Postpartum Psychosis: could genetic vulnerability to insomnia or short sleep duration be protective?

Postpartum Psychosis (PP) is a severe and understudied perinatal mental illness which disproportionately affects women with bipolar disorder (BD). A relationship between sleep disturbance and PP is often assumed, but is poorly understood. From a cohort of 2099 individuals with BD, 343 parous women were identified and screened for perinatal psychiatric complications. We compared 117 women who developed PP with 226 who did not. Polygenic Risk Scores (PRS) for BD, schizophrenia, insomnia, short sleep, long sleep, sleep efficiency and sleep duration were computed using PRS-CS. Logistic regression

Dual trajectory of insomnia and depressive symptoms in women from early pregnancy to 6 months postpartum: a prospective cohort study.

BACKGROUND: Perinatal insomnia and depression significantly impact maternal-infant health, but their co-developing trajectories are poorly understood. This study examines their heterogeneous progression, interrelationships, and predictive factors across the perinatal period. METHODS: This was part of a mother-infant cohort study conducted in the obstetrics outpatient clinic of a tertiary hospital in Wuhan, Hubei Province. Pregnant women were enrolled (N = 1034) at early pregnancy (< 14 weeks) from July 2022 to September 2023. The perinatal depressive symptoms, insomnia severity, anxiety sympt

The Management of Parkinson's Disease Before, during and after Pregnancy-an MDS Scientific Issues Committee Review.

BACKGROUND: Pregnancy after a Parkinson's diagnosis presents complex challenges. Due to the paucity of literature, there is no evidence-based guidelines and protocols for preconception care, management of pregnancy, childbirth and the postpartum period in women with early-onset Parkinson's disease (PD). Decision-making can be fraught with uncertainty for both patients and healthcare providers. OBJECTIVES: This review is aimed to provide pragmatic advice to help guide women with PD before, during and after pregnancy, and to address key gaps in the existing literature. METHODS: An interdiscipl

Prevalence of poor sleep quality before cesarean delivery and its association with postpartum quality of recovery and postpartum depressive symptoms: a prospective observational study.

PURPOSE: The primary objective of this study was to estimate the prevalence of sleep quality among patients hospitalized for cesarean delivery, with a secondary objective of assessing the impact of sleep quality on postpartum outcomes. METHODS: This prospective observational study included patients aged ≥18 years scheduled for elective cesarean delivery under spinal anesthesia admitted a day before delivery. The incidence of poor sleep quality (Richards-Campbell Sleep Questionnaire score <50) was estimated. Mean differences in obstetric quality-of-recovery-11 (ObsQoR-11) scores on postpartum

The Impact of Pregnancy on Functional Neurological Disorder (FND).

BACKGROUND: Functional neurological disorder (FND) commonly affects women of reproductive age, yet little is known about its interaction with pregnancy and childbirth. We aimed to explore FND symptom changes during pregnancy, delivery and the postpartum period and understand women's experiences of care. METHODS: We conducted a cross-sectional online survey of women with FND who had experienced pregnancy. The questionnaire was co-designed with clinicians and individuals with lived experience. RESULTS: Of 111 respondents, 61 had experienced pregnancy after FND onset. Among 88 who provided onse

Posttraumatic stress, anxiety, and depression in mothers after preterm delivery and the associated psychological processes.

BACKGROUND: Mothers of preterm infants report traumatic, anxious and depressive symptoms in the postpartum period. Many studies have focused on biological, social, and life circumstance factors to explain the emergence of these symptoms. The process model proposes to focus on psychological processes, which are mechanisms underlying mental disorders. However, the psychological processes underlying the onset of traumatic, anxious, and depressive symptoms in postpartum mothers of preterm infants had not yet been investigated. The aim of this study was to identify the most common symptoms experien

Sleep disturbance as a marker of postpartum psychosis risk: a prospective actigraphy study.

BACKGROUND: Postpartum Psychosis (PP) is a severe perinatal psychiatric disorder affecting 1-2 in 1000 individuals following childbirth. Most episodes emerge within the first two weeks postpartum and commonly present with mania and decreased need for sleep. The postnatal period is a time of profound sleep disruption and sleep deprivation is a known trigger for mania and psychosis. Despite growing recognition of the role of sleep in the onset and progression of PP, this relationship remains poorly understood. Existing research is largely retrospective, relies on self-reported data and primarily

Catatonic postpartum paternal depression as a first debut of a bipolar disorder: a case report.

