Persistent SARS-CoV-2 Positive Tests in Neonates: Clinical Outcomes, Transmission Pathways, and Immune Vulnerability-Case Series.

Background: In 2020, the World Health Organization declared a Public Health Emergency of International Concern due to the global outbreak of SARS-CoV-2. Recognized as a severe and highly contagious disease, it affected both the adult and pediatric population. However, due to the early timing of the pandemic, limited research was conducted in the perinatal field, leaving many questions regarding the true impact of maternal transmission to fetuses and its consequences during the neonatal period. Methods: In this case series, we reviewed data from ten newborns delivered in the Instituto Nacional

Understanding Patient Perspectives and Experiences in Severe Hemolytic Disease of the Fetus and Newborn: Insights from Qualitative Interviews.

Objective Maternal red cell alloimmunization is a rare pregnancy condition that can cause hemolytic disease of the fetus and newborn (HDFN), which can be serious and fatal. As subsequent alloimmunized pregnancies carry HDFN risk, families' future pregnancy decisions may be affected. Findings from qualitative interviews in alloimmunized mothers are presented to understand experiences and perspectives during their previous pregnancy affected by severe HDFN, aiming to inform patient-centered interventions. Study Design This article presents qualitative findings of a broader mixed-methods study pu

Peculiarity of preterm neonates at 33-35 weeks gestational age with mild-to-severe hypoxic-ischaemic encephalopathy: a single-centre experience.

UNLABELLED: To describe the peculiarity of preterm neonates at 33-35 weeks gestational age (GA) with mild-to-severe hypoxic-ischaemic encephalopathy (HIE). This single-centre retrospective study compared the background characteristics, treatments, and outcomes of 30 preterm HIE neonates at 33-35 weeks GA with those of 399 HIE neonates at ≥ 36 weeks GA between 2011 and 2023. Between the 33-35 weeks and 36, 37-40, or ≥ 41 weeks GA groups, HIE severity and use of therapeutic hypothermia (TH) were not significantly different. Between the 33-35 weeks and 37-40 weeks GA groups, no TH, except for mil

Multifetal Pregnancies in Women with CKD: Risk Assessment and Indications for Counseling.

KEY POINTS: CKD carries an increased risk for developing hypertensive disorders of pregnancy in multifetal pregnancies. CKD in multifetal pregnancies is associated with a shorter duration of gestation and an increased risk of having a baby small for gestational age. Multifetal pregnancies are a risk factor for hypertensive disorders of pregnancy compared with singleton gestations in patients living with CKD. BACKGROUND: CKD is a risk factor for adverse pregnancy outcomes in singleton pregnancies. Little is known about multifetal pregnancies in women with CKD. Owing to higher maternal age and

Timing of birth to improve outcomes in chronic or gestational hypertension: the WILL RCT.

BACKGROUND: For women with chronic or gestational hypertension who remain well, early term birth (at 37-38 weeks' gestation) may reduce maternal complications, caesareans and stillbirths, but it may increase neonatal morbidity compared with expectant care. Expectant care may increase costs. There are no high-quality data to guide care, which currently involves maternal-fetal surveillance and intervention for maternal or fetal compromise, which may be rapid or unexpected. OBJECTIVE: To investigate optimal timing of birth for women with chronic or gestational hypertension who reach term and rem

Management of anti-M antibody during pregnancy: a case report.

BACKGROUND: Anti-M antibodies are relatively common and naturally occurring. When anti-M antibodies cross the placenta, they may cause hemolytic disease of the fetus and newborn (HDFN). Anti-M antibodies account for less than 15 cases of HDFN reported in the published English literature. HDFN can lead to foetal anaemia, hydrops fetalis, hypoxia, heart failure, and even death. OBJECTIVE: To review the general guidelines and propose a less intensive management approach of anti-M antibody during pregnancy through the context of a case report. METHODS: We report a 25-year-old healthy pregnant G3

Risk factors of the appearance of anencephaly in Tunisia.

INTRODUCTION: Anencephaly is a serious developmental defect of the central nervous system in which the brain and cranial vault are grossly malformed. The cerebrum and cerebellum are reduced or absent, but the hindbrain is present. Anencephaly is a part of the neural tube defect spectrum. This defect results when the neural tube fails to close during the third to fourth weeks of development, leading to fetal loss, stillbirth, or neonatal death. AIM: To find out probable principal risk factors for the appearance of anencephaly. METHODS: This study was conducted to compare between pregnancies a

Characterization of severity of hemolytic disease of the fetus and newborn due to Rhesus antigen alloimmunization.

