Pediatric effects of forever chemicals may last for a long time.

Perfluorinated alkylate substances (PFASs) have been in use for about 75 years. Because of their persistence, concentrations in the environment have increased, and absorbed PFAS will remain in the human body, some compounds for many years. The fetus and the newborn infant are particularly vulnerable to PFAS toxicity due to the intense development of organs and body functions. In addition, PFASs pass the placenta so that a pregnant woman shares her accumulated PFAS burden with the progeny. In addition, PFASs are excreted in the milk, so that the resulting serum-PFAS concentration in the infant

Early Epidemiologic and Immune Predictors of Atopic Dermatitis: Reduced Cord Blood Regulatory B10 Cells in the Munich Atopy Prediction Study (MAPS).

BACKGROUND: A complex interaction between environmental and lifestyle factors, immune dysregulation, and skin barrier integrity is believed to contribute to the development of atopic dermatitis (AD). However, the precise mechanisms underlying disease onset in infants remain largely unclear. METHODS: The "Munich Atopic Prediction Study" (MAPS) is a comprehensive clinical and biological investigation of a prospective birth cohort from Munich, Germany. Information on pregnancy, child development, environmental influences, parental exposure to potential allergens, as well as illnesses affecting b

Microplastics and neurotoxicity: could prenatal exposure to microplastics boost congenital enteric neuropathies?

Microplastics (MPs) pollution represents an increasing worldwide problem and a real global challenge for human health, which also affects unborn children. Specifically, during their degradation, they can release a broad range of toxic and hormonally active agents, such as plasticizers. Thus, microplastics alone are pernicious, but they often also carry other harmful chemicals and even problematic bacteria on their surface and within their structure (heavy metals, pesticides, parabens, etc.), which amplifies their toxic potential. Due to their induction of oxidative damage, inflammation, mitoch

Health Care Use and Recurrence Rate in Hemolytic Disease of the Fetus and Newborn: Retrospective Cohort Study.

BACKGROUND: Hemolytic disease of the fetus and newborn (HDFN) is a life-threatening condition resulting from maternal-fetal erythrocyte antigen incompatibility. Although anti-Rhesus D (RhD) prophylaxis has reduced RhD-associated cases, HDFN persists due to non-RhD antibodies and gaps in prevention. Population-based data on maternal and neonatal outcomes and recurrence patterns are limited. OBJECTIVE: This study aimed to characterize maternal and neonatal outcomes, health care use patterns, and recurrence rates of HDFN across pregnancies. METHODS: We conducted a retrospective cohort study of

Insulin/C-peptide interpretation in infants with hypoglycaemia of unknown origin.

BackgroundHypoglycaemia is not a disease per se but a manifestation of underlying perturbations of glucose homoeostasis. Among the biochemical tests commonly requested in newborns with confirmed, persistent/recurrent hypoglycaemia of unknown origin is the measurement of insulin and C-peptide in a blood sample(s) taken during the period of hypoglycaemia. Accepting an elevated insulin result with/without raised C-peptide as bona fide could trigger unnecessary investigations and/or diagnostic misapplications.PurposeTo highlight the significance of the "maternal-foetal-newborn" associations and th

Clinical and multi-omics characterisation of early neurodevelopmental disorders associated with critical congenital heart disease: the prospective cohort CATAMARAN neonatal study protocol.

INTRODUCTION: Critical congenital heart disease (CHD) is associated with neurodevelopmental disorders, recognised as the most common long-term morbidity in affected children. In critical CHD, that is, CHD requiring cardiac surgery within the first 3 months of life, 30%-50% of children have lower developmental scores. Therefore, early identification of at-risk infants is crucial, yet there is no scientifically evaluated care programme in France. This study aims to evaluate early neurodevelopmental status in infants with prenatally diagnosed critical CHD and to determine how intrinsic susceptibi

Hypertrophic Cardiomyopathy as a Key Feature of MRAS-Related Noonan Syndrome: New Case and Comprehensive Literature Review.

