Pregnancy history, recency of childbirth, and outcomes in young women with early-stage breast cancer.

BACKGROUND: Breast cancer (BC) diagnosed during pregnancy or postpartum often presents with aggressive features, potentially influenced by hormonal, immunological, and tissue remodeling changes. Studies have suggested that postpartum BC may be associated with worse outcomes, though most evidence is retrospective and constrained by methodological limitations. This study evaluated associations between pregnancy history, recency of childbirth, and long-term outcomes in a prospective cohort of young patients with early-stage BC. PATIENTS AND METHODS: Patients aged ≤40 years with stage I-III BC en

Postpartum depressive symptoms and associated factors among women with lactation mastitis: a cross-sectional study.

BACKGROUND: Lactation mastitis is a common complication during breastfeeding that can negatively impact maternal well-being and breastfeeding continuation. While the physical manifestations of lactation mastitis have been widely studied, its psychological burden and related contributing factors among affected women have received comparatively less attention. This study aimed to investigate postpartum depressive symptoms and associated factors in women diagnosed with lactation mastitis. METHODS: A cross-sectional survey was conducted among 87 women with lactation mastitis at a tertiary hospita

Prognosis of Pregnancy-Associated Breast Cancer: A Systematic Review of Contemporary Observational Studies.

BACKGROUND/OBJECTIVES: Pregnancy-associated breast cancer (PABC) is uncommon but increasingly encountered as more women delay childbearing. Its prognostic impact remains controversial, particularly for cancers diagnosed in the early postpartum period. We aimed to synthesize contemporary evidence on the prognosis of breast cancer diagnosed during pregnancy or within 12 months after delivery compared with breast cancer in other young women, with a specific focus on differences between pregnancy time and postpartum disease. METHODS: We performed a systematic review of observational studies publi

Clinical and Pathological Characteristics and Prognosis of Breast Cancer During Pregnancy and Postpartum: A Multicenter Retrospective Study.

PURPOSE: The prognostic impact of the interval between the most recent childbirth and breast cancer diagnosis remains insufficiently defined, particularly when breast cancer diagnosed during pregnancy (PrBC) is distinguished from postpartum breast cancer (PPBC). In this multicenter Chinese cohort, we aimed to validate and extend prior observations regarding the association between time since recent childbirth, clinicopathological features, and outcomes. METHODS: This multicenter retrospective cohort study included 12 453 patients aged 18-55 years with stage I to IV breast cancer for descripti

Association of gestational diabetes with other lactation-related disorders: A 2-sample Mendelian randomization analysis of causal effects.

Gestational diabetes mellitus (GDM) is associated with adverse metabolic outcomes and may also be related to postpartum breast and lactation disorders. Observational studies are susceptible to confounding and reverse causation. We used a 2-sample Mendelian randomization design to examine the association between genetic liability to GDM and other disorders of the breast and lactation associated with childbirth. Genetic liability to GDM was associated with higher odds of a composite phenotype of breast and lactation disorders associated with childbirth. Replication using independent samples and

Antimicrobial Component Concentrations in the Milk of Peripartum Goats.

The aim of this study was to investigate changes in milk conditions and antimicrobial components in goats during 8 weeks prepartum and 1 week postpartum. Milk was collected weekly from seven prepartum goats 8 weeks before the due day, immediately, and 1 week after parturition. Milk color scores and the concentration of antimicrobial components (cathelicidin-7 and S100A8) were significantly increased prepartum. There was a significant decrease in milk viscosity, color scores, somatic cell count, and antimicrobial components (lactoferrin and cathelicidin-2, 7) after kidding. However, S100A7 leve

Vaccarin treats lactation insufficiency through the ALKBH5-SFRP2-Wnt/β-catenin signaling pathway.

ETHNOPHARMACOLOGIC SIGNIFICANCE: Vaccarin, a natural small molecule extracted from Gypsophila vaccaria (L.) Sm., is an active flavonoid glycoside. BACKGROUND: Lactation insufficiency refers to insufficient milk secretion in women after childbirth, which affects the feeding of infants and even their development. Our preliminary experiments showed that alkylation repair homolog protein 5 (ALKBH5) was abnormally overexpressed in mammary tissue of lactation deficiency model rats, which played an important role in regulating milk secretion, but the mechanism was not clear, and no research reports

Exploring pathogen-specific somatic cell patterns in dairy ewes during early lactation.

The postpartum period is a critical period for mastitis susceptibility in dairy ewes, yet the species-specific effects of mastitis pathogens on milk SCC and differential inflammatory cell counts (DICC) remain largely unknown. Therefore, this study investigates pathogen-specific impacts on microscopic SCC and DICC in milk and colostrum from Lacaune dairy ewes during early lactation. A total of 586 colostrum and milk samples were collected from 50 ewes (100 udder halves) at 6 time points: on the day of parturition and at 1, 3, 7, 15, and 30 d postpartum. Milk and colostrum samples were examined

Safety of dermatologic medications in pregnancy and lactation: An update - Part I: Pregnancy.

