[Early prediction of the injured regions in neonatal brain with hypoxic-ischemic encephalopathy by diffusion weighted imaging and measuring their apparent diffusion coefficient].

OBJECTIVE: To elucidate that diffusion weighted imaging (DWI) can be used to predict the injured regions of neonatal brain with hypoxic-ischemic encephalopathy (HIE) in the early phase of injury, and to measure the apparent diffusion coefficient (ADC) values in the multiple regions of the brain. METHOD: The participants in this study were twenty-six infants with HIE from neonatology ward hospitalized between July 2006 and July 2009. Nineteen patients had severe HIE, and seven had moderate HIE. DWI and conventional magnetic resonance imaging (MRI) were performed for each case within the first

Unilateral retinal hemorrhages with subarachnoid hemorrhage in a 5-week-old infant: is this nonaccidental injury?

PURPOSE: Retinal hemorrhages are an important sign in the setting of nonaccidental injury (NAI) (abusive head injury) in young infants and form a very important part of the evidence in support of the diagnosis. The diagnosis of NAI has serious social and legal implications. Other causes of retinal hemorrhages in an infant, such as birth trauma, accidental head injury, subarachnoid hemorrhage, other less common disorders of clotting, leukemia, and infections such as endocarditis, need to be considered and ruled out in making a diagnosis of NAI. METHODS: Descriptive case report. RESULTS: A 5-w

Differential effects of intraventricular hemorrhage and white matter injury on preterm cerebellar growth.

OBJECTIVE: To hypothesize that detailed examination of early cerebellar volumes in time would distinguish differences in cerebellar growth associated with intraventricular hemorrhage (IVH) and white matter injury in preterm infants. STUDY DESIGN: Preterm newborns at the University of California San Francisco (n = 57) and the University of British Columbia (n = 115) were studied with serial magnetic resonance imaging scans near birth and again at near term-equivalent age. Interactive semi-automated tools were used to determine volumes of the cerebellar hemispheres. RESULTS: Adjusting for supr

Unassisted childbirth or homicide--different appraisals of severe injuries in a newborn.

A case of a 24-year-old woman who gave birth to a mature newborn is reported. Many injuries at the head, neck and shoulders, back, mouth and throat which at least partly indicated unassisted childbirth were observed during autopsy. Some injuries, especially the different scull fractures were discussed controversially on trial. One expert postulated a coaction of unassisted childbirth and blunt head trauma to be responsible for the exitus. The other expert considered it possible that all injuries could originate from unassisted childbirth. The court consented to the opinion that all injuries co

The new conception of mechanism and sequence of damages typical for labor trauma is presented: injury of ligaments of atlas and axis and anterior atlanto-occipital membrane, ruptures of synchondrosises of cranial bones because of vertebral column pressing on occipital bone; tears of dura mater of brain upon basilar impression in the area of focal spot; subarachnoid hemorrhage and intracerebral bleeding in case of cerebral compression by bone borders where they overly override; ruptures of ligaments of cervico-occipital and intervertebral joints in case of excess flexion and overextension of he

Birth injury of the cranium and central nervous system.

Birth injury of the scalp, skull and central nervous system (CNS) is a well-recognized complication of a difficult delivery. The rate of birth trauma has dropped precipitously and now accounts for less than 2% of neonatal deaths. Despite this dramatic decrease in birth-trauma mortality significant injuries still occur. A variety of risk factors clearly predispose certain infants to birth-related injury. Recent neuroradiology studies indicate that intracranial hemorrhage, even in asymptomatic infants, is not rare. Pathologists' (neuropathologists and forensic pathologists) appreciation of the s

Leptomeningeal cyst due to vacuum extraction delivery in a twin infant.

