Maternal mortality in the United States: report from the Maternal Mortality Collaborative.

To better define the incidence, causes, and risk factors associated with maternal deaths, the Maternal Mortality Collaborative in 1983 initiated national voluntary surveillance of maternal mortality. The Maternal Mortality Collaborative reported 601 maternal deaths from 19 reporting areas for 1980-1985, representing a maternal mortality ratio of 14.1 per 100,000 live births. Overall, 37% more maternal deaths were reported by the Maternal Mortality Collaborative than by the National Center for Health Statistics for these reporting areas. Older women and women of black and other races continued

Diagnostic fetal umbilical blood sampling in the management of isoimmunization.

Current management of isoimmunization in pregnancy is predicted on the assumption that all sensitized women carry antigen-positive fetuses. In addition, management is based on indirect predictors of the magnitude of the fetal hemolytic disease. We present a preliminary report using a new approach of direct fetal blood sampling for the diagnosis and treatment of these patients. This form of evaluation provides specific information about fetal red blood cell antigen status and the degree of fetal anemia at an earlier gestational age than that validated by the Liley curves and eliminates empirici

Maternal mortality--a twelve-year survey at the University of Ilorin Teaching Hospital (U.I.T.H.) Ilorin, Nigeria.

This paper concerns an analysis of maternal death at the University of Ilorin Teaching Hospital (U.I.T.H.) Ilorin over a 12-year period (1972-1983). There were 138,577 births and 624 deaths making a maternal mortality rate of 4.50 per 1000 births. Hemorrhage, ruptured uterus and obstructed labor were the major direct obstetric causes of death. The most important indirect causes were cerebrospinal meningitis, pulmonary infections and fulminating hepatitis. The main avoidable factors were ineffective and cumbersome blood transfusion services; poor management of the third stage of labor; large nu

[Analysis of indirect maternal mortality cases].

Among 145 cases of maternal deaths from the period 1975 to 1983, whose data had been collected in a nationwide survey, 30 cases (21%) were classified as associated causes of maternal deaths. 10 deaths (33%) occurred at the birth of the child, 11 (37%) within 6 weeks after delivery, 4 (13%) within the first trimester and 5 (17%) within the second trimester. The disease that was in causal relation to the complication leading to death was known in 16 cases (53%), unknown in 13 cases (43%); in one case information is missing. In 11 cases (37%) it was a cardiac disease, in 6 cases (20%) a cerebrova

Maternal mortality in Addis Ababa, Ethiopia.

Between July and September 1983, a two-stage probability survey was conducted in Addis Ababa, Ethiopia to obtain data on pregnancy outcomes for all women aged 13-49 in 32,215 houses. The survey covered a two-year period, from 11 September 1981 to 10 September 1983. Of the 9,315 women who were pregnant during those two years, 45 died from complications of pregnancy, delivery, and the puerperium. The maternal mortality rate for 1982-83 was estimated to be 566 per 100,000 live births. Mortality was highest for nullipara, the unmarried, women employed as maids/janitresses, and students. The most c

[Peripartal mortality in an autopsy sample of the Pathologic Institute of the Department of Medicine of the Karl Marx University in Leipzig 1960-1982].

Between 1960 and 1982 we have autopsied 88 cases of peripartal mortality in the pathological institute of the department of medicine, Karl-Marx-University of Leipzig. According to the legal instruction in the GDR we have subdivided in direct and indirect peripartal death cases (direct and indirect relation between maternal mortality and pregnancy). We have compared both the groups (1960-1969, 1970-1982) and have found: The number of cases with indirect and direct relation between maternal mortality and pregnancy is decreased markedly in the second time period. The composition within the two ti

Maternal mortality in Ontario from 1970 to 1980.

This review describes trends in maternal mortality in Ontario, Canada during 1970 to 1980 inclusive. There has been a significant decline in the total and direct deaths recorded. Anesthetic complications, hemorrhage, obstetric sepsis, and non-obstetric infections are now rare causes of maternal mortality. Toxemia and emboli remain major components of direct obstetric deaths. Collagen vascular disease, cardiac disease, pulmonary emboli, and cerebrovascular accidents are consistently found as causes of indirect maternal deaths.

The demography of maternal mortality--seven Swedish parishes in the 19th century.

Complications of childbirth is a leading death cause for women of reproductive ages in the developing countries today. The unique Swedish population statistics from the 19th century permit comparable historical studies. In seven Swedish parishes 170 maternal deaths were observed - 595 per 100,000 live births. Eclampsia, obstructed labor and hemorrhage were responsible for 92 deaths, and puerperal sepsis for only 15 deaths. Indirect obstetrical deaths, like pneumonia, tuberculosis, dysentery and heart diseases, accounted for 30.8% (41/133) of the specified death causes. With a time limit of 1 y

[Mortality and fatality in cesarean section].

