FAM20B Related Skeletal Dysplasia: Expanding the Prenatal Phenotype.

FAM20B encodes glycosaminoglycan xylosylkinase, a key enzyme in proteoglycan biosynthesis. Biallelic pathogenic variants have only recently been linked to skeletal dysplasia. We report two pregnancies from one couple, resulting in three fetuses (twin sibs and younger sib) with severe skeletal anomalies. Prenatal findings included enlarged nuchal translucency, short bowed and dislocated limbs, intrauterine growth restriction, and multiple malformations. Intra-uterine fetal death occurred in Probands 1 and 2, and neonatal death of Proband 3. Postnatally, all three probands showed limb shortening

Clinical course and perinatal management of fetuses and newborns affected by trisomy 13 and 18: a retrospective single-centre cohort study.

Objectives Trisomy 13 (T13) and 18 (T18) are lethal chromosomal abnormalities. While the prognosis is generally poor, recent evidence suggests that some infants with T13 and T18 are able to survive longer than previously supposed. These findings underline the importance of an individualised approach to perinatal/neonatal care. Methods This retrospective study included 193 pregnancies with T13 and T18 in a single tertiary center between 2005 and 2023. Fetal and maternal characteristics, overall survival, and perinatal and postnatal management were analysed. Results Of 193 affected fetuses, 142

The Ethics of Clinical Research on Diseases of the Fetus and Newborn: Balancing Benefit-Risk, Autonomy, and Maternal-Fetal Interests.

OBJECTIVE: Pregnant persons historically have been excluded from clinical trials. Recently, there has been a shift from exclusion toward inclusion of pregnant persons in research while acknowledging the complexity of reproductive ethics and the intertwined interests of the pregnant person and fetus. Our objective was to use a principle-based approach to review the ethics of clinical research concerning pregnancy-related disorders that predominantly affect fetal well-being. STUDY DESIGN: Ethical principles are applied to the design of interventional trials in two rare conditions of pregnancy,

Monitoring and timing of delivery in suspected late fetal growth restriction at term.

BACKGROUND: Infants affected by late fetal growth restriction are at risk of adverse perinatal outcomes, either due to placental insufficiency or the consequences of late iatrogenic prematurity. There is no randomized trial reporting the optimal timing of delivery, and recommending delivery at 37 weeks can cause harm. Antenatal ultrasound parameters can be used to risk-stratify these pregnancies into low-risk cases, which are associated with a lower likelihood of adverse outcomes and are thus suitable for full-term delivery, and high-risk cases, which may benefit from early-term delivery. Maternal Obesity and Neonatal Death in Preterm US Pacific Islander Neonates Using 2 Analytic Approaches.

IMPORTANCE: Owing to a high prevalence of obesity, Pacific Islander individuals in the US are at higher risk for preterm birth (PTB), but outcomes after PTB remain understudied. Existing literature suggests that associations between prepregnancy obesity and neonatal mortality stratified by gestational age (GA) are modest or null, which may be a result of overlooked stratification bias. OBJECTIVE: To estimate the association between prepregnancy body mass index (BMI) and neonatal death (NND) following PTB by different degrees of prematurity in US Pacific Islander neonates using birth-based and

The Perinatal Committee report: Review of the progress of obstetric healthcare in Japan.

BACKGROUND: Japan's maternal mortality rate and perinatal mortality rate have shown one of the world's most significant declines, positioning Japan at the global forefront of the lowest levels. The nearly logarithmic annual decline in maternal and perinatal mortality rates suggests that, in addition to advances in medicine and healthcare, various types of care for mothers and newborns have played a crucial role in this achievement. METHOD: From the period of World War II to the postwar era, up to around 1980, and then every decade thereafter, the events and movements surrounding perinatal hea

Maternal Endocrine and Metabolic Health and Outcomes (MEMO): a prospective cohort study of gestational diabetes mellitus and subclinical hypothyroidism in China.

BACKGROUND: In recent years, more and more evidence has confirmed the “Developmental Origins of Health and Disease (DOHaD)” hypothesis, which states that adverse environments in early life can increase the risk of long-term metabolic diseases such as obesity and diabetes. Gestational diabetes mellitus (GDM) and subclinical hypothyroidism (SCH) during pregnancy are the most common endocrine diseases, which directly interfere with the intrauterine development environment of the fetus and thus are ideal research entry points for validating and extending the DOHaD theory. OBJECTIVE: This study aim

Recurrent pregnancy loss and fetal complete heart block as the initial manifestations of maternal systemic lupus erythematosus: a case report on the diagnostic and preventive role of anti-SSA (Ro) antibodies.