A rare case of a leptomeningeal cyst is reported in a twin male neonate delivered using a vacuum extractor, who presented a huge, non-pulsating, oedematous mass overlying the frontal fontanelle after birth. The mass was initially diagnosed as a cephalo haematoma. Ultrasonography indicated intracranial bleeding and a subsequent CT scan revealed an intraparenchymal bleeding above the left frontal horn, combined with a thin, left-sided, subdural haematoma and subarachnoid haemorrhage in the left Sylvian fissure. Apart from a bulging soft and round formation (2 x 2 x 3 cm) next to the anterior fon

[Assessment of the Doppler cerebral blood flow measurement in infants with perinatal trauma].

BACKGROUND: Birth trauma may lead in infants to developmental delay, cerebral palsy, epilepsy and complications with increasing risk of the death. Several complications are of vascular origin. The aim of the study was the assessment of the transcranial Doppler-sonographic (TCD) measurement of cerebral blood flow in infants with perinatal CNS pathology. MATERIAL AND METHODS: 14 infants, 9 females and 5 males, within 12 premature infants, with the CNS perinatal pathology were examined. In children during the neonatal period respiratory distress syndrome, sepsis, apnoe and seizures were also obs

Neonatal subgaleal hematoma: presentation and outcome--radiological findings and factors associated with mortality.

To describe the presentation and outcome of infants who develop subgaleal hematoma (SGH), we compared perinatal factors, clinical and head imaging findings, and outcome in a cohort (N = 34) of all infants admitted to Saint Louis Children's Hospital neonatal intensive care unit with SGH from January 1991 to June 2003. All except three of the infants admitted with SGH had instrumental deliveries (31 of 34; 91.2%): 21 vacuum, eight vacuum followed by forceps, two forceps). There was also a high frequency of occurrence of associated intracranial hemorrhage (17 of 34; 50%: subarachnoid hemorrhage,

Acquired unilateral coronal craniosynostosis.

Coronal craniosynostosis of both the sporadic and syndromic types have been comprehensively described and extensively investigated. Previously, there have been no cases reported of acquired unilateral coronal craniosynostosis. We present a case of a 22-month-old male who developed a left unilateral coronal craniosynostosis following multiple surgical interventions for birth-related intracranial injuries. The genesis and molecular biology of craniosynostosis are discussed; patient presentation and treatment are reviewed.

Comparison of accidental and nonaccidental traumatic head injury in children on noncontrast computed tomography.

OBJECTIVE: Mixed-density convexity subdural hematoma and interhemispheric subdural hematoma suggest nonaccidental head injury. The purpose of this retrospective observational study is to investigate subdural hematoma on noncontrast computed tomography in infants with nonaccidental head injury and to compare these findings in infants with accidental head trauma for whom the date of injury was known. PATIENTS AND METHODS: Two blinded, independent observers retrospectively reviewed computed tomography scans with subdural hematoma performed on the day of presentation on 9 infant victims of nonacc

Postnatal onset cortical dysplasia associated with infarction of white matter.

On MRI at 35 weeks of a boy born at 25 weeks, focal disorganization of the cortex was observed near a frontal venous infarct developed in the first week. Disruption of the final steps of cell migration, injury to the subplate and/or disruption of corticospinal axons are possible mechanisms behind it. Preterms with white matter lesions at or below 25 weeks postconceptional age should be scrutinized for cortical dysplasia.

Intracranial hemorrhage in asymptomatic neonates: prevalence on MR images and relationship to obstetric and neonatal risk factors.

PURPOSE: To retrospectively evaluate the prevalence of neonatal intracranial hemorrhage (ICH) and its relationship to obstetric and neonatal risk factors. MATERIALS AND METHODS: Pregnant women were recruited for a prospective study of neonatal brain development; the study was approved by the institutional review board and complied with HIPAA regulations. After informed consent was obtained from a parent, neonates were imaged with 3.0-T magnetic resonance (MR) imaging without sedation. The images were reviewed by a neuroradiologist with 12 years of experience for the presence of ICH. Medical r

Fetal and neonatal origins of altered brain development.

