Outcomes of Biliary Complications in Pediatric Liver Transplantation: Is There a Role of Anastomosis Type and Epstein-Barr Virus Viremia?

BACKGROUND: Biliary complications remain a common adverse event after pediatric liver transplantation, with distinct etiologies and management approaches based on timing. This study aimed to evaluate the incidence, risk factors, and outcomes of early and late biliary complications in a high-volume living donor pediatric liver transplant center. METHODS: We retrospectively analyzed 98 pediatric liver transplantations performed between January 2018 and February 2024. Biliary complications were categorized as early (≤90 days) or late (>90 days) post-transplant. Risk factors were assessed using u

Payor status is associated with guideline-concordant skin-to-skin contact at an urban hospital in south central United States: a retrospective cohort study.

BACKGROUND: Skin-to-skin contact (SSC) is independently associated with improved breastfeeding outcomes. We sought to evaluate guideline-concordant SSC practices at an urban hospital in central Texas, which does not have baby-friendly designation. Given racial and ethnic disparities in breastfeeding outcomes, we hypothesized that Black and Hispanic women would be less likely to receive the recommended SSC. METHODS: This was a retrospective review of births at our institution from 1 January, 2021, to 31 July, 2021. Eligible mother-baby dyads were those with spontaneous, singleton, term vaginal

Maternal and Environmental Influence on Birth Weight in Jujuy, Argentina (1991-2014).

INTRODUCTION: Birth weight (BW) is a key indicator of intrauterine growth, influenced by maternal and environmental factors, including geographic altitude. This study analyzes the influence of maternal and socio-environmental factors on BW between 1991 and 2014 in the province of Jujuy, Argentina. METHODS: A total of 310 793 live births in Jujuy were analyzed, grouped by maternal region of residence. Mean BW was estimated, and the percentage distribution of maternal and gestational variables was described. Three-level generalized linear models were applied to assess the association between BW

Early mobilization in pediatric critical care and parental psychological outcomes 3-6 months after discharge-a pilot study.

UNLABELLED: Advancements in care for critically ill children have lowered mortality but increased morbidity in survivors. Post-intensive care syndrome (PICS) in children including their families (PICS-p) underscores the need for interventions to mitigate long-term effects. Early mobilization shows promising results in adults; however, evidence in pediatrics is limited. Enhanced parental involvement may contribute to improved long-term psychological outcomes. This pilot study examines the impact of an early mobilization program on parental stress and psychological health after admission on a pe

The effect of bath applied in two different sequences on stress level and physiological parameters in term babies.

BACKGROUND: The sequence of neonatal bathing may significantly influence an infant's ability to maintain homeostasis. Traditional methods begin with the head, but deferring head washing to the final stage may reduce thermal loss and procedural distress. PURPOSE: This study compared the effects of two swaddle bathing sequences head-first versus body-first on the stress levels and physiological parameters of term newborns. DESIGN AND METHODS: This randomized controlled trial (RCT) included 104 term infants born in a Training and Research Hospital in Istanbul between July 18 and September 20, 2

The effect of age-specific stay-at-home recommendation on healthcare utilization: Evidence from Finland's COVID-19 policy.

Postponements of non-acute care during the COVID-19 pandemic commonly raised concerns about harmful health consequences and increased healthcare costs, particularly among older individuals. Using nationwide register data from Finland, we employ a regression discontinuity design to examine the effect of an age-specific stay-at-home recommendation on healthcare utilization during the first wave of the pandemic. We find that the recommendation reduced non-acute visits, such as dental care, physiotherapy, and specialized care visits, but had no effect on acute care use, including emergency departm

Assessing the impact of maternal blood pressure during pregnancy on perinatal health: a wide-angled Mendelian randomization study.

BACKGROUND: Observational studies link high blood pressure in pregnancy to numerous adverse pregnancy and perinatal outcomes; however, findings may be affected by residual confounding or reverse causation. This study aimed to assess the causal effect of blood pressure during pregnancy on a range of pregnancy and perinatal outcomes. METHODS: We performed two-sample Mendelian randomization (MR) to assess the effect of systolic and diastolic blood pressure (SBP/DBP) during pregnancy on 16 primary and eight secondary adverse pregnancy and perinatal outcomes. We obtained genetic association data f

Elevated Blood Pressure in Newborns From Hypertensive Disorders of Pregnancy During the Immediate Postnatal Period.

