Severe Late-Onset Vitamin K Deficiency Bleeding Presenting With Subdural Hematoma and Seizures in a 4-Month-Old Infant: A Case Report From a Resource-Limited Setting.

BACKGROUND: Vitamin K deficiency bleeding (VKDB) remains a preventable cause of severe hemorrhage in early infancy. Late-onset VKDB (2 weeks-6 months) most commonly presents with intracranial hemorrhage (ICH) and can result in death or long-term neurological sequelae. Although routine neonatal intramuscular vitamin K prophylaxis is highly effective, late VKDB may still occur in the presence of contributing factors such as infection or impaired vitamin K availability. CASE PRESENTATION: A 4-month-old male infant presented with fever, cough, and poor appetite. He was delivered by cesarean secti

Surgical timing and mortality in acute traumatic subdural hematoma: is the four-hour threshold confounded by injury severity?

PURPOSE: Early surgical evacuation within four hours has long been advocated for acute traumatic subdural hematoma (ASDH), yet the supporting evidence is largely observational and unadjusted for injury severity. We examined whether surgical timing is independently associated with in-hospital mortality after accounting for baseline severity. METHODS: We retrospectively analyzed 91 consecutive patients who underwent surgical evacuation for ASDH between January 2014 and December 2024. The primary outcome was in-hospital mortality; the secondary outcome was functional status at discharge measured

Birth-Related Subdural Hemorrhage in Asymptomatic Newborns: Magnetic Resonance Imaging Prevalence and Evolution of Intracranial and Intraspinal Localization.

Background: Neonatal birth-related intracranial subdural hemorrhages (SDHs) represent a form of bleeding inside the skull that occurs in newborns. This condition includes the extravasation of blood both in the encephalic parenchyma and in the extra-axial spaces. Recent studies have shown that SDH and particularly post-traumatic birth-related hemorrhages represent a frequent occurrence, but they are often asymptomatic. The gold standard for the diagnosis and follow-up of patients with SDH is multiparametric Magnetic Resonance Imaging. The aim of this study is to describe our experience by repor

The "Heart-and-Brain Interaction" in Newborns with Complex Congenital Heart Disease.

OBJECTIVES: Neonates with complex congenital heart disease (CHD), in particular newborns with hypoplastic left heart syndrome (HLHS) or transposition of the great arteries (TGA), may show brain pathologies and altered brain growth after birth. Our prospective study investigates brain volumes and immature brain structures in these patient groups compared to healthy controls. METHODS: Neonatal cerebral magnetic resonance imaging (MRI) scans were analysed by semi-automated segmentation (dHCP pipeline) in 51 children: 31 HLHS/hypoplastic left heart complex (HLHC)/UVH (61%) at stage I, in 18 neona

The impact of metal cup size on neonatal and maternal morbidity in vacuum-assisted deliveries.

OBJECTIVE: To evaluate whether metal cup size influences maternal and neonatal morbidity in vacuum-assisted deliveries (VAD). MATERIALS AND METHODS: Retrospective cohort at a single tertiary center (2011-2022) comparing 60-mm versus 50-mm Malmström cups. A 1:1 matched cohort analysis balanced maternal and obstetric variables. Primary neonatal outcomes were a birth trauma composite outcome (brachial plexus injury, cephalohematoma, subgaleal or subdural hematoma) and a severe neonatal composite outcome (neonatal intensive care unit admission, 5-min Apgar score less than 7, umbilical artery pH l

Identifying Risk Factors of Children Who Suffered Physical Abuse: A Systematic Review.

BACKGROUND: Approximately 25% of children in the United States experience child abuse or neglect, 18% of whom are physically abused. Physicians are often in a position to differentiate accidental trauma from physical child abuse. Therefore, the aim of this study was to review recent literature for risk factors associated with physical child abuse. METHODS: In this systematic review, three electronic databases were searched for articles published in the past 10 years, using the terms "abuse," "risk factors," and "children," with associated variations. A total of 1,568 articles were identified.

Infantile traumatic pericallosal aneurysm: illustrative case.

BACKGROUND: Traumatic aneurysms are a rare sequela of nonaccidental head trauma in infants. The rate of nonaccidental trauma (NAT) in the pediatric population is increasing; therefore, traumatic aneurysms are an important consideration in the evaluation of pediatric patients with abusive head trauma. OBSERVATIONS: A 24-day-old infant with no significant past medical or birth history presented with twitching and poor oral intake for 1 day. The patient was found to have bilateral subdural hematomas, multifocal contusions, and traumatic subarachnoid hemorrhage. NAT work-up was remarkable for a p

Birth-related subdural hemorrhage in asymptomatic neonates: evolution over time and differentiation from traumatic subdural hemorrhage.

BACKGROUND: Intracranial birth-related subdural hemorrhage frequently occurs in asymptomatic newborns and has no adverse long-term sequelae. It is medically and medicolegally important to differentiate birth-related subdural hemorrhage from other pathological causes of intracranial hemorrhage. There is limited literature available on the incidence of birth-related subdural hemorrhage, its imaging features, and evolution over time, mainly because asymptomatic infants do not routinely undergo cranial MRI. OBJECTIVE: To establish the incidence and distribution of birth-related subdural hemorrhag

The Use of Middle Meningeal Artery Embolization to Treat Chronic Subdural Hematoma in the Pediatric Population: A Review of the Literature.