Abnormal development of the brain during fetal life is now thought to contribute to the aetiology of many neurological disorders that manifest throughout life. Many factors are likely to underlie such abnormal development including genetic makeup and an adverse intrauterine environment. This review will focus on prenatal hypoxic/ischaemic injury, inflammatory/infective insults and preterm birth. A range of experimental models have been used to characterize lesions formed in response to these insults and to determine mechanisms of damage resulting from such events. Relatively brief periods of f

Part 1: early recognition and treatment of birth trauma: injuries to the head and face.

The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Sentinel Event Alert from July 21, 2004 states that there have been 47 cases of birth trauma-related perinatal death or permanent disability reported for JCAHO review since 1996. This report clearly illustrates the importance of birth trauma in clinical practice for neonatal and perinatal nurses. Estimates suggest that birth trauma occurs in 2% to 7% of all deliveries and is associated with an increase in both mortality and morbidity. Birth trauma to the head may result in minor superficial extracranial injuries, such as

Brain injury in the term infant.

Perinatal brain injury in the term infant is a relatively uncommon event. The principal lesions are intracranial hemorrhage including subarachnoid, subdural, intraparenchymal, intraventricular, focal cerebral infarction and hypoxic ischemic cerebral injury secondary to intrapartum hypoxia-ischemia. Both intracranial hemorrhage and focal cerebral infarction are invariably identified at the time of clinical symptoms, ie, seizures or apnea. This clearly limits the potential for prevention. The mechanisms contributing to brain injury secondary to intrapartum hypoxia-ischemia have become more clear

The value of autopsy in determining the cause of failure to respond to resuscitation at birth.

Autopsy is invaluable in identifying the causes of severe depression and very low Apgar score after birth and in assessing contributory conditions. Brain scans are increasingly used in the care of neonates who fail to respond to resuscitation at birth but their interpretation depends on the information gained from sound neuropathological studies. Asphyxia, both acute intrapartum asphyxia and chronic asphyxia, is an important cause of low Apgar scores. The gestational age and the nature of the asphyxial insult both have a profound influence on the ultimate pattern of injury. Asphyxia in the pre

Defining the nature of the cerebral abnormalities in the premature infant: a qualitative magnetic resonance imaging study.

OBJECTIVES: The aim of this study was to define qualitatively the nature and extent of white and gray matter abnormalities in a longitudinal population-based study of infants with very low birth weight. Perinatal factors were then related to the presence and severity of magnetic resonance imaging (MRI) abnormalities. METHODS: From November 1998 to December 2000, 100 consecutive premature infants admitted to the neonatal intensive care unit at Christchurch Women's Hospital were recruited (98% eligible) after informed parental consent to undergo an MRI scan at term equivalent. The scans were an

Comparison of maternal and infant outcomes between vacuum extraction and forceps deliveries.

The authors conducted a population-based historical cohort study in the Canadian province of Quebec to assess the maternal and infant outcomes associated with vacuum extraction and forceps deliveries. The study database contains information on 305,391 mother-infant dyads (linked by a common institutional code and hospital chart number) for singleton live vaginal births with a nonbreech presentation at the gestational age of 37 or more completed weeks and a birth weight between 2,500 and 4,000 g during fiscal years 1991/1992 to 1995/1996. Of the births, 31,015 were delivered by vacuum extractio

[History, controversy and pathogenesis].

In this paper, we discuss the historical and pathophysiological aspects of syringomyelia. Defined as fluid cavities extending beyond several segments within the spinal cord this pathological entity is a condition with many possible causes. Hindbrain herniation is the commonest foramen magnum abnormality associated with the so called "hindbrain related syringomyelia". A history of birth injury, a small posterior fossa, an arachnoid scarring of the basal cisterna, a segmentation abnormality of the superior cervical vertebrae, a hydrocephalus or arachnoid cyst of the posterior fossa are often pre
