Domperidone Use in Lactation and Risk of Severe Postpartum Mental Health Outcomes.

OBJECTIVE: To evaluate whether postpartum domperidone use is associated with new-onset psychosis and other severe mental health outcomes. METHODS: We conducted a retrospective cohort study of people who filled a prescription for domperidone within 56 days of delivery between March 1, 2006, and March 1, 2022, in Ontario, Canada. Those who filled a domperidone prescription were matched 1:1 based on propensity score to an equal number of those who did not. The primary outcome was any health care contact for incident psychosis in the subsequent 365 days, with a secondary outcome of any psychiatri

Psychotropic drugs in pregnancy and lactation.

INTRODUCTION: The prescription of psychotropic drugs during pregnancy is a constantly increasing practice, but its safety information remains limited. On the other side, while concerns about teratogenicity persist, it is also evident that mothers' psychiatric disorders represent a risk factor for both them and the infants and cannot be neglected or left untreated. Therefore, careful monitoring and dose adjustments are essential to minimize risks while ensuring treatment efficacy. AREAS COVERED: The present article is a narrative review of the literature on psychotropic drug use in pregnancy a

Hypoprolactinemia: Biology, Clinical Relevance, and Diagnostic Challenges.

BACKGROUND: Prolactin (PRL) is a pleiotropic hormone, traditionally associated with lactation, but now recognised for its role in reproduction, metabolism, and neuroendocrine and immune regulation. While the clinical features and consequences of hyperprolactinemia are well known, hypoprolactinemia is poorly understood. RESULTS: The causes of PRL deficiency range from genetic mutation to acquired pituitary diseases and rare isolated disorders. Its diagnostic evaluation is often challenging, due to the lack of standardised lower reference limits. Nonetheless, growing evidence links hypoprolacti

Acute Sheehan's syndrome.

OBJECTIVE: To present a case of a patient who developed acute Sheehan's syndrome, despite adequate estimated blood loss. CASE REPORT: Sheehan's syndrome is a relatively rare disease with various incidences, mainly due to different obstetric care factors in individual countries. Pathogenetic mechanisms are not fully understood. An important factor is reduced blood flow through the pituitary arteries caused by hypotension in the setting of postpartum hemorrhage. Subsequent clinical manifestations depend on the extent of damage to the pituitary gland, and consequently, on the loss of individual

Outbreak of Trypanosoma vivax in Dairy Cows: Hematologic, Immunological and Antioxidant Responses Before and After Treatment with Isometamidium Chloride.

Trypanosoma vivax infection is an emerging condition that causes damage and mortality among cattle and is transmitted by mechanical vectors or contaminated fomites. This disease has been spreading in southern Brazil, causing anemia, weight loss, diarrhea, abortion, and infertility; however, its behavior and host-parasite relationships are not yet fully understood. To clarify this issue, animals that presented clinical signs were subjected to an immunochromatographic screening test. An indirect immunofluorescence test was then performed on samples collected before treatment (the gold standard t

Effects of antioxidant-rich Lactiplantibacillus plantarum inoculated alfalfa silage on rumen fermentation, antioxidant and immunity status, and mammary gland gene expression in dairy goats.

BACKGROUND: Milk synthesis in lactating animals demands high energy metabolism, which results in an increased production of reactive oxygen metabolites (ROM) causing an imbalance between oxidants and antioxidants thereby inducing oxidative stress (OS) on the animals. To mitigate OS and postpartum disorders in dairy goats and gain insight into the impact of dietary choices on redox status during lactation, a feeding trial was conducted using alfalfa silage inoculated with a high-antioxidant strain of Lactiplantibacillus plantarum. METHODS: Twenty-four Guanzhong dairy goats (38.1 ± 1.20 kg) wer

Oxylipins biosynthesis and the regulation of bovine postpartum inflammation.

Uncontrolled or dysregulated inflammation has adverse effects on the reproduction, production and health of animals, and is a major pathological cause of increased incidence and severity of infectious and metabolic diseases. To achieve successful transition from a non-lactation pregnant state to a non-pregnant lactation state, drastic metabolic and endocrine alteration have taken place in dairy cows during the periparturient period. These physiological changes, coupled with decreased dry matter intake near calving and sudden change of diet composition after calving, have the potential to disru

Alterations in Rumination, Eating, Drinking and Locomotion Behavior in Dairy Cows Affected by Subclinical Ketosis and Subclinical Acidosis.

This study delves into the effects of subclinical ketosis (SCK) and subclinical acidosis (SCA) on various parameters related to dairy cow rumination, eating, drinking and locomotion behavior. The research hypothesized that these subclinical metabolic disorders could affect behaviors such as rumination, feeding, and locomotion. A total of 320 dairy cows, with a focus on those in their second or subsequent lactation, producing an average of 12,000 kg/year milk in their previous lactation, were examined. These cows were classified into three groups: those with SCK, those with SCA, and healthy cow

Effect of extended heat stress in dairy cows on productive and behavioral traits.

