Age, cohort and period effects on hip fracture incidence: analysis and predictions from New Zealand data 1974-2007.

UNLABELLED: Hip fractures are substantial problem worldwide. The increase in rate with age does not continue into very old age. Rates decline in successive birth cohorts. If the increasing trend in period effect continues, there will be a substantial increase in hip fracture incidence. INTRODUCTION: The purpose of this study is to (1) determine incidence rates for fracture neck of femur (FNF) for the period 1974-2007, (2) estimate age, cohort, and period effects, and (3) predict the burden of FNF in 2025. METHODS: Age adjusted incidence rates were estimated using New Zealand hospital dischar

Child abuse and orthopaedic injury patterns: analysis at a level I pediatric trauma center.

BACKGROUND: Child abuse is a serious threat to the physical and psychosocial well-being of the pediatric population. Musculoskeletal injuries are common manifestations of child abuse. There have been multiple studies that have attempted to identify the factors associated with, and the specific injury patterns seen with musculoskeletal trauma from child abuse, yet there have been no large studies that have used prospectively collected data and controlled comparisons. The purpose of our study was to describe the patterns of orthopaedic injury for child abuse cases detected in the large urban are

Ontogeny of long bone geometry in capuchin monkeys (Cebus albifrons and Cebus apella): implications for locomotor development and life history.

Studies of a diverse array of animals have found that young individuals often have robust bones for their body size (i.e. augmented cross-sectional dimensions), limiting fracture risk despite general musculoskeletal immaturity. However, previous research has focused primarily on precocial taxa (e.g. rodents, lagomorphs, bovids, goats and emu). In this study, we examined the ontogenetic scaling of humeral and femoral cross-sectional robusticity in a mixed-longitudinal sample of two slow-growing, behaviourally altricial capuchin monkeys. Results showed that, when regressed against biomechanicall

Cesarean section and the risk of fractured femur.

BACKGROUND: The rate of cesarean section is increasing and in the United States recently exceeded 30% of all deliveries. Birth injuries during CS are relatively rare. Femur fractures have a very low incidence during both vaginal delivery and CS. OBJECTIVES: To assess our 10 year experience (2008-1997) in managing fractured femur during CS, including a typical case. METHODS: We reviewed the prevalence of femur fractures in two tertiary, academic, level one trauma center hospitals in Israel (Hadassah in Jerusalem and Soroka in Beer Sheva). RESULTS: During the study period 221,939 deliveries o

Birth injuries in caesarian sections: cases of fracture femur and humerus following caesarian section.

Long bone injuries are less common during caesarian section. Sometimes, they remain unnoticed to the operating surgeon but are frequently noted by attending physician or nurses. The aim of this case study is to remind the surgeon that any forceful extraction may result long bone injuries. So, care should be given during and after delivery to rule out injuries.

Bilateral femur fracture in a newborn: an extreme complication of cesarean delivery.

INTRODUCTION: Bilateral femoral fracture due to birth trauma which is extremely rare, can occur during cesarean section. MATERIAL AND METHODS: This case represents a newborn delivered by cesarean section for breech presentation who sustained bilateral subtrochanteric fracture of the femur. RESULT: The newborn was treated with immobilization in pelvipedal cast. CONCLUSION: It has been concluded that performing accurate delivery technique and immediate evaluation-orthopedic consultation of the newborn in the presence of forced obstetrical maneuver are important.

[Fractures in full-term neonates].

It has recently been suggested in the media that fractures in full-term neonates might be caused by non-accidental injury. It is well known that a traumatic birth can result in fractures in the neonate. To evaluate the incidence of unexplained fractures in full-term neonates, we analysed data from the Netherlands Perinatal Registry, containing admission data on neonates from all 10 Neonatal Intensive Care Units and 60% of the level II hospitals. In 1997-2004, 158,035 full-term neonates were admitted. In 1174 of these (0.74%), fractures were demonstrated. In 227 of these 1174 neonates (19%), no

Comparative study of modified release alpha-blocker exposure in elderly patients with fractures.

PURPOSE: Standard formulation alpha-blocker use has been associated with an increased risk of hypotension-related adverse events, particularly with cardiovascular indications and on treatment initiation. We investigated the association between those fractures that are commonly due to falls and modified-release doxasozin exposure. METHODS: A case-control study using UK primary care records (the THIN database) showed that 6540 cases aged 50 years or older had a record of first fracture of the hip/femur, humerus or wrist after May 2001; 26 495 controls had no fracture and were matched on practic

Randomized prospective study of ACL reconstruction with interference screw fixation in patellar tendon autografts versus femoral metal plate suspension and tibial post fixation in hamstring tendon autografts: 5-year clinical and radiological follow-up results.