BACKGROUND: Clinical manifestations of hemolytic disease of the fetus and newborn include anemia, hyperbilirubinemia, hydrops fetalis, kernicterus, and fetal or neonatal demise. More than 50 antibodies are linked to hemolytic disease of the fetus and newborn, with Rhesus (including D and c) and Kell antigens carrying the highest risk of disease. To date, a multicenter, comprehensive evaluation of hemolytic disease of the fetus and newborn due to Rhesus antigen alloantibodies in the United States has not been undertaken. OBJECTIVE: This study aimed to implement a novel severity index to charac

What is the role of intrauterine transfusion after single intrauterine death in monochorionic twin pregnancies? Evidence from a systematic review and meta-analysis.

OBJECTIVE: To investigate the role of intrauterine transfusion (IUT) in affecting the outcome of the surviving twin showing sign of fetal anemia after a single intrauterine fetal death (IUFD) in monochorionic (MC) twin pregnancies. METHODS: PubMed, Medline and Embase databases were searched (2010-2024). The inclusion criteria were studies reporting the outcome of fetuses showing signs of fetal anemia, defined as the presence of the peak systolic velocity (PSV) of the middle cerebral artery (MCA) >1.5 MoM, after single IUFD receiving compared to those not receiving IUT. The outcomes observed w

Case Report: Preserved umbilical cords underscore family histories of inborn errors of immunity.

A history of susceptibility to infections and a family history of death because of unexplained infections during infancy are helpful in diagnosing inborn errors of immunity (IEIs). However, infections can occur because of various reasons, and determining whether the underlying disease is undoubtedly an IEI is implausible at present. In Japan, preservation of the umbilical cord at birth is customary. Two patients were suspected of having X-linked agammaglobulinemia (XLA); the patients were ultimately diagnosed with XLA based on the history of susceptibility to infections and family histories of

Fetomaternal Outcome in Obstetric Cholestasis in a Tertiary Care Hospital.

INTRODUCTION: Intrahepatic cholestasis is a potentially serious condition that can develop during the late half of pregnancy. It can increase the risk of preterm birth and stillbirth in fetus and postpartum hemorrhage in mothers. The main symptoms are severe pruritus. Since this condition is hormone-induced, it is a reversible condition and likely to recur in subsequent pregnancy. Early diagnosis, adequate treatment, and well-timed delivery are imperative for better maternal and fetal outcomes. MATERIALS AND METHODS: This is a prospective observational study done in a tertiary care center, RI

Customized growth charts for twin vs singleton pregnancies and their ability to identify small for gestational age-associated risk of adverse perinatal outcome.

BACKGROUND: Twin fetuses are born earlier and have slower growth than singletons. It is uncertain as to what degree this is pathological or a physiological adaptation. OBJECTIVE: We set out to develop a customized chart for twin pregnancy and compare it with the corresponding singleton chart in its ability to assess risk of adverse perinatal outcomes. STUDY DESIGN: The cohort consisted of 8457 twin pregnancies (16,914 fetuses) without information on chorionicity, recorded during routine care in 127 UK hospitals. We performed a mixed-effects linear regression analysis to calculate customized

Structural Posterior Fossa Malformations: MR Imaging and Neurodevelopmental Outcome.

Objectives: The increasing use of fetal MRI has increased the diagnosis of posterior fossa malformations, yet the long-term neurodevelopmental outcomes of affected fetuses remain unclear. This study aims to examine the long-term neurodevelopmental outcomes of fetuses with structural posterior fossa malformation diagnosed on fetal MRI. Methods: A historical cohort study was conducted at a single tertiary referral center, including fetuses diagnosed with structural posterior fossa malformations and apparently healthy fetuses who underwent fetal brain MRI between 2011 and 2019. Aflatoxin exposure during pregnancy or infancy and its effect on infant health: a narrative review.

Pregnant women are exposed to various contaminants through foods, with environmental toxicants and aflatoxin (AF) being among the major food contaminants. Therefore, this review was conducted for a better perspective on the AF exposure during pregnancy or infancy, highlighting how exposure through the mother (via placenta and breast milk) and directly through infant foods ultimately affects infant health. The literature suggests that AF exposure during pregnancy may lead to maternal anaemia, premature delivery, pregnancy loss or decreased number of live births. AF crosses through the placenta

[Perinatal consequences of eating disorders during pregnancy].

Eating disorders are a group of mental illnesses that significantly affect an individual’s emotional, psychological and physical well-being; they typically occur in women and are the most common during the reproductive years. The prevalence of eating disorders during pregnancy is 7.5–11.5%. Food aversion occurs in 70–85% of pregnant women, nausea in 100%, and increased salivation (ptyalism) in 40%. The treatment of eating disorders during pregnancy is a significant challenge, as it can lead to dangerous conditions and even maternal or fetal death. Today, there is no obstetrician who has not en
