We conducted a retrospective study of 32 patients treated for syringomyelia associated with non-traumatic arachnoid scarring. We selected the cases with documented history of arachnoiditis with pre and post-operative clinical evaluation of the neurological status and anatomical study on MRI with a minimal follow-up of one year. Extensive arachnoid scarring (Group I) was noted in 18 cases, after spinal meningitis in 15 cases (tuberculosis in 9 cases, listeria in 3 cases, pyogenic meningitis in 3 cases), subarachnoid hemorrhage in 3 cases. Focal arachnoid scarring (Group II) occurred in 10 cases

The purpose of this pictorial review is to demonstrate the imaging features of a range of cranial injuries associated with birth at term. Scalp and skull injuries include simple cephalhaematoma, osteomyelitis of the skull vault from infected cephalhaematoma, subgaleal haemorrhage and skull fracture. Intracranial injuries include extradural, subdural and subarachnoid haemorrhage, leptomeningeal cyst and brain infarct. Radiologists need to be aware of the range of cranial injuries following birth and their differentiating features.

BACKGROUND AND PURPOSE: The purpose of our study was to determine whether MR studies in the neonatal period are predictive of the neuroradiologic sequelae and clinical outcome in premature and term infants with perinatal brain injury.
METHODS: Thirty subjects (15 premature and 15 term infants) with abnormalities revealed by initial MR studies were reexamined approximately 1 year after birth with both MR imaging and a neurologic assessment. All initial MR studies were performed between 35 and 45 weeks corrected age in premature infants and within 28 days of life in term infants. The initial MR

BACKGROUND: Intracranial hemorrhage in a full-term infant is uncommon, is usually subarachnoid in type, and is usually associated with operative vaginal delivery or asphyxia.
CASE: A 15-year-old primigravid woman at 37 weeks' gestation developed a prolonged second stage of labor associated with persistent occiput posterior position. With the onset of pushing, baseline fetal heart rate (FHR) decreased and variability increased. Thirty minutes before vaginal delivery, sudden fetal tachycardia (up to 210 beats per minute) was observed, with absent variability and minimal decelerations. At birth,

We prospectively studied 653 consecutive head-injured children (less than or equal to 14 years old) treated over a 54-month period (1984-88) at the Department of Neurosurgery of the Rambam Medical Center (Haifa, Israel). Demographic and clinical data were collected, the patients were divided into five age groups (birth to 2 years, 169; 3-5 years, 194; 6-9 years, 164; 10-12 years, 77; and 13-14 years, 49), and the information relative to each was then compared. All patients (except three who died on the operating table) underwent computed tomography (CT) scans; 225 (34.6%) had intracranial path

Twenty five asphyxiated newborns (seventeen term and eight preterm) with mean gestational age of 37 weeks (range 28-48 weeks) and mean birth weight of 2.4 kg (range 0.75 kg to 3.5 kg), respectively, constituted the cases in present study. Normal CT scan was found in five term (29.4%) and two preterm babies (25%). CT abnormalities noted in term babies included hemorrhage (subarachnoid 5.8%, intracerebral 11.6%), hypodensity (mild 23.2%, moderate 11.6% severe 5.8%); hypodensity with hemorrhage 5.8% and cerebral atrophy 5.8%. In the preterm babies abnormalities included intraventricular hemorrhag

The authors report their experience in 8 cases of cranial post-traumatic superficial collections, studied by US. Traumatic cranial injuries with superficial collections are not rare in newborns; their early and complete diagnosis is necessary for a correct therapeutic approach. Sonography could be used as a first choice procedure but presents some limitations in the study of the posterior fossa. Therefore CT remains the technique of choice.

A statistic analysis of 1,421 perinatal autopsies from our institute in 30 years divided into 5-year groups, matched with clinical data was reported. Dystocia rate was 2.3 times increased mainly because the number of cesarean section was increased 6.8 times. The rate of intracranial hemorrhage has not been reduced significantly in the 6-groups. However in the last 5 years intracranial hemorrhage due to trauma as result of dystocia has decreased. The development of intracranial hemorrhage depends upon the birth weight and the degree of anoxia: the lower the birth weight, the more severe the ano

Five full-term neonates with a posterior fossa subdural hematoma caused by birth injury are reported. All of the patients were successfully treated with surgery. They all presented with biphasic ventricular dilatation as demonstrated by serial computerized tomography (CT) scanning. The initial dilatation due to obstructive hydrocephalus disappeared after removal of the hematoma; however, a second dilatation without obstruction of the ventricular system developed between 2 and 4 weeks after surgery. A ventriculoperitoneal shunt was required in one case because of progressive enlargement of the

