Maternal and fetal amino acid concentrations and fetal outcomes during pre-eclampsia.

The pathophysiology of pre-eclampsia is contested, but one hypothesis indicates that it is a heterogeneous condition in which only a subset of affected women bear small-for-gestational age (SGA) babies. In intrauterine growth-restricted (IUGR) infants, placental transport of amino acids is diminished and the resulting decrease in cord-blood amino acid concentrations is thought to contribute to their stunted growth. In contrast, the metabolic syndrome (dyslipidaemia, hyperinsulinaemia, hyperglycaemia, hypertension and obesity) which is associated with high amino acid concentrations is more prev

Endocrine-related causes and consequences of intrauterine growth retardation.

The term intrauterine growth retardation (IUGR) is assigned to newborns born with a birth weight and/or birth length below the tenth percentile for their gestational age. Intrauterine growth retardation is usually due to maternal, fetal factors, or placental insufficiency, while endocrine factors represent just a small minority in its etiology. Main endocrine-related causes of IUGR are disorders in insulin or insulin-like growth factor-I (IGF-I) secretion or action. Newborns with IUGR are at increased risk to develop a metabolic syndrome later in life, namely obesity, arterial hypertension, hy

Perinatal programming, renal development, and adult renal function.

Substantial data indicate that maternal diet during gestation, as well as perinatal events, affect organogenesis and, furthermore, may affect organ function at maturity, thus determining whether an individual is prone or resistant to a variety of health problems such as hypertension, cardiovascular disease, or renal dysfunction. This concept is called perinatal programming, or the fetal origins of adult disease (FOAD), a concept that arose after the publication of epidemiologic observations by Barker and colleagues that birth weight among apparently normal individuals studied in midlife was in

[Pregnancy and congenital heart defects].

With advanced diagnostic and therapeutic techniques in pediatric cardiology and cardiac surgery, pregnancy can be an option for patients with congenital heart disease. A low overall maternal mortality and a healthy pregnancy require interdisciplinary cooperation between the cardiologist, obstetrician and general practitioner caring for the mother. Treatment and outcome will depend on the type of cardiac malformation, on the functional impairment of the maternal heart (heart failure and/or cyanosis) and on the status of the fetus, with evidence of a better outcome for patients treated in specia

Fas and Fas ligand expression in maternal blood and in umbilical cord blood in preeclampsia.

The Fas-Fas ligand (FasL) pathway of apoptosis is abnormally activated in diseases associated with impaired immune tolerance or chronic inflammation. Pregnancy-related hypertension is a spectrum of disease that commonly causes significant morbidity in women and in their newborn infants, is associated with generalized inflammation, and may be causally related to impaired maternal-fetal tolerance. Our recent observation of enhanced trophoblast expression of FasL in one form of pregnancy-related hypertension led us to hypothesize that this group of disorders might be associated with abnormal acti

Progressive lung and cardiac changes associated with pulmonary hypertension in the fetal rat.

To determine the natural history of lung vascular remodeling and cardiac changes in the rat model of persistent pulmonary hypertension syndrome (PPHN) of the newborn, we studied fetal rats subjected to maternal indomethacin administration initiated on day 19 of gestation and continued for 2, 3, or 4 days. Animals receiving a similar volume of water or alcohol served as controls. Significant pulmonary hypertension was noted in the experimental group, as evidenced by a significantly increased right to left ventricular wall ratio to 1.6 +/- 0.1 in the 4-day treatment group, as compared with 1.2 +

[Birth weight and gestational age in relation to the course of pregnancy].

BACKGROUND: In midpregnancy, more than 3000 women in Brno filled in the questionnaires for the ELSPAC study. They informed of their health, activities, stress and psychological state during this pregnancy. METHODS AND RESULTS: Out of medical documentation, the results from routine prenatal examinations were extracted and records concerning all disorders occurred during the gravidity. After delivery, information coming from both sources were linked to the birth weight (BW) and gestational age (GA) data of newborns and analysis of variance was performed. In statistically significant relations t

Effects of fetal and neonatal environment on sympathetic nervous system development.

The sympathoadrenal system plays an important role in the regulation of metabolic and cardiovascular activity. With respect to carbohydrate metabolism, specifically, catecholamines affect both insulin secretion and insulin action. Alterations in sympathoadrenal system function have been suggested to contribute to the constellation of disorders referred to as syndrome X (obesity, hypertension, NIDDM, and dyslipoproteinemia). The origin of any such abnormalities in sympathoadrenal function is unknown. The sympathoadrenal system, like other parts of the mammalian nervous system, is susceptible to

Report from the National Transplantation Pregnancy Registry (NTPR): outcomes of pregnancy after transplantation.

Female solid organ recipients with good graft function generally tolerate pregnancy well. However, the combination of mother, fetus, transplanted organ, and immunosuppressive and other medications increases the complexity of management and raises the specter of adverse outcomes. For the mother, considerations include the nature of the original disease (i.e. genetic risk of transmission), co-morbid conditions which increase pregnancy risk (i.e. hypertension, diabetes, renal insufficiency), and long-term maternal prognosis. For the fetus, questions include the adequacy of maternal physiology (ca

Angiotensin converting enzyme inhibitors use in the first trimester of pregnancy.

