BACKGROUND: Accurate preoperative characterization of brachial plexus birth injury (BPBI) is important for counseling and surgical planning, but the prediction of root-level injury remains challenging. We evaluated the agreement between preoperative injury classification and intraoperative nerve root findings, patterns of discordance, and the influence of Horner syndrome (HS).
METHODS: A retrospective review identified 162 infants who underwent primary brachial plexus exploration and reconstruction at a single institution between 2004 and 2023. Preoperative injury classification was assigned

Brachial plexus birth injuries (BPBI) affecting the C5 to C8 nerve roots commonly impair wrist extension, making its restoration a key goal of reconstruction. However, children with persistent wrist extension paralysis have limited surgical options, particularly in the setting of extensive plexus injury. Notably, infants with C5-C8 injuries often lose wrist extension while preserving median nerve function due to intact T1 contributions. We report two infants in whom wrist extension was successfully restored using an anterior interosseous nerve-to-extensor carpi radialis brevis (AIN-ECRB) nerve

Brachial plexus birth injury (BPBI) results in individualized impairments in upper extremity (UE) mobility. A patient-specific understanding of these movement limitations is critical for optimizing decision-making and outcomes; however, current assessments may fail to capture the entirety of a patient's UE mobility. Reachable workspace quantifies an individual's global UE mobility by measuring the regions that can be reached by their hand. Despite most UE activities of daily living requiring adequate close-to-body function, current workspace approaches only assess far-from-body mobility. This

OBJECTIVE: A key residual deficit in brachial plexus birth injury (BPBI) is the lack of active glenohumeral external rotation (ER). Surgical options for reinnervating the suprascapular nerve (SSN) include nerve grafting from a proximal stump and nerve transfer using the spinal accessory nerve (XIN) as a donor. The optimal surgical strategy is unclear from the current literature. This study aimed to compare the outcomes of nerve grafting and nerve transfer for SSN reinnervation in upper trunk BPBI.
METHODS: The authors conducted a retrospective study (1990-2020) on prospectively collected data

PURPOSE: Clinical management for brachial plexus birth injuries (BPBI) is often focused on childhood, leaving a gap in understanding of how this condition impacts individuals into young adulthood. Recent studies suggest that pain persists beyond pediatric care, although the long-term trajectory of pain and function remains poorly understood. This study compares pain interference in young adults with BPBI to matched controls. Secondary objectives were to compare pain intensity between groups, examine the relationship between upper limb function and pain, and identify common areas of pain using

BACKGROUND: Children with brachial plexus birth injury often rely on compensatory movements due to limited shoulder mobility. In clinical practice, the Modified Mallet Scale (MMS) is frequently used to assess shoulder movements. However, MMS scores can be influenced by trunk contributions, preventing an accurate evaluation of pure shoulder complex motion. This study aimed to quantify the relative contributions of the extremity and trunk during MMS movements using wearable inertial sensors.
METHODS: The study included 24 children (7.20 ± 1.41 years; range 4-10 years), and three-dimensional kin

OBJECTIVES: To determine the diagnostic accuracy of MRI for diagnosing nerve injury in brachial plexus birth injury (BPBI).
METHODS: Electronic databases were searched from inception to February 15, 2023 for studies reporting the accuracy of MRI (index test) compared to surgical exploration (reference standard) in detecting the target conditions of: root avulsion, any nerve abnormality, and pseudomeningocele (as a marker of root avulsion) in children with BPBI. Meta-analysis using a bivariate model was performed where data allowed.
RESULTS: Eight studies met the inclusion criteria. In total,

Humerus de-rotational osteotomies are one of the commonly done salvage procedures in children with residual brachial plexus birth injuries. Conventionally, the degree of angular correction was measured clinically intraoperatively by the ability to reach the occiput and the belly or it can be measured by putting reference k-wires at the appropriate angles. However, accurate angular correction is essential to success, as under-correction may lead to parental dissatisfaction and over-correction may lead to loss of mid-line function. BACKGROUND: Magnetic resonance imaging (MRI) has not been routinely used for infants with brachial plexus birth injury (BPBI); instead, the decision to operate is based on the trajectory of clinical recovery by 6 months of age. The aim of this study was to develop an MRI protocol that can be performed without sedation or contrast in order to identify infants who would benefit from surgery at an earlier age than the age at which that decision could be made clinically.
METHODS: This prospective multicenter NAPTIME (Non-Anesthetized Plexus Technique for Infant MRI Evaluation) study included infa