Chronic subdural hematoma (cSDH) is rare in the pediatric population and typically arises from various causes. These include trauma (accidental, non-accidental, or birth-related injuries), coagulopathies (such as hemophilia or von Willebrand disease), vascular malformations (such as arteriovenous malformations), and complications from previous surgeries. These diverse etiologies contribute to the complexity of managing this condition. Although middle meningeal artery (MMA) embolization is proven effective in adults, limited studies have investigated its applicability in pediatrics. This study

Precision of vacuum cup placement and its association with subgaleal hemorrhage and associated morbidity in term neonates.

PURPOSE: To evaluate whether the precision of vacuum cup placement is associated with failed vacuum extraction(VE), neonatal subgaleal hemorrhage(SGH) and other VE-related birth trauma. METHODS: All women with singleton term cephalic fetuses with attempted VE were recruited over a period of 30 months. Neonates were examined immediately after birth and the position of the chignon documented to decide whether the cup position was flexing median or suboptimal. Vigilant neonatal surveillance was performed to look for VE-related trauma, including subgaleal/subdural hemorrhages, skull fractures, sc

Traumatic brain injury during pregnancy is associated with increased rate of cesarean section: a nationwide multi-register study in Finland.

OBJECTIVE: The literature on the incidence of traumatic brain injury (TBI) during pregnancy is lacking. Furthermore, only studies with small sample size have analyzed the impact of TBI during pregnancy to maternal and fetal outcomes. Thus, we aim to report the incidence of TBIs during pregnancy and study the pregnancy outcomes using nationwide high-quality registers. METHODS: This nationwide retrospective register-based matched cohort study utilized two national registers. All fertile-aged (15-49 years) women with a TBI hospitalization period during pregnancy were retrieved the Care Register

Is bridging vein rupture/thrombosis associated with subdural hematoma at birth?

BACKGROUND: The combination of bridging vein rupture/thrombosis and subdural hematoma in infants has recently gained attention as highly suggestive of abusive head trauma. While subdural hematomas are frequently observed at birth, there are no previous studies of bridging vein rupture/thrombosis prevalence in that context. OBJECTIVE: To evaluate the prevalence of bridging vein rupture/thrombosis in newborns with and without subdural hematoma. MATERIALS AND METHODS: This bicentric retrospective study (2012-2019) looked at all brain MRIs performed in neonates. We noted delivery method, demogra

Unexplained mechanism of subdural hematoma with convulsion suggests nonaccidental head trauma: A multicenter, retrospective study by the Japanese Head injury of Infants and Toddlers study (J-HITs) group.

OBJECTIVE: The medical history of injury given by parents of infants and toddlers with head trauma may not be accurate or completely true. The purpose of this study was to examine the relationship between subdural hematoma (SDH) due to nonaccidental injury and mechanisms of injury provided by caregivers. METHODS: Our multicenter study group retrospectively reviewed the clinical records of children younger than 4 years with head trauma who have been diagnosed with any finding on head computed tomography (CT) and/or magnetic resonance imaging (MRI). A total of 84 cases of subdural hematomas wit

Is external hydrocephalus a possible differential diagnosis when child abuse is suspected?

BACKGROUND: Criteria for diagnosing abusive head trauma (AHT) or "shaken baby syndrome" are not well defined; consequently, these conditions might be diagnosed on failing premises. METHODS: The authors have collected a total of 28 infants, from the US (20) and Norway (8), suspected of having been violently shaken, and their caregivers had been suspected, investigated, prosecuted or convicted of having performed this action. Among 26 symptomatic infants, there were 18 boys (69%) and 8 girls (31%)-mean age 5.1 month, without age difference between genders. RESULTS: Twenty-one of 26 symptomatic

Birth-related subdural hemorrhage: prevalence and imaging morphology.

BACKGROUND: Birth trauma accounts for 1-2% of the mortality in newborns with significant intracranial injuries presenting in the immediate postnatal period. However, a significant number of asymptomatic neonates harbor birth-related intracranial hemorrhage (ICH), with birth-related subdural hemorrhage (SDH) being a common occurrence on infant brain CT and MRI studies performed as a standard of care for a variety of reasons. Although clinically insignificant, birth-related SDH is frequently brought up in courts as an alternative explanation for SDH in suspected abusive head trauma. OBJECTIVE:

Subdural hemorrhage in a cohort with cerebral sinovenous thrombosis: Application to abusive head trauma.

BACKGROUND: Cerebral sinovenous thrombosis (CSVT) is a postulated cause of subdural hemorrhage (SDH) that is hypothesized to mimic abusive head trauma (AHT). Minimal data exists directly investigating this relationship. OBJECTIVES: To evaluate the frequency of SDH in children with CSVT, identify factors associated with CSVT and SDH, and to assess if any association supports the hypothesis that CSVT causes SDH. PARTICIPANTS AND SETTING: The International Pediatric Stroke Study (IPSS) prospectively collects data on subjects birth to 19 years of age with intracranial thrombosis. METHODS: IPSS

Extracorporeal Carbon Dioxide Removal During Continuous Renal Replacement Therapy as Adjunctive Therapy.

Lung-protective ventilation targeting low tidal volumes and plateau pressures is the mainstay of therapy in patients with ARDS. This ventilation strategy limits pulmonary strain, inflammation, and injury, but it may be associated with profound hypercapnic acidosis. In such conditions, extracorporeal CO2 removal can attenuate or normalize hypercapnia and may even facilitate ultraprotective ventilation. Almost half of patients with ARDS develop renal failure. Pathophysiological cross-talk between the injured lung and kidney may aggravate global organ failure and weighs negatively on outcomes. A