Angiotensin Converting Enzyme Inhibitors (ACE-I) have been very effective in treating hypertension. Adverse conditions in the fetus with the use of ACE-I, such as oligohydramnios, intra-uterine growth restriction (IUGR), hypocalvaria, persistent ductus arteriosus with fetal and neonatal death have been rerported. Though the pathophysiology was thought to be a problem with renal hypoperfusion in the fetus, it remained unclear whether the first trimester exposure to these drugs produced a similar pattern. We participated in a collaborative trial initiated by the Organization of Teratology Inform

[Emergency caesarean section for a patient with chronic schizophrenia].

A 39-year-old female with chronic schizophrenia underwent an emergency caesarean section under general anesthesia at her 39th week of gestation. A diagnosis of schizophrenia was made at 28 years of age and since then oral antipsychotic drugs, haloperidol and levomepromazine had been given orally. She had only taken haloperidol 4 mg per day between the 12th week of gestation and the day of surgery. At the 39th week of gestation, she developed a marked excitement which would have caused fetal distress. We decided to terminate her gestation. As the excitement was diagnosed as psychokinesis, we av

Premature delivery of small versus appropriate-for-gestational-age neonates. A comparative study of maternal characteristics.

Prematurity and growth retardation of the fetus are major factors affecting perinatal outcome. The purpose of this study was to compare maternal characteristics of 63 women who delivered a preterm, small-for-gestational-age (SGA) neonate with those of 63 women matched for parity and gestational age who delivered a preterm, appropriate-for-gestational-age (AGA) neonate. All women delivered at the Soroka Medical Center between January 1, 1985, and December 31, 1990. Mothers of SGA neonates were more likely than those of AGA neonates to be over the age of 35 years (P = .043), to have hypertensive

Preterm delivery rate and fetal outcome in structurally affected twin pregnancies: a retrospective matched control study.

Data from 23 twin pregnancies with one structurally affected fetus were compared with data from 23 twin pregnancies with proven absence of structural fetal anomalies and matched for maternal age, parity, and year of delivery. The preterm delivery rate (< 37 weeks) was high in both groups but not significantly different (57 vs. 48 per cent). Perinatal mortality was significantly higher in the structurally affected twin pregnancies (65 vs. 9 per cent). In the affected twins, birth weight of the anomalous fetus was significantly lower than that of the normal co-twin. Since there was no difference

Is lung maturation related to fetal growth in diabetic or hypertensive pregnancies?

The purpose of this study was to determine whether infants sufficiently affected by maternal diabetes or hypertension to exhibit abnormal growth (macrosomia, growth retardation) would also display significant alteration in timing of pulmonary maturity (delay or acceleration, respectively). We studied 874 consecutive women with fetal pulmonary maturity testing prior to delivery. Patients were stratified by birth weight into fetal size categories (small for gestational age [SGA], appropriate for gestational age [AGA], large for gestational age [LGA]). Cases were compared based on maternal diseas

[Activity of plasma dopamine-beta-hydroxylase in newborn infants. Implication in prematurity, maternal pathology and growth].

The aim of the study was to determine plasma dopamine-beta-hydroxylase activity (DBH) in newborn infants and possible changes with maturation, mode of delivery and maternal disease: pregnancy-induced hypertension and gestational diabetes. DBH activity was determined by high performance liquid chromatography from 82 neonates divided in 2 groups: group I: 27 neonates (age: 1 to 8 days) including 6 preterm and 21 full-term neonates; group II: 55 full-term neonates divided in 5 classes: A: vaginal delivery; B: forceps; C: elective caesarean section; D: vaginal delivery and maternal hypertension; E

The syndrome of preeclampsia.

Preeclampsia has traditionally been viewed as one of several forms of hypertension complicating pregnancy. More recently, the multisystem nature of this unique gestational disorder has been emphasized. Pathophysiologic events, including abnormal placentation and heightened vascular reactivity, may occur weeks or months prior to clinical recognition of the disease. Although most frequently presenting as hypertension and proteinuria, hepatic (abdominal pain and elevation of transaminases) and hematologic (intravascular hemolysis and thrombocytopenia) involvement may be important features of the

Ontogeny of surfactant proteins A and B in human amniotic fluid as indices of fetal lung maturity.

Surfactant proteins A and B (SP-A and SP-B) were measured in human amniotic fluid by ELISA and correlated with lecithin to sphingomyelin ratio (L/S), phosphatidylglycerol (PG), and perinatal outcome. Amniotic fluid SP-A, SP-B, and L/S increased with advancing gestation. SP-A was detected at 19 wk gestation and increased dramatically in the 3rd trimester of pregnancy. SP-B was first detectable at 31 wk gestation and increased significantly to term. SP-A was a more specific predictor of nonrespiratory distress syndrome (RDS) than L/S or SP-B; however, the sensitivity of SP-A in predicting RDS wa

The relationship of placental grade, fetal lung maturity, and neonatal outcome in normal and complicated pregnancies.

Ultrasonically diagnosed maturity changes in the placenta, Grades 0 to III, have been previously shown to correlate with fetal lung maturity. In a prospective study of 230 term and preterm complicated pregnancies, we compared the relationship between sonographic placental grading, amniotic fluid phospholipids, and neonatal outcome. The frequencies of gestational age less than 38 weeks, lecithin/sphingomyelin (L/S) ratio less than 2.0, negative phosphatidylglycerol, and neonatal hyaline membrane disease were found to decrease as placental grade advanced from 0 to III. Patients were divided into
