Fractures in the under-3-year-old age cohort.

One hundred twenty-four urban children under 3 years of age admitted for fractures were retrospectively reviewed to determine the frequency of accidental and nonaccidental causes in this population. The fractures were categorized according to their mechanisms: motor-vehicle passenger or pedestrian accident, other accidents, or child abuse. There were no differences in the frequency of fractures by race, date of birth, or season in which the injury occurred. Skull fractures were most frequent (62%), followed by femur fractures (11%). There was a 26% increase in fractures between 1987 and 1989,

Frontal plagiocephaly: synostotic, compensational, or deformational.

Plagiocephaly is a term commonly used to describe congenital forehead asymmetry. Sixty patients with frontal plagiocephaly were evaluated retrospectively and separated into three types: synostotic (N = 24), compensational (N = 3), and deformational (N = 33). Categorization of frontal plagiocephaly as synostotic or deformational was reliably made by physical examination, focusing on the supraorbital rims, nasal root, ears, and malar eminences. Other anatomic parameters useful in the differential diagnosis included chin point, palpebral fissures, and facial height. This study documented that bir

Locking and reverse molding of the fetal skull.

In 1972 Emery described a condition that he called "locking and reverse moulding of the fetal skull." Crossing and fixation (locking) of the fetal cranial bones prevented normal skull molding, thus giving rise to pressure on the basal structures of the brain. We describe 7 such cases and compared them with the 9 reported by Emery. Multiple hemorrhages were found, frequently at the base of the brain. All pregnancies had gone near or to term. Most mothers were primiparous and labor was often complicated. In several of our cases there was clinical evidence for cephalopelvic disproportion. The bab

Experimental odontogenic tumors produced by ethylnitrosourea injections and mechanical injuries.

The present study was carried out to investigate odontogenic tumor induction in the rats by injections of carcinogen N-ethyl-N-nitrosourea (ENU) coupled with incisional wounds. The animals which received local injections of ENU in the region of incisor tooth germ of the right mandible every other day for 19 days after birth coupled with incisional wounding in the same region at 2 and 8 days, developed odontogenic carcinomas. However, the animals which were given local injections of ENU in the region of incisor tooth germ but did not receive incisional wounds, showed no pathologic changes. The

The association between skull fracture, intracranial pathology and outcome in pediatric head injury.

We prospectively studied 653 consecutive head-injured children (less than or equal to 14 years old) treated over a 54-month period (1984-88) at the Department of Neurosurgery of the Rambam Medical Center (Haifa, Israel). Demographic and clinical data were collected, the patients were divided into five age groups (birth to 2 years, 169; 3-5 years, 194; 6-9 years, 164; 10-12 years, 77; and 13-14 years, 49), and the information relative to each was then compared. All patients (except three who died on the operating table) underwent computed tomography (CT) scans; 225 (34.6%) had intracranial path

An exploratory case-control study of brain tumors in children.

An exploratory case-control study of childhood brain tumors was conducted in southern Ontario between 1977 and 1983, on 74 cases and 138 age- and sex-matched population controls. A significantly elevated risk (perhaps due to early case symptoms) was seen for skull X-rays at least 5 years prior to diagnosis, and for head or neck injuries which required medical attention. However, no evidence of an increased risk appeared for exposure to sick pets or to pesticides, maternal or paternal history of smoking, and various birth characteristics or antenatal exposure of the child, though these have pre

Biphasic ventricular dilatation following posterior fossa subdural hematoma in the full-term neonate.

Five full-term neonates with a posterior fossa subdural hematoma caused by birth injury are reported. All of the patients were successfully treated with surgery. They all presented with biphasic ventricular dilatation as demonstrated by serial computerized tomography (CT) scanning. The initial dilatation due to obstructive hydrocephalus disappeared after removal of the hematoma; however, a second dilatation without obstruction of the ventricular system developed between 2 and 4 weeks after surgery. A ventriculoperitoneal shunt was required in one case because of progressive enlargement of the

[Differential roentgenodiagnosis of congenital craniocerebral hernias].

Ninety-three patients with congenital craniocerebral hernia were subjected to clinical and X-ray examination. Signs characteristic of each type of congenital craniocerebral hernias were detected. Congenital craniocerebral hernia was combined with other developmental anomalies of the skull and brain in 31% of cases. Differential X-ray diagnosis is made between congenital craniocerebral hernias and traumatic birth hernias, traumatic midline hernias, cholesteatoma, dermoids and epidermoids of the orbit, polyps, papilloma, and fibrous tumors. It was established that X-ray examination plays a very

Posterior fossa hemorrhage in term newborns.

