Maternal Mortality Revisited: The Underrecognized Burden of Incidental and Traumatic Deaths in a Forensic Autopsy Series.

Maternal deaths are among the most critical global health indicators and directly reflect the quality of obstetric care. In this context, it is crucial to define and evaluate maternal mortality accurately. The aim of this study was to identify the factors leading to maternal deaths, and to assess the burden of trauma-related deaths. The autopsies of maternal deaths conducted at the Morgue Department of the Ankara Group Presidency of the Council of Forensic Medicine between 2011 and 2025 were retrospectively recorded through the National Judicial Network system. A total of 63 cases were identif

Preconceptual counselling for women with epilepsy - is it happening? A retrospective observational medical records review.

Epilepsy in pregnancy is common, but it remains a major risk factor for maternal morbidity and mortality. Over the last decade, neurological causes have remained in the top three indirect causes of maternal death in the UK. National reports, including the recent maternity care bundle, have emphasised the importance of preconceptual counselling to optimise maternal health and reduce complications for epileptic women during pregnancy and postpartum. This study is a retrospective medical record review, examining records of epileptic women of childbearing age, taken from a general practitioner (GP

HDAC5 stabilization by tubeimoside I suppresses cervical cancer metastasis via inhibiting H3K27ac/KPNA2 axis.

BACKGROUND: Cervical cancer poses a significant threat to women's health, and its metastasis is one of the leading causes of cancer-related deaths. Tubeimoside I (TBMS1) is a traditional Chinese medicinal herb with anticancer properties. We previously demonstrated the anticancer effect of TBMS1 in cervical cancer by inducing autophagy-related cell death. Nevertheless, whether and how TBMS1 prevents cervical cancer metastasis remain unclear. METHODS: Wound healing, transwell assays, and a lung metastasis mouse model were used to evaluate cell metastasis. Immunoprecipitation combined with site-

Maternal Mortality Among Black Women in Brazil: A Retrospective Cohort Study.

BACKGROUND: Maternal mortality in Brazil remains a critical indicator of social and racial inequalities, reflecting structural failures in access to and quality of obstetric care. Black women, particularly those categorized as black or brown, are at a higher risk of dying during pregnancy, childbirth, or the postpartum period. This is the result of the intersection of institutional racism, poverty, and social vulnerabilities. This study aimed to analyze trends and associated factors of maternal mortality among black women in Brazil from 2000 to 2020. METHODS: This is a retrospective cohort an

Introduction of confidential enquiry into maternal deaths in Ethiopia: Implementation and methodological considerations.

BACKGROUND: Despite having high maternal mortality, no recent confidential enquiry into maternal deaths (CEMD) has been implemented in Ethiopia. This paper outlines the introduction of the CEMD, major findings, and key methodological considerations. METHODS: We embedded this CEMD in the ongoing Ethiopian Obstetric Surveillance System (EthOSS), a regional system that monitors a range of major obstetric conditions in eastern Ethiopia. Multiple methods (both qualitative and quantitative) were used to collect, analyse and report the data. A multidisciplinary committee was established and trained

Avian Influenza H5N1 Infection During Pregnancy: Preparing for the Next Flu Pandemic and Improving Perinatal Outcomes.

Influenza (flu) is a common respiratory virus with seasonal global spread. Zoonotic viruses can occasionally cross species, leading to pandemic-level spread, and for flu viruses, this is considered an "antigenic shift". The flu can be particularly severe during pregnancy due to immune system adaptations that occur during pregnancy, with prior global pandemics causing excess hospitalizations, deaths, and other complications in the mothers and the neonates. We aim to review the current literature with respect to novel avian H5N1 and the potential impact of infection with flu during pregnancy. A

Clinical factors associated with severe maternal outcomes in two South African hospitals: A case-control study.

BACKGROUND: A persistently high maternal mortality ratio (MMR) from direct pregnancy causes of maternal deaths in a district of the Gauteng Province prompted a detailed audit to identify clinical factors associated with severe maternal outcomes (SMOs). METHODS: A cross-sectional matched case control study design was used to identify clinical factors associated with severe maternal outcomes in two regional hospitals in an urban district in Gauteng. 175 sequential SMO cases (near misses and maternal deaths) were matched to two different controls each. The first control was of women with complic

Immunology of pregnancy and sepsis: shared and specific pathways guiding future precision care.

