[A case of lymphocutaneous sporotrichosis].

We report a case of lymphocutaneous sporotrichosis in a 69-year old man who had nodular-ulcerated lesions on the right hand and forearm. Small nodules remained on the right hand after 8 weeks of 0.5 g daily treatment with potassium iodide. Alternatively, terbinafine therapy (125 mg/day) resulted in healing with atrophic scars after 9 weeks without side effects. We reviewed 67 patients of cutaneous sporotrichosis in Japan from 1993 to 1999. Those cure rates (and mean durations of treatments in parentheses) are 90.9% (8.1 weeks) with potassium iodide, 86.6% (12.6 weeks) with itraconazole (100 mg

Sporotrichosis in Manipur: report of two cases.

Two cases of lymphocutaneous sporotrichosis are reported from Imphal. Sporothrix schenekii was isolated from pus from the lesion and identity was confirmed by mycelial to yeast conversion at 37 degrees C and mice pathogenicity test. One patient gave history of injury with bone of fermented fish (Ngari). Both the patients were treated successfully with oral administration of potassium iodide. These two cases are the first authentic cases of sporotrichosis from Manipur.

Potassium iodide in dermatology: a 19th century drug for the 21st century-uses, pharmacology, adverse effects, and contraindications.

Potassium iodide (KI) is a useful drug in the dermatologic armamentarium. It is successfully used for inflammatory dermatoses, most notably erythema nodosum, subacute nodular migratory panniculitis, nodular vasculitis, erythema multiforme, and Sweet's syndrome. KI is also successfully used for cutaneous and lymphocutaneous sporotrichosis. The precise mechanism by which KI acts is unknown. Although many minor side effects are common with this drug, major side effects can occur in pregnant patients and those with a history of kidney or thyroid disease. This article reviews the pharmacology, mech

Practice guidelines for the management of patients with sporotrichosis. For the Mycoses Study Group. Infectious Diseases Society of America.

UNLABELLED: The recommendations for the treatment of sporotrichosis were derived primarily from multicenter, nonrandomized treatment trials, small retrospective series, and case reports; no randomized, comparative treatment trials have been reported. Most cases of sporotrichosis are non life-threatening localized infections of the skin and subcutaneous tissues that can be treated with oral antifungal agents. The treatment of choice for fixed cutaneous or lymphocutaneous sporotrichosis is itraconazole for 36 months. The preferred treatment for osteoarticular sporotrichosis also is itraconazole,

Fine-needle aspiration cytologic diagnosis of lymphocutaneous sporotrichosis: a case report.

A case of lymphocutaneous sporotrichosis initially diagnosed by fine-needle aspiration (FNA) cytology and confirmed by tissue biopsy and culture study is presented. Asteroid bodies and yeast cells with budding, highly suggestive of the disease, were seen in the cytologic and histologic preparations. The pathology of this unusual fungal disease and the role of cytology in the diagnosis are discussed. This is the first case of lymphocutaneous sporotrichosis reported in the cytologic literature as diagnosed by FNA cytology.

Lymphocutaneous sporotrichosis: a case report and unconventional source of infection.

A 32-year-old white man had a 5-month history of a progressively worsening rash on the dorsal aspect of his left foot. He stated that he engaged in self-tattooing of the left foot prior to the onset of the rash. Further questioning revealed that he had mowed the lawn wearing only sandals on the same day that he had tattooed his foot. The rash was diagnosed as lymphocutaneous sporotrichosis based on clinical appearance, biopsy examination, and fungal culture. Clearing of the lesions was documented following 4 months of therapy with itraconazole. The remaining granulomatous lesions were flattene

[Atypical cutaneous sporotrichosis. The response to itraconazole and surgery].

Sporotrichosis is a subcutaneous (rarely extracutaneous) fungal disease caused by Sporothrix schenckii. Cutaneous sporotrichosis has two well-defined forms: lymphocutaneous and stable. In lymphocutaneous sporotrichosis, lesions arise along lymphatic vessels, generally in limbs. The stable form is rarer and consists of a single, generally facial, lesion. We report the case of a 70-year-old male farmer with a single lesion on the right nostril that had been removed repeatedly. After 5 years, new, linearly distributed lesions appeared. Fungal cultures of biopsy tissues revealed S. schenckii. The

Terbinafine: broad new spectrum of indications in several subcutaneous and systemic and parasitic diseases.

