Neonatal myotonic dystrophy associated with prenatal ventriculomegaly. A case report.

Myotonic dystrophy is an autosomal dominant neuromuscular disease in which the maternal manifestations may be mild or subclinical. However, the congenital form, transmitted only from affected mothers, is reported to have a neonatal mortality rate as high as 50%. Ultrasonographic features of pregnancies with affected fetuses include hydramnios and decreased fetal movement, breathing motion and swallowing. Several reports have also noted an occasional infant with ventriculomegaly or hydrocephalus. It appears that prenatal ventriculomegaly may also be an occasional ultrasonographic finding herald

Severe neonatal centronuclear (myotubular) myopathy: an X-linked recessive disorder.

Prenatal onset and rapidly fatal course of centronuclear myopathy are described in four male newborns including two brothers. Diagnosis was established by muscle biopsy within the first week of life in two and at autopsy in the two other patients: Central nuclei, central aggregation of oxydative enzyme activity in the majority of muscle fibers and type 1 fibre hypotrophy were demonstrated. Prenatal manifestation included polyhydramnios, reduced fetal movements and breech presentation. All four newborns developed respiratory insufficiency requiring artificial ventilation immediately after birth

The antenatal ultrasonographic detection of the Holt-Oram syndrome.

The Holt-Oram syndrome is an autosomal dominant disease with 100% penetrance. No correlation exists between the maternal clinical expression and that of the affected offspring. The syndrome includes a wide range of cardiac and skeletal malformations. Real-time ultrasound, with a detailed study of the fetal heart and skeletal system, can play a crucial role in the counselling of affected pregnant women. This study describes the ultrasonographic findings of 2 affected fetuses at risk (at 34 and 14 weeks' gestational age). Ultrasonography detected and correctly estimated the severity of the cardi

Homozygous alpha-thalassaemia: clinical presentation, diagnosis and management. A review of 46 cases.

Forty-six hydropic infants with homozygous alpha-thalassaemia born during a period of 10 years have been reviewed. The incidence was 1:1550 total births, and accounted for 81% of all non-immune hydrops. The male to female ratio was 1:1.4. There was increased incidence of anaemia, pregnancy induced hypertension, antepartum haemorrhage, malpresentation, prematurity, fetal distress, difficult vaginal delivery, caesarean section, retained placenta, postpartum haemorrhage and congenital abnormalities. Antenatal diagnosis by DNA hybridization with subsequent abortion of the affected fetuses is the b

Myotonic dystrophy in pregnancy. Prenatal, neonatal and maternal considerations.

Five pregnancies occurred in three women with myotonic dystrophy. Four of the five infants had generalized hypotonia, respiratory failure, swallowing difficulties and arthrogryposis; two of the affected infants died despite intensive treatment. The surviving affected children had a gradual improvement in respiratory and swallowing symptoms but have remained hypotonic and developmentally delayed. The fifth infant was entirely normal. Polyhydramnios was the most constant feature of the affected pregnancies. Three of the four affected fetuses weighed less than 2,500 gm. Other obstetric complicati

Recurrent idiopathic fetal hydrops: results of prenatal therapy.

A case of nonimmune hydrops recurring within a sibship is reported. Progressive severe fetal ascites and hydramnios at 31 weeks' gestation led to fetal paracentesis for both therapeutic and diagnostic purposes. A successfully placed indwelling peritoneal-amniotic diversion shunt functioned well but became dislodged, and rapid recurrence of ascites and hydramnios required two subsequent drainage procedures. Neonatal death occurred from pulmonary failure after delivery at 34 weeks' gestation, as had happened in the similarly affected sibling. Amniotic and fetal peritoneal pressures, biochemical

What affects fetal pulmonary maturation during diabetic pregnancy?

Previous studies differ as to whether the fetus of the woman with diabetes mellitus has altered formation of lung surfactant. The factors responsible for these differences remain unclear. In this study, measures of blood glucose control, including birth weight percentile and the presence of factors potentially influencing fetal pulmonary maturation, such as diabetic class, maternal chronic hypertension, and preeclampsia, were compared with the amniotic fluid lecithin/sphingomyelin ratio, phosphatidylglycerol, and phosphatidylinositol within a group of 90 diabetic pregnancies. The factors were

Prenatal ultrasonic findings in congenital chloride diarrhoea.

