Evaluation of the galactogogue effect of silymarin on mothers of preterm newborns (<32 weeks).

Hypogalactia has a relative high frequency in women having delivered preterm infants, who often have difficulties in maintaining a sufficient production of milk for their infants' needs over prolonged periods of time. Recent studies have shown a potential galactogogue effect of silymarin on milk production in animal models (cows and rats) and in humans (mothers of term newborns); nonetheless, none of the studies conducted on humans consisted of double-blind randomized clinical trials and no data are available concerning mothers who delivered preterm infants. The aim of our study was to assess

[Prevalence of factors associated with the duration of exclusive breastfeeding during the first 6 months of life in the INMA birth cohort in Gipuzkoa].

OBJECTIVE: To estimate the prevalence of exclusive breastfeeding (EB) during the first 6 months of life in the Gipuzkoa birth cohort, identify the reasons for abandonment of EB, and establish the associated factors. METHODS: The study population consisted of 638 pregnant women from the INMA-Gipuzkoa (Infancia y Medio Ambiente, www.proyectoinma.org) birth cohort, who were followed up from the third trimester of pregnancy until the child was aged 14 months. To determine the factors related to abandonment of EB, logistic regression models were used in two different stages (4 months or early stag

Invited review: piglet mortality: management solutions.

Preweaning mortality varies greatly among herds and this is partly attributed to differences in farrowing house management. In this review, we describe the various management strategies than can be adopted to decrease mortality and critically examine the evidence that exists to support their use. First, we consider which management procedures are effective against specific causes of death: intrapartum stillbirth, hypothermia, starvation, disease, crushing, and savaging. The most effective techniques include intervention to assist dystocic sows, measures to prevent and treat sow hypogalactia, g

Presentation of an infant with nutritional deficiency dermatitis as the initial manifestation of cystic fibrosis.

INTRODUCTION: Cystic fibrosis (CF) is a multisystemic autosomal recessive disease most frequently recognized by characteristic respiratory and/or digestive manifestations. Exceptionally rare, as is the case with the infant we are presenting, the initial sign of the disease can be nutritional deficiency dermatitis (NDD). CASE OUTLINE: A three-month-old male infant of young and healthy non-consanguineous parents, born at term after the first uneventful pregnancy, was hospitalized due to atopic dermatitis (AD)-like skin changes, failure to thrive and normochromic anemia (Hb 60 g/L). As exclusive

[Breastfeeding: the importance of intervening].

BACKGROUND: Breast milk is considered by the WHO the ideal food for the first months of life. Although health professionals are aware of recommendations, high rates of drop-outs have been identified in Portugal. A false notion of hypogalactia is the major factor for early termination, which is allied to the technical difficulties of the feeding. Health professionals, often lacking training in the area, may have difficulty in reassuring mothers in these situations. In Portugal, at the 3rd month, most mothers stop breastfeeding by indication of their medical assistant. OBJECTIVES: Gather eviden

Clinical observation on the treatment of post-cesarean hypogalactia by auricular points sticking-pressing.

OBJECTIVE: To explore the effect of auricular points sticking-pressing (APSP) in treating post-cesarean hypogalactia (PCH). METHODS: A randomized, controlled, single-blinded clinical trial on 116 patients with PCH was carried out. They were equally assigned to the treatment group and the control group. The treatment group received APSP, with the pellets pressed for 4 times daily, while the control group was only asked to do lactation to meet infant demand. The therapeutic efficacy and the changes in scores of traditional Chinese medicine (TCM) syndrome, volume of milk secretion, supplementary

[A 6-day-old girl with weight loss].

Newborn children normally lose up to 10% of their birth weight during the first week of life. With greater weight loss one must consider low intake and pathological losses. We describe a 6-day-old girl with nearly 19% weight loss and hypertonic dehydration caused by hypogalactia. Principles for treatment of hypertonic dehydration and strategies for finding these children are discussed.

Hypogalactia treated with hand expression and translactation without the use of galactagogues.

This case study demonstrates the possibility of treating hypogalactia without using galactagogues. The focus is a female child who, in her first 54 days of life, had a weight gain of only 610 g, with a daily average of 11 g. The child suffered from improper latch-on, insufficient sucking, and consequent excessive drowsiness. During the first clinical visit by the mother, hand expression produced only 5 mL of breast milk. Mother and daughter were assisted daily with the hand expression and translactation method, without the use of any other milk, feeding bottles, or galactagogues. After 2 month

[Justifications for formula supplementation in low-risk newborns at a Baby-Friendly Hospital].

The Baby-Friendly Hospital Initiative recommends not giving newborn infants any food or drink other than breast milk unless medically indicated. This study investigated the prevalence and alleged reasons for giving formula supplementation to rooming-in newborns at a Baby-Friendly Hospital. Participants were 300 formula-supplemented, exclusively rooming-in newborns at a Baby-Friendly Hospital in Rio de Janeiro, Brazil. Reasons for formula supplementation were classified as acceptable or unacceptable in accordance with the WHO/UNICEF Baby-Friendly Hospital Initiative guidelines. A supplementatio

Clinical efficacy, safety and tolerability of BIO-C (micronized Silymarin) as a galactagogue.

BACKGROUND AND AIM OF THE WORK: The authors have previously reported the use of Silymarin (a Silybum marianum standardized extract) as a promoter of milk production in cows. Due to the important psychological impact of hypogalactia in women after delivery, we evaluated the role of Silymarin as a safe and effective galactogogue for human species. METHODS: 50 healthy women during lactation were enrolled in order to verify the galactogogue role played by an oral treatment with micronized Silymarin (420 mg/day) in comparison with an undistinguishable placebo product. RESULTS: Women orally treate

Impact of the mother's zinc deficiency on the woman's and newborn's health status.

The results of studying the prevalence of zinc deficiency in pregnant women and children at birth are presented. About 77% of the mothers and, in average, every third-fifth child depending on gestational age at birth have decreased serum zinc concentration (below 13 micromol/L). Malnutrition regarding macro- and micronutrients increases the risk of zinc deficiency in examined women (AP = 28%; RR = 1.4; CI = 0.9-3.5). Zinc deficiency increases the risk of pregnancy complications, exacerbations of chronic diseases with gestation as background, labor activities disorders, with hypogalactia and de

[Breastfeeding and nutrition beliefs].

Hypogalactia has been one of the reasons that lead mothers to early weaning. Beliefs and taboos influence breastfeeding during lactation. This study aims at identifying regional lactagogues used as a support to breastfeeding based on taboos and beliefs. It is a case report which uses prose analysis. The findings show that feeding oneself with certain types of food or not, having experienced breastfeeding, beliefs transmitted by family members and physicians and the child's growth and development were factors which stimulated breastfeeding.