Noonan syndrome (NS) is a rare multisystemic condition among the RASopathy group, characterized by a broad phenotypic spectrum and genetic variability. It results from pathogenic variants in genes regulating the RAS/MAPK pathway, affecting cell proliferation and differentiation. While the PTPN11 gene accounts for approximately 50% of cases, other genes, including MRAS, have been implicated. NS presents with features such as facial dysmorphism, short stature, and cardiac anomalies. Hypertrophic cardiomyopathy (HCM) is a major contributor to mortality, with specific variants conferring higher ri

Patient perspectives on hemolytic disease of the fetus and newborn and fetal and neonatal alloimmune thrombocytopenia: common experiences and insights.

INTRODUCTION: Hemolytic disease of the fetus and newborn (HDFN) and fetal and neonatal alloimmune thrombocytopenia (FNAIT) are two rare alloimmune diseases of pregnancies where maternal alloantibodies target fetal blood components, potentially leading to risks of serious health conditions and developmental delays for babies, and sometimes loss of the baby. HDFN and FNAIT often have similarly complex journeys, from diagnosis through to burdensome treatments such as intrauterine transfusions and intravenous immunoglobulin. OUR STORIES: We, the authors, are two mothers with lived experience of t

Effect of intact cord resuscitation versus conventional cord clamping on clinical outcomes in very preterm neonates: a randomized controlled trial from Eastern India.

UNLABELLED: Early umbilical cord clamping during delivery-room resuscitation may interrupt placental transfusion during cardiopulmonary transition, increasing neonatal morbidity and mortality. This study aimed to evaluate whether intact cord resuscitation (ICR) improves delivery-room stabilization and clinical outcomes in very preterm neonates. In this single-centre, open-label randomized controlled trial at a tertiary-care hospital in Eastern India, neonates born at 26⁺⁰-31⁺⁶ weeks' gestation requiring delivery-room resuscitation were randomized (1:1) to ICR until physiological placental expu

Timing of birth to improve outcomes in chronic or gestational hypertension: the WILL RCT.

BACKGROUND: For women with chronic or gestational hypertension who remain well, early term birth (at 37-38 weeks' gestation) may reduce maternal complications, caesareans and stillbirths, but it may increase neonatal morbidity compared with expectant care. Expectant care may increase costs. There are no high-quality data to guide care, which currently involves maternal-fetal surveillance and intervention for maternal or fetal compromise, which may be rapid or unexpected. OBJECTIVE: To investigate optimal timing of birth for women with chronic or gestational hypertension who reach term and rem

Global metabolomic alterations associated with endocrine-disrupting chemicals among pregnant individuals and newborns.

BACKGROUND: Gestational exposure to non-persistent endocrine-disrupting chemicals (EDCs) may be associated with adverse pregnancy outcomes. While many EDCs affect the endocrine system, their effects on endocrine-related metabolic pathways remain unclear. This study aims to explore the global metabolome changes associated with EDC biomarkers at delivery. METHODS: This study included 75 pregnant individuals who delivered at the University of Cincinnati Hospital from 2014 to 2017. We measured maternal urinary biomarkers of paraben/phenol (12), phthalate (13), and phthalate replacements (4) from

The Utility of a Critical Antibody Titer in Anti-K Alloimmunized Pregnancies: A Systematic Review and Meta-Analysis of Diagnostic Test Accuracy.

Anti-Kell (anti-K) alloimmunization is a known cause of severe hemolytic disease of the fetus and newborn (HDFN), yet the utility of a critical maternal antibody titer in guiding clinical management remains debated. We conducted a systematic review and meta-analysis to evaluate the diagnostic accuracy of a maternal anti-K titer threshold of ≥8 for predicting the need for intrauterine intervention due to severe anti-K-mediated HDFN. In parallel, we characterized all reported cases of severe HDFN occurring in the setting of low maternal anti-K titers (<8). Studies were excluded if they lacked re

The Health of Pregnant Women and Their Unborn Children- Neglected in Vaccine Development.

