Peculiarity of preterm neonates at 33-35 weeks gestational age with mild-to-severe hypoxic-ischaemic encephalopathy: a single-centre experience.

UNLABELLED: To describe the peculiarity of preterm neonates at 33-35 weeks gestational age (GA) with mild-to-severe hypoxic-ischaemic encephalopathy (HIE). This single-centre retrospective study compared the background characteristics, treatments, and outcomes of 30 preterm HIE neonates at 33-35 weeks GA with those of 399 HIE neonates at ≥ 36 weeks GA between 2011 and 2023. Between the 33-35 weeks and 36, 37-40, or ≥ 41 weeks GA groups, HIE severity and use of therapeutic hypothermia (TH) were not significantly different. Between the 33-35 weeks and 37-40 weeks GA groups, no TH, except for mil

Immunoglobulin therapy for the fetus and neonate.

Maternal immunity is modulated during pregnancy at the placental interface to prevent alloreactivity with the developing fetus. Importantly, however, maternal immunoglobulin G (IgG) freely crosses the placenta, and the presence of pre-existing alloreactive antibodies can lead to injury of fetal tissues and/or cells. Because maternal IgG continues to circulate up to 6 months after birth, these antibodies can also continue to affect the newborn, causing a variety of disease conditions including haemolytic disease of the newborn, neonatal alloimmune thrombocytopenia, neonatal lupus, neonatal Grav

Intrauterine exposure to HBeAg exerts modest effect on neonatal immune repertoire.

BACKGROUND: The paucity of information on fetal immune repertoire dynamics following intrauterine hepatitis B related antigens exposure precludes elucidation of its effects on fetal immune system. METHODS: Totally 34 cord blood (CB) samples from singleton full-term neonates and 29 paired maternal peripheral blood (PB) samples were collected at delivery and divided into four groups based on maternal hepatitis B condition and antiviral intervention history. Clonal information in complementary determining region 3 (CDR3) of T cell receptor β (TCRβ) chain and B cell receptor (BCR) heavy chain enc

Birth asphyxia outcomes and associated factors among newborns admitted to a tertiary hospital in Eastern Uganda: A prospective cohort study.

BACKGROUND: Birth asphyxia (BA) is a significant global health challenge, contributing to an estimated 23% of neonatal deaths worldwide and a substantial burden of long-term disabilities. It results from interrupted blood flow and gas exchange to the fetus, leading to neuronal injury and short or long-term outcomes. While most affected newborns recover fully, a notable proportion develop hypoxic-ischemic encephalopathy (HIE), associated with high morbidity and mortality. This study aimed to describe Birth asphyxia outcomes and associated factors among newborns admitted at Mbale Regional Referr

Effect of timing of umbilical cord clamping on cerebral regional tissue oxygenation: a secondary analysis of the COSGOD III trial.

AIM: To determine how different cord clamping strategies affect cerebral oxygenation in the first 15 min after birth in preterm infants. METHODS: A post-hoc secondary outcome analysis of a multicentre prospective randomised clinical trial (COSGOD III) conducted between October 2017 and October 2021 in 11 tertiary neonatal intensive care units in six countries in Europe and in Canada. In the present ancillary study, all included premature neonates (<32 weeks gestation) were retrospectively assigned to three groups according to the timing of cord clamping (G1<30 s, G2 30-60 s, G3>30 s). The aim

Outcome of monochorionic diamniotic twin pregnancy with selective fetal growth restriction and continuous or intermittent absent or reversed end-diastolic umbilical artery flow: international multicenter cohort study.

OBJECTIVES: Monochorionic diamniotic (MCDA) twins with selective fetal growth restriction (sFGR) and either continuous (cAREDF) or intermittent (iAREDF) absent or reversed end-diastolic flow in the umbilical artery face significant fetal and neonatal risks. This study evaluated fetal and neonatal outcomes in these cases and compared outcomes for the larger twin following selective reduction (SR) vs expectant management (EM). METHODS: This was an international retrospective cohort study of MCDA twin pregnancies with sFGR and cAREDF or iAREDF from five fetal medicine centers over 7 years (2016-

Anesthetic Dilemma in a Full-term Parturient with Facial Trauma and Suspected Head Injury Presenting as Difficult Airway Posted for Emergency LSCS in a Limited Resource Setting: Spinal versus General Anesthesia.

