A Tiny Heart, a Massive Challenge: Case Presentation of Pericardial Effusion and Cardiac Tamponade in an Extremely Preterm Neonate and Corresponding Literature Review.

Massive pericardial effusion requiring pericardiocentesis in an extremely preterm, extremely low birth weight (ELBW) neonate is exceedingly rare and infrequently described in neonatal literature. We report the successful management and favorable neurodevelopmental outcome of a 24-week gestation male baby (birth weight 640 g) who developed hemodynamically significant, massive pericardial effusion during a neonatal intensive care unit course. The infant's clinical course was complicated by severe respiratory distress syndrome, persistent pulmonary hypertension of the newborn (PPHN), prolonged ve

Immunization Gaps Among High-Risk Preterm Infants in Kazakhstan.

BACKGROUND: Advances in neonatal intensive care in Kazakhstan have improved the survival of preterm infants, including those with very low birth weight (VLBW) and extremely low birth weight (ELBW). However, after hospital discharge, these medically vulnerable children may remain insufficiently protected against vaccine-preventable infections because of prolonged temporary medical exemptions, clinical caution, parental concerns, and fragmented coordination between specialized follow-up services and primary health care. OBJECTIVE: This study aimed to assess vaccination coverage in a 10-year coh

Late-Onset Sepsis in the Periviable Neonate.

A female infant born at 23 0/7 weeks' gestation with a birth weight of 570 g was admitted to the NICU due to extreme prematurity, extremely low birth weight, and respiratory distress syndrome. At approximately 2 weeks of life, she underwent an evaluation for late-onset sepsis because of escalating respiratory support requirements. Empiric antibiotics were initiated while awaiting culture results. Sequential broadening of antibiotic coverage occurred in response to worsening clinical status. Despite maximal medical therapy, the infant continued to deteriorate and ultimately died following an ac

Determinants of time to full enteral nutrition and its association with short-term outcomes in very and extremely low birth weight infants: a multicenter study in China.

BACKGROUND: Achieving full enteral nutrition (FEN) is a critical milestone for very low birth weight (VLBW) and extremely low birth weight (ELBW) infants. However, determinants of FEN timing and its clinical implications remain insufficiently characterized in China. To identify factors associated with time to FEN and evaluate its relationship with short-term outcomes in VLBW and ELBW infants. METHODS: This multicenter retrospective study included 773 preterm infants admitted between January 2019 and June 2023. Infants were stratified into VLBWI (n = 659) and ELBWI (n = 114). Clinical characte

Diagnostic Pitfalls in Meconium-Related Ileus: Red Flags for Surgical Pathologies in Extremely Low Birth Weight Infants.

AIM: Meconium-related ileus (MRI) often mimics other surgical pathologies in extremely low birth weight infants (ELBWIs), leading to diagnostic difficulties. This study aimed to clarify the clinical presentation and management outcomes of enema therapy in ELBWIs with a specific focus on the clinical indicators of underlying non-MRI surgical pathologies. METHODS: A retrospective review was conducted of 31 ELBWIs (birth weight < 1000 g) who received enema therapy for suspected MRI at a single tertiary center between 2008 and 2024. Infants were categorized into Success (n = 20) and Failure (n =

Case Report: Usefulness of transcutaneous bilirubin measurement for the early detection of severe hyperbilirubinemia due to late-onset hemolysis in an extremely low birth weight infant.

BACKGROUND: Late-onset hemolysis is a rare but potentially severe complication in extremely preterm infants and may cause rapid increases in bilirubin levels requiring intensive treatment, including exchange transfusion. Phototherapy for neonatal jaundice is guided by total serum bilirubin (TSB) and unbound bilirubin (UB) levels; however, in extremely low-birth-weight infants (ELBWIs), defined as neonates with a birth weight of less than 1,000 g, bilirubin neurotoxicity may occur at lower thresholds, and severe hyperbilirubinemia can develop unexpectedly beyond the acute neonatal phase. Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Surgical treatment of pulmonary hydatidosis: six years of experience and results at the National Hospital Guillermo Almenara Irigoyen, Lima, Peru.

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

The effects of early enteral feeding, fortification, and rapid feeding advancement in extremely low birth weight infants.

BACKGROUND: Preterm infants are at risk for necrotizing enterocolitis (NEC) and sepsis. Optimal strategies of preterm feeding to achieve full enteral feeding early with minimal duration of central lines without increasing the risk of NEC remain uncertain. We aimed to evaluate if new enteral feeding strategies reflecting early initiation, fortification, and more rapid advancement is beneficial without increasing the risk of NEC. METHODS: We performed a retrospective cohort study including 122 infants born with birth weight < 1000 g during Period 1 (old feeding protocol, n = 61) and Period 2 (n