BACKGROUND: Randomized Controlled Trials of vaccines given in pregnancy aimed at benefitting the unborn child began in 2015. Their use for licensing purposes now appears established. These trials generate data on possible benefits and harms to infants but also on maternal health impacts. The International Council on Regulations for Pharmaceutical Use in Humans has realized that current safety regulations are not adequate for clinical trials in the second half of pregnancy. They are now drawing up improved guidelines for the conduct of these trials. AIMS: To focus attention on maternal and fet

Fetal treatment and long-term neonatal outcomes in severe maternal red cell alloimmunization - a single-centre experience.

OBJECTIVES: Haemolytic disease of the fetus and newborn (HDFN) occurs due to maternal IgG alloantibodies that actively cross the placenta and bind to paternally derived fetal antigens on the erythrocytes. The aims of this study were to describe the Slovenian cohort of patients with severe HDFN, who required fetal treatment, to review the fetal treatment strategies, and to describe pregnancy and neurodevelopmental outcomes. CASE SERIES PRESENTATION: Data on patients who developed severe HDFN between 2006 and 2021 and were treated at our institution were collected retrospectively. Primary care

Oropouche Virus (OROV) and Breastfeeding Safety: Analysis of Related Orthobunyaviruses for Mother-Infant Vertical Transmission in Breast Milk.

The discovery that the Oropouche virus (OROV) can be transmitted vertically from an infected pregnant mother to the fetus, resulting in fetal and placental OROV infection, miscarriage, stillbirth, and congenital malformations including microcephaly, has emphasized its public health significance. Because of the importance of breastfeeding in those areas affected by the Oropouche fever outbreak, public health agencies have continued to encourage nursing among mothers who have had OROV infection or who reside or travel in endemic regions. However, the basis for this recommendation has not been st

Pregnancy and Neonatal Outcomes in Maturity-Onset Diabetes of the Young: A Systematic Review.

Maturity-onset diabetes of the young (MODY)-a monogenic form of diabetes-accounts for approximately 1-2% of all diabetes cases, with GCK-MODY being the second most commonly diagnosed type. Although the inherited nature of the disease implies that the interplay between maternal glycemia and fetal genotype directly influences neonatal outcomes, clinical guidelines for MODY-complicated pregnancies remain underdeveloped. A systematic literature search in the PubMed, Scopus, Web of Science, and Cochrane databases was conducted following the PRISMA guidelines. The study protocol has been logged in t

Restoring antenatal health services in a war-affected hospital: a quality improvement project.

BACKGROUND: Health service restoration involves reinstating all previous health service components to a facility after a period of disruption caused by war, conflict or other disasters. As defined by the WHO, antenatal care (ANC) is the care provided by skilled healthcare professionals to pregnant women and adolescent girls to ensure optimal health conditions for both mother and fetus during pregnancy. Before the restoration process, the hospital senior management team, maternal and newborn health (MNH) staff and Institute for Healthcare Improvement (IHI) improvement advisors had discussions o

Machine learning demonstrates normal fetal Doppler velocimetry associated with reduced risk of necrotizing enterocolitis among preterm infants with growth restriction.

BackgroundNecrotizing enterocolitis (NEC) is an intestinal ischemic disease that affects preterm infants with fetal growth restriction (FGR). The role of fetal Dopplers in stratifying risk for developing NEC is unclear but the innovative use of machine learning technology may aid in identifying their contribution.MethodsThis is a single center retrospective cohort of 164 infants born before 33 weeks' gestation with FGR from 2016 to 2019. We used machine learning to classify NEC and to evaluate the predictive values of gestational age (GA), birth weight (BW), and the presence of abnormal umbili
