An Atypical Case of Exophiala Bergeri Chromoblastomycosis in an Immunosuppromised Individual.

Chromoblastomycosis (CBM) is a rare chronic, granulomatous mycosis affecting the skin and subcutaneous tissue. It is a deep, slow growing, fungal infection that most often presents with epidermal changes that clinically appear as a hypertrophic and verrucous plaque. We describe an atypical presentation of an immunocompromised male patient, age 76 years, who presented with a 4-month history of painless subcutaneous firm nodules of the left fifth finger. The fungal culture grew Exophiala bergeri, and histopathology showed medlar bodies with lack of pseuodepitheliomatous hyperplasia or epidermal

A Case of Chromoblastomycosis Caused by Knufia epidermidis Presenting as Multiple Dark Brown Papules in the Nasal Area.

A Japanese man in his 70s presented with multiple blackish-brown papules on the nasal wing region, which had appeared 3 months prior to his initial visit. He was initially treated at a local clinic with clindamycin phosphate gel, but showed poor improvement. Subsequently, he visited our hospital for further evaluation. A skin biopsy was performed, granulomas consisting of histiocytes were observed in the tissue, and spore-like brown cells were observed sporadically within the granulomas. Fungal culture of biopsy tissue showed the development of black villiform colonies. Molecular biological id

From neglected to notable: A growing public health challenge driven by hospitalization for sporotrichosis in Pernambuco, Northeast Brazil (2016-2024).

Opportunistic and endemic mycoses represent a significant global health concern, particularly in Brazil. In the northeastern state of Pernambuco (PE), sporotrichosis has become a rapidly escalating public health challenge, evidenced by a notable increase in hospital admissions, despite its mostly outpatient nature. The present study, therefore, aimed to investigate the epidemiological and clinical profile of mycosis-related hospitalizations in PE from 2016 to 2024, with a primary focus on the burden of sporotrichosis. This statewide surveillance study used data from Brazil's Unified Health Sys

Elephantiasis and verrucous lesions in an Ethiopian man living in Italy.

Elephantiasis refers to chronic, severe lymphedema caused by the abnormal accumulation of interstitial fluid due to reduced lymph transport and lymph stasis. This condition is observed secondary to injury, infection, cancer or cancer-related treatment, or congenital abnormalities of the lymphatic system. One of the several forms of elephantiasis is Elephantiasis Nostras Verrucosas (ENV), where "nostras" refers to a non-filarial form of lymphedema that occurs primarily in non-tropical areas. We describe a case of ENV in an Ethiopian man living in Italy, presenting with fever, hypotension, woody

Calcineurin regulates morphological development, stress responses and virulence in Fonsecaea monophora.

Fonsecaea monophora is a major cause of chromoblastomycosis (CBM) in southern China. While calcineurin is known to be a virulence factor in several fungi, its role in F. monophora remains poorly understood. In this study, we characterized the function of calcineurin in F. monophora by examining mutants of the two calcineurin subunit genes (cnaA and cnaB). The mutants exhibited significant defects in conidiation, germination, morphogenesis (including the transformation into muriform cells), resistance to various stressors, and increased susceptibility to triazole drugs. Importantly, the mutants

Chromoblastomycosis in an Immunocompetent Child: A Case Report.

Chromoblastomycosis is a chronic cutaneous infection caused by dematiaceous pigmented fungi. Lesions may begin as an erythematous papule, which progresses to a hyperkeratotic plaque. Management of chromoblastomycosis is usually challenging. We report a case of a 12-year-old immunocompetent male child with chromoblastomycosis, characterized by warty, hyperkeratotic plaques on the face. The diagnosis was made by histopathology and clinical examination. Histopathology confirmed the presence of a brownish, dematiaceous, fungal organism with internal septations. The patient was treated with oral it

In Vitro Activities of Five Antifungal Drugs Against Conidia and Sclerotic Cells of Chromoblastomycosis Agent Fonsecaea spp.

Chromoblastomycosis (CBM) is a chronic skin and subcutaneous infection mainly caused by Fonsecaea pedrosoi, a dematiaceous fungus with various morphotypes. Characteristic sclerotic cells-globe-shaped, multiseptated and pigmented-are found in lesions of infected individuals, though their differentiation in the host remains poorly understood. To evaluate in vitro activity of five antifungal drugs-itraconazole (ITZ), posaconazole (PCZ), voriconazole (VCZ), fluconazole (FCZ), and caspofungin (CAS)-against Fonsecaea spp. conidia or sclerotic cells, assessing their minimum inhibitory concentration (

Microbial biodegradation of polyethylene terephthalate microplastics by an indigenous Candida tropicalis strain and biocompatibility evaluation of microplastics-degraded metabolites in GIFT Tilapia.

