Brain injury in congenital heart disease: pathophysiology, MRI patterns, and perioperative risk factors.

BACKGROUND: Congenital heart disease (CHD) affects approximately 1% of live births and remains a major cause of neonatal neurological morbidity. Despite advances in perioperative management, both pre- and postoperative brain injuries continue to occur frequently and contribute to adverse long-term neurodevelopmental outcomes. The aim of this review is to synthesize current MRI-based cohort studies to describe the prevalence and patterns of pre- and postoperative brain injury in CHD, elucidate the underlying pathophysiological mechanisms, and identify maternal-fetal, perioperative, and postoper

Neurodevelopmental Outcomes of Isolated Cerebellar Pathologies in Extremely Preterm Infants.

BACKGROUND AND OBJECTIVES: Cerebellar pathologies, especially cerebellar hemorrhage (CBH), in extremely preterm infants has been increasingly detected through cerebral magnetic resonance imaging (cMRI). We evaluated prevalence, associated risk factors, and neurodevelopmental outcomes of isolated CBH in a contemporary cohort of high-risk infants without major supratentorial brain injuries. METHODS: This observational cohort study included infants born at less than 28 weeks' gestation during 2017-2022 who underwent cMRI at term-equivalent age. Cerebellar pathologies, including low-grade (≤3 mm)

Large intracerebral hemorrhage in the setting of vitamin K prophylaxis refusal.

BACKGROUND: Vitamin K is a vital component of prophylactic treatment in newborns, as deficiency leads to inadequate synthesis of coagulation factors. Vitamin K deficiency bleeding (VKDB) can lead to gastrointestinal, mucocutaneous, or intracranial hemorrhage. Unfortunately, the rise in vaccine hesitancy has been paralleled by a rise in refusal of vitamin K prophylaxis at birth. Case report Here, we present the case of a 2-month-old who presented to the emergency department with a large ICH due to a lack of vitamin K prophylaxis. The patient presented to the emergency room (ER) unresponsive wit

Diagnostic Challenges and Surgical Outcomes of Miyazaki Syndrome: A Report of Two Cases and a Systematic Review.

Miyazaki syndrome is a rare and underrecognized complication of long-standing cerebrospinal fluid (CSF) diversion. It usually occurs in patients with chronic CSF overdrainage and is caused by enlargement of the cervical epidural venous plexus, which may compress the spinal cord and cause progressive myelopathy or radiculopathy. Because symptoms develop slowly and may resemble degenerative cervical myelopathy or other neurological conditions, diagnosis is often challenging. We report two illustrative cases from our institution. The first patient was a 57-year-old man with a ventriculoperitoneal

Preconception Counseling for People With Disabilities.

Over 70 million Americans, including 35 million women, are living with a disability including ~10% to 12% of reproductive-age women. Having a disability does not preclude a desire to or ability to have children. Studies have demonstrated that an increased risk of adverse outcomes, including hypertensive disorders, hemorrhage, and preterm birth, occurs among pregnant people with physical, intellectual, developmental, and sensory disabilities. This review aims to evaluate pregnancy outcomes, outline general preconception care, and review disability-specific considerations with the goal of improv

Impact of a minimal handling care protocol on intraventricular hemorrhage in preterm infants: a retrospective cohort study.

BACKGROUND: Premature birth is associated with a high risk of mortality and neurodevelopmental morbidity, with intraventricular hemorrhage (IVH) remaining a major cause of brain injury in preterm infants. Neuroprotective care bundles focusing on minimal handling have been proposed to reduce physiological stress and cerebral hemodynamic instability. While such bundles have been associated with reduced rates of severe intraventricular hemorrhage, evidence regarding the isolated impact of minimal handling-particularly on other neonatal morbidities-remains limited. METHODS: This retrospective com

Severe Late-Onset Vitamin K Deficiency Bleeding Presenting With Subdural Hematoma and Seizures in a 4-Month-Old Infant: A Case Report From a Resource-Limited Setting.

BACKGROUND: Vitamin K deficiency bleeding (VKDB) remains a preventable cause of severe hemorrhage in early infancy. Late-onset VKDB (2 weeks-6 months) most commonly presents with intracranial hemorrhage (ICH) and can result in death or long-term neurological sequelae. Although routine neonatal intramuscular vitamin K prophylaxis is highly effective, late VKDB may still occur in the presence of contributing factors such as infection or impaired vitamin K availability. CASE PRESENTATION: A 4-month-old male infant presented with fever, cough, and poor appetite. He was delivered by cesarean secti

Surgical timing and mortality in acute traumatic subdural hematoma: is the four-hour threshold confounded by injury severity?

PURPOSE: Early surgical evacuation within four hours has long been advocated for acute traumatic subdural hematoma (ASDH), yet the supporting evidence is largely observational and unadjusted for injury severity. We examined whether surgical timing is independently associated with in-hospital mortality after accounting for baseline severity. METHODS: We retrospectively analyzed 91 consecutive patients who underwent surgical evacuation for ASDH between January 2014 and December 2024. The primary outcome was in-hospital mortality; the secondary outcome was functional status at discharge measured

CMRI-detected brain injuries and clinical key risk factors associated with adverse neurodevelopmental outcomes in very preterm infants.

Neurological impairment is high after preterm birth. This study evaluates the impact and interplay of cMRI-detected brain injuries (BI) and clinical risk factors on neurodevelopmental outcomes and extracts the most important key factors. A retrospective analysis was conducted on risk factors (perinatal/neonatal, cMRI-detected BI) for adverse motor (MO) and cognitive (CO) outcomes (Bayley Scales of Infant Development, 24 months corrected age) in a tertiary center cohort (2009-2018) of very preterm infants (< 32 weeks of gestation) using uni-/multivariable regression models. Cranial Ultrasonography-Standards in Diagnosis of Intraventricular Hemorrhage and Ventricular Dilatation in Premature Neonates.

Intraventricular hemorrhage (IVH) is a common complication encountered in extremely-low-birth-weight (ELBW) and very-low-birth-weight (VLBW) premature babies. The neurologic outcome of these patients is influenced by the magnitude of the hemorrhagic process that damages the involved anatomic structures but also by the impaired circulation of cerebrospinal fluid (CSF) through the ventricular system, leading to posthemorrhagic ventriculomegaly (PHVM). Cranial ultrasound (CUS) performed by neonatologists (point-of-care ultrasound-POCUS) facilitates the early diagnosis of IVH and PHVM and can obje

Developmental Stage-Dependent Transcriptomic Responses to Neonatal Intraventricular Hemorrhage.

Neonatal intraventricular hemorrhage (IVH) is a major complication of preterm birth, yet how developmental stage influences the brain's response to injury remains unclear. We performed single-nucleus RNA sequencing on rat brains 24 hours after IVH at postnatal day 2 (PND2) or day 5 (PND5) to define transcriptional responses across cell types. We identified 42 distinct cell populations and found that PND5 brains exhibited a markedly stronger immune and inflammatory response to IVH, with a threefold increase in differentially expressed genes compared to PND2. Microglia were the most perturbed ce
