Beyond biometry: the role of fetal venous doppler in discriminating fetal growth restriction from small for gestational age and predicting adverse perinatal outcomes.

BACKGROUND: The distinction between fetal growth restriction (FGR) from small for gestational age (SGA) remains a major clinical challenge due to overlapping biometric features. The aim of this study was to evaluate the role of fetal arterial and venous Doppler parameters in distinguishing FGR from SGA and in predicting adverse perinatal outcomes. METHODS: In this prospective case-control study conducted at a tertiary referral center, 120 singleton pregnancies between 32 and 39 weeks’ gestation were classified into FGR, SGA, and appropriate for gestational age (AGA) groups. Doppler examination

Common and recurrent dysregulated molecular network of placental hypoxia and associated vasculogenesis and angiogenesis in fetal growth restriction.

Fetuses with fetal growth restriction (FGR) and selective FGR (sFGR) face elevated health risks both before and after birth. Although the underlying pathomechanisms remain unclear, placental dysfunction is recognized as a major contributing factor. By integrating untargeted transcriptomic data from FGR/sFGR placentae, this study identified 69 differentially expressed mRNAs (DEmRs) and eight differentially expressed miRNAs (DEmiRs). Functional enrichment analysis demonstrated significant enrichment in the angiogenesis and vasculogenesis pathways, with the hypoxia-related genes HIF1A and VEGFA s

Low-dose aspirin for preventing intrauterine growth restriction and pre-eclampsia in sickle cell pregnancy in Nigeria (PIPSICKLE): a randomised controlled trial.

BACKGROUND: Pregnant women with sickle cell disease are at increased risk of intrauterine growth restriction (IUGR), pre-eclampsia, and sickle-related conditions. Low-dose aspirin might reduce some of these complications but has not been tested in sickle cell pregnancy. We aimed to investigate the effectiveness and safety of low-dose aspirin for preventing complications during pregnancy. METHODS: In a double-blind randomised controlled trial in 16 public health facilities in southwest Nigeria, pregnant women (aged ≥18 years) with haemoglobin SS (HbSS) or SC (HbSC) and carrying a singleton fet

Placental DNA methylation dysregulation underlies fetal growth restriction associated with maternal pre-pregnancy underweight.

BACKGROUND: While maternal obesity is a well-established risk factor for adverse fetal outcomes, the impact of pre-pregnancy underweight remains incompletely characterized. This study aimed to investigate the association between maternal pre-pregnancy underweight and fetal growth restriction (FGR) and, more critically, to elucidate the underlying mechanisms. METHODS: We conducted a retrospective cohort study of 7,504 singleton deliveries at a tertiary obstetric and gynecology hospital in Shanghai. Genome-wide DNA methylation profiling (using GM-seq, a bisulfite-free genomic methylation sequenc

From Fetal Growth Restriction to Adolescent Cardiometabolic Risk: The Impact of Catch-Up Growth and Adiposity.

Background/Objectives: Fetal growth restriction (FGR) represents a model of adverse intrauterine programming associated with an increased risk of cardiometabolic disorders later in life. We examined the relationships between birth weight, catch-up growth, adipokine signaling, and early cardiometabolic risk in adolescents. Methods: This cross-sectional study included 80 term-born adolescents (40 FGR, 40 controls) matched for age and sex. Anthropometry, blood pressure, lipid profile, fasting glucose, adipokines (leptin, adiponectin), and ghrelin levels were assessed. Associations between birth w

Clinicopathologic and molecular landscape of villitis of unknown etiology: insights from a large-scale case-control study in PR China.

Villitis of unknown etiology (VUE) is a chronic placental inflammatory lesion characterized by lymphohistiocytic infiltration and destruction of villous architecture in the absence of infection. Although VUE is well recognized for its association with fetal growth restriction and adverse pregnancy outcomes, its clinicopathologic correlates and molecular basis remain poorly understood. VUE cases were identified from the pathology database of Jinan Maternal and Child Health Hospital (2020-2023) and classified as high-grade or low-grade according to the Amsterdam criteria. Clinical data, maternal

Oxygen and glucose supplementation increases rates of linear growth and cell proliferation in growth-restricted fetal sheep.

Placental insufficiency lowers oxygen and glucose concentrations in the fetus, which causes fetal growth restriction (FGR). Previously we showed that FGR fetuses chronically supplemented with oxygen and glucose (OG) have improved glucose tolerance. However growth was not evaluated. Here we test the hypothesis that sustained OG supplementation to an FGR fetus will increase linear growth rates, raise anabolic hormone concentrations and promote proliferation rates in pancreatic β-cells and skeletal muscle satellite cells, which are two tissues involved in glucose regulation. FGR was induced in sh

Liposomal encapsulation of L-arginine and L-citrulline enhances pharmacokinetics and therapeutic effects in a model of preeclampsia and fetal growth restriction.

