Risk and protective factors for birth-related clavicle fracture and brachial plexus birth injury: a national cohort study.

OBJECTIVE: Clavicle fracture is the most common birth-related fracture, resulting in concerns and dissatisfaction, while brachial plexus birth injury (BPBI) is a severe complication with potential lifelong sequelae. The study investigated how the incidence of reported clavicle fracture and BPBI has changed over time in Sweden. DESIGN: A register-based national cohort study (2010-2022). SETTING: The Swedish Medical Birth Register. PATIENTS: All term singleton live births in Sweden with birth-related clavicle fracture or BPBI were compared to infants without these diagnoses. Elective caesarea

Fetal parietal bone fracture in late 3rd trimester secondary to traffic accident with improper seatbelt use: a case report.

Approximately half of the women in developed countries drive motor vehicles and the consequences of road traffic-related injuries involving pregnant women can be severe. This case report describes a rare injury of a parietal bone fracture in a fetus resulting from improper seatbelt use during a motor vehicle accident. A 36-year-old woman at 35 weeks of gestation was involved in a traffic accident while seated in the front seat wearing a seatbelt, but in an improper way for pregnant women. Transabdominal ultrasonography revealed a displaced fracture of the fetus's right parietal bone with an ov

Incidence, pattern and mechanisms of injuries and fractures in children under two years of age: a population-based study.

BACKGROUND: Epidemiological data on injuries and fractures in children under two years of age is limited, thus, we aimed to explore the incidence, patterns and mechanisms. METHODS: Retrospective, population-based cross-sectional study including children under two, seen at the A&E department due to a trauma, or having a radiograph due to birth trauma. All radiographs were re-reviewed. RESULTS: Four hundred thirty children (49.8% female), mean age 14.7 months (SD 6.8), were included, of whom four had a high-energy trauma and 42 were birth-related. Of 388 children (50.3% female) (mean age 16.3

Congenital depressed skull fracture in a neonate without obstetric trauma.

UNLABELLED: Congenital depressed skull fractures (ping-pong fractures) without obstetric trauma are rare. A term male neonate delivered via uncomplicated caesarean section, demonstrated a right parieto-temporal skull depression (5 cm × 5 cm) at birth. Computed tomography revealed a 4 mm parietal depression without intracranial injury. No instrumental delivery or maternal trauma were present. The likely aetiology was intrauterine compression ('faulty foetal packing'). The patient was managed conservatively with close follow-up. CONTRIBUTION: This case underscores the importance of perinatal hi

Congenital Vault Depression in an Infant Requiring Manual Pelvis Disengagement During Delivery.

Congenital vault depression (CVD) is a rare neonatal cranial finding that typically results from chronic intrauterine pressure and remodeling rather than acute delivery trauma. To the best of our knowledge, this report is the first that describes CVD in a term infant delivered via Caesarean section requiring manual pelvic disengagement due to a significantly impacted fetal head. Upon delivery, a 4 x 3 cm depression was noted in the right temporoparietal bone; however, the infant remained neurologically intact, and imaging via X-ray and computed tomography confirmed a concave deformity without

Impacted Fetal Head at Cesarean Birth: A Clinical Nursing Review.

Fetal head impaction (IFH) occurs when the presenting part becomes wedged in the maternal pelvis during a cesarean section, requiring additional maneuvers to complete birth. As cesarean rates have risen, so has IFH, estimated to occur in 1.5% of all births annually in the United States. Maternal complications include uterine extension, hemorrhage, bladder injury, and intensive care unit admission. Neonatal complications include low cord pH, skull fracture, intracranial hemorrhage, hypoxic-ischemic encephalopathy, and death. These outcomes are not inevitable. Management technique, clinician edu

Honeybee comb geometry emerges from local redistribution of angular space by worker construction activity.