55 deaths following a Caesarean section occurred in Austria during 1975-1982 (41,7% of all maternal deaths). 45 deaths (82%) were directly related to the Caesarean section, in 9 cases (16%) a severe underlying disease was found which had contributed to the fatal outcome (indirect deaths). In one case the diagnosis could not be clarified. Among the direct deaths the indication for the Caesarean section was in 12 cases (27%) severe bleeding, in 12 cases gestosis, in 12 cases predominantly foetal causes, in 6 cases previous Caesarean section and in 3 cases poor obstetric history. Maternal mortality in the American University of Beirut Medical Center (AUBMC) 1971-1982.

Maternal deaths were reviewed at the American University of Beirut Medical Center (AUBMC) during an 11-year period, 1971-1982. There were 35,058 live births and 45 deaths making a maternal mortality rate of 128 per 100,000 live births. Hemorrhage, sepsis and toxemia were the main direct obstetric causes of death. The most important indirect causes were cerebrovascular accidents and heart disease. In this review, an analytic discussion of the direct and indirect causes of maternal death in Lebanon are presented and preventive measures are discussed.

Maternal mortality in a major referral hospital, 1926 to 1980.

This review describes the changes in the causes of maternal deaths in a major referral hospital over a span of 55 years. There has been a significant decline in direct maternal deaths from infection, hemorrhage, and toxemia. Continued vigilance is needed since 58% of direct obstetric deaths in our hospital during the last 30 years were considered to have been preventable. Heart disease and nonobstetric infection as indirect causes of maternal deaths have decreased also. Greater effort is necessary to identify those patients with conditions that predispose to indirect deaths and to provide appr

Maternal mortality in an Israeli hospital: a review of 23 years.

In spite of great strides in obstetrics, maternal mortality has been completely eliminated. Possible changes in the causes of mortality are examined for three periods of time (1954-1961, 1962-1971 and 1972-1976). The overall incidence was 3.6/10 000, changing through the three periods from 4.9 to 4.3 and finally to 3.0/10 000. Vascular accidents were the cause of death in almost one third of the cases, emerging as the most important etiologic factor. Older age and higher parity did not seem to influence the incidence of obstetric deaths. Cesarean section was involved in ten of 23 cases in whic

Fetal outcome in trial of antihypertensive treatment in pregnancy.

242 women completed a controlled trial of methyldopa ('Aldomet') for moderate hypertension in pregnancy. Active treatment was associated with a significantly improved fetal outcome, due in part to a reduced number of mid-pregnancy abortions. There were 9 pregnancy losses in the control group, which included 4 mid-pregnancy abortions, and 1 fetal loss in the treated group. The birthweight and maturity of viable infants were similar in treated and control groups, and a detailed multivariate analysis confirmed that hypotensive treatment had no effect on fetal growth in utero. The better outcome a

Maternal mortality in a teaching hospital in southern India. A 13-year study.

During the 13 years 1960-1972, in a teaching hospital that serves a predominantly rural and semiurban population in southern India, there were 74,384 deliveries and 1245 maternal deaths, a maternal mortality rate of 16.7 per 1000 births. Direct obstetric factors caused 854 (65.5%) of these deaths. The leading indirect or associated causes of maternal deaths were anemia, cerebrovascular accidents, and infectious hepatitis. During the past 13 years, monthly maternal mortality meetings have helped to reduce the incidence of avoidable factors in maternal deaths among patients from the city but not

An analysis of maternal deaths in British Columbia: 1963 to 1970.

There were 132 maternal deaths in British Columbia in the years 1963 to 1970. The mean maternal mortality rate for these eight years was 0.317. Sixty of these deaths (45.5%) were due to direct obstetrical causes. Indirect and nonrelated deaths accounted for 21.2 and 33.3% of the total, respectively. The most common causes of direct obstetrical deaths were hemorrhage, infection and vascular accidents, in that order; pre-eclampsia ranked a distant fourth. Ninety-five percent of direct obstetrical deaths were probably avoidable. Approximately 27% of all direct obstetrical deaths were abortion-rel

MATERNAL MORTALITY IN BRITISH COLUMBIA: A STUDY OF 145 DEATHS FROM 1955 TO 1962.

Between 1955 and 1962, 145 maternal deaths were reported in the Province of British Columbia. One hundred of them were due to obstetrical causes. Of these deaths, hemorrhage was by far the commonest cause (40 cases), followed in frequency by vascular accidents (23 cases), infections (17 cases), toxemia (10 cases), anesthetic deaths (five cases), and other causes (five cases). Significant avoidable factors were noted in 80%. Indirect obstetrical deaths accounted for 29 cases, or 20% of all maternal mortalities. The most frequently encountered causes of indirect obstetrical deaths were cardiac (