Maternal sepsis remains a leading cause of preventable death globally, yet its underlying biology within the context of pregnancy is poorly understood. This review examines the extent to which pregnancy and sepsis share physiological, immunological, and regulatory pathways. We propose a unifying model for maternal sepsis that integrates immune, transcriptomic, epigenetic, and metabolic frameworks to advance a precision medicine approach, restoring immune equilibrium, controlling pathological inflammation while preserving pregnancy-specific adaptations. We address key knowledge gaps in current

Determinants of maternal mortality in the Province of Maniema, Democratic Republic of the Congo.

Maternal mortality remains a major public health problem in the Democratic Republic of the Congo. This case-control study, conducted in four health zones of the province of Maniema, aimed to identify the independent determinants of maternal death. A total of 197 maternal death cases were matched with 394 controls (surviving women). Multivariate analysis showed that lack of attendance at antenatal care visits, non-use of the partograph, receiving antenatal consultations in community health centers, as well as delivery in these facilities, were significant risk factors for maternal mortality. In

The Hidden Burden: Global, Regional, and National Trends in Indirect Cause of Maternal Mortality from 1980 to 2021: A Global Analysis Using the Global Burden of Disease 2021 Data.

INTRODUCTION: Worldwide, lowering maternal mortality is a top priority. While significant efforts have been made to address direct obstetric complications, indirect causes such as pre-existing medical conditions that are aggravated by pregnancy remain a major cause of mortality but are often unrecognized. Understanding indirect maternal mortality prevalence and patterns is crucial for creating focused interventions. METHODS: Data were explored from the Global Burden of Disease (GBD) study 2021. Maternal mortality ratio (MMR), maternal mortality rate (MM Rate), disability-adjusted life years (

Evolution of modern cesarean delivery: historic perspective and new challenges.

From a procedure of last resort that caused the death of most mothers and babies, the cesarean delivery has become a lifesaving delivery method and the most common major surgical procedure performed worldwide. The first modern cesarean deliveries, based on accurate anatomic knowledge, were performed 250 years ago. The development of new surgical and anesthesia techniques, antibiotics, uterotonics, blood transfusion, and neonatal care over the last 100 years has transformed the perinatal outcomes of cesarean delivery. The epidemiology of cesarean delivery has changed over the last 30 years with

[Factors associated with maternal deaths at Tengandogo University Hospital, Ouagadougou, Burkina Faso, 2017-2022].

INTRODUCTION: Burkina Faso ranks among the African countries with high maternal mortality rates, with a ratio of 198 per 100,000 live births. Maternal health remains a major concern in this country. Several factors contribute to this mortality rate. Our study aimed to identify factors associated with maternal deaths at Tengandogo University Hospital, a referral hospital in Burkina Faso. METHODS: This was a retrospective case-control study. We selected records pertaining to women admitted to the maternity and intensive care units between January 1, 2017, and December 31, 2022. Postoperative venous thromboembolism following cesarean delivery: prevalence, pathophysiology, diagnosis, treatment, and prevention.

Venous thromboembolism contributes to approximately 10% of pregnancy-associated deaths in the United States, with the highest incidence rate in the immediate postpartum period. Cesarean deliveries are associated with 4-fold relative risk of venous thromboembolism vs vaginal delivery. While most patients have some evidence of thrombosis in the pelvic veins after cesarean delivery, only a small proportion of patients (2.6-4.3 per 1000 deliveries) develop a clinically relevant venous thromboembolism. We outline the normal physiologic process of postcesarean hemostasis and the pathways by which ab

Intrauterine exposure to maternal diabetes and the risk of developing epilepsy in children: a national cohort study of 2.3 million children.

BACKGROUND: Approximately half of the epilepsy cases lack a documented cause. Despite growing evidence linking maternal diabetes to pregnancy complications and impaired child neurodevelopment, little is known about its potential association with child epilepsy. This study aims to determine whether intrauterine exposure to maternal pre-existing type 1 diabetes mellitus (T1DM), type 2 diabetes mellitus (T2DM), or gestational diabetes mellitus (GDM) increases the risk of epilepsy in children. METHODS: This study included live-born children without major malformations in Sweden from 1998 to 2021.
