, we examined PA prevalence and its relationship to hypertension trajectory. METHODS: Adults across the US meeting guideline-recommended screening criteria for PA were prospectively tested. Women with a self-reported history of HDP completed a questionnaire examining the relationship between PA and hypertension trajectory. RESULTS: Of 3

Screening for β-Thalassemia Trait in Pregnant Women With Microcytic Hypochromic Anemia and Its Impact on Pregnancy Outcomes.

Background Thalassemia is a common autosomal recessive hemoglobinopathy and a major public health concern in India. In pregnancy, β-thalassemia trait often presents as microcytic hypochromic anemia and may mimic iron deficiency anemia, leading to misdiagnosis and inappropriate management. Early and accurate differentiation is essential to prevent unnecessary iron therapy and reduce adverse maternal and neonatal outcomes. The study was designed to evaluate the prevalence, diagnostic differentiation, and pregnancy outcomes associated with β-thalassemia trait in this population. Methods This pros

Factors associated with inadequate iron supplementation among pregnant women in the Brazilian Unified Health System: a cross-sectional study.

BACKGROUND: Gestational anemia remains an important public health problem, especially in developing countries, and is associated with adverse maternal and perinatal outcomes. Prophylactic iron supplementation during pregnancy is a simple and effective strategy to prevent this condition and is routinely recommended during prenatal care. In Brazil, supplements are provided free of charge through the public health system; however, adherence to treatment remains variable. This study aimed to identify the sociodemographic and healthcare-related factors associated with inadequate iron supplementatio

Assessment of safety and immunogenicity of a protein subunit COVID-19 vaccine (ABDALA) in pregnant women: a single-cohort, multicenter, observational ESPIRTA study.

BACKGROUND: Pregnant and lactating women are generally excluded from clinical trials during vaccine development. While the safety, inmunogenicity, efficacy, and effectiveness of the Abdala vaccine against COVID-19 have been demonstrated in the general population, there is a lack of specific information regarding its benefits for pregnant women. Consequently, this study was undertaken to evaluate the safety of the Abdala vaccine in pregnant women and their newborns, as well as to assess the maternal immune response elicited by the vaccination and its capacity for passive immunity transfer to th

Myelin Oligodendrocyte Glycoprotein Antibodies in Breast Milk in a Lactating Woman With Severe Enterovirus-Associated MOGAD.

OBJECTIVES: Myelin oligodendrocyte glycoprotein antibody (MOG-IgG)-associated disease (MOGAD) is a distinct neuroinflammatory disorder, which may be preceded by infections. Transmission of MOG-IgG from blood into breast milk has not been reported previously. METHODS: We present the case of a 38-year-old lactating woman with a severe manifestation of MOGAD after enterovirus gastroenteritis, who was still lactating at that time. Alongside with detailed clinical characteristics, we measured MOG-IgG in breast milk and serum from the breast-fed infant. RESULTS: Our patient experienced a severe pr

Chronic Headache and Visual Field Loss: Manifestations in a Patient With Chronic Brucellosis Meningitis: Case Report With Brief Literature Review.

Brucellosis is a zoonotic infection caused by Brucella species, often leading to serious complications and presenting diagnostic challenges. A 45-year-old female patient presented with a headache that had persisted for two months and was recently accompanied by visual disturbances. Notably, the patient's headache began 6 weeks postpartum, and she had no prior history of illness, medication use, or headaches. Imaging studies were normal, and cerebrospinal fluid (CSF) analysis revealed meningitis, with a positive Wright test. After excluding other potential causes and considering the patient's c

Schistosomiasis in pregnancy and lactation.

Almost 140 million individuals are infected with one of three primary species of schistosomes globally, including an estimated 35 million women of reproductive age. Women of reproductive age also bear the burden of pregnancy, with additional risks of schistosomiasis to both pregnant women and their offspring. Both prospective studies and randomised controlled trials have implicated schistosomiasis in a range of maternal and newborn adverse outcomes. Schistosomiasis has most clearly been associated with maternal risk for anaemia and iron insufficiency. Three randomised controlled trials conduct

Successful vaginal delivery in very severe malarial thrombocytopenia: a case report.

BACKGROUND: Very severe thrombocytopenia, characterized by a platelet count below 20,000/µL, in pregnant women with malaria, particularly Plasmodium falciparum infection, is a significant concern due to its association with increased mortality. While not always associated with bleeding, it can be a sign of severe disease and may require prompt intervention. CASE PRESENTATION: The author presents a rare case of a successful vaginal delivery in a 22-year-old, Para II, woman from Western Ethiopia, who had very severe malarial thrombocytopenia. She started treatment with artesunate for acute mala

Gaps in the Hepatitis C Prenatal and Postpartum Care Cascade: Rationale for Treatment in Pregnancy.

BACKGROUND: Hepatitis C virus (HCV) infections have increased among younger populations, including pregnant people. While universal screening guidelines have improved case-finding, studies suggest low postpartum linkage-to-care rates. Better understanding of the peripartum HCV care cascade at the population level is needed to inform optimal management, including the role of treatment in pregnancy. METHODS: A retrospective cohort study linking pregnant individuals with HCV test records to health administrative data in Ontario, Canada. We examined the HCV care continuum, including during subseq

PregInPoxVac maternal protocol: a phase 3 trial evaluating maternal immunogenicity and safety of the MVA-BN vaccine in the Democratic Republic of the Congo.

INTRODUCTION: Mpox remains endemic in the Democratic Republic of the Congo (DRC), with pregnant women at increased risk of severe disease and adverse outcomes. The WHO has called for data on mpox vaccination in high-risk groups, but no clinical trial has assessed the safety or immunogenicity of mpox vaccines in this population. The modified vaccinia Ankara-Bavarian Nordic (MVA-BN) vaccine, a third-generation, highly attenuated, non-replicating, live virus vaccine, has shown an excellent safety profile in adolescents and adults, including immunocompromised individuals, and is expected to be saf
