Caffeine consumption and the risk of reproductive hazards.

Recent reports on the relationship between caffeine consumption by pregnant women and their infants' risk of miscarriage, low birth weight, preterm delivery and congenital malformations were reviewed. The evidence continues to support the view that moderate consumption of caffeine by pregnant women does not adversely affect their fetuses.

Cocaine use in pregnancy: perinatal morbidity and mortality.

With the increasing use of cocaine in the United States, there has been growing concern regarding its effects on the fetuses and neonates of pregnant cocaine abusers. Fifty-two cocaine-using women enrolled in a comprehensive perinatal addiction program were evaluated and compared with 73 women who had used narcotics in the past and were maintained on methadone during pregnancy. The groups were similar in maternal age, socioeconomic status, number of pregnancies and cigarette, marijuana and alcohol use. The cocaine-using women had a significantly higher rate of premature labor, precipitous labo

Effects of extradural anaesthesia on human fetal blood flow in utero. Comparison of three local anaesthetic solutions.

Twenty-seven women, scheduled for elective Caesarean section under extradural anaesthesia were allocated randomly to one of three groups: group Ea received 1.5% etidocaine with adrenaline, group Bp 0.5% bupivacaine plain, and group Ba 0.5% bupivacaine with adrenaline. There was no difference in the quality and distribution of sensory blockade between the three groups. Motor blockade was most profound in group Ea. Maternal heart rate and arterial pressure were only slightly affected in the three groups. Before induction of extradural anaesthesia, and 15 and 30 min after, fetal umbilical and aor

Caffeine-cigarette interaction on fetal growth.

The influence of caffeine and cigarette consumption on fetal growth during pregnancy was studied retrospectively in 913 newborn infants. Analysis of variance was adjusted for length of gestation and sex. A significant caffeine-cigarette interaction was found on birth weight (F = 85.4, p less than 0.01). Among women smoking 15 cigarettes or more per day, birth weight was 206 gm (+/- 57.7 gm) lighter for babies whose mothers consumed 300 mg or more of caffeine per day. Head circumference and length of the newborns were affected only by cigarette smoking. Another caffeine-cigarette interaction wa

Maternal and fetal characteristics affecting alcohol's teratogenicity.

Thousands of children are born each year with alcohol-related birth defects. However, despite comparable levels of prenatal alcohol exposure, some children are affected less severely than others, whereas others do not appear to be affected at all. Epidemiological investigations and experimental studies in animals suggest that maternal or fetal genetic factors, maternal age or parity, and pattern of drinking are important risk factors affecting alcohol's actions on the embryo/fetus. Identifying these and other risk factors may provide a means for cost-effective prevention efforts.

Selenium in pregnancy: effect of maternal drinking.

To study the effect of drinking alcohol during pregnancy on serum selenium, selenium concentrations were measured in 41 women (ten abstinent, 31 problem drinkers) throughout pregnancy as well as in 24 nonpregnant abstinent women. In abstinent pregnant women, serum selenium decreased with advancing gestational age, a pattern not affected by moderate maternal drinking (42 to 126 g of ethanol weekly, N = 14). Seventeen women drank heavily (more than 140 g of ethanol weekly), and nine of them (53%) gave birth to infants with fetal alcohol syndrome. The women with infants having fetal alcohol syndr

Morphometric-interferometric investigations of neurons in the medial thalamic nucleus of newborn rats following maternal treatment with ethanol.

The nucleus and cell size, as well as the dry weight and protein contents of neurons in the medial thalamic nucleus of newborn rats, exposed to ethanol in utero, were examined on the first day of postnatal life. Comparison was made with a control group of animals, not exposed to ethanol. No significant changes in the protein content and dry weight values were observed. The perikarya of neurons from animals exposed to ethanol were significantly larger than those of the controls. The findings indicate that the nucleus and perikaryon are affected differently by alcohol, following intrauterine exp

Intrauterine growth and postnatal fasting metabolism in infants of obese mothers.

We compared neonatal fasting metabolism in 12 infants of obese women and 10 control infants born to lean mothers. Weight gain during pregnancy was less among obese women (7.2 vs 13.6 kg). Term infants born to obese women were heavier (3.9 bs 3.2 kg), had greater triceps and midscapular skinfold thicknesses, and had heavier placentae (687 vs 572 gm). Length and head circumference were not affected. After birth, fasting blood glucose concentrations declined in obese infants to hypoglycemic levels at 60 minutes of the study (26.3 vs 63.1 mg/dl). Insulin levels were not statistically different bet

The effect of maternal diabetes on the synthesis and secretion of phosphatidylcholine in fetal and maternal rat lungs in vitro.

Incorporation of (methyl-14C)choline into phosphatidylcholine and the release of prelabelled phosphatidylcholine was investigated in vitro using lung slices from pregnant rats and their offspring near term. Tissue from normal, diabetic and insulin-treated diabetic pregnant rats and their offspring was utilized to assess the effects of maternal diabetes on fetal lung maturation. The results show that the synthesis of phosphatidylcholine in fetal/newborn lungs through the cytidine 5'-diphosphocholine pathway was not affected by maternal diabetes. However, secretion of phosphatidylcholine from th

Drug use during pregnancy and breast-feeding. A classification system for drug information.

