In the case of 17 obstetrical facial paralyses occurring in a total of 20,409 births, the authors have studied the circumstances of their appearance and the prognosis for this lesion. It is a rare lesion favoured by asymmetric purchase when using articulated forceps. The authors insist that use of Thierry's forceps do not harm the facial nerve. They emphasise the possibility that progression may not always be favourable.

We present a newborn with a unilateral complete facial palsy caused by birth trauma, and discuss the differential diagnosis, pathophysiology, and management of this common, usually benign, condition. This child made a rapid, complete, spontaneous recovery despite a severe initial injury. Surgical exploration of the facial nerve should be considered only for infants with complete paralysis, clinically and electrophysiologically, who demonstrate no improvement by 5 weeks of age.

We have been able to find 291 cases of obstetric trauma when we analysed the 20,409 deliveries which took place in the Pitié maternity between 1971 and 1980. It has been shown that bony lesions did well in the long and short term but nerve lesions had a far less good prognosis. This pathology should become far less when it is possible to recognize better when the fetus is large and when such traumatising manoeuvres as high forceps and assisted deliveries of large breech babies are stopped.

The authors retrospectively studied 13,870 singleton full-term consecutive live births at a major teaching hospital and assessed the frequency of brachial plexus injury, clavicular fracture, and facial nerve injury in newborns delivered from January 1974 through December 1977 and from January 1979 through September 1981. Predisposing factors relating to mechanical birth trauma were defined, and a risk assessment profile was developed to identify the fetus at risk. Mechanical injuries were identified according to the International Classification of Diseases codes 761. The incidence of selected

Forty-four cases of neonatal facial paralysis have been treated in the ENT department for children in the hôpital Trousseau, paris. In nearly half of the cases the etiology was related to nervous or muscular aplasia, while in the other cases of traumatic, infective, or doubtful etiology, the severity and course of the paralysis served as a guide to choice of therapy. Surgery was indicated in only 13 cases and recovery was obtained in 4 infants, with more or less marked sequelae. Prognosis is very poor in cases treated at a late stage, especially when the nerve appears to be greatly altered or

Neither clinical examination nor electrical or radiological investigations were able to predict, pre-operatively, the malformatory or traumatic etiology in 9 cases of neonatal facial paralysis. Surgical exploration demonstrated that the paralysis was due to trauma in 3 cases and to malformations in the other 6. Exploration of the intrapetrous portion of the facial nerve in these 9 cases was not satisfactory, as in 6 patients nerve agencies was detected. Simultaneous investigation of the parotid region in 5 of these 6 infants demonstrated absence of the intraparotid part of the facial nerve. Wh

The differential diagnosis in 170 patients between birth and 18 years of age is reviewed. There are a number of obvious physical findings and historical features that allow one to make a diagnosis rather quickly. Pain, vesicles, a red pinna, vertigo, and sensorineural hearing loss suggest herpes zoster oticus. Slow progression beyond three weeks, recurrent facial paralysis involving the same side, facial twitching, weakness, or no return of function after six months indicate a neoplasm. Bilateral simultaneous facial paralysis indicates a cause other than Bell's palsy, such as Guillain-Barré sy

Intratemporal lesions of the facial nerve can be divided into operative lesions, lesions due to birth trauma, trauma through the external auditory canal, gunshot injuries, lesions caused by fractures of the temporal bone and lesions due to intratemporal tumours. Bell's palsy and palsies due to inflammation are not included in this paper. The lesions caused by fractures are discussed in more detail. In longitudinal fractures the nerve is always damaged at the site of the geniculate ganglion and not at the pyramidal segment, as accepted until now. Of the lesions caused by tumours the neurinomas
