Prenatal diagnosis of renal agenesis in a twin gestation.

Bilateral renal agenesis is a lethal congenital anomaly. It appears to be transmitted in a polygenic pattern. The prenatal ultrasound findings consist of severe oligohydramnios, absence of the fetal bladder, and failure to identify fetal kidneys. Twin gestations with renal agenesis have been described in the paediatric literature. We detail a case of a patient with two prior affected pregnancies with bilateral renal agenesis. Her latest pregnancy was diagnosed prenatally, with one fetus with bilateral and the other fetus with unilateral renal agenesis. The ultrasound findings should be differe

Renal failure in the neonate associated with in utero exposure to non-steroidal anti-inflammatory agents.

In utero exposure to non-steroidal anti-inflammatory agents (NSAIAs) can produce combinations of oligohydramnios, a bleeding diathesis, ileal perforation, premature closure of the ductus, and acute or chronic renal injury. NSAIAs induce renal dysgenesis in fetal monkeys and renal structural abnormalities in the developing human fetus. We report oligohydramnios and renal failure associated with in utero exposure to early, prolonged, high-dose indomethacin in four neonates, and to ibuprofen in one neonate. Four of the affected neonates were one of twins. In each set of twins, only one of the pai

Fetal obstructive uropathy.

Congenital obstructive uropathy is a relatively common developmental malformation, and severely affected fetuses die soon after birth from oligohydramnios-induced pulmonary hypoplasia or renal failure. Prenatal ultrasonography can reliably diagnose the specific anatomic defect, and using fetal urine sampling we can determine the extent of renal damage in utero with reasonable certainty. With these diagnostic tools and an understanding of the natural history of congenital obstructive uropathy we can now make rational decisions regarding treatment. Clinical experience has demonstrated that the s

Does reduction of amniotic fluid affect fetal movements?

The effect of the amount of amniotic fluid on the form of fetal general movements was studied longitudinally in 19 pregnancies complicated by premature rupture of the amniotic membranes (PROM). Before birth, general movements were studied weekly by means of 1-h ultrasound observations, performed under standardized conditions. In the early postnatal period, 11 of these infants were followed with video recordings of their spontaneous movements. In the fetus, speed and amplitude of general movements were directly related to the reduction in amniotic fluid. A moderate reduction of amniotic fluid w

Antenatal ultrasound for fetal anomalies: importance of perinatal autopsy.

Sixty-one instances of congenital anomalies identified prenatally by ultrasound were reviewed to determine whether autopsy provided important additional information. An important finding was defined as one which would affect: 1) genetic counseling; 2) diagnosis of a syndrome; 3) determination of etiology or pathogenetic mechanism of the anomaly; or 4) interpretation of severity of the anomalies. In 28 cases (46%), post-mortem examination provided such information. All of these infants had multiple anomalies; correlations with oligohydramnios and poor fetal outcome were noted. Autopsy provided

Single umbilical artery. A report of 159 cases.

We retrospectively reviewed the records of 56,919 infants born from 1966 through 1986 to determine the incidence, significance, and possible associations of single umbilical artery (SUA). One hundred fifty-nine infants were affected. The sex distribution was approximately equal. Fourteen infants (8.8%) were twins who were discordant for SUA. The smallest twin was the one affected in all but two cases. Of the 159 infants, 16 were stillborn and 19 died neonatally. The mean birth weight of all infants with SUA was 2.47 kg, and the mean gestational age was 35.9 weeks. Young primipara and older mul

Intrapartum and delivery room management of premature rupture of membranes complicated by oligohydramnios.

Intrapartum management of PROM is affected as much by quantity of amniotic fluid remaining in the uterine cavity as gestational age at which PROM occurs. Strategies for PROM must take into account the unique mechanical and developmental fetal risks, as well as greater infectious morbidities associated with severe oligohydramnios. Perinatal management should include close fetal monitoring, timely intervention, and the provision of skilled neonatal resuscitation once the infant is delivered. Etiology of cardiopulmonary distress in the neonate may initially be difficult to determine. Surfactant d

Third trimester ultrasonic presentation of infantile polycystic kidney disease.

