Sustained alterations in postnatal respiratory function following sub-optimal intrauterine conditions.

This paper reviews recent evidence from epidemiological, follow-up and experimental studies that sub-optimal conditions during gestation can cause alterations in respiratory function that persist during postnatal life. Several studies indicate that placental insufficiency, which can be associated with fetal substrate deprivation, hypoxia and low birthweight, may be followed by evidence of respiratory compromise in later life. Similarly, it is becoming evident that maternal smoking affects fetal lung development and that the effects can persist into postnatal life. A reduced period of fetal dev

Antenatal drugs affecting vitamin K status of the fetus and the newborn.

Coumarin derivatives and anticonvulsants administered during pregnancy enter the fetal circulation, interfering with the action of vitamin K. Vitamin K plays a crucial part in the gamma-carboxylation of glutamic acid residues of the vitamin K-dependent coagulation factors prothrombin, FVII, FIX, and FX. Other vitamin K-dependent proteins in the coagulation cascade are protein C and protein S. Vitamin K-dependent bone proteins are osteocalcin and gamma-carboxyglutamate matrix protein. Administration of coumarol derivatives results in under carboxylation of the vitamin K-dependent proteins. Anti

Effects of maternal protein malnutrition on fetal growth, plasma insulin-like growth factors, insulin-like growth factor binding proteins, and liver insulin-like growth factor gene expression in the rat.

We examined the effects of maternal dietary protein restriction on fetal growth and expression of IGF-I and -II, and IGF-binding proteins (IGFBP). We sought to dissociate the respective effects of maternal protein versus calorie restriction on growth indices and IGF synthesis by the neonates of protein-restricted dams. Pregnant Wistar rats (six to eight per group) fed a low (5%) protein diet throughout gestation had impaired body weight gain compared with controls fed a normal (20%) protein diet (by 45%, p < 0.001). Their serum and liver IGF-I concentrations and liver IGF-I mRNA concentrations

[Fetal megalosomia: clinical and prognostic value of main risk factors].

Fetal megalosomia associates with an increased risk of maternal and perinatal pathology. However it is right to make a clear distinction between "physiologic" megalosomia, expression of a high genetic potential of growth from the fetus, and megalosomia referable to any maternal pathology. While the first one entails problems of obstetric management at the most, the second one compels to take measures in order to reduce its incidence. A sample of 45 fetuses with megalosomia was studied retrospectively and compared with a standard sample, randomly selected among the fetuses, term delivered, who

Effects of alcohol use, smoking, and illicit drug use on fetal growth in black infants.

STUDY OBJECTIVES: To compare the effects of prenatal exposure to alcohol, smoking, and illicit drugs on birth size. DESIGN: Prospective, longitudinal correlational study, with statistical control for confounding. PARTICIPANTS: Four hundred seventeen black infants. Mothers recruited at first prenatal clinic visit on the basis of moderate-to-heavy use of alcohol or cocaine or both, plus a 5% random sample of lower-level drinkers and abstainers. MAIN RESULTS: Alcohol, smoking, opiates, and cocaine were each correlated with smaller birth weight, length, and head circumference (median r = -0.21;

The disorders induced by iodine deficiency.

This paper reviews present knowledge on the etiology, pathophysiology, complications, prevention, and therapy of the disorders induced by iodine deficiency. The recommended dietary allowances of iodine are 100 micrograms/day for adults and adolescents, 60-100 micrograms/day for children aged 1 to 10 years, and 35-40 micrograms/day in infants aged less than 1 year. When the physiological requirements of iodine are not met in a given population, a series of functional and developmental abnormalities occur including thyroid function abnormalities and, when iodine deficiency is severe, endemic goi

Protein restriction during pregnancy affects postnatal growth in swine progeny.

Protein deficiency during pregnancy affects fetal development. The critical period, when the fetus is most susceptible to maternal protein deficiency and its effect on neonatal growth, is unknown. Therefore, we studied the effect of a protein-restricted diet during early and late pregnancy and throughout pregnancy on growth of pigs from birth to market weight. Sows were fed a control (13% protein) or protein-restricted (0.5% protein) diet throughout pregnancy or protein-restricted diet from d 1 to 44, then control diet to term or control diet from d 1 to 81, then the protein-restricted diet to

Maternal risk factors for low birth weight and intrauterine growth retardation in a Guatemalan population.

Hospital data on 306 mother-infant pairs in Cobán, Guatemala, were studied to assess factors responsible for low infant birth weights involving the two general kinds of intrauterine growth retardation (IUGR), these being Type I (symmetric stunting) and Type II (asymmetric stunting or wasting). The results indicate that the Type I IUGR observed arose primarily from long-term poverty, malnutrition, and chronic poor living conditions, while much of the Type II IUGR seemed attributable to maternal disorders affected by short interbirth intervals.

Malnutrition and drug metabolism in fetal and neonatal animals.

Protein-calorie malnutrition in experimental animals resulted in a complex picture of changes in various drug-metabolic pathways. The need for these studies is discussed in the context that undernutrition is prevalent in many countries and coincides with many intercurrent illnesses in the affected populations. The need of these affected populations for appropriate therapies is recognized, and these therapies should be based on scientific principles rather than on empiricism.

[Role of zinc deficiency in the etiology of neural tube malformations].

The authors study the hypothesis of zinc deficiency as an etiology of malformations of the neural tube. This hypothesis is based on several facts: decrease of maternal zinc levels in this disease found by many authors, malformations of the neural tube induced by experimental deficiency in several animal species, zinc membrane transfer disorders found in fibroblasts cultures in affected newborns. Numerous zinc related biochemical mechanisms could explain this effect. But the most interesting would a disturbance of folate metabolism. In fact, neither a strict isolated zinc deficiency nor a stric

Neonatal rheumatic disorders. View of the pediatrician.