Intracranial hemorrhage in neonates is often found in either the subependymal area or the subdural space. The former is observed particularly in premature infants and is attributable to damage of the germinal matrix layer. The latter usually occurs in the vicinity of the falx and tentorium cerebri of full-term neonates and is thought to be caused by birth injury. Two cases of intracerebral hemorrhage in full-term newborn babies are reported. In both the hematoma was located at the left frontal area under the coronal suture. Angiograms revealed no causative signs, such as arteriovenous malforma

A woman in labor and not wearing a seat belt sustained multiple fractures of the pelvis and femur while in an automobile accident. The fetus suffered fractures of the parietal and occipital bones. To minimize maternal pelvic and fetal injury, delivery by cesarean section was performed. The infant had generalized cerebral edema and intraventricular and subarachnoid hemorrhage, diagnosed shortly after birth. Motor abnormalities and residual injury to the periventricular white matter were evident at age 4 months. Use of seat belts may significantly diminish or prevent such injuries.

Posterior fossa hemorrhage detected by computed tomography (CT) in seven term newborns was managed by nonsurgical means. All infants were developmentally normal two to four years later with the exception of one infant who died with a massive posterior fossa hemorrhage. Two infants with extensive hemorrhages developed hydrocephalus. Spinal fluid drainage and diuretic therapy resulted in the resolution of hydrocephalus in one infant and delayed the placement of a ventriculoperitoneal shunt in the other. This series documents normal outcome in nonsurgically managed neonatal posterior fossa hemorr

Fifty-eight (6,2%) cases of hemorrhages into the cerebellum (HC) that were found among 899 perinatal and neonatal necropsies were studied; hemorrhages less than 0,5 cm in diameter were not included into the material examined. HC were prodominantly observed in premature newborns. The main cause of death of newborns and intranatally dying fetuses with HC was a skull trauma occurring during delivery. HC were subdivided into 3 groups: 1) those provoked by pressure on the upper surface of cerebellum hemispheres followed by vessel compression; 2) those provoked by blood stasis in cerebellum venae as

Over a five-year period we identified 22 term newborns with intracranial hemorrhage by computed tomography in an intensive care unit for newborns. Primary subarachnoid hemorrhage (diffuse or focal) was the most common type of hemorrhage. Diffuse subarachnoid hemorrhage was caused either by traumatic delivery or severe hypoxic-ischemic encephalopathy and caused seizures on the first day. Focal subarachnoid hemorrhage was associated with cerebral infarction. Intraventricular hemorrhage was always accompanied by bloody CSF. Somewhat more than half the newborns with intraventricular hemorrhage had

Four hundred and thirty-one vacuum extraction deliveries were reviewed during a five-year period from 1978 to 1983. All cases involved the use of the Kobayashi silastic cup vacuum extractor with nine participating obstetricians performing the procedures. Specific attention was directed toward maternal and fetal morbidity associated with the use of the silastic cup extractor. No maternal or fetal deaths occurred. Vaginal and cervical lacerations accounted for a 4% maternal morbidity rate. Seventy-five percent of the infants had Apgar scores greater than 7 at one minute, and 97% had scores great

An intracranial haemorrhage was diagnosed by computerised tomography in 35 term neonates. Subarachnoid haemorrhage was found in 23, intracerebral haemorrhage in 9 and subdural haematoma in 3 patients. Perinatal risk factors indicating trauma or hypoxic events were evaluated in more than 50%. Clinical symptoms were independent of the site of the bleeding. An additional hypoxic-ischaemic brain injury seemed to be the cause of the observed clinical symptoms in many cases. The outcome was significantly better in patients with subarachnoid haemorrhage than in intracerebral haemorrhage or subdural h

In the literature, there has been no report of spontaneously growing calcified chronic subdural hematoma (C-C-SDH). The authors report a five-year-old boy with spontaneously growing C-C-SDH. The patient was admitted to author's department on July 24, 1979, because of progressive right hemiparesis, seizure and mental as well as physical retardation. He had a birth injury caused by forceps delivery resulting in skull fracture and subarachnoid hemorrhage. Three months after birth, gradual enlargement of his head circumference was noticed. A ventriculoperitoneal shunt was performed under the diagn

A review of the current knowledge about a variety of aspects of infantile hydrocephalus suggests that one cause of this problem is subarachnoid blood from birth trauma interacting with an anatomically immature cerebrospinal fluid-absorbing system. To test this hypothesis and to create a model that could be used to devise various methods of prophylaxis and therapy, we injected autologous blood into the cisterna magna of puppies, followed the size of their ventricles with serial computerized tomographic (CT) scans, and evaluated the scanning electron microscopic (SEM) appearance of the ventricul
