Fenton vs INTERGROWTH-21st charts in preterm infants < 32 weeks: impact of chart selection on growth classification.

UNLABELLED: Monitoring postnatal growth in preterm infants is essential to detect extrauterine growth restriction (EUGR) and guide nutritional management. The Fenton and INTERGROWTH-21st (IG-21) charts are commonly used for this purpose, but their agreement and clinical relevance remain uncertain. This study aimed to compare postnatal growth assessment using Fenton and IG-21 charts in infants born before 32 weeks' gestation and to evaluate differences in static and dynamic growth classification. We conducted a prospective single-center cohort study of preterm infants < 32 weeks admitted betwee

Conservative versus liberal oxygenation targets in critically ill children: the Oxy-PICU RCT.

BACKGROUND: The optimal target for systemic oxygenation in critically ill children is unknown. Liberal oxygenation is widely practised but is associated with harm in observational studies. OBJECTIVES: To evaluate the clinical and cost-effectiveness of a conservative oxygenation target of peripheral oxygen saturation 88-92% compared with peripheral oxygen saturation > 94% in critically ill children admitted to paediatric intensive care unit as an emergency. DESIGN AND SETTING: A pragmatic, open, multicentre, parallel-group, randomised clinical trial conducted in 15 National Health Service pae

Risk factors and clinical features of feeding intolerance in preterm infants and the effect of multimodal oral-sucking-oromotor training: a single-center retrospective analysis.

BACKGROUND: Feeding intolerance (FI) is a common gastrointestinal problem among hospitalized preterm infants that can delay establishment of enteral nutrition and increase the risk of complications. OBJECTIVE: To identify factors associated with FI and characterize its clinical manifestations in preterm infants, and to evaluate the association between addition of multimodal oral-sucking-oromotor training to routine care and FI incidence and short-term outcomes, including subgroup analyses by weight for gestational age. METHODS: We performed a single-center retrospective cohort study that inc

Routine use of a spike-in DNA in-process control for foetal RHD genotyping: Testing the real-world effectiveness of this 'canary'.

BACKGROUND AND OBJECTIVES: Non-invasive foetal RHD (fRHD) genotyping is widely implemented to prevent unnecessary administration of antenatal anti-D prophylaxis. Reliable assay performance is critical. In line with expert recommendations, we validated and implemented an artificial spike-in extraction control in our previously published assay. In this study, we report on assay verification and its performance in a 2-year cohort. STUDY DESIGN AND METHODS: fRHD typing was performed with cell-free DNA isolated from maternal plasma from gestational age week 27 or later. A circular plasmid with a f

Metformin use in gestational diabetes mellitus and neonatal outcomes: a systematic review and meta-analysis on the risk of small for gestational age.

PURPOSE: It remains unclear whether maternal metformin use in gestational diabetes mellitus (GDM) is associated with an increased risk of small-for-gestational-age (SGA) newborns. METHODS AND RESULTS: A systematic literature search was conducted across PubMed, Embase, and the Cochrane Library up to September 1, 2024. Nineteen studies (n = 115,192 participants), comprising randomized controlled trials and cohort studies, were included. Two evaluators independently assessed eligibility and bias risk. Maternal use of metformin for GDM was not significantly associated with SGA incidence in newbor

Application of artificial intelligence in diagnosis and management of fetal growth disorders: a comprehensive review.

Fetal growth disorders, including both fetal growth restriction and macrosomia, remain major contributors to perinatal morbidity and long-term health risks in adulthood. While ultrasound is the most frequently employed technique for the diagnosis of intrauterine growth abnormalities, its efficacy is constrained by the operator's experience and variable accuracy. This review explores the role of artificial intelligence (AI) in advancing the detection and management of fetal growth disorders. We conducted a comprehensive literature search of major databases to identify original and review articl

A cross-sectional examination of immune adaptations during pregnancy in the ECHO Cohort.

BACKGROUND: Pregnancy requires finely tuned immune changes that support implantation, placental development, maternal-fetal tolerance, and preparation for labor, yet the normative trajectories of circulating inflammatory proteins across gestation remain poorly defined. This cross-sectional study investigates how circulating inflammatory proteins vary with gestational age in pregnancy and examines the impacts of fundamental biological characteristics, such as gravidity and fetal sex. METHODS: Data were drawn from 1154 pregnant individuals from six study sites of the National Institutes of Heal

Customized birth weight percentiles for identification of SGA short statue.