Neonatal lupus is a unique clinical entity characterized primarily by cutaneous or cardiac injury. Dermatitis usually resolves without significant residual effects but heart block may be irreversible and life threatening during the neonatal period. SS-A/Ro and/or SS-B/La antibodies of maternal origin are present in the serum of the mother and affected infant and are markers for this syndrome. For many mothers breast feeding is the preferred choice for infant nutrition. With proper guidance, lactating mothers may safely use several antirheumatic medications such as ibuprofen, piroxicam, flurbip

Intrauterine growth and postnatal fasting metabolism in infants of obese mothers.

We compared neonatal fasting metabolism in 12 infants of obese women and 10 control infants born to lean mothers. Weight gain during pregnancy was less among obese women (7.2 vs 13.6 kg). Term infants born to obese women were heavier (3.9 bs 3.2 kg), had greater triceps and midscapular skinfold thicknesses, and had heavier placentae (687 vs 572 gm). Length and head circumference were not affected. After birth, fasting blood glucose concentrations declined in obese infants to hypoglycemic levels at 60 minutes of the study (26.3 vs 63.1 mg/dl). Insulin levels were not statistically different bet

Intrauterine malnutrition in the rat: alterations of fetal glycerol metabolism.

This investigation intended to clarify the effects of malnutrition in utero on enzymes of glycerol metabolism and the stores of phosphorylated glycerol intermediates in liver, striated muscle, and brain in the rat. Pregnant Wistar rats were restricted to an intake of 50% (M) of ad libitum fed controls (C) from the seventh day of gestation onward. Fetuses were removed 2 days (-2), or 1 day (-1), before term, or at the day of birth (DOB) The M fetuses and newborn rats were stunted. Their hepatic alpha-glycerophosphate oxidase (GPO) levels were lower than those of C in utero (mean +/- SEM: M = 23

Disorders of calcium homeostasis in the fetus and neonate.

The physiological mechanisms involved in the alterations in calcium homeostasis during pregnancy are complex. The fetal acquisition of calcium, for skeletal growth, is obtained by an increase in intestinal calcium absorption in the mother with transplacental calcium transfer to the fetus. The regulation of calcium homeostasis during the transition from the intrauterine to the extrauterine environment is complex and poorly understood. Within the first few hours of life the serum calcium concentrations begins to fall progressively reaching a "trough" value by the second or third day of life and

Effect of maternal canine starvation on fetal and neonatal liver metabolism.

Heptic glycolytic and gluconeogenic intermediates from fasted newborns of five control and five 3-day starved canine mothers (MCS) were studied at 0, 3, 6, 9, and 24 h of age. MCS did not affect fetal hepatic glycogen concentration; however, a significant increase in uridine diphosphate glucose (UDPG) (0.186 vs. 0.106 mumol/g), fructose 6-phosphate (0.084 vs. 0.034), pyruvate (0.321 vs. 0.126), and citrate (0.190 vs. 0.140) concentrations occurred. At 3 h, the intrahepatic glucose concentration among the MCS newborns declined (3.09 vs. 6.34) and remained lower than the controls for up to 9 h.

Fetal and neonatal responses to maternal canine starvation: circulating fuels and neonatal glucose production.

Pregnant dogs were starved for 72 hr while controls were fasted overnight. Maternal starvation significantly reduced fetal birth weight (269 +/- 7.2 versus 294 +/- 4.4 g). Total caloric deprivation had no effect on maternal or fetal blood glucose concentration at the time of delivery; however, fasting neonatal blood glucose levels were depressed during the first 9 hr of life. Starvation produced a large elevation of maternal free fatty acids (1.68 +/- 0.39 versus 0.74 +/- 0.2 mM) and ketone bodies (2.99 +/- 0.70 versus 1.04 +/- 0.48). Although fetal free fatty acids increased minimally (0.39 +

Effects of cigarette smoking on maternal nutrition and the fetus.

Available evidence supports the importance of including facts about smoking and pregnancy in all teaching to women in the childbearing years. The incidence of low birthweight infants is possibly almost twice as high among smoking mothers, and these low birthweight babies are known to be at risk on several counts. Compromised utilization of vitamins and minerals is presented as a causative factor: calcium, vitamin B12, and vitamin C levels are lower in smokers. Other vitamin levels may be affected. Poor nutrition and other factors connected with smoking may result also in stillbirths, spontaneo

Banting Lecture 1980. Of pregnancy and progeny.

A profile characteristic of fuel economy in the mother during normal pregnancy has been delineated. The evidence indicates that pregnancy changes the metabolism of every class of foodstuff. The mechanisms by which the conceptus may be implicated are reviewed. The gestational interactions create a pattern of "accelerated starvation" whenever food is withheld, especially in late pregnancy, and they tend to "facilitate anabolism" when food is ingested. The consequent heightened metabolic oscillations during the shuttlings from fed to fasted state provide a basis for more aggressive therapy with e

Influence of folic acid on birthweight and growth of the erythroblastotic infant. I. Birthweight.

The birthweights of 100 infants with erythroblastosis were carefully matched as to sex, gestational age, and parity with the birthweights of 200 control infants born during the same period. At all gestational ages the average birthweight of the affected infants was below that of the controls, the average reduction being 227 g. The more severely affected infants tended to be at a lower centile for birthweight than were the mildly affected ones. The relationships between maternal serum folate, cord blood serum folate, and centile for birthweight among affected infants were also studied. There wa
