Clinical and multi-omics characterisation of early neurodevelopmental disorders associated with critical congenital heart disease: the prospective cohort CATAMARAN neonatal study protocol.

INTRODUCTION: Critical congenital heart disease (CHD) is associated with neurodevelopmental disorders, recognised as the most common long-term morbidity in affected children. In critical CHD, that is, CHD requiring cardiac surgery within the first 3 months of life, 30%-50% of children have lower developmental scores. Therefore, early identification of at-risk infants is crucial, yet there is no scientifically evaluated care programme in France. This study aims to evaluate early neurodevelopmental status in infants with prenatally diagnosed critical CHD and to determine how intrinsic susceptibi

Cobalamin-Related Remethylation Disorders: Pregnancy Outcomes and Prenatal Treatment-New Cases and a Literature Study.

Cobalamin-related remethylation disorders (cbl-RD) are rare, heterogenous conditions with a wide phenotypic spectrum, including prenatal and early infantile manifestations. Data on pregnancy outcomes in affected women and the efficacy and safety of prenatal treatment of affected fetuses remain limited. Here we report (1) five women with cbl-RDs (3 cblC/epi-cblC, 1 cblD, 1 cblE) who had in total nine pregnancies and (2) eight fetuses with cbl-RD (7 cblC, 1 cblF) diagnosed and treated prenatally. All affected mothers were treated with hydroxocobalamin (OHCbl) during pregnancy and two also receiv

Effect of maternal body mass index on inflammatory biomarkers and dynamic thiol-disulfide homeostasis during pregnancy.

BACKGROUND: The aim of this study was to investigate the relationship between maternal body mass index (BMI), a modifiable factor during the reproductive period, and inflammation and oxidative stress by assessing dynamic thiol-disulfide homeostasis (TDH) in both the mother and fetus. METHOD: This prospective cohort study was conducted between May and June 2024 at a tertiary obstetric care center. The inclusion criteria consisted of healthy pregnant women aged over 18 years, between 37 and 41 weeks of gestation, who had not used medications other than iron and folic acid supplements, with newb

A narrative review of metformin in pregnancy: Navigating benefit and uncertainty.

Metformin is well-established as a treatment for type 2 diabetes in non-pregnant individuals. The low cost, acceptability and broad tolerability of metformin have also made it an attractive option for research into the treatment of other conditions associated with insulin resistance. Despite almost 50 years of clinical experience with the use of metformin to treat diabetes in pregnancy, many questions remain regarding its precise effectiveness in different maternal subgroups, as well as potential short-term and long-term effects on the offspring. In this narrative review, we present the curren

Macronutrients, Micronutrients, and Malnutrition: Effects of Nutrition on Immune Function in Infants and Young Children.

The function of the immune system is not only dependent on factors like genetics, age, the environment, and exposure to infectious agents and allergens but also on our microbiota and our diet. It has been known for centuries that food can influence health and vulnerability to infection. This is especially true for infants, young children, and the elderly. This review focuses on how nutrition can support immune function from gestation to school-aged children. Immune support begins during pregnancy by the mother's diet and transfer of nutritional components as well as antibodies to her fetus. Af

Adipokines as Potential Biomarkers in Pregnancy: A Naturalistic Study of Adipokines in Pregnant Women and Newborns.

Maternal obesity is an escalating public health concern that adversely affects pregnancy outcomes. Adipokines play a key role in regulating metabolism and fetal development, but their dynamic changes during pregnancy remain inadequately understood. Objective: This study investigates maternal and fetal adipokine variations throughout pregnancy and their associations with maternal body mass index (BMI), abdominal wall thickness, and neonatal outcomes. Methods: A prospective case-control study was conducted involving 74 pregnant women categorized by BMI. Maternal blood samples were collected at m

Maternal Obesity and Neonatal Death in Preterm US Pacific Islander Neonates Using 2 Analytic Approaches.

IMPORTANCE: Owing to a high prevalence of obesity, Pacific Islander individuals in the US are at higher risk for preterm birth (PTB), but outcomes after PTB remain understudied. Existing literature suggests that associations between prepregnancy obesity and neonatal mortality stratified by gestational age (GA) are modest or null, which may be a result of overlooked stratification bias. OBJECTIVE: To estimate the association between prepregnancy body mass index (BMI) and neonatal death (NND) following PTB by different degrees of prematurity in US Pacific Islander neonates using birth-based and

Maternal Vitamin D Status and Its Association with Neonatal Health: Clinical Implications and Influencing Factors.

Introduction: Adequate maternal vitamin D status during pregnancy is essential for fetal skeletal development and neonatal vitamin D reserves. Evidence from Central and Eastern Europe on maternal deficiency, its determinants, and supplementation effectiveness in late pregnancy is limited. This study assessed the impact of 2000 IU/day and 4000 IU/day maternal vitamin D supplementation during the third trimester, compared to no supplementation, on maternal and neonatal 25-hydroxyvitamin D [25(OH)2D] levels at birth, and explored sociodemographic, obstetric, dietary, and lifestyle factors affecti

Application of group-based trajectory models to evaluate the association of fetal growth trajectories and childhood overweight and obesity: A longitudinal study with 2-year follow-up.

OBJECTIVE: This study investigated whether fetal growth trajectory (FGT) can predict adverse childhood growth outcomes. The correlation between maternal metabolic factors (e.g., body mass index (BMI), diabetes during pregnancy) and FGT was evaluated. METHOD: 1525 women with singleton pregnancies were included in this study. Fetal growth was assessed at least three times at 20-24 weeks of gestation, 28-32 weeks, and ≥36 weeks. Children's growth was monitored from birth to 2 years old. A population-based trajectory model was used to analyze the changes in FGT with gestational age and the associ

Incretin receptor agonism during pregnancy: implications for mother and baby.