Infantile polycystic kidney disease (Potter's Type 1) is an autosomal recessive disorder that affects the kidneys and liver. Use of ultrasound to make the diagnosis prenatally is well documented and, in fact, it is advocated as a screening device for second-trimester identification of potentially affected fetuses. The sonographic appearance is characterized by enlarged hyperechoic kidneys, enlarging fetal abdominal circumference, and oligohydramnios. It is suggested that a ratio of the kidney circumference to the abdominal circumference (KC/AC) be used as method of quantifying renal size and a

Oligohydramnios sequence (Potter's syndrome): case clustering in northeastern Tennessee.

The oligohydramnios sequence (OS) is manifest in newborns when prolonged oligohydramnios has been present during pregnancy. The most important signs are an infant small for gestational age, with wrinkled skin, Potter facies, compression deformities of the limbs, and respiratory distress caused by pulmonary hypoplasia. The recurrence rate of kidney conditions implicated in OS depends on the severity of the condition in the previously affected sib. While absent or encysted kidneys (Potter types II and IV) are incompatible with life, chronic leakage of amniotic fluid may result in a viable infant

Does an expanding fetal abdominal mass produce pulmonary hypoplasia?

Fetal pulmonary hypoplasia has been related to multiple factors. In an effort to define which fetuses may benefit from prenatal intervention to prevent or reverse pulmonary hypoplasia, we studied the relative contribution of an enlarging abdominal mass in the fetus. We produced abdominal masses in fetal rabbits at 24 days gestation by two methods. In one group, a small cylindrical chip of Takasen, (a synthetic polymer that expands to 50 times its size in 1 week; Grobeast, Pop Art Co, Cleveland, OH) was inserted into the peritoneal cavity of the fetal rabbit; in another group, the bladder neck

Outcomes in patients with unusually high maternal serum alpha-fetoprotein levels.

In a study group of 166 patients with unusually high maternal serum alpha-fetoprotein values greater than or equal to 5 multiples of the median, 110 (66%) patients had a condition affecting obstetric care compared with 14% in the 2.5 to 2.9 range, 26% in the 3.0 to 3.9 range, and 30% in the 4 to 4.9 range of multiples of median. Fetal anomalies composed a significantly greater proportion (p less than 0.0001) of positive findings in the study group than the group with maternal serum alpha-fetoprotein values of greater than or equal to 2.5 to 4.9 multiples of the median. The effect of prolonged oligohydramnios on fetal lung development, maturation and ventilatory patterns in the newborn guinea pig.

We drained the amniotic fluid surrounding guinea pig fetuses between days 45 and 65 of gestation (term is 67 days). The fetuses were delivered by Cesarean section and the impact of prolonged oligohydramnios on lung growth, maturation and postnatal ventilatory pattern was measured. Untouched littermate fetuses served as controls. Neither fetal body, liver nor brain weights were significantly affected by the experimental situation. When expressed in percent of control values, lung weight (63%), lung/body weight ratio (70%), lung volume (67%), total lung DNA content (63%) and lung DNA per gram of

Perinatal management of fetal hydronephrosis with normal bladder.

This report covers 30 cases of prenatal diagnosis of uni- or bilateral hydronephrosis not associated with an overdistended bladder. Oligohydramnios was observed only in four cases while polyhydramnios occurred in three affected fetuses. In no case was fetal urine aspirated or drained prenatally. Early delivery was performed in four cases with oligohydramnios. Sixteen newborns required surgical correction of the lesion as it was confirmed by urography, following at least two sonographic examinations which confirmed a moderate to severe hydronephrosis after birth. The most frequent lesion was py

Discordant amniotic band sequence in monozygotic twins.

