Systemic outcomes following maxillomandibular advancement for obstructive sleep apnea: systematic review and meta-analysis.

Obstructive sleep apnea (OSA) is a common sleep disorder associated with an elevated risk of systemic comorbidities. Maxillomandibular advancement (MMA) surgery can reduce upper airway obstruction and improve sleep parameters, but its systemic health benefits remain less well characterized. This systematic review and meta-analysis was performed to synthesize evidence on the systemic health outcomes associated with MMA in patients with OSA. A comprehensive literature search was conducted in PubMed, Embase, Cochrane CENTRAL, Scopus, and Web of Science from inception through May 20, 2026. Inclusi

Persistent Sleepiness After Effective Positive Airway Pressure Therapy Deserves Independent Clinical Evaluation.

Positive airway pressure (PAP) therapy often improves excessive daytime sleepiness in obstructive sleep apnea (OSA), yet effective control of sleep-disordered breathing does not invariably restore normal daytime alertness. Persistent sleepiness after PAP therapy should therefore be regarded as a clinical observation requiring explanation rather than automatically attributed to treated OSA. The term "residual excessive daytime sleepiness" may become misleading when it is interpreted as an etiologic conclusion rather than a descriptive label. Evaluation should distinguish PAP adherence from trea

Maternal and neonatal outcomes in women with inflammatory bowel disease following different biologic utilization trajectories in pregnancy.

OBJECTIVE: The 2025 Global Consensus recommends continuing biologics throughout pregnancy in women with inflammatory bowel disease. Real-world evidence on biologic treatment patterns and outcomes remains limited. This study compared maternal and neonatal outcomes across different biologic use trajectories during pregnancy. METHODS: This retrospective study included pregnant women with inflammatory bowel disease treated or delivering at University Hospitals Leuven, Belgium, between 2017 and 2025. Patients were categorized as biologic continuers, discontinuers, non-users, or initiators during p

Regional blood-brain barrier dysfunction maps to lower performance in cognitive ageing.

Age-related cognitive decline is a common and growing public health challenge, associated with loss of independence, reduced quality of life, increased caregiver burden and substantial healthcare and societal costs. Yet cognitive decline in ageing is heterogeneous, and current clinical constructs do not reliably capture the underlying biological mechanisms or predict progression to dementia. This cross-sectional study examined whether blood-brain barrier dysfunction (BBBD) provides a biologically supported correlate of cognitive performance beyond clinical diagnosis. The study included 46 pati

Fingered Citron Extract Ameliorates Diabetes-Related Depression via Targeting SRC-Mediated Insulin Signaling and Neuroinflammation.

Diabetes-related depression (DD) is a challenging comorbidity characterized by self-reinforcing interactions between metabolic dysfunction and affective disturbance, while conventional antidepressants and glucose-lowering drugs often yield suboptimal outcomes with adverse effects. This study investigated the therapeutic potential of fingered citron extract (FCE), traditionally used for metabolic and inflammatory conditions, in alleviating DD and elucidated its underlying mechanisms. Using a DD mouse model induced by high-fat diet, streptozotocin, and chronic unpredictable mild stress, we admin

AI-powered risk prediction models for preventable maternal mortality in rural settings: a systematic review.

BACKGROUND: Maternal mortality remains disproportionately high in low- and middle-income countries, particularly in rural settings with limited access to skilled obstetric care. Artificial intelligence and machine learning models offer promise for early risk prediction, yet their methodological rigour, applicability, and deployment feasibility in resource-constrained rural contexts remain inadequately synthesised. This systematic review evaluated AI-powered risk prediction models for preventable maternal mortality, with an emphasis on their suitability for rural and low-resource settings. MET

Summary cerebral small-vessel disease score mediates white-matter microstructural and perfusion changes in mild cognitive impairment: a diffusion kurtosis imaging and arterial spin labeling study.

PURPOSE: The summary cerebral small-vessel disease (CSVD) score reflects brain-damage severity, but its relationship with white-matter (WM) integrity and cerebral perfusion remains unclear. Here, we aimed to explore how this score links WM microstructure and cerebral blood flow (CBF). METHODS: We included 31 patients with CSVD and mild cognitive impairment (CSVD-MCI) and 31 matched healthy controls. Diffusion kurtosis imaging (DKI) and three-dimensional arterial spin labeling were used with tract-based spatial statistics to assess group differences in axial kurtosis, mean kurtosis (MK), radia

Molecular and histopathological evaluation of intestinal mucosal damage associated with coccidiosis in naturally infected lambs.

Coccidiosis, caused by Eimeria species, is a major cause of economic loss in sheep farming, leading to diarrhea, growth retardation, and mortality in young animals. This study aimed to characterize the intestinal pathology of naturally acquired coccidiosis in lambs and to evaluate the molecular response of the intestinal mucosa through the expression of five target genes. Lambs were classified into Control, Mild, Moderate, and Severe groups (n = 6/group) based on histopathological scoring of seven parameters: villus morphology, epithelial integrity, parasitic burden, epithelial hyperplasia, cr

Persistent postnatal alterations in microRNA expression in neonatal encephalopathy.

