Protocols for Diagnosis and Management of Arthropod Bite Allergies in Humans.

Allergic diseases have increased significantly in recent decades, with a higher incidence of severe manifestations such as anaphylaxis. This increase is attributed mainly to environmental factors associated with the Western lifestyle that alter the epithelial barrier, promoting sensitization to allergens, such as those from arthropods. Hymenoptera (bees and wasps) are the most implicated arthropods. The sting introduces a venom composed of highly sensitizing proteins that are capable of triggering reactions ranging from local and mild to potentially fatal systemic reactions. The incidence is h

AA in Hypertension and Preeclampsia.

Human essential hypertension is driven by a network of interacting nutritional, metabolic, inflammatory, and endothelial mechanisms in which AA and other PUFAs play a critical role. Beyond excess sodium intake, population data supports the role of inadequate calcium, potassium, and magnesium intake and low antioxidant vitamin status in shaping blood pressure regulation. These nutrients influence vascular smooth muscle tone and act as cofactors that support Δ6 and Δ5 desaturase activities, thereby governing tissue availability of AA, EPA, and DHA and downstream formation of vasodilator and anti

Extraperitoneal Single-Port Robotic-Assisted Radical Cystectomy with Orthotopic Ne-obladder: Technique and Surgical Considerations.

INTRODUCTION: Radical cystectomy (RC) is the standard therapy for muscle-invasive bladder cancer (MIBC) and refractory high-risk non-muscle-invasive bladder cancer (1-3). Single-port robot-assisted radical cystectomy (RARC) offers notable minimally invasive advantages (4-6), whereas conventional transperitoneal approaches are associated with intestinal and gastrointestinal complications (7). This study evaluated an optimized extraperitoneal single-port RARC with orthotopic neobladder reconstruction for improved surgical safety and clinical outcomes. MATERIALS AND METHODS: A 57-year-old male p

Cost-effectiveness of the hepatitis B birth-dose vaccine in South Africa: a mathematical modelling analysis.

BACKGROUND: In 2022, South Africa had an estimated hepatitis B prevalence of 4·7-6·0%. In an effort to eliminate viral hepatitis as a public health threat, a targeted (ie, selective) hepatitis B birth-dose vaccination (HepB-BD) policy was introduced in 2023. Under this policy, only infants born to mothers with confirmed hepatitis B positivity are eligible for HepB-BD, which is given in addition to peripartum antiviral prophylaxis (PAP) for mothers from the second or third trimester of pregnancy. This policy differs from the WHO recommendation that all newborns should receive a HepB-BD vaccine

Updated recommendations for the design of therapeutic trials for neonatal seizures.

The International Neonatal Consortium Seizure Working Group of the Critical Path Institute provides an update to the original recommendations for design of clinical trials to treat neonatal seizures based on recent experiences from several trials and developments in the field. Although there aren't sufficient new data to inform definitions of optimal efficacy endpoints, the Working Group recommended inclusion of alternate measures of seizure burden reduction as secondary or exploratory endpoints, to elucidate clinically meaningful efficacy endpoints for future trials. It was recommended to inc

Area level measures of socioeconomic deprivation and their impact on very preterm birth and very preterm birth outcomes: A retrospective cohort study.

OBJECTIVE: We hypothesised that socioeconomic deprivation as determined by an area measure would be an important risk factor for preterm birth in a contemporary cohort. METHODS: A retrospective study was carried out across two tertiary maternity hospitals in Ireland between 2018 and 2022 representing approximately 27% of all births in Ireland. Non stillborn infants whose address could be matched to a small area deprivation index (Haase-Pratschke (HP) Index) were included in the analysis. A combination of non-parametric and parametric tests were used to examine the impact of socioeconomic depr

Advancing equitable postpartum depression screening in a level-IV NICU: a quality improvement approach shaped by planned experimentation.

BACKGROUND: Given the incidence and impact of postpartum depression (PPD) on families in the NICU, we aimed to increase the percentage of birthing parents screened for PPD from 0 to 90% over 18-months and provide psychosocial support. METHODS: Planned experimentation was used to design an effective screening framework in a level IV NICU. Interventions included process standardization, electronic health record (EHR) support, and data visualization for feedback. Data were displayed on a control chart and stratified for race and language to ensure equitable delivery. RESULTS: Provider screening

You can read something that is delightful to you: a scoping review of reading interventions in neonatal and paediatric critical care settings.

OBJECTIVES: This scoping review evaluates the physiological, neurodevelopmental, and psychosocial effects of reading interventions on critically ill infants and young children. INTRODUCTION: Critically ill children, including premature infants and those with critical CHDs, are at elevated risk for neurodevelopmental complications that may be exacerbated by noxious stimuli in hospital settings. Given the increased awareness of the significance of developmental care in the critical care environment and the well-established role of reading and literacy interventions in healthy children, a potent

Digestive enzyme replacement in very preterm infants: a prospective observational study.