Immunocompromised patients are at risk of contracting serious fungal infections. The emergence of acquired resistance to azole treatment by opportunistic fungal organisms is increasing and poses a major therapeutic challenge. Treatment of some deep cutaneous and subcutaneous mycoses remains unresolved, relapses are frequent, lack of tolerability of the antifungal drugs becomes an obstacle, and, unfortunately, surgery is, in some cases, the last option. The development of allylamine antifungals, of which terbinafine is the most effective to date, may help to resolve this situation. In vitro, te

Case report. Sporotrichosis successfully treated with itraconazole in Japan.

A case of lymphocutaneous sporotrichosis that had developed from the dorsum nasi to the left buccal region of a 65-year-old woman was treated with itraconazole 100 mg day-1. The lesion healed in 10 weeks after starting treatment leaving crusts, mild erythema and pigmentation, and the treatment was completed 16 weeks after the start of treatment. Neither adverse reactions nor abnormal clinical laboratory values were noted. Until the present time, 1 year and 2 months after the completion of treatment, no recurrence has been observed. In Japan, 43 cases of sporotrichosis have been treated with it

Itraconazole therapy in lymphocutaneous sporotrichosis: a case report and review of the literature.

A 24-year-old white man had a six-month history of nontender nodules on the dorsal aspect of the right hand. The lesions were diagnosed as lymphocutaneous sporotrichosis based on clinical appearance, histopathologic examinations, and fungal culture. Therapy with itraconazole was started. Considerable improvement was recorded after two months, when all of the lesions diminished in size and were no longer nodular in appearance.

Epidemic cutaneous sporotrichosis: report of 16 cases in Queensland due to mouldy hay.

We report an epidemic of sporotrichosis in a south-east Queensland rural community. Sporotrichosis is a fungal infection due to the organism Sporothrix schenckii, typically presenting with cutaneous lesions. Sixteen cases of cutaneous sporotrichosis were seen over a 9 month period in the Darling Downs region of Queensland. All patients had had contact with a batch of mouldy hay presumed to be contaminated by Sporothrix schenckii. Nine of 16 patients were male; the youngest patient was aged 11 and the oldest was 67 years of age. Lymphocutaneous sporotrichosis was seen in 50% of patients; the re

Outbreak of sporotrichosis among tree nursery workers.

In spring 1994, an outbreak of sporotrichosis occurred at a tree nursery in Florida; 9 (14%) of 65 workers involved in production of sphagnum moss topiaries developed lymphocutaneous sporotrichosis. A cohort study of all 65 employees was conducted to identify risk factors for sporotrichosis, and an environmental investigation was done. The risk of sporotrichosis increased significantly with the duration of working with sphagnum moss (P < .05), in particular with filling topiaries (P < .05), and with having less gardening experience (P < .05). Wearing gloves was protective (P < .005). Sporothri

Treatment of lymphocutaneous and visceral sporotrichosis with fluconazole.

Thirty patients with documented sporotrichosis were treated with 200-800 mg of fluconazole daily. Fourteen patients had lymphocutaneous infection; only five (36%) of these patients had any underlying illnesses. Sixteen patients had osteoarticular or visceral sporotrichosis; 12 (75%) of these patients had underlying diseases, mostly alcoholism, diabetes mellitus, and chronic obstructive pulmonary disease. Eleven of the 30 patients had relapsed after prior antifungal therapy. Most patients were treated with 400 mg of fluconazole; however, four received 200 mg of fluconazole daily for the entire

Lymphocutaneous sporotrichosis.

Lymphocutaneous sporotrichosis presented in a 10 year old child 1-2 weeks after an abrasion. A series of nodules, two of which ulcerated, appeared along the arm with tender unilateral axillary lymphadenopathy in the absence of systemic symptoms. Biopsy showed a granulomatous infiltrate but failed to reveal the organism; however, culture was positive for Sporothrix schenckii. The primary lesion healed with a scar after 3 months of systemic therapy with potassium iodide.

Old and new therapies for sporotrichosis.

The old therapies for sporotrichosis--saturated solution of potassium iodide (SSKI) and amphotericin B--have largely been supplanted by itraconazole treatment. Although SSKI is effective for the treatment of lymphocutaneous sporotrichosis, it is difficult to administer and is frequently associated with side effects; response rates of >90% are associated with itraconazole therapy for lymphocutaneous sporotrichosis. Patients with osteoarticular sporotrichosis rarely have systemic symptoms and can be effectively treated with a prolonged course of itraconazole, thus obviating the need for intraven
