Impact of Intravitreal Ranibizumab on Retinopathy of Prematurity: A Retrospective Analysis.

Purpose The purpose of this study was to evaluate the impact of intravitreal ranibizumab (IVR) introduction on treatment patterns for retinopathy of prematurity (ROP) and the incidence of severe complications, including retinal detachment (RD), in a real-world setting. Methods We retrospectively analyzed 785 infants born at ≤1800 g or ≤34 weeks of gestation between January 2016 and December 2022 at two tertiary hospitals in Japan. IVR (0.2 mg/0.02 mL) was introduced in August 2020 at Tohoku University Hospital and in December 2019 at Miyagi Children's Hospital. Infants meeting Early Treatment

Peritoneal drainage versus primary laparotomy in premature newborns with intestinal perforation: a retrospective cohort study evaluating an etiology-driven surgical approach.

BACKGROUND: The surgical management of intestinal perforation in extremely low birth weight (ELBW) infants, stemming from necrotizing enterocolitis (NEC) or spontaneous intestinal perforation (SIP), is a subject of enduring debate. The comparative effectiveness of initial bedside peritoneal drainage (PD) versus formal primary laparotomy (PL) on mortality and major morbidity is not fully resolved. OBJECTIVE: To compare 90-day mortality and comprehensive morbidity outcomes between PD-first and PL-first strategies in a homogeneous cohort of preterm infants with intestinal perforation, with rigor

Meconium-related ileus in extremely low birthweight infants: Growth and neurodevelopmental outcomes.

PURPOSE: Meconium-related ileus (MRI) is a common gastrointestinal complication in extremely low birthweight infants (ELBWIs) that may delay enteral nutrition and adversely affect growth and neurodevelopment. This study aimed to determine the incidence of MRI in ELBWIs, identify associated clinical factors, and investigate short- and long-term impacts on growth and neurodevelopment outcomes. METHODS: We retrospectively reviewed medical records of ELBWIs admitted to a single neonatal intensive care unit between January 2015 and August 2021. We investigated the prevalence of MRI and examined cl

The maternal-fetal anesthetic considerations of ex utero intrapartum treatment (EXIT) procedure: Experience from a tertiary referral center and literature review.

The ex utero intrapartum treatment (EXIT) procedure is a specialized surgical technique designed to safely establish cardiopulmonary support by maintaining placental circulation during cesarean delivery. It is most commonly indicated for fetuses with compromised airways caused by cervical masses, congenital high airway obstruction syndrome (CHAOS), or tracheal occlusion. Additional indications include resection of thoracic abnormalities, extracorporeal membrane oxygenation (ECMO) cannulation, management of conjoined twins, and resuscitation of extremely low birth-weight neonates. The primary p

Intraoperative Hemodynamic Collapse During Patent Ductus Arteriosus Ligation in an Extremely Low-Birth-Weight Infant: A Case Report.

Background and Clinical Significant: Patent ductus arteriosus (PDA) is a common cardiovascular disorder in extremely low-birth-weight (ELBW) infants, for which surgical ligation is indicated when pharmacologic closure fails. Sudden increases in afterload combined with immature myocardial contractility can lead to post-ligation cardiac syndrome (PLCS), which usually occurs within hours after surgery. However, acute intraoperative hemodynamic collapse during PDA ligation has rarely been described. Case Presentation: A preterm infant born at 24 weeks and 3 days of gestation with a birth weight of

Iatrogenic esophageal perforation in extremely preterm newborn with multiple comorbidities: case report and ethical considerations.

Neonatal esophageal perforation (EP) is a rare but potentially life-threatening complication, particularly in extremely low birth weight (ELBW) infants exposed to repeated invasive procedures. Although conservative management has progressively improved survival rates, mortality remains significant among the most vulnerable neonates. Literature studies predominantly describe favorable outcomes, possibly underrepresenting early lethal cases and overlooking the ethical implications of iatrogenic injury in infants at the threshold of viability. We report a fatal case of iatrogenic EP in an ELBW pr

Non-invasive ventilation in the delivery room.

