Miyazaki syndrome is a rare and underrecognized complication of long-standing cerebrospinal fluid (CSF) diversion. It usually occurs in patients with chronic CSF overdrainage and is caused by enlargement of the cervical epidural venous plexus, which may compress the spinal cord and cause progressive myelopathy or radiculopathy. Because symptoms develop slowly and may resemble degenerative cervical myelopathy or other neurological conditions, diagnosis is often challenging. We report two illustrative cases from our institution. The first patient was a 57-year-old man with a ventriculoperitoneal

BACKGROUND: Premature birth is associated with a high risk of mortality and neurodevelopmental morbidity, with intraventricular hemorrhage (IVH) remaining a major cause of brain injury in preterm infants. Neuroprotective care bundles focusing on minimal handling have been proposed to reduce physiological stress and cerebral hemodynamic instability. While such bundles have been associated with reduced rates of severe intraventricular hemorrhage, evidence regarding the isolated impact of minimal handling-particularly on other neonatal morbidities-remains limited.

Etanercept, a selective inhibitor of the cytokine tumor necrosis factor (TNF), delivered by perispinal administration, has been used clinically for 15 years to treat chronic, intractable, post-stroke and post-brain injury neurological dysfunction. We report the case of a 19-year-old patient with cerebral palsy (CP) who experienced rapid and sustained neurological improvement after treatment with perispinal etanercept (PSE). The patient was born at 25 weeks gestation following a twin pregnancy complicated by abruptio placentae and premature onset of labor. The initial head ultrasound, performed

Background/Objectives: Severe brain injury (SBI) in very preterm infants includes heterogeneous lesions with distinct timing, burden and outcomes. We used cranial ultrasound (CUS) to describe SBI entity, documented timing, three-domain burden, deaths following documented withdrawal, withholding or non-escalation of life-sustaining treatment for poor neurological prognosis (neuro-WWLST), and survivor outcomes. Methods: Retrospective single-center cohort (1991-2020) of 2841 very preterm infants (<32 weeks' gestation and/or birth weight ≤ 1500 g) with complete CUS within 48 h after birth. CUS was

OBJECTIVE: Bronchopulmonary dysplasia (BPD) remains a common pathology in very preterm infants. The risk of complications increases with the severity of the disease. The study aimed to determine the factors affecting the formation of moderate/severe BPD in the modern population of very preterm infants.
METHODS: Data from 201 very low birth weight infants < 32 weeks of gestation were used in a retrospective cohort study. Infants were retrospectively divided into two groups based on the type of respiratory support at 36 weeks of postmenstrual age (PMA) - mild BPD (133 infants) and moderate/sever

Neurological impairment is high after preterm birth. This study evaluates the impact and interplay of cMRI-detected brain injuries (BI) and clinical risk factors on neurodevelopmental outcomes and extracts the most important key factors. A retrospective analysis was conducted on risk factors (perinatal/neonatal, cMRI-detected BI) for adverse motor (MO) and cognitive (CO) outcomes (Bayley Scales of Infant Development, 24 months corrected age) in a tertiary center cohort (2009-2018) of very preterm infants (< 32 weeks of gestation) using uni-/multivariable regression models. Intraventricular hemorrhage (IVH) is a common complication encountered in extremely-low-birth-weight (ELBW) and very-low-birth-weight (VLBW) premature babies. The neurologic outcome of these patients is influenced by the magnitude of the hemorrhagic process that damages the involved anatomic structures but also by the impaired circulation of cerebrospinal fluid (CSF) through the ventricular system, leading to posthemorrhagic ventriculomegaly (PHVM). Cranial ultrasound (CUS) performed by neonatologists (point-of-care ultrasound-POCUS) facilitates the early diagnosis of IVH and PHVM and can obje

Neonatal intraventricular hemorrhage (IVH) is a major complication of preterm birth, yet how developmental stage influences the brain's response to injury remains unclear. We performed single-nucleus RNA sequencing on rat brains 24 hours after IVH at postnatal day 2 (PND2) or day 5 (PND5) to define transcriptional responses across cell types. We identified 42 distinct cell populations and found that PND5 brains exhibited a markedly stronger immune and inflammatory response to IVH, with a threefold increase in differentially expressed genes compared to PND2. Microglia were the most perturbed ce

BACKGROUND AND OBJECTIVES: We determined whether white matter injury (WMI) severity and location on early-life vs term-equivalent age (TEA) brain MRI were more strongly associated with 36-month neurodevelopment.
METHODS: Very preterm infants were recruited across 3 tertiary NICUs and underwent early-life and TEA MRI. Moderate-severe WMI severity and anterior or posterior location were scored. 36-month neurodevelopmental assessments were completed with Bayley Scales of Infant Development, Third Edition; delay was defined as a composite score <85 points. BACKGROUND: Intraventricular hemorrhage (IVH) most commonly occurs in infants born very preterm (<32 weeks' gestation). There are mixed findings on whether infants small for gestational age (SGA) or with suspected fetal growth restriction (FGR) are at higher risk for IVH. Understanding the relationship between SGA or FGR and IVH is critical to inform clinical care.
OBJECTIVE: The primary aim was to determine the rates of IVH in very preterm newborns, with SGA or suspected FGR, and to stratify for severity of both FGR and IVH. The secondary aim was to identify risk factors for IVH in a large c

BACKGROUND: Survivors of acute kidney injury (AKI) in very low birth weight (VLBW) infants may be at risk of developing chronic kidney disease in the long term. Large cohort studies examining risk factors associated with AKI in VLBW infants are scarce.
METHODS: We retrospectively investigated the risk factors for the development of AKI by comparing the file records of VLBW infants (<1500 g) born between 2019 and 2022 and admitted to the neonatal intensive care unit with and without AKI.
RESULTS: Of the 218 VLBW infants enrolled, 35 (16%) developed AKI. The AKI group had lower gestational age

BACKGROUND: Acute kidney disease (AKD) is defined as kidney dysfunction lasting 7-90 days after the onset of acute kidney injury (AKI). Although widely recognized in adults and children, its significance in neonates is unclear. We aimed to investigate the risk factors and outcomes of neonatal AKD in a cohort of neonates with AKI.
METHODS: We performed a retrospective cohort study at a tertiary NICU in Japan. Among 345 neonates with AKI between 2014 and 2024, 280 were included after excluding cases with congenital kidney disease, early death (< 7 days), or missing data. AKD was defined by KDIG