Microplastic pollution is known to impact various abiotic and biotic components of ecosystems. Among the various microplastic remediation methods, biological approaches using microbes have gained significant attention due to their high efficacy and eco-friendly nature. In this study, the degradation of PET-based microplastics was carried out by the indigenous yeast strain Candida tropicalis in in vitro batch degradation experiments. The degradation was measured by monitoring the optical density of the yeast grown on a medium containing microplastic pieces, as well as assessing weight reduction

When to suspect and how properly early detect and treat patients with endemic mycoses.

Endemic mycoses are caused by dimorphic fungi and eventually molds, as the case of implantation mycoses. In general, these diseases are acquired through trauma or inhalation of fungal elements in the environment, and less frequently by zoonotic acquisition or transmitted during organ transplantation. The target population for endemic mycoses is usually represented by normal hosts with low-income and intensive outdoor activities. Awareness of these diseases remains limited, even in regions with high prevalence, resulting in delayed diagnosis, and affecting the quality of life and outcomes of pa

Potassium Iodide in Dermatology- Recent Advances in Mechanism of Action, Preparation, Uses and Adverse Effects.

Potassium iodide (KI) is a simple yet versatile drug. It has been successfully used for the treatment of various diseases in dermatology, but there are very limited studies on the exact mechanism of action, preparation, and dosage of potassium iodide, causing concerns regarding its toxicity. This article reviews the multimodal mechanism of action of KI and standardises the method of preparation of a saturated solution of potassium iodide (SSKI) on an OPD basis. It also lists the existing and newer indications, optimal dosages, adverse effects, and contraindications of potassium iodide, thereby

Subcutaneous mycoses - The hidden mould, a case series.

Subcutaneous mycoses are a heterogenous group of infections common in subtropical and tropical regions. These infections have an insidious onset and slowly evolve as the pathogen survives and adapts to the host tissue environment. Common subcutaneous fungal infections include Sporotrichosis, Chromoblastomycosis, Mycetoma, Subcutaneous Zygomycosis, Subcutaneous Phaeohyphomycosis, Lobomycosis and Rhinosporidiosis. Even with rapid urbanization, the incidence of subcutaneous mycoses is on the rise. Confirmation by histopathological examination is necessary.

Concurrent chromoblastomycosis and eumycetoma: a unique case of dual neglected tropical fungal diseases in Asia.

Chromoblastomycosis (CBM) and mycetoma, as implantation mycoses, have been listed as neglected tropical diseases (NTDs) by the World Health Organization. The concurrent occurrence of these two NTDs in a single patient is extremely rare. A 69-year-old female patient presented with papules on the dorsum of her left hand for over 5 months and nodules on the left lower limb accompanied by ulceration and pain for 20 days. Histopathological examination of the papule on the dorsum of the left hand revealed muriform cells and fungal culture of the tissue identified Fonsecaea monophora. Microscopic exa

Approach to Diagnosing and Managing Implantation Mycoses.

Chromoblastomycosis, subcutaneous phaeohyphomycosis, and mycetoma are implantation mycoses that cause substantial morbidity and decreased quality of life. Limited data on the global epidemiology of these diseases are available in addition to a lack of robust diagnostic and clinical options. The skin is the key starting point for the diagnosis and management of these infections, the steps for which are outlined in this article.

Widespread Verrucous Plaques of Chromoblastomycosis: A 12-Year Diagnostic Odyssey.

Chromoblastomycosis is a deep fungal infection of the skin and subcutaneous tissue. This case underscores the critical importance of thorough laboratory investigations, particularly histopathology, in chronic dermatological conditions that mimic other granulomatous diseases. Misdiagnosis, such as treating chromoblastomycosis as cutaneous tuberculosis without confirmatory testing, can delay appropriate therapy and prolong morbidity.

The Diagnostic Value of Autofluorescence in Hematoxylin-Eosin-Stained Sections for Cutaneous Sporotrichosis.

BACKGROUND: Previous studies suggest that autofluorescence of pathogenic fungi in hematoxylin-eosin (HE) stained sections under ultraviolet (UV) light may offer rapid screening for fungal infections. AIMS: To evaluate the diagnostic utility of autofluorescence in HE-stained sections for cutaneous sporotrichosis and compare it with Periodic acid-Schiff (PAS) and Gomori's methenamine silver (GMS) staining. METHODS: A retrospective analysis included 49 fungal culture-confirmed sporotrichosis cases and eight chromoblastomycosis controls from 2022 to 2024. HE-, PAS-, and GMS-stained sections were
