Indicators for availability, utilization, and quality of emergency obstetric care in Ethiopia, 2008.

OBJECTIVE: To report on the availability and quality of emergency obstetric and newborn care (EmONC) in Ethiopia. Critical care in pregnancy.

Childbirth is a major event in the lives of mothers and their families. Critical illness in pregnancy is uncommon but may arise from conditions unique to pregnancy, conditions exacerbated by pregnancy and coincidental conditions. According to the latest Confidential Enquiry into Maternal Deaths in the UK, haemorrhage remains a leading direct cause of mortality; however, there has been an increase in mortality due to indirect causes. The obstetric population has changed over the past decade and we are caring for much older mothers with pre-existing disorders and advanced chronic medical conditi

[Impact of HAPO study findings on future diagnostics and therapy of gestational diabetes].

The Hyperglycemia and Adverse Pregnancy Outcome (HAPO) Study is a trial on a high evidence level that included 25,000 women recruited in 15 centers all over the world who underwent a 75-gram oral glucose tolerance test (oGTT) at 24-32 weeks of gestation. Data remained blinded if the fasting plasma glucose level was below 105 mg/dl (5.8 mmol/l) and the 2-hour plasma glucose level was below 200 mg/dl (11.1 mmol/l). The aim of the study was to clarify whether maternal hyperglycemia less severe than that in diabetes mellitus is associated with increased risks of adverse pregnancy outcomes. The res

A review of cardiovascular complications of pregnancy.

With recent advances in prenatal care, the incidence of direct causes of maternal death has declined and indirect causes have gained significant importance. Thromboembolism, hypertension and cardiovascular diseases are the most common indirect causes of maternal death. Acute myocardial infarction, stroke, venous thromboembolism, peripartum cardiomyopathy aortic dissection and amniotic fluid emboli are responsible for the majority of the maternal deaths from cardiovascular causes. The issue of pregnancy of heart transplant--and Turner syndrome--patients requires extensive research. Obstetrician

Stillbirth workup and delivery management.

Evaluation for etiology is vital in cases of stillbirth to facilitate emotional closure and for counseling regarding subsequent pregnancies. Patients should be treated in a sensitive manner and encouraged to allow an evaluation within the boundaries of their cultural and individual values. The most important components of this workup include a complete medical history, autopsy, placental pathology, and karyotype. Other tests that may be valuable include Kliehauer-Betke; antiphospholipid antibodies, parvovirus, and syphilis serology; toxicology; and indirect Coombs. Further testing should be gu

Strengthening HIV services for pregnant women: an opportunity to reduce maternal mortality rates in Southern Africa/sub-Saharan Africa.

Reliable data from South Africa emanating from WHO recommendations for the Safe Motherhood programme underscores HIV/AIDS as the most common cause of maternal deaths. The strengthening of HIV services for pregnant women especially in countries with a high burden of HIV infection will reduce HIV-related and un-related maternal mortality rates. High-quality and complete data on maternal deaths is a critical foundation for reliably monitoring temporal trends in maternal deaths, and causes thereof, but needs substantial strengthening in many resource-constrained settings. HIV/AIDS is an increasing
