Psychosocial stress factors in families with preterm infants during the Covid-19 pandemic: a cross-sectional study.

BACKGROUND: The Covid-19 pandemic restrictions severely impacted parents' and children's mental and physical health. Families with pre-existing stress factors may have been particularly affected. Parental stress following premature birth is well acknowledged. The addition of the pandemic restrictions to stressors related to premature birth may constitute an especially high-risk factor for family stress and thereby neuropsychological development. Accessibility to special preterm follow-up care programs is important for neuropsychological development and faced additional relevance in the face of

Risk of neurodevelopmental impairment in Swedish preterm children treated for necrotizing enterocolitis: retrospective cohort study.

BACKGROUND: As the survival of preterm infants has increased, the management of long-term complications, especially neurodevelopmental impairment, becomes increasingly important. The aim of this study was to investigate the risk of neurodevelopmental disorders in preterm babies receiving medical or surgical treatment for necrotizing enterocolitis, compared with other preterm babies and preterm babies who received abdominal surgery for other indications. METHODS: In this nationwide Swedish cohort study, including all liveborn preterm babies born between 1998 and 2019, the risk of attention def

Heavy prenatal alcohol exposure and overall morbidities: a Danish nationwide cohort study from 1996 to 2018.

BACKGROUND: Heavy prenatal alcohol exposure is harmful and can lead to fetal alcohol spectrum disorders. A systematic review and meta-analysis identified 428 comorbidities in individuals with fetal alcohol spectrum disorders, and reported pooled prevalence estimates. We aimed to investigate overall risk of morbidities in heavy prenatal alcohol-exposed children by estimating risk of the identified comorbidities, and previously unidentified diseases and health-related problems. METHODS: Our Danish nationwide register-based cohort study included all singleton births. Individuals were followed up

Bronchopulmonary Dysplasia: Then, Now, and Next.

Bronchopulmonary dysplasia (BPD) has evolved considerably since its first description over 50 years ago. This review aims to provide a historical framework for conceptualizing BPD and a current understanding of the changing definition, epidemiology, pathophysiology, treatment, and outcomes of BPD. The transdisciplinary approach that led to the initial phenotypic description of BPD continues to hold promise today. Investigators are refining the definition of BPD in light of changes in clinical care and increasing survival rates of very preterm infants. Despite improvements in perinatal care the

Associations between household incense burning and delayed motor development among preterm infants modified by gestational age and maternal educational status.

BACKGROUND: Household incense burning is a common ritual behavior in the Asia-Pacific region but has been associated with inferior developmental outcomes in term infants. We aimed to examine these associations among preterm infants. METHODS: Information from 1190 mother-infant pairs during 6- and 18-month follow-up to the Taiwan Birth Cohort Study was examined for associations between household incense burning exposure and infant neurodevelopmental milestone achievement using multivariable Cox proportional hazard model with propensity score weighting, along with stratified, sensitivity, and d

Quantitative Handheld Swept-Source Optical Coherence Tomography Angiography in Awake Preterm and Full-Term Infants.

PURPOSE: To compare retinal vascular parameters acquired by handheld swept-source optical coherence tomography angiography (SS-OCTA) between nonsedated preterm and full-term infants. METHODS: Preterm and full-term infants at the University of Washington Medical Center were enrolled. Retinal angiograms (nominal size ∼7 × 7 mm2) were obtained at each routine retinopathy of prematurity (ROP) screening session for preterms and once during the first 72 hours of life for full-terms. Macular vessel area density and nonperfusion area were evaluated on the binarized vasculature map in both small (1.5

Adverse neonatal outcomes associated with pregestational diabetes mellitus in infants born preterm.

BACKGROUND: Pregestational diabetes mellitus is associated with a higher risk of adverse pregnancy outcomes. Based on the available data, it is unclear whether infants born preterm to mothers with pregestational diabetes mellitus are at a higher risk of adverse outcomes than other preterm infants. OBJECTIVE: This study aimed to quantify the neonatal complications associated with pregestational diabetes mellitus in infants born preterm. STUDY DESIGN: This was a retrospective cohort study of all nonanomalous singleton neonates born in Texas from 2006 to 2014. Analysis was limited to births bet

Moderate to late preterm intrauterine growth restriction: A restrospective, observational study of the indications for delivery and outcomes in an Australian perinatal centre.