Patellar tendon graft has been the most frequently used material in anterior cruciate ligament (ACL) reconstruction, but the hamstring tendons have been increasingly used as well; however, which graft is to be preferred is not adequately supported by existing clinical studies. In this prospective randomized clinical trial, the study hypothesis was that the hamstring tendons are equally good graft material as the patellar tendon in ACL reconstruction. Ninety-nine patients with laxity due to a torn ACL underwent arthroscopically assisted reconstruction with graft randomization according to their

Peak bone mass after exposure to antenatal betamethasone and prematurity: follow-up of a randomized controlled trial.

UNLABELLED: Small birth size is associated with reduced adult bone mass. We determined if antenatal betamethasone exposure, birth weight, or prematurity affects peak bone mass in 174 adults. Antenatal betamethasone exposure did not. Lower birth weight and prematurity predicted reduced adult height. Slower fetal growth rather than prematurity predicted lower bone mass, but this lower bone mass was appropriate for reduced adult height. INTRODUCTION: Small size at birth is reported to be associated with lower bone mass in adulthood. However, previous studies have not distinguished the relative c

Degenerative knee joint disease in mice lacking 3'-phosphoadenosine 5'-phosphosulfate synthetase 2 (Papss2) activity: a putative model of human PAPSS2 deficiency-associated arthrosis.

OBJECTIVE: Murine brachymorphism (bm) results from an autosomal recessive mutation of the Papss2 gene that encodes 3'-phosphoadenosine 5'-phosphosulfate synthetase 2, one of the principal enzymes required for the sulfation of extracellular matrix molecules in cartilage and other tissues. A spondyloepimetaphyseal dysplasia has been identified in Pakistani kindred having a mutation of PAPSS2. In addition to skeletal malformations that include short stature evident at birth due to limb shortening, brachydactyly, and kyphoscoliosis, affected individuals demonstrate premature onset degenerative joi

Fetal femur fracture and external cephalic version.

BACKGROUND: Femoral fractures due to birth trauma are extremely rare. External cephalic version is considered a relatively safe alternative when dealing with breech presentation, but it can be associated with complications. CASE: This patient underwent elective cephalic version for breech presentation at 36 weeks of gestation. Due to concerns of fetal distress after the version, a cesarean delivery was performed. The newborn had a bruised leg at delivery, and X-ray studies confirmed a distal femoral corner fracture. CONCLUSION: This case demonstrates another risk of external cephalic version

Birth-related fractures of long bones.

OBJECTIVE: Birth-related fractures of the long bones are not rare. METHODS: This study presents a retrospective analysis of birth-related fractures of long bones seen at the King Fahd Hospital of the University, Al-Khobar, Saudi Arabia. RESULTS: There were 21 fractures (clavicle 11, femur 6, humerus 3 and radius 1) during the 10-year period, with an overall incidence of 0.67 per 1000 live births. Neonates with fractures had higher birth-weight (p<0.001) as compared to the control group and other parameters like gestational age and mode of delivery were not significant. CONCLUSION: It is rec

Birth-associated femoral fractures: incidence and outcome.

Femoral fractures resulting from birth injuries are rare. The authors undertook a study to assess their incidence and outcome. Seven patients from a total of 55,296 live births suffered 8 femoral fractures (incidence 0.13 per 1,000 live births). Twin pregnancies, breech presentations, prematurity, and disuse osteoporosis were associated with the occurrence of a fracture. The typical injury pattern was a spiral fracture of the proximal half of the femur, which was held in an extended position. A variety of treatment modalities were used, including gallows traction, spica cast, and Pavlik harnes

Ebb and flow rickets in a premature infant: the Afghan turban sign.

The pattern of alternating dense and lucent bands, with straight outer edges, at the youngest metaphysis of long bones, the "Afghan turban" sign, occurs when incompletely treated rickets has recurred and been retreated. Recognition of this, and other, rickets patterns allows the radiologist to influence treatment, as described in a very low birth weight infant.

Use of toenail fluoride levels as an indicator for the risk of hip and forearm fractures in women.

The relation between fluoride intake and risk of osteoporotic fractures remains unclear. The lack of individual measures of long-term fluoride intake has limited epidemiologic studies. We used toenail fluoride in this study as a measure of long-term intake to evaluate the relation between fluoride intake and subsequent risk of hip and distal forearm fractures. Between 1982 and 1984, we collected toenail clippings from 62,641 women in the Nurses' Health Study who were free from cancer, heart disease, stroke, and previous hip or forearm fracture. We identified fracture cases (53 proximal femur a

The development of the growth plate after birth: a study by osteocalcin immunocytochemistry.

The post natal development of the growth plate was studied in the rabbit lower limb using an immunocytochemical technique which localized osteocalcin in situ. At birth, there is poor columnar organization of the physis and bone is not produced. Instead the predominant activity is cartilage remodelling. The columnar architecture develops later when endochondral ossification is also observed.