In 3 fetuses affected by congenital chloride diarrhoea there were characteristic prenatal ultrasonic findings. In all cases the fetal abdominal cavity was filled with distended loops of fetal intestine, the fetal stomach was of normal size and polyhydramnios was present. The prenatal ultrasonic findings in this metabolic inherited disease are identical to those caused by mechanical obstruction of the distal fetal intestine. Whenever these findings are encountered prenatally, the possibility of chloride diarrhoea should be considered, because therapy with electrolytes is needed rather than the

Intra-uterine growth and fatal fetal abnormality.

To study intra-uterine growth in pregnancies complicated by fatal abnormality of the fetus, 73 pregnancies involving fetal or neonatal death caused by malformation, chromosomal aberration or Mendelian disease were evaluated. Small-for-gestational age newborns were found in 45% of the patients, and this finding was typical of 18-trisomies, amnion adhesion syndrome fetuses and multimalformed fetuses. Pregnancies affected by a Mendelian disease or a single fetal malformation mostly presented normal intra-uterine growth. In 25% of the patients, the symphyseal-fundal growth was retarded; this retar

Neonatal myotonic dystrophy as a cause of hydramnios and neonatal death. A case report and literature review.

A woman had three consecutive pregnancies complicated by hydramnios and early neonatal death. The identical prenatal and neonatal course, family history and, ultimately, maternal diagnosis supported the diagnosis of recurrent severe neonatal myotonic dystrophy. Ultrasonographic observation of the last fetus affected suggested and predicted the neonatal features of the disease.

Idiopathic hydrops fetalis report of 4 patients including 2 affected sibs.

We report four patients with idiopathic hydrops fetalis (IHF), two being affected sibs; the latter represent the first reported familial occurrence. A review identified 45 additional cases that seem to represent 1/3 to 2/3 of all cases of hydrops fetalis of nonimmunologic origin (NIHF). Our patients and the other adequately documented cases permit delineation of "idiopathic" fetal hydrops; ie, that form of the condition which is not associated with any detectable fetal or maternal disorders. These fetuses are usually premature, often the product of a gestation complicated by pre-eclampsia, occ

Fetal lung hypoplasia: biochemical and structural variations and their possible significance.

Quantitative biochemical criteria for lung growth and maturation were compared with the histological appearances in hypoplastic lungs from 20 fetuses and newborn infants. Cases associated with oligohydramnios showed a characteristic series of changes with narrow airways, retardation of epithelial and interstitial growth, delay in development of blood-air barriers, and low concentrations of phospholipid phosphorus, lecithin phosphorus, total palmitate, and lecithin palmitate. The growth and maturation arrest appeared to affect the peripheral part of the acinus. Examples of other types of lung h

Diabetes and thyroid disease during pregnancy.

In a prospective study of 100 consecutive diabetic pregnancies, 1 was complicated by hyperthyroidism and 4 by hypothyroidism. The association of hypothyroidism with diabetes mellitus merits special attention as this combination of diseases affected 4 of 20 (20%) White's class D and F diabetics. Hydramnios and/or spontaneous premature labor were features of diabetic pregnancies complicated by compensated hypothyroidism. Delayed diagnosis of hypothyroidism may have contributed to the fatal congenital malformations in one of these fetuses, but the remaining infants survived, including 1 infant su

Myotonia dystrophica: obstetric complications.

We describe the course and outcome of 23 pregnancies in six women affected by myotonia dystrophica in a large Labrador family. A seventh patient had 14 pregnancies, so that the infertility commonly described in this disorder did not apply to this family. The rate of complications was high, particularly in respect of polyhydramnios, premature onset of labor, cesarean section, postpartum hemorrhage, and neonatal deaths. Polyhydramnios indicated that the fetus was abnormal. Direct observation of an atonic uterus at cesarean section supported other evidence that uterine muscle may be affected. We