BACKGROUND: The management of preterm intrauterine growth restriction is limited to fetal surveillance and timely delivery. Despite the existence of evidence-based guidelines, uncertainty regarding the optimal timing of delivery is common, and management remains individualised for each patient. AIMS: To provide recent Australian data on the indications for delivery of moderate to late preterm growth restricted infants and the outcomes of these deliveries. MATERIALS AND METHODS: Retrospective study of singleton live births delivered between 32 + 0 and 36 + 6 weeks gestation over a three-year

Platelet indices in late preterm newborns.

BACKGROUND: The current study objective was to compare blood platelet indices in late preterm newborns (LPN) and full term newborns (FTN). MATERIALS AND METHODS: We recruited 58 LPN and 71 FTN. Platelet indices were estimated in blood samples collected from the umbilical artery. RESULTS: LPN demonstrated a decreased count of blood platelets (249 × 10³/μL) as compared to FTN (295 × 10³/μL), p < 0.001. Platelet hematocrit (PCT) also showed substantial differences in both groups (LPN = 0.2% vs. FTN = 0.23%; p < 0.001). Mean platelet volume (MPV) was found to be nearly the same (LPN = 7.98fl, FT

Feeding problems in preterm infants of preeclamptic mothers.

AIM: Maternal disease can cause prematurity and neonatal complications, notably feeding problems. To determine the relationship between maternal disease and the nature and severity of neonatal feeding problems, we compared feeding profiles, time to demand feeding and length of hospital stay between preterm infants of preeclamptic mothers, mothers with amniotic infection and mothers with other disease causing prematurity. METHODS: The retrospective study used labour ward data collected from 2002 to 2005 in a tertiary university centre to analyse three groups of singletons born at <32 completed

School performance and behaviour in extremely preterm growth-retarded infants.

OBJECTIVE: To describe school performance and behaviour of extremely preterm, growth-retarded infants. DESIGN: Cohort study at two tertiary care centres. Included were all surviving, singleton infants (N= 127) with fetal growth retardation due to placental insufficiency. All were delivered by caesarean section because of signs of fetal distress before the beginning of labour at a gestational age of 26 to 32 weeks during the years 1984-1989. Main outcome measures were special education, mainstream education below the appropriate age level and behaviour according to attention-deficit hyperactiv

Intrauterine growth and gestational age in preterm infants with cerebral palsy.

In a case-control study, gestational age and intrauterine growth of 191 preterm singleton infants 1971-82 with cerebral palsy were compared to all preterm live-born singletons in Denmark in 1982 (N = 2203). The distribution of gestational age among preterm cases was slightly bimodal with maximum values at 29 and 32 weeks. The risk for cerebral palsy was highest in the infants with gestational age 28-30 weeks (OR = 5.6 (4.0-7.8), 95% confidence interval). Birth weight deviation, in the 34-36 weeks infants, expressed as the number of standard deviations from the mean birth weight for gestational

Behaviors of preterm infants with and without chronic lung disease when alone and when with nurses.

Many clinicians believe that infants with chronic lung disease display more irritability and react more negatively to care than other preterm infants. Therefore, the sleep-wake states and behaviors displayed by 31 high-risk preterm infants without chronic lung disease and 20 infants with chronic lung disease during four-hour observations conducted in intermediate care between 32 and 36 weeks postconceptional age were compared. Separate analyses were conducted for times when infants were alone and when they were with nurses to determine whether behavioral differences might be greater during han

Baseline respiratory sinus arrhythmia and heart rate responses during sustained visual attention in preterm infants from 3 to 6 months of age.

Three groups of preterm infants were studied longitudinally at 14, 20, and 26 weeks of age (corrected for gestational age). The groups included infants with no perinatal medical complications, those with mild respiratory problems requiring ventilatory assistance, and those with respiratory distress syndrome. Baseline heart rate and respiratory sinus arrhythmia (RSA) were recorded for 5 min, and heart rate was also recorded while the infants engaged in sustained visual attention to stimuli presented on video monitors. The heart rate response during stimulus orienting and sustained attention was

Is it time to reassess the risk for the growth-retarded fetus with normal Doppler velocimetry of the umbilical artery?

OBJECTIVE: Previous studies demonstrate an association between abnormal umbilical artery Doppler velocimetry and the birth of a small-for-gestational-age infant and between abnormal result and adverse neonatal outcome. The hypothesis is that preterm growth-retarded infants with normal antenatal velocimetry have outcomes similar to other preterm infants, whereas preterm small-for-gestational-age infants with abnormal Doppler results define a subgroup with increased morbidity. STUDY DESIGN: For 100 live-born infants, at risk for fetal growth retardation and undergoing antenatal Doppler and targ