Obesity has been described by the WHO as the largest health threat facing mankind. More than 55% of pregnancies in the United Kingdom occur in women who are overweight or living with obesity. Obesity in pregnancy increases the risk of developing gestational diabetes mellitus (GDM), a condition that affects one in seven pregnancies globally and is associated with short- and long-term risks for both mother and baby. Therefore, optimising treatment to effectively treat both obesity and GDM in the perinatal period could have wide-ranging benefits for mother and child. Stabilised analogues of gluca

Characteristics of Successful Third-Line Eradication of Helicobacter pylori based on Antibiotic Susceptibility.

BACKGROUND: Regimens using vonoprazan (VPZ), amoxicillin (AMX), and sitafloxacin (STFX) have been reported to be effective for the third-line eradication of Helicobacter pylori and are widely used in Japan. However, the outcomes of these therapies have only been reported by a limited number of institutions. We aimed to investigate the backgrounds and treatment outcomes of patients who underwent successful third-line eradication of H. pylori. METHODS: This was a single-center, retrospective, observational study included patients who underwent antibiotic susceptibility testing and third-line er

Exploring mitochondrial heteroplasmy in neonates: implications for growth patterns and overweight in the first years of life.

BACKGROUND: Mitochondrial heteroplasmy reflects genetic diversity within individuals due to the presence of varying mitochondrial DNA (mtDNA) sequences, possibly affecting mitochondrial function and energy production in cells. Rapid growth during early childhood is a critical development with long-term implications for health and well-being. In this study, we investigated if cord blood mtDNA heteroplasmy is associated with rapid growth at 6 and 12 months and overweight in childhood at 4-6 years. METHODS: This study included 200 mother-child pairs of the ENVIRONAGE birth cohort. Whole mitochon

The influence of maternal prepregnancy weight and gestational weight gain on the umbilical cord blood metabolome: a case-control study.

BACKGROUND: Maternal overweight/obesity and excessive gestational weight gain (GWG) are frequently reported to be risk factors for obesity and other metabolic disorders in offspring. Cord blood metabolites provide information on fetal nutritional and metabolic health and could provide an early window of detection of potential health issues among newborns. The aim of the study was to explore the impact of maternal prepregnancy overweight/obesity and excessive GWG on cord blood metabolic profiles. METHODS: A case control study including 33 pairs of mothers with prepregnancy overweight/obesity a

Association between vitamin D levels with IL-6 and IL-10 in umbilical cord blood of infants.

A worldwide issue, vitamin D deficiency affects pregnant mothers and babies everywhere, including Indonesia. It involves the adaptive immune system by controlling the production of pro-and anti-inflammatory cytokines and the balance between humoral (Th2) and cell-mediated (Th1) immunity. The aim of this study was to investigate the relationship between vitamin D and the cytokines IL-6 and IL-10 in infants. It also examined the relationship between ferritin and IL-6/IL-10 in newborns. The study collected 114 umbilical cord blood samples from term-born mothers without clinical symptoms. IL-6 and

Maternal and fetal/neonatal outcomes in pregnancy, delivery and postpartum following bariatric surgery and comparison with pregnant women with obesity: a study protocol for a prospective cohort.

BACKGROUND: Being obese can lead to various complications during pregnancy, such as Gestational Diabetes Mellitus (GDM), pregnancy induced hypertension (PIH), Pre-Eclampsia (PE), and Large Gestational Age (LGA). Although bariatric surgery is an effective way to treat obesity, it can also result in complications and may be linked to having small for gestational age (SGA) babies. This cohort study protocol aims to compare the maternal and fetal/neonatal outcomes of two groups of Iranian pregnant women: those who have undergone bariatric surgery and those who are obese but have not had bariatric

Defining poor growth and stillbirth risk in pregnancy for infants of mothers with overweight and obesity.

BACKGROUND: Mothers who are obese carry heavier fetuses and have lower rates of small for gestational age (<10th birthweight centile) infants. However, their infants may be growth-restricted (with an increased risk of stillbirth) at a higher birthweight centile compared with infants from healthy-weight women. OBJECTIVE: This study aimed to quantify the birthweight centile at which the risk of stillbirth in infants born to obese women equaled that of <10th-centile infants born to healthy-weight women, and clarify the relationship between maternal body mass index, infant size, and stillbirth.

Vitamin B12 status in term pregnant women and the cord blood samples of their newborns: A cross-sectional study.

Background and aim: In India, total vitamin B12 (Vit B12) and its active form (active B12) have not been studied in mother's blood and cord blood. We hypothesized that total and active B12 levels are sufficiently maintained in cord blood despite low levels in mothers. Methods: Two hundred term pregnant mother's blood and cord blood was collected and analyzed for total Vit B12 (radioimmunoassay method) and active B12 levels (enzyme-linked immunosorbent assay). Mean values of constant or continuous variables (hemoglobin (Hb), packed cell volume (PCV), mean corpuscular volume (MCV), white blood c

Obesity Induces an Impaired Placental Antiviral Immune Response in Pregnant Women Infected with Zika Virus.

Obesity is increasing in incidence worldwide, especially in women, which can affect the outcome of pregnancy. During this period, viral infections represent a risk to the mother, the placental unit, and the fetus. The Zika virus (ZIKV) outbreak in Brazil has been the cause of congenital Zika syndrome (CZS), with devastating consequences such as microcephaly in newborns. Herein, we analyzed the impact of maternal overweight/obesity on the antiviral factors' expression in the placental tissue of Zika-infected mothers. We accessed placentas from women with and without obesity from 34 public healt