Posterior fossa hemorrhage detected by computed tomography (CT) in seven term newborns was managed by nonsurgical means. All infants were developmentally normal two to four years later with the exception of one infant who died with a massive posterior fossa hemorrhage. Two infants with extensive hemorrhages developed hydrocephalus. Spinal fluid drainage and diuretic therapy resulted in the resolution of hydrocephalus in one infant and delayed the placement of a ventriculoperitoneal shunt in the other. This series documents normal outcome in nonsurgically managed neonatal posterior fossa hemorr

Congenital unilateral orbital fibrosis with suspected prenatal orbital penetration.

Congenital fibrosis of the extraocular muscles includes various abnormalities ranging from isolated fibrosis of a single muscle to bilateral involvement of all extraocular muscles. Congenital unilateral orbital fibrosis is a specific fibrosis syndrome with only five cases previously reported. Although these reports postulated that a prenatal orbital inflammatory process caused the fibrosis, no etiology was proposed, nor was a mechanism suggested for entrance of the inflammation into the orbit. We discuss three additional cases in which evidence suggested prenatal orbital penetration. In one, a

Clinical assessment of the Kobayashi vacuum extractor.

Four hundred and thirty-one vacuum extraction deliveries were reviewed during a five-year period from 1978 to 1983. All cases involved the use of the Kobayashi silastic cup vacuum extractor with nine participating obstetricians performing the procedures. Specific attention was directed toward maternal and fetal morbidity associated with the use of the silastic cup extractor. No maternal or fetal deaths occurred. Vaginal and cervical lacerations accounted for a 4% maternal morbidity rate. Seventy-five percent of the infants had Apgar scores greater than 7 at one minute, and 97% had scores great

Intrauterine depressed skull fractures of the newborn.

Intrauterine depressed skull fractures are reported only occasionally. In reviewing the literature it seems clear that pressure of the fetal head against the maternal bony structures, mainly the sacral promontory, accounts for most of the so-called "spontaneous" congenital depressed skull fractures. Rather than true fractures, they are focal congenital moulding depressions. On the other hand, trauma to the mother's abdomen and traumatic delivery are accepted pathological mechanisms for such lesions; most commonly they are ascribed to inexpert application of the forceps blades or undue force by

Deviated nasal septum. Incidence and etiology.

Septal deformity is of two kinds, which may occur independently, or together: 1) anterior cartilage deformity of the quadrilateral septal cartilage, caused by direct trauma or pressure at any age; and 2) combined septal deformity involving all the septal components, caused by compression across the maxilla from pressures occurring during pregnancy or parturition. This is part of a facial deformity. The incidence of septal deformity was investigated in 2,380 Caucasian infants at birth, 2,112 adult skulls of five ethnic groups (European, Indian [Asian], Chinese, African and Australian Aborignal)

Abnormal fetal growth patterns. Ultrasonic diagnosis and management.

Diagnostic ultrasound is a useful tool in the management of high-risk pregnancies and provides valuable information about abnormal fetal growth patterns including excessive fetal size and intrauterine growth retardation. Two patterns of growth retardation can be distinguished ultrasonically. Fetuses exhibiting "reduced growth potential" type patterns have little risk of fetal distress and are readily recognized as small for gestational age at birth. Fetuses suffering from uteroplacental insufficiency frequently exhibit a pattern of biparietal growth arrest in the third trimester. These fetuses

Craniofacial surgery: present and future.

The possibilities for radical craniofacial restructuring have increased dramatically in the past 6 years with the development of craniofacial surgery. The field developed from a background of patients with major craniofacial birth defects allowing orderly planning and expansion to correction of a multitude of other craniofacial structural problems. The procedures concentrate upon changing the skeletal structures using extensive subperiostial dissection of soft tissue, and adding bone to fill in areas of deficiency. There are three grades of complexity in craniofacial procedures. After extensiv

[The management of intratemporal facial nerve lesions (author's transl)].

Intratemporal lesions of the facial nerve can be divided into operative lesions, lesions due to birth trauma, trauma through the external auditory canal, gunshot injuries, lesions caused by fractures of the temporal bone and lesions due to intratemporal tumours. Bell's palsy and palsies due to inflammation are not included in this paper. The lesions caused by fractures are discussed in more detail. In longitudinal fractures the nerve is always damaged at the site of the geniculate ganglion and not at the pyramidal segment, as accepted until now. Of the lesions caused by tumours the neurinomas