This study evaluates the response of dairy cows to short and extended heat stressing conditions (from 1 to 28 days), as expressed in changes in their behavior. Due to climate change, heat stress and strong heat waves are frequently affecting the productivity and behavior of dairy cows. In the five years under study from 2018 to 2022, two were characterized by extremely strong heat waves occurring in the region analyzed in this study (Northern Italy). The dairy cattle farm involved in this study is located in Northern Italy and includes about 1 600 Holstein Friesian lactating dairy cows. Phenot

The Complex Interplay of Insulin Resistance and Metabolic Inflammation in Transition Dairy Cows.

During the transition period, dairy cows exhibit heightened energy requirements to sustain fetal growth and lactogenesis. The mammary gland and the growing fetus increase their demand for glucose, leading to the mobilization of lipids to support the function of tissues that can use fatty acids as energy substrates. These physiological adaptations lead to negative energy balance, metabolic inflammation, and transient insulin resistance (IR), processes that are part of the normal homeorhetic adaptations related to parturition and subsequent lactation. Insulin resistance is characterized by a red

Women with Gaucher Disease.

Gaucher disease is an inherited disorder in which there is a deficiency of the enzyme glucocerebrosidase, which leads to the accumulation of glucosylceramide. Although much scientific evidence is now available, there is still limited data on the impact on the different life stages of women with this disease. Among other alterations, a delay in menarche has been described, although it has not been related to fertility problems. Menorrhagia is relatively frequent, being related to the presence of thrombocytopenia, thrombocytopathies or coagulation disorders. On the other hand, pregnancy planning

Dysphoric Milk Ejection Reflex: Characteristics, Risk Factors, and Its Association with Depression Scores and Breastfeeding Self-Efficacy.

Background: Dysphoric milk ejection reflex (D-MER) is a phenomenon that occurs before milk letdown and is described as a wave of negative or devastating emotions, ranging from mild to severe and lasting for seconds to minutes. To date, there has been little research regarding this phenomenon. This study aims to determine the prevalence of D-MER in our population as well as its association with postnatal depression scores and breastfeeding self-efficacy. Methods: Lactating persons between 4 and 12 weeks postpartum at our institution were invited to complete an anonymous 59-question survey via a

Combining reproductive outcomes predictors and automated estrus alerts recorded during the voluntary waiting period identified subgroups of cows with different reproductive performance potential.

The objective was to compare differences in reproductive performance for dairy cows grouped based on the combination of data for predictors available during the prepartum period and before the end of the voluntary waiting period (VWP), automated estrus alerts (AEA) during the VWP, and the combination of both factors. In a cohort study, data for AEA and potential predictors of the percentage of cows that receive AI at detected estrus (AIE), pregnancies per AI (P/AI) for first service, and the percentage of cows pregnant by 150 DIM (P150) were collected from -21 to 49 DIM for lactating Holstein

Periparturient Mineral Metabolism: Implications to Health and Productivity.

Mineral metabolism, in particular Ca, and to a lesser extent phosphorus (P) and magnesium (Mg), is altered with the onset of lactation because of extensive irreversible loss to synthesize colostrum and milk. The transient reduction in the concentration of Ca in blood, particularly when it lasts days, increases the risk of mineral-related disorders such as hypocalcemia and, to a lesser extent, hypophosphatemia. Although the incidence of clinical hypocalcemia can be reduced by prepartum dietary interventions, subclinical hypocalcemia remains prevalent, affecting up to 60% of the dairy cows in th

Genomic prediction of blood biomarkers of metabolic disorders in Holstein cattle using parametric and nonparametric models.

BACKGROUND: Metabolic disturbances adversely impact productive and reproductive performance of dairy cattle due to changes in endocrine status and immune function, which increase the risk of disease. This may occur in the post-partum phase, but also throughout lactation, with sub-clinical symptoms. Recently, increased attention has been directed towards improved health and resilience in dairy cattle, and genomic selection (GS) could be a helpful tool for selecting animals that are more resilient to metabolic disturbances throughout lactation. Hence, we evaluated the genomic prediction of serum

Association between hoof trimming frequency in primiparous cows and hoof health and survival in second lactation.

Lameness, mostly resulting from claw lesions, causes major welfare problems in the dairy industry. One way to prevent claw lesions is hoof trimming, but the overall effect and the optimal hoof trimming frequency are unknown. In a retrospective cohort study, we investigated the association between hoof trimming frequency in primiparous cows and hoof health and culling in the second lactation. We based our analysis on breed, calving, and culling data in the period 2015 to 2018 for 30,613 primiparous cows in 202 Swedish dairy herds. Of the cows enrolled, 20% were not hoof-trimmed, 30% were trimme

Developmental pyrethroid exposure in mouse leads to disrupted brain metabolism in adulthood.

Environmental and genetic risk factors, and their interactions, contribute significantly to the etiology of neurodevelopmental disorders (NDDs). Recent epidemiology studies have implicated pyrethroid pesticides as an environmental risk factor for autism and developmental delay. Our previous research showed that low-dose developmental exposure to the pyrethroid pesticide deltamethrin in mice caused male-biased changes in the brain and in NDD-relevant behaviors in adulthood. Here, we used a metabolomics approach to determine the broadest possible set of metabolic changes in the adult male mouse