Trauma frequently occurs during pregnancy and occurs 1 in 12 pregnant women affecting maternal mortality, morbidity, and also pregnancy outcome. Hence, it is essential to consider pregnancy in every female trauma patient of reproductive age. Motor vehicle accidents and domestic/intimate partner violence account for most cases of major maternal trauma. Most of the causes mentioned above can lead to facial injuries/head injuries making it challenging for the anesthesia provider. We report a parturient who presented to us with multiple facial injuries with a suspected head injury following a road

Effects of gestational diabetes mellitus on offspring: A literature review.

Gestational diabetes mellitus (GDM) is a health problem that is becoming increasingly serious worldwide. It has a high incidence and strongly affects both mothers and children. In recent years, more extensive and in-depth studies have been conducted. In the present study, we systematically reviewed the effects of GDM on offspring, including the short- and long-term outcomes. Related articles were indexed in PubMed, Web of Science, Science Direct, and the Cochran Library up to June 2024. Recommendations were extracted from the identified articles and collated as themes. Many studies, which are

Gestational alloimmune liver disease treated with intravenous immunoglobulin resulting in maternal drug-induced liver injury.

Gestational alloimmune liver disease (GALD) is a rare pregnancy complication involving maternal antibody-mediated fetal liver injury that recurs in 90% of affected pregnancies. The fetal and neonatal mortality rate is high, up to 82%, and affected fetuses can develop fetal hydrops, growth restriction, and oligohydramnios. Intravenous immunoglobulin (IVIG) administration can prevent GALD recurrence. We present a patient whose three live births were impacted by GALD. Her first pregnancy was an elective termination. Her second pregnancy resulted in neonatal demise due to autopsy-proven GALD. Her t

Fetal brain injury in the pump twin of a TRAP sequence with favorable postnatal outcome: a case report.

BACKGROUND: Prenatal diagnosis of abnormal brain lesions in the pump twin of a twin reversed arterial perfusion (TRAP) sequence is exceptionally rare, with only three cases documented in the literature. In this report, we present a case of prenatally diagnosed brain injury in the pump twin, correlating with defined cerebral territory, along with postnatal cranial ultrasound, developmental and clinical outcomes. CASE PRESENTATION: A 22-year-old pregnant woman was initially misdiagnosed with a monochorionic diamniotic (MCDA) twin pregnancy complicated by single fetal demise. The viable twin cor

Foetal disruptive brain injuries: Diagnosing the underlying pathogenetic mechanisms with cranial ultrasonography.

Antenatal destructive events affecting the central nervous system of the foetus lead to disruptive brain lesions that are often associated with impaired neurodevelopment. The pathogenesis of these lesions encompasses a range of causes, including haemorrhagic, embolic, or other vascular events; exposure to teratogens, such as drugs or substance abuse; congenital brain infections; genetic conditions; and metabolic disorders. Cranial ultrasonography is the first-line imaging modality to diagnose these antepartum brain lesions in the newborn infant; it is often complemented by brain magnetic reson

The Perinatal Committee report: Review of the progress of obstetric healthcare in Japan.

BACKGROUND: Japan's maternal mortality rate and perinatal mortality rate have shown one of the world's most significant declines, positioning Japan at the global forefront of the lowest levels. The nearly logarithmic annual decline in maternal and perinatal mortality rates suggests that, in addition to advances in medicine and healthcare, various types of care for mothers and newborns have played a crucial role in this achievement. METHOD: From the period of World War II to the postwar era, up to around 1980, and then every decade thereafter, the events and movements surrounding perinatal hea

Mini-laparotomy to avoid vascular injury during fetoscopic laser ablation in a case of twin-twin transfusion syndrome with anterior wall placenta: Report of a case complicated with renal dysplasia and cord-around-trunk in donor cotwin and review of the literature.