BACKGROUND: Despite causing severe neurological morbidity, neonatal encephalopathy (NE) lacks reliable biochemical markers for early diagnosis and intervention. This study investigated if alterations in microRNA (miRNA) expression, previously identified in umbilical cord blood in NE, remain altered in postnatal whole blood samples. METHODS: Term infants with NE (and matched controls) born in Cork University Maternity Hospital (CUMH) between 2017 and 2020 were recruited. Whole blood was collected within the first 6 (T0), 24 (T24) and 48 (T48) hours after delivery via dried blood spot microsamp

Association of Cholinergic Stimulation with Improved Functional Performance and Neurotrophic Markers in Mild Cognitive Impairment.

Mild cognitive impairment (MCI) is frequently accompanied by early impairment in gait and postural control. Whether commonly prescribed cognitive therapies differentially influence physical function and neurotrophic signaling remains unclear. We followed 99 older adults with MCI for six months while receiving either the cholinergic agent Donepezil (n = 47, age = 68.3 ± 4.1 years) or the glutamatergic modulator Memantine (n = 52, age = 70.1 ± 3.9 years). Postural balance and gait speed were assessed using the Short Physical Performance Battery. Plasma brain-derived neurotrophic factor (BDNF) an

Precise immunomodulation by mesenchymal stem cells mitigates hepatitis B virus-specific CD8⁺ T cell-mediated acute liver injury without compromising viral clearance.

Hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF) is associated with a high mortality rate. Although mesenchymal stem cell (MSC) therapy markedly improves liver function and survival in patients with HBV-ACLF, it remains unclear whether the immunomodulatory effects of MSCs suppress antiviral CD8⁺ T cell function and trigger HBV reactivation. Here, we developed a novel in vitro HBV-specific CD8⁺ T cell-mediated acute liver injury organoid model, which was co-cultured with either menstrual blood-derived MSCs (MenSCs) or umbilical cord-derived MSCs (UC-MSCs). We found that both

A 78-Year-Old Man With a Previously Stable Atherosclerotic Abdominal Aortic Aneurysm Presenting With Impending Rupture Following COVID-19.

BACKGROUND Abdominal aortic aneurysm (AAA) rupture is a life-threatening condition. Although elective repair is generally guided by aneurysm size, impending rupture can occur when rapid aneurysmal expansion or periaortic inflammatory changes develop before overt hemodynamic deterioration. This report describes the case of a 78-year-old man with a previously stable 43-mm atherosclerotic AAA who presented with impending rupture after coronavirus disease 2019 (COVID-19). CASE REPORT A 78-year-old Japanese man with a known AAA under imaging surveillance developed fever and was diagnosed with COVID

Sodium-dependent multivitamin transporter deficiency caused by novel SLC5A6 mutations: a clinical case and personalized mice model study.

The SLC5A6 gene encodes the sodium-dependent multivitamin transporter (SMVT), which transports biotin, pantothenate, and lipoic acid. Mutations of the SLC5A6 gene result in a hereditary syndrome whose symptoms range from early-onset, life-threatening systemic disorder to relatively mild, late-onset motor neuropathy. We describe a clinical case of a patient with previously unknown compound heterozygous missense mutations R280W and P437Q in the SLC5A6 gene. The patient exhibited a severe metabolic disorder accompanied by an elevated blood concentration of 3-hydroxyisovalerylcarnitine (C5OH), lik

Mini-Laparotomy Enterolithotomy for Gallstone Ileus in an Elderly Patient: A Case Report.

Gallstone ileus is an uncommon cause of mechanical small-bowel obstruction resulting from migration of a gallstone through a biliary-enteric fistula and occurs predominantly in older adults. A 70-year-old man with treated hypertension and recurrent, conservatively managed cholecystitis presented with three days of abdominal pain, vomiting, and progressive distension. He was febrile (38°C), tachycardic (110 beats/min), and had a blood pressure of 100/60 mmHg. Laboratory testing showed mild acute kidney injury, mild hypokalemia and hypochloremia, and a moderate inflammatory response. After initi

Preclinical Evaluation of [225Ac]Ac-NM600 Therapeutic Potential for Localized and Metastatic Triple-Negative Breast Cancer.

The absence of estrogen receptor, progesterone receptor, and HER2 expression limits the utility of targeted therapies in triple-negative breast cancer (TNBC). Chemotherapy is the standard of care for advanced TNBC but offers limited responses and an overall poor prognosis. We have developed several tumor-selective alkylphosphocholine analogs (NM600) for radiopharmaceutical therapy (RPT) of TNBC. Herein, we investigate the preclinical [225Ac]Ac-NM600 therapeutic efficacy and safety in two murine models of TNBC. NM600 was radiolabeled with 225Ac following previous methods.