UNLABELLED: Non-invasive respiratory support in the delivery room (DR) is a relatively new concept. Now, instead of immediately intubating most babies in respiratory distress, initial use of nasal CPAP or intermittent positive pressure ventilation (NIPPV) is recommended, to prevent lung injury. How to best implement non-invasive ventilation is still under investigation. What are the best levels of nasal CPAP, is NIPPV better to employ, and how can we synchronize breaths with the infant's efforts are all questions that are not yet answered. CONCLUSION: This article reviews the history of DR re

Pelvic Stability During Total Hip Arthroplasty: A Comparison of Traction and Standard Surgical Tables Using Augmented Reality Navigation.

Intestinal parasites are known to cause changes in blood count parameters and immunological markers among various populations. Prison inmates often have poor living conditions and limited access to healthcare, thus infections with intestinal parasites could negatively impact on their overall health and wellbeing. This study assessed the effects of intestinal parasitic infections on the haemato-immunological profile of prison inmates in Southern Ghana. A cross-sectional study using stratified sampling technique was conducted among 347 male prison inmates in three major prisons in Southern Ghana

Neonatal diabetes due to a truncating RFX6 mutation in an extremely low birth weight preterm infant with Mitchell-Riley syndrome.

OBJECTIVES: Mitchell-Riley syndrome (MRS) is a rare autosomal recessive disorder caused by pathogenic variants in the RFX6 gene and characterized by neonatal diabetes, pancreatic hypoplasia with exocrine insufficiency, intestinal atresia, and hepatobiliary abnormalities. Despite increasing recognition of the syndrome, the number of reported cases remains limited and clinical management remains challenging. We aimed to report a preterm infant with an extremely low birth weight and a truncating RFX6 mutation to highlight the severe metabolic instability that may occur in such patients. CASE PRE

Safety and efficacy of restrictive red blood cell transfusion strategy in extremely low birth weight infants: a before-after comparative study.

BACKGROUND: Extremely low birth weight (ELBW) infants frequently require red blood cell (RBC) transfusions, yet optimal hemoglobin thresholds remain debated. Recent evidence supports restrictive transfusion strategies, but real-world implementation data in Chinese neonatal intensive care units (NICUs) are limited. This study aimed to evaluate the safety and efficacy of implementing a restrictive RBC transfusion protocol in ELBW infants. METHODS: This multicenter before-after comparative study enrolled ELBW infants (birth weight < 1,000 g) admitted to two tertiary NICUs between January 2021 an

Congenital multidrug-resistant tuberculosis (MDR-TB) in an extremely low birth weight neonate: diagnostic challenges, management and a narrative review.

BACKGROUND: Congenital tuberculosis(TB) is rare and often fatal, particularly with multidrug-resistant strains. Neonatal diagnosis is challenging due to nonspecific features and low-yield microbiology. Management in extremely preterm infants is further complicated by limited evidence on second-line drug dosing, safety, and monitoring. METHODS: We describe a single patient case alongside a narrative review of the relevant literature. Literature search was conducted in major databases using predefined keywords. Cornelia de Lange syndrome in an extremely low birth weight infant: a case report.

Cornelia de Lange syndrome (CdLS) is a rare genetic disorder characterized by distinctive facial features, fetal growth restriction, intellectual disability, and hirsutism. Few reports have described extremely low birth weight (ELBW) infants with CdLS. We report an ELBW infant diagnosed with CdLS. The infant was delivered by emergency cesarean section at 34 weeks and 1 day of gestation, weighing 994 g (-3.7 SD score). A diagnosis of CdLS was suspected based on dysmorphic features, including forehead hirsutism, cleft palate, long philtrum, wide nostrils, syndactyly of toes, femoral hirsutism, a

Early-onset non-oliguric hyperkalaemia in a moderate preterm infant following maternal magnesium sulfate therapy.

Early neonatal hyperkalaemia is uncommon but may cause life-threatening arrhythmias if untreated. While most cases occur in extremely low-birth-weight infants after 24 hours of life, hyperkalaemia within the first few hours after birth is unusual.Magnesium sulfate, commonly used for pre-eclampsia, crosses the placenta and can cause neonatal hypermagnesaemia, which may disrupt potassium balance by inhibiting sodium-potassium adenosine triphosphatase activity and renal outer medullary potassium channel-mediated distal tubular potassium secretion.A preterm infant born at 33+4 weeks' gestation dev
