[Practical Recommendations from Clinical Experts for Optimizing Screening, Diagnosis, and Treatment of Iron Deficiency during Pregnancy in Portugal].

Iron deficiency is a clinical condition characterized by the depletion of iron stores, potentially affecting up to 95% of pregnant women in Portugal. If not identified and treated early, this condition can progress from fatigue, tiredness and reduced work productivity to anemia, increasing the risk of maternal-fetal complications. Therefore, early detection of iron deficiency during pregnancy is crucial for promptly addressing iron deficits and preventing complications during childbirth and the postpartum period. Scientific evidence and international guidelines recognize serum ferritin as the

Exploring pathogen-specific somatic cell patterns in dairy ewes during early lactation.

The postpartum period is a critical period for mastitis susceptibility in dairy ewes, yet the species-specific effects of mastitis pathogens on milk SCC and differential inflammatory cell counts (DICC) remain largely unknown. Therefore, this study investigates pathogen-specific impacts on microscopic SCC and DICC in milk and colostrum from Lacaune dairy ewes during early lactation. A total of 586 colostrum and milk samples were collected from 50 ewes (100 udder halves) at 6 time points: on the day of parturition and at 1, 3, 7, 15, and 30 d postpartum. Milk and colostrum samples were examined

Risk factor analysis and development of a nomogram prediction model for Plasma Cell Mastitis.

BACKGROUND AND OBJECTIVE: The risk factors for plasma cell mastitis (PCM) remain unclear. Understanding and mitigating these factors to prevent PCM before its onset has become a significant concern. This study identifies PCM risk factors, develops a predictive nomogram, and offers insights for targeted prevention and awareness in high-risk groups. METHODS: We retrospectively analyzed the clinical data of 82 patients diagnosed with PCM at Hangzhou Women's Hospital's Breast Surgery Department from 01/01/2019 to 01/01/2022. A control group was randomly selected, consisting of 82 healthy women ag

FIGO good practice recommendations on anemia in pregnancy, to reduce the incidence and impact of postpartum hemorrhage (PPH).

Anemia affects 32 million pregnant women globally, contributing annually to more than 115 000 maternal deaths and 591 000 perinatal deaths worldwide. Low- and middle-income countries (LMICs) bear the highest burden of anemia in pregnancy, with nearly 50% of affected pregnant women. It is now 2025, which is WHO's target year for a 50% reduction in maternal anemia, and the global prevalence of anemia in pregnancy remains more than twice the target of 15%. This calls for a renewed global focus on optimal approaches for reducing the burden and complications of anemia in pregnancy. In this FIGO Chi

A specific metabolomic and lipidomic signature reveals the postpartum resolution of gestational diabetes mellitus or its evolution to type 2 diabetes in rat.

Gestational diabetes mellitus (GDM) represents a major public health concern due to adverse maternal postpartum and long-term outcomes. Current strategies to manage GDM fail to reduce the maternal risk to develop later impaired glucose tolerance (IGT) and type 2 diabetes (T2D). In a rodent model of diet-induced GDM without obesity, we explored the perinatal metabolic adaptations in dams with gestational IGT followed by either persistent or resolved postpartum IGT. Female Sprague-Dawley rats were fed a high-fat high-sucrose (HFHS) or a chow [control group (CTL)] diet, 1 wk before mating and thr

Effect of Mode of Delivery on Incidence of Urinary and Anal Incontinence after Childbirth: A Prospective Cohort Study of Nigerian Women.

BACKGROUND: Urinary incontinence (UI) and anal incontinence (AI) are common postpartum complications that significantly affect quality of life. However, evidence on how mode of delivery influences their incidence remains limited. AIM: To assess the effect of mode of delivery on the incidence of UI and AI among postpartum women. METHODS: This prospective cohort study involved postpartum mothers at a Nigerian tertiary hospital. Women who had cesarean section (CS) formed the exposed group, while those with vaginal delivery (VD) served as controls. The CS group was subdivided into elective CS (A

BACKGROUND: Pregnancy after a Parkinson's diagnosis presents complex challenges. Due to the paucity of literature, there is no evidence-based guidelines and protocols for preconception care, management of pregnancy, childbirth and the postpartum period in women with early-onset Parkinson's disease (PD). Decision-making can be fraught with uncertainty for both patients and healthcare providers. OBJECTIVES: This review is aimed to provide pragmatic advice to help guide women with PD before, during and after pregnancy, and to address key gaps in the existing literature. METHODS: An interdiscipl

Therapeutic plasma exchange in severe postpartum HELLP syndrome: a promising treatment.