BACKGROUND: Systemic Lupus Erythematosus (SLE) is a chronic autoimmune disease that causes multi-organ damage and primarily affects women of reproductive age. Although pregnancy in these patients carries increased risks, advances in management have significantly improved outcomes for both the mother and the fetus. CASE PRESENTATION: A 32-year-old woman with a history of two stillbirths and an infant death due to a complete heart block (CHB) was referred at 17 weeks of gestation for fetal echocardiography, which showed no abnormalities. Further clinical evaluation revealed systemic features in

A Clinical Practice Guideline for the Management of Pregnancy Alloimmunized to Red Blood Cell Antigens.

IMPORTANCE: Red blood cell alloimmunization is typically associated with the transplacental transfer of incompatible fetal red blood cells into maternal circulation. Subsequent pregnancies can be affected by fetal anemia, hydrops fetalis, and perinatal death. Most cases of Rhesus D (RhD) alloimmunization due to pregnancy can be prevented by the proper administration of Rhesus immune globulin. However, an emerging practice of using low-titer, O, RhD-positive whole blood (LTOWB) in cases of life-threatening hemorrhage has the potential to increase the exposure of the female population to a new s

Estimating the impact of maternal anemia on low-birth-weight in Sub-Saharan African countries: propensity score matching analysis.

BACKGROUND: Low birth weight is the main cause of death, illness, and disability in infancy and childhood and has a lasting effect on adult healing here are several studies that indicate the correlation between low birth weight and anemia using an observational study based on the Demographic and Health Survey (DHS) but there are limited studies to show the direct effect of anemia on low birth weight. Therefore, we used the propensity score-matching (PSM) analysis to estimate the actual impact of maternal anemia on low birth weight babies. The study aims to reveal the effect of maternal anemia

Design of a Phase 3, Multicenter, Randomized, Placebo-Controlled, Double-Blind Study of Nipocalimab in Pregnancies at Risk for Fetal and Neonatal Alloimmune Thrombocytopenia.

Nipocalimab, a neonatal Fc receptor blocker, inhibits transplacental transfer of maternal immunoglobulin G (IgG) and lowers circulating maternal IgG levels. In a phase 2 study, nipocalimab demonstrated evidence of safety and efficacy in delaying or preventing fetal anemia in early-onset severe hemolytic disease of the fetus and newborn, suggesting a potential benefit in other IgG alloantibody-mediated perinatal diseases, including fetal and neonatal alloimmune thrombocytopenia (FNAIT). The phase 3 FREESIA-1 study aims to evaluate the safety and efficacy of nipocalimab in at-risk FNAIT pregnanc

Autoimmune congenital heart block: a case report and review of the literature related to pathogenesis and pregnancy management.

Autoimmune congenital heart block (ACHB) is a passively acquired immune-mediated disease characterized by the presence of maternal antibodies against components of the Ro/SSA and La/SSB ribonucleoprotein complex that mainly affects the cardiac conducting system. ACHB occurs in 2% of women with positive anti-Ro/SSA and anti-La/SSB antibodies and causes a high risk of intrauterine fetal death, neonatal mortality, and long-term sequelae. In this review, we first describe a case of ACHB to provide preliminary knowledge. Then, we discuss the possible pathogenic mechanisms of ACHB; summarize the pre

Successful management of severe Kell alloimmunization in pregnancy with intravenous immune globulin.

Hemolytic Disease of the Fetus and Newborn (HDFN) is a condition that affects 1 to 2 out of 1000 patients during pregnancy (1). When an alloantibody is present, it is essential to identify its nature in order to organize appropriate follow-up. Kell-mediated HDFN is rare; it occurs in about 5% of Kell alloimmunized pregnant women. It is important to note that in case of anti-Kell immunization, the severity of HDFN is not correlated with maternal antibody titers, and anemia tends to occur earlier and more severely. Therefore, early diagnosing and management of this condition is crucial. In the m

The role of measuring peak systolic velocity of the middle cerebral artery blood flow and anti-K1 titre during pregnancy to detect foetuses with severe anaemia, foetal hydrops, and the requirement of intrauterine transfusion: A systematic review and meta-analysis.

The clinical manifestation of foetal anaemia caused by maternal Kell alloantibodies differs from that caused by non-Kell alloantibodies. Severe anaemia develops in the foetus in the early weeks of gestation; therefore, proper management and early intervention are important. A systematic review and meta-analysis was performed to determine whether the anti-K1 titre can determine the sequelae of Kell alloimmunised pregnancies. Prospective and retrospective cohort studies were used to conduct a systematic review following a comprehensive literature search, in accordance with the Preferred Reportin

Postnatal cord care practices and associated factors in Arba Minch town, southern Ethiopia.

BACKGROUND: Cord care is one component of essential newborn care that reduces newborn morbidity and mortality. Poor cord care practice leads to a large amount of neonatal death due to infection. In Ethiopia, many women give birth at home, where neonates are exposed to unclean cord care practices or application of different traditional substances. Despite the severity of the problem, studies related to postnatal cord care practice are scarce. Hence, this study is aimed to assess umbilical cord care practices and associated factors in the postnatal period in Arba Minch town. OBJECTIVE: To asses