Paternal postpartum depression (PD) is considered an affective disorder that affects fathers during the months following childbirth. Interestingly, it has been observed that during these months the chances of a male parent suffering from depression are double that for a non-parent male counterpart. We present the case of a 34-year-old man with no relevant medical history in who, overlapping her daughter's birth, several depressive symptoms emerged, such as fatigue, lack of concentration, sleeping disturbances and abandonment of care of the newborn. Prior to consultation, patient refused to eat

Do postpartum insomnia, fatigue and depression affect the maternal role of primiparous women?

Many factors negatively affect the motherhood role. Fatigue, insomnia and depression, which are among these factors, have not been investigated together in any study. Therefore, we carried out this study to determine the effect of insomnia, fatigue and depressive symptoms on the motherhood role in primiparous mothers. In this descriptive and cross-sectional study, we included 153 women who met the inclusion criteria using the simple, random sampling method. To determine the independent variables affecting the motherhood role, we used the backward regression model. In this study, we determined

Catatonia during pregnancy and the postpartum period.

While the psychopathology of mental disorders during pregnancy and the postpartum period is a growing area of research, the prevalence and significance of catatonic symptoms has been relatively neglected. To address this gap in knowledge, a systematic review of articles on catatonia occurring during pregnancy and the postpartum period was conducted. PubMed, Excerpta Medica, (later EMBASE) databases were queried for articles published in English from their inception in 1966 and 1946, respectively to May 31. 2022 using the terms "catatonia", AND "perinatal", "puerperal", "postpartum", "antepartu

Severe maternal morbidity and other perinatal complications among women with physical, sensory, or intellectual and developmental disabilities.

BACKGROUND: Little is known about severe maternal morbidity (SMM) among women with disabilities. OBJECTIVE: We assessed differences in SMM and other perinatal complications by presence and type of disability. We hypothesised that SMM and other complications would be more common in births to women with disabilities than to women without disabilities. METHODS: We conducted a retrospective cohort study of California births from 2000 to 2012, using birth and death certificate data linked with hospital discharge data. We included singleton deliveries with gestational age of 23-42 weeks. We classi

Psychological impacts of the COVID-19 pandemic on one-month postpartum mothers in a metropolitan area of Japan.

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic has recently become the most important issue in the world. Very few reports in Japan have examined the impact of the COVID-19 pandemic on peripartum mental health. We examined the status of postpartum mental health before and during COVID-19 pandemic from a consecutive database in a metropolitan area of Japan. METHODS: The subjects were women who had completed a maternity health check-up at a core regional hospital in Yokohama during the period from April 1, 2017, to December 31, 2020. We collected the subjects' scores for the Edinb

Migraine associated with early onset postpartum depression in women with major depressive disorder.

Major depressive disorder (MDD) and migraine are both more common among women than men. Women's reproductive years are associated with increased susceptibility to recurrence of both conditions, suggesting a potential role of sex hormones in aetiology. We examined associations between comorbid migraine and clinical features of MDD in women, including relationships with lifetime reproductive events such as childbirth. Lifetime clinical characteristics and reproductive events in a well-characterised sample of 222 UK women with recurrent MDD, with (n = 98) and without (n = 124) migraine were compa

Mid-Pregnancy Insomnia and its Association with Perinatal Depressive Symptoms: A Prospective Cohort Study.

Objective/Background: Insomnia and depression are disorders that affect many perinatal women and that often are interrelated. The present study aimed to examine concurrent and prospective associations between mid-pregnancy insomnia and depression during mid-pregnancy and 8 weeks postpartum. Furthermore, differences in depression and in the sleep-related characteristics insomnia, chronotype, and sleep efficiency were explored between the two time points (mid-pregnancy versus 8 weeks postpartum), and between primiparous and multiparous participants.Participants/Methods: The study was part of the

Factor structure of the Edinburgh Postnatal Depression Scale in the Japan Environment and Children's Study.

The Edinburgh Postnatal Depression Scale (EPDS) is frequently used to screen for postpartum depression. However, its factor structure exhibits noticeable inconsistencies between studies. We examined the EPDS at two postpartum time points using a large dataset from outside Western countries. Participants were 91,063 mothers in an ongoing birth cohort of the Japan Environment and Children's Study. One-, two-, and three-factor structures of the EPDS at 1- and 6-months postpartum were extracted using exploratory factor analysis (EFA) with oblique rotation.