OBJECTIVE: HELLP syndrome is a life-threatening obstetric complication marked by microangiopathic hemolytic anemia, elevated liver enzymes, and thrombocytopenia and potentially affecting multiple organ systems. While definitive treatment is typically achieved through delivery, in rare instances, both clinical and laboratory parameters may worsen in the postpartum period. In such critical cases, therapeutic plasma exchange (TPE) can serve as a potentially life-saving intervention. METHODS: This study analyzed the laboratory and clinical responses of 21 patients diagnosed with severe HELLP syndr

A Study on Feto-Maternal Outcome Among Elderly Pregnant Women During the Intrapartum and Postpartum Period in a Tertiary Care Center.

INTRODUCTION: The rising incidence of pregnancies among older women, typically defined as those aged 35 and above, has raised concerns regarding both maternal and fetal health outcomes. Advanced maternal age is linked to various pregnancy complications, such as a higher risk of chromosomal abnormalities, gestational hypertension, preterm birth, and other adverse outcomes. This prospective cohort study aimed to investigate maternal and fetal outcomes in this population of elderly pregnant women, shedding light on the risks and complications they face during pregnancy. The study also explored th

Intrahepatic cholestasis of pregnancy and coagulation: a dual risk of hypercoagulability and bleeding.

INTRODUCTION: Intrahepatic cholestasis of pregnancy (ICP) is a hepatobiliary disorder characterized by elevated bile acid levels and liver dysfunction and usually occurs in the third trimester. Although the ICP has been associated with various fetal complications, its effects on maternal coagulation are poorly understood. Recent studies suggest that ICP may both cause hypercoagulability and increase bleeding tendency by impairing the synthesis of clotting factors. The aim of this study was to evaluate the relationships between ICP and coagulation parameters and to examine their potential clini

An unusual case of Kippel-Trenaunay syndrome complicated with intrahepatic cholestasis of pregnancy and disseminated intravascular coagulation.

BACKGROUND: Klippel-Trenaunay syndrome (KTS) is a rare congenital vascular disorder characterized by capillary malformation, varicose veins, and limb hypertrophy. Women with KTS face increased risks of thromboembolism and hemorrhage during pregnancy, delivery, and the postpartum period. Intrahepatic cholestasis of pregnancy (ICP), the most common pregnancy-related liver disorder, is associated with adverse perinatal outcomes. However, coexistence of KTS and ICP has not been previously reported. CASE PRESENTATION: We describe the case of a 29-year-old pregnant woman with known KTS who developed

Extreme resistance to thyroid hormone caused by a novel mosaic thyroid hormone receptor beta mutation.

ABSTRACT: Oocyte in vitro maturation (IVM) is an evolving component of assisted reproductive technology (ART) that offers a less invasive and cost-effective alternative to conventional controlled ovarian stimulation. It is particularly beneficial for patients at risk of ovarian hyperstimulation syndrome (OHSS), those with polycystic ovary syndrome (PCOS), and individuals requiring urgent fertility preservation. Despite these advantages, clinical uptake has remained limited owing to concerns about the developmental competence and quality of IVM-derived oocytes. To address this, we conducted a c

Prospective cohort study of eDiary acute pain assessments in sickle cell disease during third trimester and postpartum.

Pregnant women with sickle cell disease (SCD) are at higher risk of SCD-related morbidity and mortality than after pregnancy. Existing data from health care use suggest increased acute vaso-occlusive pain events during pregnancy, particularly in the third trimester and puerperium (6 weeks after childbirth). Many acute vaso-occlusive pain events are managed at home and may not capture the full scope of pregnancy-related morbidity. To date, to our knowledge, no studies have examined daily self-reported acute vaso-occlusive pain events during pregnancy and after pregnancy to assess their occurren
