Area level measures of socioeconomic deprivation and their impact on very preterm birth and very preterm birth outcomes: A retrospective cohort study.

OBJECTIVE: We hypothesised that socioeconomic deprivation as determined by an area measure would be an important risk factor for preterm birth in a contemporary cohort. METHODS: A retrospective study was carried out across two tertiary maternity hospitals in Ireland between 2018 and 2022 representing approximately 27% of all births in Ireland. Non stillborn infants whose address could be matched to a small area deprivation index (Haase-Pratschke (HP) Index) were included in the analysis. A combination of non-parametric and parametric tests were used to examine the impact of socioeconomic depr

Small-for-gestational-age and suspected developmental coordination disorder in preschool children:A national cross-sectional study highlighting vulnerability in full-term births.

OBJECTIVE: Small-for-gestational-age (SGA) birth has been associated with adverse neurodevelopmental outcomes. However, its relationship with suspected developmental coordination disorder (DCD) in preschool children remains unclear. We therefore examined whether SGA was independently associated with suspected DCD risk in a national population-based sample. STUDY DESIGN: We analyzed CNCMD data from 101,505 preschool children aged 3-5 years. Birthweight-for-gestational-age status was classified using national sex- and gestational-age-specific standards, and suspected DCD was assessed using the

Biochemical and anthropometric assessment of nutritional status at discharge in very-low-birth-weight infants: a retrospective cohort study.

OBJECTIVES: To evaluate nutritional status at discharge in VLBW infants using biochemical and composite nutritional indices, and to examine their relationship with extrauterine growth restriction (EUGR; also termed "growth faltering" in more recent literature). METHODS: This retrospective cohort study included 124 very low birth weight infants (≤32 weeks, ≤1,500 g) admitted to a tertiary NICU (2021-2025). Anthropometric, nutritional, and biochemical parameters were recorded from birth to discharge. Four composite biochemical deficiency outcomes were constructed: protein (CPDO), nutritional (C

Low-Carbohydrate Diets, Nutritional Adequacy, Glycemia, and Perinatal Outcomes in Women With Gestational Diabetes Mellitus: A Secondary Analysis of the DiGest Trial.

OBJECTIVE: The optimal dietary carbohydrate intake for pregnant women is disputed internationally. Many authorities recommend a minimum of 175 g/day of carbohydrates primarily to support maternal brain glucose requirements and normal fetal brain development. We assessed whether low (<175 g/day) or very low (<120 g/day) carbohydrate intake was associated with altered nutrient intake, glycemia, or pregnancy outcomes in women with gestational diabetes mellitus (GDM). RESEARCH DESIGN AND METHODS: Women recruited to the Dietary Intervention in Gestational Diabetes Trial (DiGest) completed masked c

Perinatal health outcomes in relation to principal applicant and live-in caregiver status among economic class immigrants to Canada: A retrospective analysis of births from 1993 to 2017.

OBJECTIVES: Economic class immigrants to Canada tend to have better health outcomes than other immigrant groups. However, variation may exist even within this population. METHODS: We conducted a retrospective cohort analysis of data from the Migrant Maternal and Infant Morbidity and Mortality (MMIMM) dataset, which linked Canadian birth and infant death records (1993-2017) with maternal immigration records (1952-2017). All singleton births to economic class immigrant women at least 18 years of age at immigration were analyzed. Women were classified as principal applicants (PA), non-principal

Maternal exposure to PM2.5 and its components and risk of small for gestational age in women undergoing in vitro fertilization: a propensity score-matched case-control study in Guangzhou, China.

BACKGROUND: Pregnant women conceiving via in vitro fertilization (IVF) represent a uniquely vulnerable population, yet the impact of ambient fine particulate matter (PM2.5) and its specific chemical components on their fetal growth remains under-researched. This study aimed to investigate the associations between prenatal exposure to PM2.5 and its key components-sulfate (SO42-), nitrate (NO3-), ammonium (NH4+), organic matter (OM), and black carbon (BC)-with the risk of small for gestational age (SGA) in IVF pregnancies. METHODS: A propensity score-matched case-control study was conducted in

Sex-Specific Associations of Small, Vulnerable Newborns With Mental Capital in Early School-Aged Children.

BACKGROUND: Although mental capital is increasingly recognised as a foundation for children's later well-being, its perinatal determinants remain unclear because traditional classifications such as preterm birth and small for gestational age (SGA) capture only partial and overlapping aspects of vulnerability. OBJECTIVES: Using the integrated small vulnerable newborn (SVN) framework with mutually exclusive categories, we aimed to assess associations between SVN categories and mental capital at ages 6-7 years by sex. METHODS: We analysed data from six waves of the Adachi Child Health Impact of

AI-powered risk prediction models for preventable maternal mortality in rural settings: a systematic review.

BACKGROUND: Maternal mortality remains disproportionately high in low- and middle-income countries, particularly in rural settings with limited access to skilled obstetric care. Artificial intelligence and machine learning models offer promise for early risk prediction, yet their methodological rigour, applicability, and deployment feasibility in resource-constrained rural contexts remain inadequately synthesised. This systematic review evaluated AI-powered risk prediction models for preventable maternal mortality, with an emphasis on their suitability for rural and low-resource settings. MET

Deficiency of placental GDF15 causes fetal growth restriction by inhibiting fetal hepatic STAT3-IGF2 axis.

Fetal growth restriction (FGR) is a major cause of perinatal morbidity and mortality, yet its underlying mechanism remains elusive. Here, we show that growth differentiation factor 15 (GDF15) levels are significantly reduced in the placenta and fetal circulation in mouse and pig FGR models and correlate positively with fetal weight. Placenta-specific GDF15 deficiency causes FGR, while placenta-specific Gdf15 mRNA delivery through targeted lipid nanoparticle (LNP) effectively alleviates FGR. Mechanistically, placental GDF15 is a major contributor to the fetal circulating GDF15 pool and enters t

FUBP1 lactylation suppresses trophoblast function in fetal growth restriction.

BACKGROUND: Fetal growth restriction (FGR) is a common complication of pregnancy; however, its exact molecular mechanisms remain unclear. Excessive lactate accumulation in the placenta is often observed during FGR. High concentrations of lactate can regulate the proliferation of pathological cells through lactylation of key proteins. Therefore, we aimed to screen key lactylation modification-related genes (LMRGs) and provide new directions for the diagnosis and treatment of FGR. METHODS: The Differentially expressed LMRGs (DE-LMRGs) in FGR were evaluated using the Gene Expression Omnibus data

Persistent postnatal alterations in microRNA expression in neonatal encephalopathy.

BACKGROUND: Despite causing severe neurological morbidity, neonatal encephalopathy (NE) lacks reliable biochemical markers for early diagnosis and intervention. This study investigated if alterations in microRNA (miRNA) expression, previously identified in umbilical cord blood in NE, remain altered in postnatal whole blood samples. METHODS: Term infants with NE (and matched controls) born in Cork University Maternity Hospital (CUMH) between 2017 and 2020 were recruited. Whole blood was collected within the first 6 (T0), 24 (T24) and 48 (T48) hours after delivery via dried blood spot microsamp
