Unsafe abortion: the preventable pandemic.

Ending the silent pandemic of unsafe abortion is an urgent public-health and human-rights imperative. As with other more visible global-health issues, this scourge threatens women throughout the developing world. Every year, about 19-20 million abortions are done by individuals without the requisite skills, or in environments below minimum medical standards, or both. Nearly all unsafe abortions (97%) are in developing countries. An estimated 68 000 women die as a result, and millions more have complications, many permanent. Important causes of death include haemorrhage, infection, and poisonin

Maternal mortality in seven districts of Uttar Pradesh--an ICMR task force study.

Maternal mortality is a major health and development concern. The available information on maternal mortality in rural India is inadequate and scanty. This study presented maternal mortality data from the demographically and developmentally (including for health) poor performing state of Uttar Pradesh. A descriptive, cross-sectional survey was conducted utilizing a stratified cluster sampling design between 1989-90 in eight districts of Uttar Pradesh. Four good performing districts namely, Agra, Farrukhabad, Ghaziabad and Badaun from the western region and four poor performing districts from t

Determining the contribution of asphyxia to brain damage in the neonate.

Studies in the research laboratory have demonstrated the complex relationship between fetal and newborn asphyxia and brain damage, a balance between the degree, duration and nature of the asphyxia and the quality of the cardiovascular compensatory response. Clinical studies would support the contention that the human fetus and newborn behave in a similar manner. An accurate diagnosis of asphyxia requires a blood gas and acid base assessment. The clinical classification of fetal asphyxia is based on a measure of metabolic acidosis to confirm that fetal asphyxia has occurred and the expression o

Aneurysmal subarachnoid haemorrhage in pregnancy.

Although uncommon, aneurysmal subarachnoid haemorrhage (SAH) in pregnancy can be devastating for both mother and baby. It is the leading cause of other indirect maternal death in England and Wales accounting for 60 deaths in the decade 1988-1999. No single obstetric or neurosurgical unit has sufficiently large database or experience in managing this condition in pregnancy. With significant improvements in antenatal care and management of deliveries, non-obstetric causes of maternal death such as aneurysmal subarachnoid haemorrhage are likely to become increasingly significant. The clinical fea

Averting maternal deaths in spite of resource constraints: an Indian rural experience over two decades.

The present study was conducted to analyse the changing trends in maternal deaths and the factors affecting them in a rural institution in India. Analysis of case records of maternal deaths over a period of 20 years, between April 1982 and March 2002 was performed in relation to the age, parity and disorders leading to death. It was revealed that the maternal mortality ratio has declined significantly, from 890.7 between April 1982 and March 1987 to 259.4 between April 1997 and March 2002. Most of the deaths were in young women (20-29 years) and primigravidae. Hypertensive disorders (27.4%) an

Maternal deaths and their causes in Ankara, Turkey, 1982-2001.

This study was carried out to determine the incidence and causes of maternal deaths about a 20-year period at the Zekai Tahir Burak Women's Health Education and Research Hospital (ZTBWHERH), Ankara, Turkey. All maternal deaths from January 1982 to July 2001 were reviewed and classified retrospectively. Using a computer-generated list, 348 patients admitted to the Labour Department of ZTBWHERH during 1982-2001 were selected as controls. Medical records were reviewed for demographic data, history of antenatal care, route of delivery, referral history, and perinatal mortality. Cases and controls

Pituitary and adrenal disorders complicating pregnancy.

PURPOSE OF REVIEW: Confidential enquiries of maternal deaths in the UK for 1997-1999 revealed that the number of indirect deaths from medical conditions exacerbated by pregnancy was greater than deaths from conditions directly caused by pregnancy. Indirect maternal deaths accounted for 36% of all deaths. Although neoplastic disorders of the pituitary and adrenal glands are very rare during pregnancy, they can be potentially fatal. The rarity of these disorders may pose difficulty in formulating an optimum plan of management. Biochemical tests that can be performed in the non-pregnant state may

[Maternal mortality in Switzerland 1985-1994].

The maternal mortality rate in the west has dropped drastically in the last 100 years. As yet there are no detailed numbers of the actual causes of death for Switzerland. We re-evaluated the anonymous patient documentation for the years 1985-1994 using the officially registered (ICD-8) cases of maternal mortality. 45 deaths out of 812,825 births remained from the original 76 registered cases, which corresponds to a direct maternal mortality of 5.4/100,000 live births. The other cases had to be reclassified (indirect, late or pregnancy-related obstetric deaths). The majority of the deaths were

[Maternal mortality: epidemiology, risk factors and evitability. About ten cases].

OBJECTIVES: To determine maternal mortality rate during the last decade as revealing the quality of obstetrical follow-up and the necessary measures to be taken. PATIENTS AND METHODS: A retrospective study conducted in the department of gynaecology in the military hospital in Tunis between 1990 and 2001, permitted to count 10 cases of maternal death. RESULTS: Maternal mortality rate was about 33.72/100,000 live births. Mean age of patients was 31.2 years. Primiparity, multiparity, poor socio-economic conditions, high-risk pregnancies and bad follow-up were responsible in 5, 1, 6 and 3 cases

Mothers infected with HIV.

The HIV/AIDS epidemic intersects with the problem of maternal mortality in many circumstances. The extent of the contribution of HIV/AIDS to maternal mortality is difficult to quantify, as the HIV status of pregnant women is not always known. HIV infection and AIDS-related deaths have become one of the major causes of maternal mortality in many resource-poor settings. HIV impacts on direct (obstetrical) causes of maternal mortality by an associated increase in pregnancy complications such as anaemia, post-partum haemorrhage and puerperal sepsis. HIV is also a major indirect cause of maternal m

Maternal haematocrit and pregnancy outcome in Nigerian women.

Anaemia is the most common medical disorder in pregnancy and a direct or indirect cause of maternal and perinatal mortality; therefore antenatal care should be concerned with early detection and management. The incidence of anaemia in 735 normal singleton pregnant patients at booking in the University College Hospital, UCH, Ibadan, between the 1st of June 2000 and 31 of May 2001 was 15%. Six hundred and thirty-three of the patients (86.1%) presented for delivery at this centre. Five hundred and sixty seven (89.6%) of the patients were not anaemic when seen in labour, reducing the incidence of

Evidence for a 'healthy pregnant woman effect' in Niakhar, Senegal?

BACKGROUND: Although it is generally believed that pregnancy exposes women to a wide variety of excess health risks that go beyond the direct obstetric complications of pregnancy, the epidemiological evidence in support of such excess indirect risks is inconclusive. In this article we attempt to document the contribution of indirect causes of death to maternal mortality in rural Senegal by using an epidemiological approach whereby the time spent during pregnancy and postpartum is considered a transient period of exposure to the health hazards of childbearing. METHODS: We use data from an ongo

Deaths attributable to childbearing in Matlab, Bangladesh: indirect causes of maternal mortality questioned.

Little is known about the nature of diseases aggravated by pregnancy or the magnitude of mortality from causes indirectly related to pregnancy. This study aims at clarifying the contribution of indirect causes to maternal mortality by analyzing the problem from an epidemiologic perspective, using population-based data from Matlab, Bangladesh, for the period 1976-1993. The time spent during pregnancy and the puerperium was considered a transitory exposure period in women's lives, and death rates were calculated for women aged 15-44 years, while exposed and while not exposed. During or shortly a
