Rural-Urban Differences in Coverage Gains Under Continuous Postpartum Medicaid Eligibility.

BACKGROUND: Rural residents face higher rates of obstetric complications, yet are more likely to lose insurance after childbirth, limiting access to essential postpartum care. This study examined whether continuous Medicaid eligibility during the COVID-19 public health emergency (PHE) differentially impacted postpartum insurance coverage in rural versus urban populations. METHODS: Using data from the 2017-2022 American Community Survey, we evaluated how state-level variation in pregnancy-related Medicaid eligibility during the PHE influenced coverage among rural and urban residents in the pos

BACKGROUND: Pregnant women with dilated cardiomyopathy (DCM) face high risks of complications and maternal death due to hemodynamic overload, withdrawal of teratogenic but essential therapies, and limited treatment options during pregnancy. To evaluate maternal and fetal outcomes in women with DCM during pregnancy and up to 12 months postpartum, across different etiologies, and identify predictors of maternal death. METHODS: Prospective cohort of pregnant women with confirmed DCM enrolled in the InCor Pregnancy and Heart Disease Registry. All received standardized cardio-obstetric care. Left

OBJECTIVE: To assess socioeconomic inequality and identify key determinants of PPFP utilization among Millennial and Generation Z women in Indonesia. METHODS: We conducted

METHODS: A cross-sectional survey was conducted among 87 women with lactation mastitis at a tertiary hospita

Enhanced peripartum hemostatic management does not decrease postpartum hemorrhage incidence in hemophilia carriers: the Pregnancy and Inherited Bleeding Disorders study.

BACKGROUND: Pregnant hemophilia carriers receive prophylactic factor [F]VIII or FIX concentrate based on third trimester FVIII/FIX activity to reduce the risk of severe postpartum hemorrhage (PPH, ≥ 1000 mL). Due to persistently high severe PPH rates, Dutch guidelines were revised in 2018. Consensus was reached to increase the third trimester threshold for prophylaxis from < 50 to < 80 IU/dL and peak target levels during childbirth from ≥ 100 to ≥ 150 IU/dL. OBJECTIVES: To assess the severe PPH incidence after guideline revision. METHODS: Pregnant hemophilia carriers were prospectively enrol

This project aims to implement and evaluate the effectiveness of a continuous midwifery care model during pregnancy and delivery, as well as its impact on various maternal and neonatal outcomes for socially disadvantaged women in Iran. METHODS/DESIGN: This research will utilize a convergent parallel mi

The PROMISE study: protocol for a community-based cohort study and co-creation of a reproductive health intervention in Somaliland.

Somaliland faces persistently high burdens of maternal and perinatal mortality, with limited population-based data on pregnancy complications, sociocultural influence on maternal and perinatal health, and women's reproductive health needs across the continuum of pregnancy, childbirth, and postpartum. Also, available health materials, such as for assessing maternal near misses (MNM) or strengthening healthcare literacy, often appear unfit in the contextual realities. In response, the objective of this study is to unfold the physical, cultural, and psychosocial strengths and challenges experienc

Women's perspectives on health service integration for multi-morbidity in pregnancy, childbirth, and postpartum in a high-burden setting.

BACKGROUND: Maternal and infant health outcomes in low- and middle-income countries (LMICs) remain below global targets. Greater integration of health services within routine antenatal care and infant immunization is a priority but has had variable success when implemented. We aimed to understand coexisting health needs for integrated services among women and infants and elevate women's views on health service delivery and planning. METHODS: We conducted a longitudinal study of 699 pregnant women and their infants in Papua New Guinea from first antenatal visit to 1 year postpartum. We collect

Pregnancy as a journey of care and connection: Indigenous women's experiences navigating health systems and community support.

INTRODUCTION: Pregnancy and childbirth are opportunities for Indigenous communities to welcome new life, integrate meaningful cultural practices, and tangibly strengthen communal wellbeing. Simultaneously, pregnant American Indian and Alaska Native (AIAN) women must navigate complex healthcare systems while experiencing structural inequities that contribute to elevated risks of complications and poor birth outcomes. This study explored the pregnancy experiences of AIAN women, examining how participants navigated healthcare systems, social supports, and structural challenges. METHODS: We condu

A Qualitative Ecological Exploration of Black Perinatal Mental Health and Wellbeing.

BACKGROUND: In the United States, Black women are three to four times more likely to die from pregnancy-related complications than White women and experience disproportionately higher rates of maternal mental health issues across pregnancy, childbirth, and the postpartum period. Structural racism, healthcare bias, and inequitable access to culturally responsive care position Black women among the most underserved and undertreated populations in the nation and contribute to intergenerational impacts on Black families and communities. OBJECTIVE: This study aimed to understand how Black communit

Individually tailored physiotherapy interventions for pregnancy-related pelvic girdle pain: functioning, physical activity, and treatment satisfaction four months postpartum. A cross-sectional study.

BACKGROUND: Pregnancy-related pelvic girdle pain (PPGP) affects more than 50% of pregnant women, and for some it may lead to persistent postpartum pain. The factors affecting the prevalence of PPGP postpartum are unclear. Recommended interventions for PPGP include exercise, pelvic belts, acupuncture, and promotion of physical activity. However, individually tailored interventions based on treatment guidelines are necessary given the multifaceted nature of PPGP, but this has not been evaluated in a clinical setting. The aim of this study was to describe levels of functioning, physical activity,

Moving Toward a Positive Birth: The Protective Role of Psychological Capital in Shaping Childbirth Experiences.

PURPOSE: Positive childbirth experiences (CBEs) impact maternal and child well-being, but many women, particularly those of low socioeconomic status, report negative or traumatic births. This mixed-methods longitudinal study investigated whether and how psychological capital-a psychological construct composed of hope, self-efficacy, resilience, and optimism-moderates the associations between birth complications, birth trauma, and perceived own capacity, a critical indicator of positive CBEs. METHODS: Mixed-methods data were extracted from a prospective longitudinal perinatal cohort study of b

The Hidden Burden: Global, Regional, and National Trends in Indirect Cause of Maternal Mortality from 1980 to 2021: A Global Analysis Using the Global Burden of Disease 2021 Data.

INTRODUCTION: Worldwide, lowering maternal mortality is a top priority. While significant efforts have been made to address direct obstetric complications, indirect causes such as pre-existing medical conditions that are aggravated by pregnancy remain a major cause of mortality but are often unrecognized. Understanding indirect maternal mortality prevalence and patterns is crucial for creating focused interventions. METHODS: Data were explored from the Global Burden of Disease (GBD) study 2021.