The breadth of therapeutic options for the management of dermatologic skin conditions continues to expand rapidly as exemplified by biologics and small molecule drug development. While dermatologists and health care providers are aware of the underlying mechanisms and indications for these therapeutics, there is a recognized practice gap due to an incomplete understanding of the safety of these medications in women of childbearing age during the prepartum, antepartum, and postpartum phases. Although a two-part continuing medical education review was published regarding the prescribing practice

Postpartum onset anti-MDA5 antibody-positive clinically amyopathic dermatomyositis; case-based review of perinatal onset anti-MDA5 antibody-positive dermatomyositis.

Anti-melanoma differentiation-associated protein 5 (MDA5) antibody positive clinically amyopathic dermatomyositis (CADM) is a subtype of inflammatory myopathy associated with a distinct clinical phenotype, characterized by rapidly progressing interstitial lung disease and limited muscle involvement. Although cases with onset of anti-MDA5 antibody positive CADM during pregnancy or the postpartum period are rare, they present unique challenges due to a potential pregnancy complications and the possible severity of the disease course. We present a case of anti-MDA5 antibody positive CADM that dev

Physical therapy intervention for breast symptoms in lactating women: a randomized controlled trial.

BACKGROUND: Therapeutic ultrasound, education, and massage are the most common physical therapy interventions provided to mothers with breast symptoms. However, there is insufficient evidence on the effectiveness of the combination of these interventions. This study aimed to explore the effects of the combination of therapeutic ultrasound, education, and massage on breast symptoms in lactating women. METHODS: This study was a single-blind randomized controlled trial. Postpartum lactating women aged from 21 to 45 with breast symptoms were recruited and randomly allocated to one of three groups

Predictive Risk Factors for Childbirth-Associated Breast Infections in the United States: A 10-Year Perspective.

Infectious mastitis is a common condition that affects up to 33% of lactating women. Several risk factors have been suggested to be strongly associated with breast abscess, nipple infection, and non-purulent mastitis associated with childbirth. In this retrospective cohort study, we gathered data from the National Inpatient Sample (NIS) between 2005 and 2014 and utilized data stratification and backward linear regression to analyze the predictive factors associated with patients hospitalized with breast infection after childbirth, with special consideration of risk factors affecting hospital l

Bacteriological investigation of milk samples and biopsies from the mammary gland of healthy sows.

OBJECTIVE: Milk samples from clinically healthy sows often contain a similar bacterial content as samples from mastitic sows. To verify whether contamination during sample collection is a possible reason for this or bacterial content in the mammary gland postpartum is a regular finding, the aim of the present study was to compare the suitability of milk samples and mammary gland biopsies for assessing the bacteriological status of healthy sows. MATERIAL AND METHODS: Twenty-five clinically healthy sows of different parities were selected. The mammary skin and teats were cleaned and disinfected

Lactation Activity and Mechanism of Milk-Protein Synthesis by Peptides from Oyster Hydrolysates.

Oyster meat has a tender texture and delicate flavor, and the oyster is an aquatic shellfish with high nutritional and economic values. As they are rich in protein, oysters serve as a good source for the preparation of bioactive peptides. However, research on the lactation effect and mechanism of the synthesis of polypeptides from oyster hydrolysates is yet to be observed. This study aimed to analyze the lactation activity of the fraction UEC4-1 and explore its mechanism. The results show that, in an in vivo experiment, UEC4-1 could significantly increase the concentration of PRL in the serum

Pregnancy-associated breast cancer: evaluating maternal and foetal outcomes. A national study.

PURPOSE: Pregnancy-associated breast cancer (PABC) is defined as breast cancer diagnosed during the gestational period (gp-PABC) or in the first postpartum year (pp-PABC). Despite its infrequent occurrence, the incidence of PABC appears to be rising due to the increasing propensity for women to delay childbirth. We have established the first retrospective registry study of PABC in Ireland to examine specific clinicopathological characteristics, treatments, and maternal and foetal outcomes. METHODS: This was a national, multi-site, retrospective observational study, including PABC patients tre

Breast cancer diagnosed in the post-weaning period is indicative for a poor outcome.

BACKGROUND: In young women, a breast cancer diagnosis after childbirth increases the risk for metastasis and death. Studies in rodents suggest that post-weaning mammary gland involution contributes to the poor prognosis of postpartum breast cancers. However, this association has not been investigated in humans, mainly because of missing information on the patient's lactation status at diagnosis. PATIENTS AND METHODS: Clinicopathological data of 1180 young women with primary invasive breast cancer, diagnosed within 2 years postpartum (PP-BC), during pregnancy (Pr-BC), or nulliparous (NP-BC), w