BACKGROUND: Mirtazapine has a unique mechanism of antidepressant action, and thus is thought to have a different profile of adverse events from that of other antidepressants. OBJECTIVE: To present a methodologically rigorous systematic review of the adverse event profile of mirtazapine and point to possible problems with safety reporting in randomized controlled trials (RCTs) of the acute-phase treatment of major depression in adults with mirtazapine in comparison with other types of antidepressant. METHODS: The Cochrane Collaboration Depression, Anxiety and Neurosis Controlled Trials Regist

Risk Factors for Neonatal Clavicular Fractures: A Population-Based Case-Control Study.

BACKGROUND: Neonatal clavicular fractures represent the most common fracture during delivery. We aimed to define risk factors associated with these fractures in a large population-based database. METHODS: Data were extracted from Clalit Health Services' electronic health records from 2000 to 2020. Newborns with clavicular fractures were compared with a healthy control group. The following parameters were compared-for the newborns: sex, birth weight, birth height, and head circumference; for the delivery process: assisted delivery, cesarean section, use of epidural, birth week, and number of f

Femur Fracture in a Newborn During Emergency Cesarean Delivery: A Case Report.

Birth injury occurs when the delivery process is not appropriately attended, and the use of improper techniques or maneuvers while conducting the delivery. Cesarean delivery is considered safe as compared to vaginal for the breech presentation. However, this case reports a case of femur fracture of a newborn that occurred during an emergency cesarean section performed for breech presentation. This highlights cesarean section for malpresentation is not completely safe.

Risk factors for shoulder dystocia-related brachial plexus injuries: A case-control study.

INTRODUCTION: Brachial plexus injury (BPI) is one of the major complications associated with shoulder dystocia. This study aimed to identify risk factors for shoulder dystocia-related BPI (SD BPI) in the Finnish population. MATERIAL AND METHODS: The study included all deliveries in the Hospital District of Helsinki and Uusimaa between 2006 and 2015 (n = 181 352). Singleton deliveries complicated by shoulder dystocia, BPI, and clavicle fracture were identified using ICD-10 codes O66.0, P13.4, P14.0, and P14.1 (n = 1708). Following a thorough review of medical records, 374 cases of BPI were ide

Birth-Related Traumatic Proximal Femoral Physeal Separation: A Case Report.

CASE: A full-term baby, born through emergency cesarean section, presented with pain and limited movement in his left leg. Radiographs at day 5 demonstrated superolateral displacement of the left femur, initially interpreted as a congenital hip dislocation. Ultrasound imaging demonstrated a normally positioned femoral head with no acetabular dysplasia, and repeat radiographs at 2-weeks revealed callus formation. Separation of the proximal femoral epiphysis was suspected and subsequently confirmed with magnetic resonance imaging. No active intervention was recommended, and 1-year radiographs de

[Forensic assessment of birth trauma in a full-term newborn: a case report].

The article describes a forensic medical examination of combined birth craniocerebral trauma in a full-term newborn, with a double, tight umbilical cord entanglement around the neck. The manifestations of the injuries in this case were: craniocerebral and spinal injuries, a fracture of the left clavicle. The connection between the injury and the use of an obstetric remedy - removal/cutting of the umbilical cord loops from the fetus's neck - has been proven.

Neonatal Physeal Fracture: What Treatment?

BACKGROUND: The management of diaphyseal fractures in newborns is well established as conservative, involving immobilization without reduction. Newborn physeal fractures (NPF) are rarer lesions, and their management is poorly codified. In the literature, treatment varies from simple monitoring to surgical reduction and osteosynthesis. The aim of this study was to describe the long-term evolution of NPFs according to their location and initial management, and to establish treatment guidelines. METHODS: The first part of the study involved a systematic review of the literature according to PRIS

A Rare Case of Femoral Shaft Fracture Linked to Childbirth.

Birth injuries occur rarely during Cesarean section thanks to antenatal care, skilled obstetricians, and decreased instrumental maneuvers. Most traumatic fractures during childbirth involve the clavicle and humerus, which are related to malposition and malpresentation. We report a rare case of femoral shaft fracture at childbirth. A 19-year-old primigravida with a normal antenatal history delivered a term newborn with a 2.14 kg birth weight by Cesarean section. A day after birth, the mother noticed a swelling on the baby's right thigh with mild deviation of the right limb compared to the left