Multiple congenital anomalies were identified at 16 weeks gestation in one fetus of an unsuspected twin pregnancy while ultrasound examination was performed before routine genetic amniocentesis. Further sonographic studies documented the amniotic band sequence (ABS) and transient oligohydramnios in the affected fetus. The latter finding supports the theory of amnion rupture followed by amniotic fluid leakage through an ineffective chorion barrier as the pathogenesis of compression related anomalies in this syndrome. Extensive craniofacial involvement including hydrocephalus, encephalocele, and

Prenatal diagnosis of Meckel syndrome: case reports and literature review.

We report on 2 couples at risk to have Meckel syndrome (MS) offspring. Three pregnancies were monitored by ultrasonography and alpha-fetoprotein (AFP) assay in midtrimester. Of the 2 involving an affected fetus, one was thought to be unaffected. A literature survey was undertaken. Sources of error seem to lie in the heterogeneity of clinical manifestations and the variable onset and rate of progression of clinical components of MS and limitations in ultrasonographic resolution inherent in the scanner or due to the associated oligohydramnios. The following observations/suggestions are made conc

The diagnosis of congenital renal anomalies with ultrasound. II. Infantile polycystic kidney disease.

Infantile polycystic kidney disease in an autosomal recessive disorder which in its severe form is characterized by bilateral renal enlargement and renal failure. The present study was undertaken to assess the diagnostic accuracy of antenatal sonography in a population at risk. Nineteen patients with fetuses at risk for infantile polycystic kidney disease were referred for ultrasound examination to the Perinatal Unit at Yale-New Haven Hospital. Ten infants had infantile polycystic kidney disease (53%). A positive antenatal sonographic diagnosis was made by the presence of oligohydramnios, an a

Experimental pulmonary hypoplasia due to oligohydramnios and its reversal by relieving thoracic compression.

To investigate the role of thoracic compression in the etiology of pulmonary hypoplasia associated with oligohydramnios, we shunted amniotic fluid into the maternal abdominal cavity at 25-days gestation in one group of fetal rabbits and occluded the bladder outlet in another. Bladder-neck obstruction produced severe bilateral hydronephrosis. Both procedures produced oligohydramnios (amniotic fluid volume reduced, p less than .001) at reexploration on day 28. At term (day 31) newborns undergoing these procedures had significantly decreased lung weights (p less than .01). Lung histology was not

Genetics, pathoanatomy and prenatal diagnosis of Potter I syndrome and other urogenital tract diseases.

We report on 12 early prenatal diagnoses of "Potter syndrome" and other severe fetal kidney diseases by ultrasonic observations of persistent oligohydramnios and absence of kidney function; seven pregnancies were terminated. These cases are discussed together with others observed in a total of 23 families. Special attention is given to the difficulty of classifying the various kidney diseases pathoanatomically, and also of determining the recurrence risk in the affected families. For genetic counselling, the possibility of early prenatal diagnosis is a practical solution to this problem. Routi

Fetal lung hypoplasia: biochemical and structural variations and their possible significance.

Quantitative biochemical criteria for lung growth and maturation were compared with the histological appearances in hypoplastic lungs from 20 fetuses and newborn infants. Cases associated with oligohydramnios showed a characteristic series of changes with narrow airways, retardation of epithelial and interstitial growth, delay in development of blood-air barriers, and low concentrations of phospholipid phosphorus, lecithin phosphorus, total palmitate, and lecithin palmitate. The growth and maturation arrest appeared to affect the peripheral part of the acinus. Examples of other types of lung h

A new dimension in the diagnosis of posterior urethral valves in children.

Infants with posterior urethral valves may seem to have diverse and unrelated symptoms when, in fact, the clinical findings are all related to the primary effect of the valves during various stages of early growth. In some, prenatal urinary obstruction leads to such severe oligohydramnios that the fetus is stillborn. Others, somewhat less affected, are born alive but have severe respiratory distress from hypoplastic (stiff) lungs and die of respiratory problems. Still others can be associated with massive ascites and urinomas, and be stillborn or die soon after birth. In less severe cases the