OBJECTIVES: Very preterm infants may display poor postnatal growth despite receiving recommended enteral intakes. This has been linked to transient exocrine pancreatic insufficiency, indicated by low fecal pancreatic elastase-1 (FPE-1) concentrations. We aimed to determine whether growth velocity may be improved by exogenous digestive enzyme replacement. METHODS: In this bicentric prospective observational study, changes in growth velocity were assessed in very preterm infants (<32 weeks gestational age, <1500 g birth weight) with low FPE-1 concentrations (<200 µg/g). Infants received 350 U l

Target Versus Modified-Standard Fortification of Human Milk and Growth of Very Low Birth Weight Infants: A Double-Blind Randomized Trial.

Background/Objectives: Individualized ("target") fortification of human milk is promoted to improve growth and subsequent neurodevelopment in very low birth weight (VLBW) infants, because standard fortifiers may not provide sufficient protein given the considerable intra- and interindividual variability of human-milk composition. We compared modified-standard fortification (MSF), an optimized high-protein standard regimen, with high-protein target fortification (TFO). Methods: In a single-center, double-blind randomized trial, VLBW infants (24 + 0 - 31 + 6 weeks) receiving 100 mL/kg/d of stand

Neonatal Sequential Organ Failure Assessment Score (nSOFA) for Prediction of Mortality Among Preterm Neonates with Respiratory Distress Syndrome: A Prospective Observational Study.

Preterm birth is a major contributor to neonatal morbidity and mortality. This study evaluated the neonatal Sequential Organ Failure Assessment (nSOFA) score for predicting mortality in preterm neonates (< 34 weeks) with respiratory distress syndrome. Among 113 neonates, median (q1, q3) nSOFA scores at 0-6 h of life were higher in non-survivors than survivors [4 (4, 6) vs 0 (0, 2); P = 0.001], with an area under the curve (AUC) of 0.80 and a cutoff of 3. Predictive performance improved at 24 ± 3 h (AUC 0.93), with higher scores in non-survivors [8 (5, 11) vs 0 (0, 2); P < 0.001] and a cutoff o

Nasal high-frequency oscillatory ventilation as primary respiratory support in preterm infants with respiratory distress syndrome: current evidence and clinical implications.

Continuous positive airway pressure (CPAP) is the standard initial therapy for respiratory distress syndrome (RDS) and associated with failure rates of up to 50% in extremely preterm infants. Nasal intermittent positive pressure ventilation (NIPPV) has demonstrated consistent benefits over CPAP in reducing respiratory failure and intubation. Nasal high-frequency oscillatory ventilation (nHFOV) has also shown reduced treatment failure compared with CPAP, particularly in extremely preterm infants. However, direct head-to-head comparisons between nHFOV and NIPPV as primary support are lacking, an

Small-for-gestational-age and suspected developmental coordination disorder in preschool children:A national cross-sectional study highlighting vulnerability in full-term births.

OBJECTIVE: Small-for-gestational-age (SGA) birth has been associated with adverse neurodevelopmental outcomes. However, its relationship with suspected developmental coordination disorder (DCD) in preschool children remains unclear. We therefore examined whether SGA was independently associated with suspected DCD risk in a national population-based sample. STUDY DESIGN: We analyzed CNCMD data from 101,505 preschool children aged 3-5 years. Birthweight-for-gestational-age status was classified using national sex- and gestational-age-specific standards, and suspected DCD was assessed using the

Biochemical and anthropometric assessment of nutritional status at discharge in very-low-birth-weight infants: a retrospective cohort study.

OBJECTIVES: To evaluate nutritional status at discharge in VLBW infants using biochemical and composite nutritional indices, and to examine their relationship with extrauterine growth restriction (EUGR; also termed "growth faltering" in more recent literature). METHODS: This retrospective cohort study included 124 very low birth weight infants (≤32 weeks, ≤1,500 g) admitted to a tertiary NICU (2021-2025). Anthropometric, nutritional, and biochemical parameters were recorded from birth to discharge. Four composite biochemical deficiency outcomes were constructed: protein (CPDO), nutritional (C

Low-Carbohydrate Diets, Nutritional Adequacy, Glycemia, and Perinatal Outcomes in Women With Gestational Diabetes Mellitus: A Secondary Analysis of the DiGest Trial.

OBJECTIVE: The optimal dietary carbohydrate intake for pregnant women is disputed internationally. Many authorities recommend a minimum of 175 g/day of carbohydrates primarily to support maternal brain glucose requirements and normal fetal brain development. We assessed whether low (<175 g/day) or very low (<120 g/day) carbohydrate intake was associated with altered nutrient intake, glycemia, or pregnancy outcomes in women with gestational diabetes mellitus (GDM). RESEARCH DESIGN AND METHODS: Women recruited to the Dietary Intervention in Gestational Diabetes Trial (DiGest) completed masked c

Perinatal health outcomes in relation to principal applicant and live-in caregiver status among economic class immigrants to Canada: A retrospective analysis of births from 1993 to 2017.

OBJECTIVES: Economic class immigrants to Canada tend to have better health outcomes than other immigrant groups. However, variation may exist even within this population. METHODS: We conducted a retrospective cohort analysis of data from the Migrant Maternal and Infant Morbidity and Mortality (MMIMM) dataset, which linked Canadian birth and infant death records (1993-2017) with maternal immigration records (1952-2017). All singleton births to economic class immigrant women at least 18 years of age at immigration were analyzed. Women were classified as principal applicants (PA), non-principal