Hypertensive disorders of pregnancy (HDP) impact up to 22% of pregnancies. Offspring from HDP, face an increased risk of future cardiovascular disease, with elevated blood pressure reported as early as childhood. Additionally, primary vascular endothelial cells derived from HDP demonstrate dysfunction in vitro. These data suggest that HDP affect fetal development in ways that impair long-term vascular function, supporting the developmental origins of health and disease (DOHaD) theory that health risks begin before birth. However, it remains unclear at what point these physiological changes fir

Seroprevalence of antibodies against diphtheria, tetanus, and pertussis over a 12-year period in children in Kilifi, Kenya (2009-2021).

OBJECTIVES: In Kilifi, pentavalent coverage remains below the 90% target, with no reported diphtheria or tetanus cases and sporadic pertussis. However, absence of disease does not guarantee immunity. To characterize age-specific gaps and waning protection not captured by routine surveillance, we conducted serial seroprevalence studies of diphtheria, pertussis, and tetanus. METHODS: We analyzed randomly selected participants from multiple cross-sectional surveys within the Kilifi Health and Demographic Surveillance System. Immunoglobulin G antibodies were measured using a fluorescent bead-base

Programming the Infant Gut: How Maternal and Early Life Nutrition Shape the Infant Microbiome and Long-term Health-A Narrative Review.

Childhood malnutrition, including undernutrition, obesity, and micronutrient deficiencies, remains a major global health burden. Emerging evidence points to the gut microbiome as a critical mediator linking maternal, prenatal, and early-life nutrition to long-term offspring health outcomes. From conception and through the first years of life, maternal diet, metabolic state, and environmental exposures shape offspring microbial colonization and maturation. Breastfeeding and consumption of fiber-rich and fermented foods (maternal and post-weaning) support beneficial microbiota, while high-fat, h

Prognostic value of modified KDIGO staging for acute kidney injury in neonates: a prospective observational study in a level IIIB NICU.

BACKGROUND: Acute kidney injury (AKI) is an increasingly recognized complication in neonates admitted to intensive care, contributing significantly to short-term morbidity and mortality. Early diagnosis and risk stratification using standardized criteria such as modified neonatal Kidney Disease Improving Global Outcomes (KDIGO) remain critical, particularly in resource-constrained tertiary units. Limited Indian studies have evaluated AKI across both term and preterm neonates in Level IIIB Neonatal Intensive Care Unit (NICU) with post-discharge growth follow-up. METHODS: This prospective observ

Analysis of factors affecting postoperative feeding intolerance in neonates following intestinal surgery.

BACKGROUND: Postoperative nutritional support is critical for neonatal recovery; however, this vulnerable population frequently experiences feeding intolerance during the postoperative period. A comprehensive analysis of the neonatal perioperative period is therefore essential to identify risk factors associated with postoperative feeding intolerance. OBJECTIVE: This study aimed to describe perioperative data in neonates and identify risk factors for postoperative feeding intolerance. METHODS: A retrospective analysis was performed on 694 neonates who received enteral nutrition (EN) followin

Association of Pre-Fontan Hemodynamics With Long-Term Outcomes After Fontan Palliation: A Study From the Pediatric Cardiac Care Consortium.

BACKGROUND: Long-term outcomes after Fontan vary widely. Although pre-Fontan hemodynamics predict early failure, their association with long-term outcomes remains unclear. We hypothesized that pre-Fontan hemodynamics predict long-term risk of death or transplantation. METHODS: We analyzed data from the Pediatric Cardiac Care Consortium, a US-based multicenter registry including patients undergoing first-time Fontan with pre-Fontan catheterization and long-term follow-up. Patients undergoing a Fontan procedure before 18 years of age at any time between 1982 and 2011 were included in the study.

Aperiodic parameters of the fMRI power spectrum associate with preterm birth and neonatal age.

Perinatal period is a critical time for brain development and premature-born children have an elevated likelihood for neurodevelopmental conditions. While strong associations of structural magnetic resonance imaging with preterm birth and post-menstrual age (PMA) have been reported, results with functional MRI (fMRI) have been considerably weaker. Using the task-free neonatal fMRI dataset from the Developing Human Connectome Project (dHCP), we first studied the associations of the aperiodic component of the blood-oxygen-level-dependent (BOLD) signal power spectrum from pre- and postcentral gyr

Exploring the cause of the pertussis resurgence in England following the COVID-19 pandemic: a mathematical modelling study.

BACKGROUND: In 2024 England, in common with many other countries, experienced a pertussis resurgence the cause of which is unclear. We used a pertussis transmission model, previously developed to investigate the cause of the pertussis resurgence in England in 2012, to explore potential factors contributing to the increase in pertussis cases observed in England in 2024. METHODS: An age-stratified dynamic transmission model fitted to pertussis notification data from England between 1953 and 2013 was run until 2034 with and without changes in social mixing as estimated from Google mobility and sc
