Alcohol and abnormal outcomes of pregnancy.

Heavy alcohol consumption by the mother during pregnancy has long been suspected of being a risk factor for abnormalities in the fetus or infant. Only during the last decade have these assumptions been supported by scientific studies. A clustering of fetal defects observed in some cases has been labelled the fetal alcohol syndrome. The syndrome involves prenatal and postnatal growth retardation, central nervous system involvement and craniofacial abnormalities, some of which are characteristic of the syndrome. Fetal alcohol syndrome is relatively rare, affecting from 1 in 300 to 1 in 2000 infa

Development of histaminergic pathways in brain as influenced by maternal alcoholism.

The development of histaminergic pathways has been studied in the rat brain specially as influenced by maternal alcoholism. Maternal ethanol consumption during pregnancy results in an increase in the cerebral histamine levels of the fetus. An increase in the brain histamine levels is also observed in the newborn rats suckling on the ethanol-fed mothers compared to the corresponding controls. Brain histamine levels are highest towards latter part of the pregnancy and lowest in the adult brains, whereas the brain histidine decarboxylase activity is lowest in the fetal brains and highest in the a

Steroid and growth hormone levels in premature infants after prenatal betamethasone therapy to prevent respiratory distress syndrome.

Prenatal maternal therapy with glucocorticoid reduces the incidence of respiratory distress syndrome (RDS) in premature infants. To investigate the effects of this treatment on the fetal endocrine system, we determined serum concentrations of betamethasone, cortisol, dehydroepiandrosterone sulfate, growth hormone, and prolactin in cord blood of 215 treated infants and 117 untreated infants of 26--36 wk of gestation. Cortisol levels are suppressed within 6 hr of betamethasone treatment, decrease to 45% of the concentration in untreated infants (8.4 micrograms/dl), and return to normal by 7 days

Drug kinetics in childbirth.

Drugs from a wide range of pharmacological classes are commonly given to women in childbirth, either for a maternal effect or a fetal/neonatal effect. A number of striking physiological and biochemical changes occur during labour and delivery that might alter drug kinetics. The rate of drug absorption from the gastrointestinal tract may be normal in labour provided that narcotic analgesics are not administered concurrently. Altered blood flow characteristics in the extremities could modify drug absorption from intramuscular injection sites. Drug distribution might be altered as a result of the

Environmental pollution and pregnancy: risks and uncertainties for the fetus and infant.

Numerous environmental contaminants can affect the developing embryo, fetus, or infant. This essay explores such questions as these: What is the importance in mutagenesis and teratogenesis of macroenvironmental pollutants such as the heavy metals, dioxin derivatives, polychlorinated diphenyl compounds, and pesticides? What is the significance of microenvironmental pollutants (or social environmental factors) such as tobacco smoke, alcohol, and pharmacologic agents over which exposed individuals have considerable control? What are some of the ethical and legal implications of these toxins of wh

Insulin levels in amniotic fluid. Management of pregnancy in diabetes.

A correlation between high insulin levels in amniotic fluid and the appearance of diabetogenic fetal morbidity was found in radioimmunoassays of 487 samples of the fluid. The mean insulin level in metabolically normal pregnancies rose from 9 muU/ml (Week 27) to 15 muU/ml (Week 40). The insulin level in amniotic fluid of diabetic patients was elevated up to 27 times the mean. Insulin levels in the umbilical cord and urine of newborns of diabetic mothers were also elevated, to 29 and 21 times the mean, respectively. Elevation of insulin levels in amniotic fluid portends diabetogenic fetal morbid

Effects of maternal ethanol ingestion on amine uptake into synaptosomes of fetal and neonatal rat brain.

Pregnant rats were kept on an ethanol-containing (6.8% v/v) liquid diet either from the 13th or 18th day of gestation. In pups exposed to ethanol from the 13th day of gestation, synaptosomal uptake of 3H-tyramine and its conversion to 3H-octopamine were increased initially and exposure to ethanol from the 18th day of gestation produced increases in synaptosomal uptake and conversion for longer periods. These changes were not observed in adult rats administered ethanol. Brain tyrosin hydroxylase activity in the developing rats was unaltered by continuous exposure to ethanol. Withdrawal from eth

Umbilical cord plasma cortisol levels in association with pregnancy complications.

Cortisol concentrations in umbilical cord plasma at birth were determined in association with pregnancy complications which may affect the time of pulmonary maturation of the fetus or otherwise influence its survival. No statistically significant change in these cortisol levels was observed in either term or premature pregnancies with the following complications: maternal disorder (preeclampsia, diabetes, pruritus), prolonged rupture of membranes, antenatal isoxsuprine treatment, maternal cigarette smoking, and neonatal asphyxia. Significant increases were observed in premature births in assoc

[Induction of malformations by N-methyl-N-nitrosourea (MNU) (author's transl))].

UNLABELLED: After observing multiple malformations in a stillborn infant whose mother was in contact with MNU during pregnancy, the inhalation of evaporated MNU solution or diazomethane was regarded as a possible cause. An attempt was made to reproduce the same conditions in experiments with rats. MATERIAL AND METHODS: The experiments were carried out with 46 pregnant rats yielding 370 fetuses. With the aid of an atomizer a solution of 50 mg MNU was atomized in a container of 12 liters. In each test up to 3 rats were exposed for 30 min (group A1). In other groups (B1--3) rats placed into the

The release of a bradykinin-like pulmonary vasodilator substance in foetal and new-born lambs.

1. Exposure to glass of whole blood from anaesthetized foetal lambs (at 0.6-0.95 of term) causes the rapid development of a potent pulmonary vasodilator agent.2. The formation of the vasodilator agent on exposure to glass, its disappearance with time, and the inhibition of its production by soya bean trypsin inhibitor, suggest that it is bradykinin.3. Small doses of bradykinin ( approximately 1 ng/kg) cause pulmonary vasodilatation on close arterial injection. Neither this, nor the vasodilatation caused by glass-exposed whole blood, are affected by agents which block the vasodilator actions of