Since 1978 the Swedish catalogue of registered pharmaceutical specialties (FASS) has carried a special section entitled "Pregnancy and breast-feeding" in each product presentation, intended to form an aid for the prescription of drugs to women during childbearing and lactation. After a brief review of transplacental transport and milk secretion, reproduction-toxicology studies in animals, and methods for clinical evaluation of drugs for use during pregnancy, the classification system is presented. On the basis of available data with regard to effects on early and late stages of pregnancy and l

Maternal and umbilical cord plasma noradrenaline concentrations during labour with and without segmental extradural analgesia, and during caesarean section.

Serial measurements of maternal and umbilical cord plasma noradrenaline concentrations were obtained in 10 patients undergoing normal vaginal delivery with segmental extradural analgesia, in 10 patients undergoing normal vaginal delivery without extradural analgesia, and in 12 patients undergoing elective Caesarean section under general anaesthesia. Maternal noradrenaline concentrations increased significantly during delivery in all three groups, the peak concentrations occurring at delivery. However, the increase in the maternal noradrenaline concentration during delivery in the extradural gr

Neonatal metabolic effects of oral ritodrine hydrochloride administration.

Neonatal hypoglycemia and hyperinsulinemia have been reported following maternal ritodrine administration, but no prospective controlled study of the neonatal metabolic and cardiovascular effects of maternal ritodrine is available. We conducted a double-blind prospective study in 35 patients with preterm labor and/or ruptured membranes. Patients in premature labor received ritodrine (max dose, 350 mcg/min) or a placebo intravenously for 12 hours, and then orally (20 mg every 4 hours) until labor ensued. Patients with ruptured membranes received only oral therapy. Only patients who were maintai

Effect of propranolol treatment in pregnant rats on motor activity and avoidance learning of the offspring.

Rats born to mothers treated with propranolol, during days 8-22 of gestation, displayed hyperactivity in the open field which lasted up to 60 days of age and an impairment of avoidance in the shuttle box which was more marked in the male rats. Females exhibited hyperactivity in the open field but developed impaired avoidance learning only when exposed prenatally to both propranolol and hypoxia. Propranolol administration during the last term of pregnancy (days 18-22) affected mostly shuttle box performance. In contrast, hyperactivity could be induced by treatment during various stages of pregn

Intrauterine malnutrition in the rat: alterations of fetal glycerol metabolism.

This investigation intended to clarify the effects of malnutrition in utero on enzymes of glycerol metabolism and the stores of phosphorylated glycerol intermediates in liver, striated muscle, and brain in the rat. Pregnant Wistar rats were restricted to an intake of 50% (M) of ad libitum fed controls (C) from the seventh day of gestation onward. Fetuses were removed 2 days (-2), or 1 day (-1), before term, or at the day of birth (DOB) The M fetuses and newborn rats were stunted. Their hepatic alpha-glycerophosphate oxidase (GPO) levels were lower than those of C in utero (mean +/- SEM: M = 23

Effects of maternal alcohol intake and smoking on neonatal electroencephalogram and anthropometric measurements.

Anthropometric data and computerized electroencaphalogram analysis during quiet, indeterminate, and active sleep were obtained from infants of mothers of four groups: (1) heavy drinking mothers (greater than 2 ounces of alcohol per day); (2) nondrinking mothers; (3) smoking, nondrinking mothers; (4) nonsmoking, nondrinking mothers. Infants in groups 1 and 2 were matched as closely as possible for postconceptional age, sex, race, and socioeconomic status. Infants in groups 3 and 4 were matched similarly. Infants of alcoholic mothers had a significantly lower birth weight, length, and head circu

A clinical neuropathological study of the fetal alcohol syndrome.

Five patients with the clinical diagnosis of fetal alcohol syndrome (FAS) died at the ages of 8 and 4 months and 17, 4 and 2 days. Neuropathological examination revealed microencephalic brains in all cases, without morphological evidence of maturation delay. One of them showed agenesis of the corpus callosum and hypoplasia of the cerebellar vermis. Five of them had only small dysgenetic changes, consisting mainly of glio or glioneuronal meningeal or parenchymal heterotopias. Our findings indicate that the brain is commonly but not affected in FAS. The influence of alcohol and its metabolites,

Direct current conversion of maternal supraventricular tachycardia developed during the treatment of a pregnant heroin addict with ritodrine.

During ritodrine (UtoparR) treatment for premature labor, a chronic heroin-addicted patient developed supraventricular tachycardia which was treated by direct current (d.c) conversion, d.c. conversion did not affect the fetus, as judged by the cardiotocogram. As heroin addicts run an increased risk or developing cardiac arrhythmia, caution is advised when treating these patients with beta-mimetics.

Effects of alcohol on the fetus: impact and prevention.

In the spectrum of adverse effects on the fetus or infant associated with maternal drinking during pregnancy the most dramatic is the fetal alcohol syndrome, a pattern of malformation that has been associated with maternal alcohol abuse. Other undesirable outcomes of pregnancy linked to alcohol exposure in utero include growth deficiency, major and minor anomalies, decrements in mental and motor performance, and fetal and perinatal wastage. Alcohol, like other teratogens, does not uniformly affect all those exposed to it. Rather, there seems to be a continuum of effects of alcohol on the fetus