OBJECTIVE: Twin-twin transfusion syndrome (TTTS) is a special complication of monochorionic (MC) twins, with unbalanced flows through vascular anastomoses within the placenta, causing hemodynamic derangement that can adversely affect perinatal outcome. The initial clinical manifestations of TTTS may be hypovolemic and oliguric of the donor cotwin (the "struck" twin), and hypervolemia and polyureic of the recipient cotwin under prenatal ultrasound. However, the mortality rate can reach 90 % if left untreated. Fetoscopic laser ablation (FLA) is considered the golden treatment standard of TTTS. H

Congenital cytomegalovirus infection and brain injury in a newborn following maternal non-primary infection: case report of an unexpected diagnosis.

BACKGROUND: Congenital cytomegalovirus (cCMV) infection leads to a significant burden on the health system. Relevant insights have been reached in the understanding of primary infection (PI) during pregnancy. However, knowledge gaps still exist related to maternal non-primary infections (NPI). Severe neurologic damage and hearing loss are the possible outcomes in the 17-20% of affected children. Furthermore, neither risk prevention strategies nor management are currently available for these NPI patients. CASE PRESENTATION: We report on a male term newborn showing in the first days of life hyp

The association of exposure to polychlorinated biphenyls with lipid profile and liver enzymes in umbilical cord blood samples.

Evidence on prenatal exposure to polychlorinated biphenyls (PCBs) and its effects on newborns and potential biological mechanisms is not well defined yet. Therefore, this study aimed to examine whether PCBs are associated with lipid profile and non-invasive markers of hepatocyte injuries in samples of blood obtained from the umbilical cord. This study included 450 mothers-newborn pairs. Umbilical levels of PCBs were measured using Gas Chromatography/Mass Spectrophotometry (GC/MS). Lipid profile including low-density lipoprotein (LDL-C), total cholesterol (TC), triglycerides (TG), and high-dens

Ameliorative mechanism of dietary vitamin d and magnesium on newborn's pulmonary toxicity induced by cadmium.

Cadmium (Cd) exposure in mothers can cause respiratory issues in newborns, but the exact toxicity mechanisms are not fully understood. Vitamin D deficiency in Cd-exposed rats is associated with increased cadmium accumulation in tissues. Finding a cost-effective medication that is vital for the body while also reducing the effects of poisoning is crucial in treating poisonings. To investigate the mechanisms of Cd-induced lung toxicity, we examined the impact of prolonged Cd exposure in female rats before pregnancy on newborn lung health, focusing on sera TNF-α level, lung P53, Foxo1 mRNA, and l

ITRAQ Based Proteomics Reveals the Potential Mechanism of Placental Injury Induced by Prenatal Stress.

Maternal stress experienced during prenatal development is recognized as a significant risk factor for neurodevelopmental and neuropsychiatric disorders across the offspring's lifespan. The placental barrier serves a crucial function in safeguarding the fetus from detrimental exposures during gestation. However, previous investigations have not yet comprehensively elucidated the extensive connections between prenatal stress and the expression of placental proteins. In this study, we used iTRAQ-based quantitative proteomics to elucidate the placental adaptive mechanisms of pregnant rats in resp

Timing of induction of labor in suspected macrosomia: retrospective cohort study, systematic review and meta-analysis.

OBJECTIVES: Large-for-gestational age (LGA) is associated with several adverse maternal and neonatal outcomes. Although many studies have found that early induction of labor (IOL) in case of a LGA fetus reduces the incidence of shoulder dystocia, no current guidelines recommend this particular clinical strategy, owing to concerns about increased rates of Cesarean delivery (CD) and neonatal complications. The purpose of this study was to assess whether the timing of IOL in LGA fetuses affected maternal and neonatal outcomes in a single center, and to combine these results with evidence reported

Impact of screening for large-for-gestational-age fetuses on maternal and neonatal outcomes: a prospective observational study.

OBJECTIVES: Debates on the management of macrosomia are still current. We have to consider the consequences of screening to contribute to these discussions. Our aim is to study the consequences of the 3rd trimester fetal macrosomia screening protocols used in several centres in the same French region in order to determine whether this screening affects maternal and neonatal outcomes: mode of delivery, maternal complications (haemorrhage, perineal lesions), neonatal health (pH, Apgar score) and the occurrence of neonatal trauma during delivery. METHODS: Prospective observational, multicenter c