BACKGROUND: Birth brachial plexus palsy (BBPP) can severely impair shoulder function by restricting external rotation and abduction, often leading to posterior subluxation or dislocation. Progressive shoulder dysplasia, a secondary condition of BBPP, further exacerbates functional disability by limiting shoulder function. Several techniques have been developed to address these challenges, including microsurgery, muscle transfer, and bony procedures. Recent reports on lower trapezius muscle transfer to the infraspinatus footprint demonstrate promising results in restoring shoulder biomechanics

This retrospective study aimed to compare two methods of reconstruction of the suprascapular nerve in neonatal brachial plexus palsy. Patients who underwent suprascapular nerve reconstruction for neonatal C5-C6 (±C7) brachial plexus palsy were included if they had at least 3 years follow-up. The results obtained after nerve surgery alone and at the end of the follow-up (after shoulder release surgery) were analysed. The primary outcome was the range of external rotation with the arm at the side. Thirty-three patients were included (19 nerve transfers, 14 nerve grafting). At last follow-up (mea

Abduction is one of the important shoulder functions that may be limited after an obstetric brachial plexus injury. We investigated the progress of spontaneous recovery of active abduction in a cohort of 68 conservatively treated obstetric brachial plexus injury patients without full recovery and with less than 90° abduction at 3 months. Of these 65 (96%) recovered at least 90° and 32 (47%) a full 170° of abduction. The median age of recovery to 90° was 9 months (IQR 7-12, range 5-65 and to 170° 24 months (IQR 12-36, range 5-84). The presence of active antigravity elbow flexion ≥90° at 4 month

Neonatal brachial plexus palsy (NBPP) is caused by injury to the C5 to T1 nerve roots, which are responsible for both sensory and motor functions in the upper limbs. Importantly, NBPP is not always linked to delivery-related trauma. We present the case of a 16-day-old neonate who was evaluated in the emergency department for fever, lethargy, and skin pallor. In the context of an infection, the infant developed desaturation and a progressive loss of spontaneous mobility in the upper limbs, with the arms positioned in extension, adduction, and pronation. Suspecting an infectious complication in

Birth brachial plexus palsy (BBPP) can lead to significant functional impairment of the upper limb, particularly affecting hand function. Despite advancements in primary nerve reconstruction, many patients require secondary procedures to optimise hand use. This study evaluates surgical strategies aimed at restoring hand function in BBPP, emphasising the timing of intervention, nerve transfer options and reconstructive techniques. A comprehensive review of direct root transfers, secondary tendon transfers, along with an analysis of sensory re-education and long-term functional outcomes. Early n

BACKGROUND: Obstetric brachial plexus palsy (OBPP) may result in lifelong shoulder dysfunction despite nerve repair surgery. Tendon transfer has emerged as a surgical option to restore external rotation and abduction in OBPP sequelae. Single and double tendon transfer techniques have been developed, but the optimal approach remains debated. This meta-analysis is the first to compare the range of motion outcomes and complications, particularly loss of midline function (LOM), between the two techniques in managing OBPP sequelae.
METHODS: On January 2025, systematic literature search was perform

Brachial plexus birth injury (BPBI), one of the most common nerve injuries in children, often leads to impaired shoulder development, resulting in sustained postural and bone deformity and muscle weakness. Despite the substantial long-term consequences, clinical consensus is lacking for what BPBI treatments are optimal in terms of timing and approach, primarily because BPBI sequelae are complex, involving stunted muscle growth, muscle denervation, and limb disuse that can disrupt glenohumeral joint development. Injury can occur as nerve rupture (postganglionic injury) or nerve avulsion (pregan

Background: Primary neuroma excision and autografting are the gold-standard surgical treatments for brachial plexus birth injury (BPBI). With the rising popularity of distal nerve transfers, it is more frequently utilised to manage BPBI. However, distal transfers for BPBI remain under investigation. We reviewed the updated evidence for nerve transfers compared to nerve grafting as a primary approach for shoulder and elbow reconstruction in Narakas I and II neonatal brachial plexus palsy. Methods: A systematic review of the literature was performed according to the PRISMA guidelines. PubMed and