L-arginine and L-citrulline improve vascular health in preeclampsia (PE) and fetal growth restriction (FGR), but their short half-life limits efficacy. This study evaluated pharmacokinetics, delivery, and therapeutic outcomes of liposomal encapsulation of these amino acids in a rat model of PE and FGR. Pharmacokinetics of liposome-encapsulated L-arginine were compared with free L-arginine (Free L-arg) in normal pregnant (NP) dams. Therapeutic effect on maternal blood pressure and fetal weight were studied in NP rats and the reduced uterine perfusion pressure (RUPP) model. Treatment groups rece

Single-cell insights into trophoblast heterogeneity and adaptive dysfunction in selective fetal growth restriction.

Selective fetal growth restriction (sFGR) in monochorionic diamniotic twins (MCDA) reflects placental dysfunction, but trophoblast adaptation mechanisms remain unclear. Using single-cell RNA sequencing on placental tissues from three paired sFGR, we demonstrate that villous cytotrophoblasts (VCT) in growth-restricted placentas undergo a transition from VCT_TP63, which expresses barrier-associated TP63/SOX6 and maintains cytoskeletal integrity, to VCT_LDHA, a metabolically reprogrammed phenotype marked by LDHA/YY1/RELA activation. Trajectory analysis shows diminished syncytial precursors, sugge

Ring chromosome 6: A case report and literature review.

Ring chromosome 6 is a rare structural chromosomal anomaly with a variable phenotype that includes growth restriction, congenital malformations, and neurological impairment. We present a term newborn female with a prenatal history of fetal growth restriction (FGR) and central nervous system malformations. At birth, she exhibited dysmorphic facies, bilateral corneal opacity, hypotonia, and a congenital heart defect. Cytogenetic analysis revealed a karyotype of 46,XX,r(6)(p25q27)[27]. The infant died on day 18 days of life due to a respiratory complication. Conclusions: Ring chromosome 6 is asso

Research progress on histone epigenetic modifications in placental trophoblast syncytialization.

During pregnancy, the placenta, as a temporary organ connecting the fetus to the mother, plays an essential role in fetal development. In the basic structural unit of the human placenta, the chorionic villus, proliferative mononuclear cytotrophoblasts cease division and spontaneously fuse to form non-proliferative multinucleated syncytiotrophoblasts a process termed trophoblast syncytialization. The expression of key genes is predominantly regulated by epigenetic modifications during syncytialization. Previous studies have reported that dysregulation of placental epigenetic modification is clo

Uterine artery Doppler assessment in pregnancy.

The prediction of major obstetric syndromes has driven advancements in non-invasive prenatal assessment. Among these, Doppler ultrasound has notably improved the detection of obstetric pathologies. By evaluating uteroplacental circulation, Doppler studies can identify both physiological and pathophysiological changes throughout pregnancy, providing crucial insights into placental insufficiency. This condition affects up to 15% of pregnancies and contributes to preeclampsia, fetal growth restriction, and preterm delivery, leading causes of preventable perinatal morbidity and mortality. Uterine

Predicting outcomes in selective fetal growth restriction of monoChOrioNic Twins: an inteRnAtional observational cohort STudy protocol (CONTRAST study).

INTRODUCTION: Selective fetal growth restriction (sFGR) is a major cause of perinatal morbidity and mortality in monochorionic diamniotic (MCDA) twin pregnancies. Current management relies on umbilical artery Doppler patterns in the smaller twin. These patterns are, however, inconsistent and do not represent a reliable severity scale, complicating clinical decision-making and parental counselling. This study aims to improve risk stratification by identifying predictors of adverse outcomes, while also evaluating the pathophysiology and multi-organ impact of sFGR in early childhood. METHODS AND

Planned mode of delivery and neonatal outcomes in pregnancies complicated by late-onset fetal growth restriction: a retrospective cohort study.

BACKGROUND: Fetal growth restriction (FGR) is a major contributor to perinatal morbidity and mortality. While guidelines address timing of delivery, the optimal mode-induction of labor (IOL) versus planned cesarean delivery (CD)-remains uncertain. OBJECTIVE: To evaluate the association between planned mode of delivery and neonatal outcomes in pregnancies complicated by late onset FGR (LOFGR). STUDY DESIGN: We conducted a retrospective cohort study at a tertiary Canadian center (2017-2022). Singleton pregnancies with LOFGR (> 34 weeks' gestation), defined by Society for Maternal-Fetal Medicin

Oligomeric lactic acid nanoplastics induce intrauterine growth restriction in mice by disrupting GATA2-mediated placental vascular development.

Humans are increasingly exposed to "eco-friendly" biodegradable microplastic pollution, whose usage in packaging and medical applications is growing exponentially. The bioplastic polylactic acid (PLA) has recently been demonstrated to release large quantities of oligomeric lactic acid (OLA) nanoplastics causing adverse health effects. No research has reported on intrauterine biodistribution of OLA, and how gestational exposure may impact on early development of the fetus. Here, we reveal that OLA plastics can readily breach the placental barrier and accumulate in various fetal organs in a mous