OBJECTIVES: About 10-15 % of small for gestational age (SGA) children have persistent short stature (SGA-SS) and may benefit from growth hormone therapy. Customized percentiles that consider maternal height and weight, among other factors, appear to more accurately identify pathologically small newborns at higher risk of perinatal morbidity. We aim to determine whether the consideration of maternal height and weight in definition of SGA has an impact on the frequency of SGA-SS diagnosis and potential therapy. METHODS: Birth data of 1321 infants with a gestational age of ≥30 weeks of gestation

Sex-Specific Variation in Maternal Serum Screening Markers Across the First and Second Trimesters: Evidence from 10,384 Screened Pregnancies.

Background: Maternal serum screening remains widely implemented for prenatal aneuploidy assessment despite increased uptake of cell-free DNA testing. Evidence suggests that fetal sex may influence placental endocrine function and maternal serum biomarker levels; however, available studies are inconsistent and often limited by sample size or incomplete adjustment for maternal factors. Objective: The aim of this study was to determine whether fetal sex independently modifies first- and second-trimester maternal serum marker Multiple of the Median (MoM) values and whether sex-related biochemical

Analysis of metabolic status and risk factors of small for gestational age children with catch-up growth in East China.

PURPOSE: Approximately 85-90% patients with small for gestational age (SGA) experience catch-up growth (CUG) by the age of 2, with their height reaching - 2 standard deviation scores (SDS) or 3% of the height of children of the same age and sex. However, SGA patients with CUG (CUG-SGA) tend to be at a higher risk of developing insulin resistance, obesity, metabolic syndrome, and cardiovascular diseases. This study explored the metabolic conditions of CUG-SGA patients in East China and analyzes the risk factors that may contribute to metabolic issues. METHODS: This multi-center study in East C

Understanding the impact of temporary childbirth migration on maternal and child health: study protocol of a community-based prospective cohort study in India.

INTRODUCTION: Temporary childbirth migration (TCM), where women return to their natal homes for pregnancy, delivery or postpartum for a limited duration, is a long-standing sociocultural practice in India. While often motivated by familial support and traditional norms, its implications for maternal and child health and health system engagement remain poorly understood. This study aims to quantify the impact of TCM on maternal and newborn outcomes and to explore how continuity of perinatal care and social support mediate these relationships. METHODS AND ANALYSIS: We are conducting a three-sit

Parenteral nutrition using corrected weight to the 10th percentile improves weight gain in preterm neonates: A randomized controlled trial.

BACKGROUND: Parenteral nutrition (PN) plays a crucial role in providing nutritional support to premature and small for gestational age (SGA) neonates. In this randomized controlled study, we evaluated PN administration in preterm SGA neonates using the corrected weight estimated at the 10th percentile, as guided by a clinical decision support system (CDSS). METHODS: A total of 100 SGA neonates were randomly assigned to either the Control group (n = 50) or the Intervention group (n = 50). Both groups received PN support using a specialized CDSS. In the Control group, the CDSS calculated the PN

An Observational Study Protocol for Assessing Lactation Intensity and Reduction in the Prevalence of Metabolic Syndrome After a Maternal Complication of Pregnancy (LEMON Study).

BACKGROUND: Major pregnancy complications, such as hypertensive disorders of pregnancy, gestational diabetes mellitus, delivery of a small for gestational age infant, preterm delivery, and placental abruption, are associated with the development of Type II diabetes mellitus and cardiovascular disease. Improving breastfeeding longevity may reduce long-term risk for cardiovascular disease for those with major pregnancy complications. Metabolic syndrome (i.e., the presence of at least three specific cardiovascular disease risk factors) is an appropriate marker to assess future cardiovascular risk

Association between birthweight discordance and extrauterine growth restriction among preterm twins: a national multi-center study in China.

BACKGROUND: This multicenter study investigated the association between birthweight discordance (BWD) and extrauterine growth restriction (EUGR) in preterm twins, and evaluated the modifying role of small for gestational age (SGA). METHODS: Data from 2,496 infants (1,248 twin pairs) admitted to 22 Chinese NICUs (2018-2020) were analyzed. BWD was calculated as the percentage difference in birthweight between larger and smaller twins, categorized into four groups (≤15%, 15%-20%, 20%-25%, >25%). EUGR was defined as discharge weight below the 10th percentile for corrected gestational age and sex
